1.Expression and significance of podoplanin,?-catenin and vascular endothelial growth factor receptor-3 in tissue of esophageal squamous cell carcinoma

Objective To investigate the expression of podoplanin,?-catenin and vascular endothelial growth factor receptor-3(VEGFR-3) in lymphatic vessels of human esophageal squamous cell carcinoma(ESCC) tissue and the normal tissues adjacent to the carcinoma,and explore the markers which can specifically show the lymphatics with metastasis.Methods 63 surgical specimens of esophageal squamous cell carcinoma and 20 normal tissue specimens were obtained and examined.Immunohisochemical measures were employed to detect the expressions of podoplanin,?-catenin and VEGFR-3.The difference in expression of VEGFR-3 between ESCC tissue with lymphatic metastasis and the tissue without metastasis was statistically assessed.The lymphatic microvessel density(LMVD) positively stained by podoplanin was observed and counted under microscope with ?200 magnification.Results Based on the morphological characters of lymphatic vessels and capillary vessels observed under microscope,it was confirmed that podoplanin(positively stained as yellow-brownish discontinuous lines) expressed only on the cellular membrane of endothelial cells of lymphatic and microlymphatic vessels.VEGFR-3 was positively expressed in the endothelial cells of lymphatic vessels as well as vessel wall and the cytoplasm of carcinoma cell.There existed a significant difference in VEGFR-3 expression between the ESCC tissue with lymphatic metastasis and the tissue without metastasis,and the positive rate was 68.57% and 28.00%,respectively(P
2.Clinical study on treatment of acute biliary tract infection with biliary drainage via endoscope combined with Chinese drug medication.

OBJECTIVETo observe the curative effects of biliary drainage via endoscope (ENBD) combined with Chinese drug medication on acute biliary tract infection (ABTI) and the influence of treatment on complement 3 (C3), C-reactive protein (CRP) and interleukin-6 (IL-6).
METHODSSixteen patients with ABTI were randomly assigned to two groups: the 9 patients in the combined treatment group (CTG) treated with ENBD combined with Chinese medicine and the 7 patients in routine treatment group (RTG) treated with ENBD alone. Another 18 patients with simple gallbladder stone were taken as the control group (CG). The curative effect was observed and the serum concentrations of C3, CRP and IL-6 were determined before and after treatment.
RESULTSBefore treatment, the concentrations of CRP and IL-6 were significantly higher and C3 lower in all ABTI patients than those in patients with simple gallbladder stone (P < 0.05 or P < 0.01). After treatment, the general condition of patients was improved, the recovery time of intestinal tract function was shortened and the concentrations of C3, CRP and IL-6 significantly decreased in CTG, with the effects better than those in RTG respectively (P < 0.01 or P < 0.05).
CONCLUSIONENBD combined with Chinese drug medication shows favorable curative effects on ABTI. Treatment with Chinese medicine according to syndrome differentiation could decrease blood level of pro-inflammatory cytokines and promote recovery of the injured immune function.

7.Comparative analysis of POCT and routine test in BNP testing

Objective To analyze the relevance between the point-of-care testing (POCT) and routine test in BNP testing .Meth-ods The whole blood samples or plasma samples from 40 inpatients were detected brain natriuretic peptide (BNP) by the Alere Triage? MeterPro fluorescence immunoassay analyzer (POCT ) or the Beckman Coulter Access ?2 chemiluminescence analyzer (routine test) ,respectively .The acquired data were subjected to comparative analysis according to the CLCS EP 9-A2 .Results The linear regression of the BNP content in the blood samples detected by POCT and the routine test was good ,the correlation coeffi-cient(r) was 0 .999 7 .Conclusion POCT and the routine test have good correlation in BNP testing .POCT for BNP testing has higher reliability and is applicable for clinical detection .
8.Congenital intestinal malrotation in children

Objective To summarize reasonable therapeutic measures by analyzing the characteristics of tardive intestinal malrotation in childhood. Methods Clinical data of 23 definitely diagnosed cases of intestinal mal-rotation from 1998 to 2008 were studied retrospectively. Results All of 23 patients were processed through plain ab-dominal radiograph;9 were examined with barium meal;2 were examined with barium enema;20 were examined by abdominal Doppler and 13 were examined by abdominal computer tomography. There were 18 patients who got final diagnosis preoperatively and the rate of final diagnosis was 78.3%. All of the cases were cured with Ladd's operative method. Conclusion Because tardive intestinal malrotation has various clinical manifestations,it is more difficult to diagnose than congenital intestinal malrotation in children. The main causes of death are intestinal volvulus and/or necrosis. At present,surgery is a traditional therapeutic way. Ladd procedure is the main traditional way to treat mal-trotation.
9.Risk factors of early rebleeding after endoscopic variceal ligation and/or endoscopic injection of fibrin tissue adhesive

Objective To study the independent risk factors of early rebleeding after endoscopic variceal ligation (EVL) and/or endoscopic injection of fibrin tissue adhesive.Methods Data of 370 patients who had cirrhosis and accepted 396 procedures of EVL and/or endoscopic injection of fibrin tissue adhesive were retrospectively studied.Independent risk factors for early rebleeding were determined by Logistic regression analysis.Results Results of all the factors that were significantly different between the re-bleeding and non-rebleeding patients,the portal vein diameter,ascites volume,Child-Pugh score and serum albumin were independent ones of early rebleeding after EVL and/or endoscopic injection of fibrin tissue adhesive ( P < 0.05).Larger volume of ascites,wider portal vein diameter,reduced albumin,Child-Pugh score greater than 10 were indicative factors of rebleeding.Conclusion The early rebleeding rate after EVL and/or endoscopic injection of fibrin tissue adhesive is determined by portal vein diameter,ascites volume,ChildPugh score,and serum albumin.
