Lateral thoracic flap is a free flap for the reconstruction of soft tissue defects and has been previously used in oral and maxillofacial surgeries. We reported a case involving a huge soft tissue defect on the parotid region caused by the resection of parotid epithelial myoepithelial carcinoma. We discussed the anatomy and application of the lateral thoracic flap.

3.Analysis on Effect of Free-frying Granules of Traditional Chinese Medicine on Treatment of Chronic Prostatitis

This study was aimed to evaluate the efficacy and safety of free-frying granules of traditional Chinese medicine(TCM) on treatment of chronic prostatitis. A total of 120 chronic prostatitis patients that were diagnosed with the dampness-heat and blood-stasis syndrome and kidney-deficiency syndrome were divided into the TCM group and western medicine group by random digit table. Category II and ⅢA chronic prostatitis were treated with oral administration of Levofloxacin Mesylate Tablets (0.2 g, bid) and the category ⅢB chronic prostatitis was treated with oral administration of Terazosin Hydrochloride Tablets (2 mg, qd). In the TCM group, the dampness-heat and blood-stasis syndrome was treated with Tongluo Qingjie formula ( which was self-made formula containing Rhizoma Dioscoreae Hypoglaucae, Herba Patriniae, Semen Plantaginis, and etc.) and the kidney-deficiency syndrome was treated with Jiawei Wuzhi Yanzong pills (which containing Semen Cuscutae, Chinese Wolfberry, Radix Rehmanniae Preparata, Fructus Rubi and Schisandra chinensis, and etc.). Four weeks was one course of treatment. The results showed that the total effective rate of TCM group was better than that of western medicine group (P< 0.05). Effect of TCM group was obviously better than that of western medicine group in the improvement of prostatitis NIH-CPSI score, the quality of life and symptoms of the patients. TCM may increase lecithin corpuscle in prostatic fluid of ⅢA chronic prostatitis (P< 0.05). Meanwhile, no obvious toxiferous and side reactions were found. It was concluded that the free-frying granules of TCM have good efficacy and safety in the treatment of chronic prostatitis.
4.Validation of relationship between vancomycin trough concentration and nephrotoxicity:A Meta-analysis based on bivariate model

Objective To evaluate the association between vancomycin trough concentrations and incidence of nephrotoxicity . Methods Pubmed, EMbase, Cochrane Library, China National Knowledge Infra-structure ( CNKI ) , WanFang Data and Chinese Bio Medical Literature Database ( CBM) were systematically searched from inception to February 2014.Specificity, sensitivity and predictive odds ratios with 95% confi-dence intervals were calculated.Results Seventeen observational stu-dies and 2802 patients were included in the Meta-analysis.While cut-offs of trough concentrations were 10 , 15 or 20 mg? L-1 , pooled sensi-tivity were 0.83, 0.69 or 0.43, pooled specificity were 0.35, 0.61 or 0.83 , pooled predictive odds ratios were 2.65 , 3.60 or 3.85. Conclusion There is definitive association between vancomycin trough concentrations ( ascending from 10 to 20 mg? L-1 ) and incidence of nephrotoxicity.Trough concentration is an appropriate parameter of thera-peutic drug monitoring of vancomycin.
5.The prediction of maternal HBV transmission by breast milk of postpartum women with chronic HBV infection

Objective To investigate the prediction of maternal HBV transmission by breast milk of postpartum women with chronic HBV infection.Methods HBV DNA levels in serum and breast milk weredetected by fluorescent quantitative polymerase chain reaction in 64 postpartum women with chronic HBV infection.HBV DNA≥1.0×103copies/ml was defined as positive,and correlation analysis was conducted.Results HBV DNA positive rate was 78.1%and 62.5%in serum and breast milk respectively,with a HBV DNA range of 1.05×103~3.87 ×104copies/ml in breast milk.When HBV DNA in serum was 1.0×105~1.0×107copies/ml,the HBV DNA positive rate in breast milk reached to 94.9%;however,when HBV DNA in serum was 1.0×103~1.0×104copies/ml,the positive rate in breast milk was only 18.2%.Conclusion The HBV DNA positive rate of breast milk in postpartum women with chronic HBV infection is correlated with the HBV DNA levels in serum;and breast-feeding should be avoided for postpartum women with HBV DNA≥1.0×105copies/ml in the serum.So serum HBV DNA detection is necessary in antenatal care for women with chronic HBV infection.
6.Expression NOS in acute lung injury following limb ischemia/reperfusion and its significance in rats.

AIMTo investigate the expression and role of inducible NOS (iNOS) and endothelial NOS (eNOS) in acute lung injury following limb ischemia/reperfusion (4h/4h).
METHODSWistar rats were randomized into four groups: control group, ischemia/reperfusion (I/R) group, L-Arginine (L-Arg) pretreatment group, Aminoguanidine (AG) pretreatment group. The lung tissue of each group was subjected to assay of content of MDA, MPO, W/D and NO2-/NO3-. The expression of iNOS and eNOS was examined with immunohistological staining. The pulmonary morphologic changes were observed under microscope respectively.
RESULTSThe acute lung injury existed after limb ischemia/reperfusion. The eNOS downregulation and iNOS upregulation among I/R, L-Arg and AG groups were observed contrasted to the control group. There was no expressional and statistical difference of iNOS between I/R group and L-Arg group. The expression of eNOS was similar between IR and AG but iNOS expression was downregulated in AG. The parameters of MDA, MPO, W/D and NO2-/NO3- in pulmonary tissue were significantly increased in I/R groups compared with those of the control group. The parameters of L-Arg and AG pretreatment groups in comparison with those of the I/R group showed significantly difference. Based on the results of pulmonary pathology, the congestion and infiltration of inflammatory cells existed obviously in IR group. L-Arg played definite role in militating lung injury and AG might make lung injury aggravated.
CONCLUSIONThe NO definite production from iNOS is possible to play a competitivly protective role in acute lung injury following limb ischemia/reperfusion and antagonist of iNOS may aggravate the lung injury.

7.The protect effect of ischemic preconditioning on the gastric mucosal injury following ischemia/reperfusion of hind limbs of rats.

AIMTo observe the degree of gastric mucosal injury following limb ischemia/reperfusion (LI/R), and to investigate the mechanism of gastric mucosal injury and the protection of ischemic preconditioning (IPC) on gastric mucosal injury.
METHODSThe model rats which underwent 4 hours of ischemia and 4 hours of reperfusion of hind limbs were made. Then we respectively observed and determined the histologic lesion score after I/R and IPC + I/R. The gastric barrier mucus in mucus were measured in different groups. The values of MPO, SOD, MDA and XOD in gastric mucosa and the values of MDA, XOD, SOD, LDH in plasma were detected.
RESULTSIn the LI/R group, the histologic lesion score increased significantly. The content of gastric barrier mucus in mucus decreased significantly. The value of MPO, MDA, XOD in gastric mucosa and the values of MDA, XOD, LDH in plasma increased remarkably and SOD activity in gastric mucosa and in plasma decreased. However in the IPC group, the histologic lesion score decreased significantly and the content of gastric barrier mucus in mucus increased significantly and the value of MPO MDA XOD LDH in gastric mucosa or in plasma decreased remarkably and the SOD activity increased compared to LI/R group.
CONCLUSIONLI/R will lead to the development of stress ulcer, oxygen free radicals play an important role in it. IPC can alleviate the damage of gastric mucosa following ischemia/reperfusion of hind limbs. The decrease of OFR is one of the protection mechanism of IPC.

8.Effects of yogurt supplementation on the growth of preschool children in Beijing suburbs.

OBJECTIVETo investigate the effect of yogurt supplementation on the growth of preschool children in Beijing suburbs.
METHODSFour hundred and two preschool children (217 males, 185 females), aged 3-5 years, whose height for age and/or weight for age were less than the reference level, were selected as subjects from 7 kindergartens in Beijing Fangshan District. The subjects were divided randomly into control group (CG, 201) and yogurt supplemented group (YG, 201). Each subject in YG was given one serving of yogurt (125 g) for 5 days a week from March to December in 2001, while nothing additional was provided to CG. All subjects kept their usual diet during the study. Anthropometry (body height and weight and upper-arm circumference) and the bone mineral density (BMD) of forearm were measured every 3 months. Disease status and dietary intake were also recorded and assessed.
RESULTSThe intake of calcium, zinc, and vitamin B2 in YG was significantly higher than that in CG. The incidence and duration of upper-respiratory infection and diarrhea of children in YG were significantly less than those in CG. The height gain of children in YG was significantly higher than that in CG after yogurt was supplemented for 3, 6, and 9 months (P<0.05) (1.90+/-0.49 cm vs 1.77+/-0.54 cm, 3.83+/-0.57 cm vs 3.64+/-0.66 cm and 5.43+/-0.69 cm vs 5.24+/-0.76 cm, respectively). The weight gain of children in YG was significantly higher than that in CG after yogurt was supplemented for 3, 6, and 9 months (P<0.05) (0.70+/-0.43 kg vs 0.49+/-0.35 kg, 0.98+/-0.62 kg vs 0.80+/-0.60 kg and 1.42+/-0.76 kg vs 1.20+/-0.67 kg, respectively). The BMD of children in YG was significantly higher than that in CG after yogurt was supplemented for 9 months (P<0.05) (0.415+/-0.058 g/cm2 vs 0.400+/-0.065 g/cm2).
CONCLUSIONYogurt is beneficial to the improvement of calcium, zinc, and vitamin B2 intake, the decreasing of the incidence and duration of upper-respiratory infection and diarrhea, and the promotion of the health and the growth and development of preschool children.

Erectile dysfunction attributable to testosterone deficiency is less common in young males, and the effect of estradiol on erectile function in eugonadal young males is unclear. We analyzed data from 195 male participants, including 143 eugonadal patients with erectile dysfunction and 52 healthy men. To distinguish psychogenic and organic erectile dysfunction, penile rigidity was measured using the nocturnal penile tumescence rigidity test. Serum levels of sexual hormones were quantified by electrochemiluminescence, and penile vascular status was assessed by penile color Doppler ultrasound. Both serum estradiol levels and the ratio of estradiol to testosterone were higher in patients with organic erectile dysfunction than in patients with psychogenic erectile dysfunction or healthy controls. Organic erectile dysfunction was negatively associated with estradiol levels and the ratio of estradiol to testosterone, and estradiol was the only significant risk factor for organic erectile dysfunction (odds ratio: 1.094; 95% confidence interval: 1.042-1.149, P = 0.000). Moreover, serum estradiol levels were negatively correlated with penile rigidity. Serum estradiol levels were higher and penile rigidity was lower in patients with venous erectile dysfunction than in patients with nonvascular erectile dysfunction. We conclude that elevated serum estradiol levels may impair erectile function and may be involved in the pathogenesis of organic erectile dysfunction in eugonadal young men.
10.Diagnosis and treatment of ejaculatory duct obstruction: Current status and advances.

Ejaculatory duct obstruction (EDO) is one of the obstructive factors for 1-5% of all cases of male infertility and it is, however, surgically correctable. Congenital developmental abnormality is a most common cause of EDO. The clinical manifestations of EDO are varied, typically with the decline of four semen parameters. Transrectal ultrasonography is an important imaging method for the diagnosis of EDO and guidance in its surgery. MRI provides high-resolution images of the reproductive system as evidence. Transurethral resection of the ejaculatory duct (TURED) is a classical operation, the application of transurethral seminal vesiculoscopy has become a new trend of minimally invasive surgery in the treatment of EDO, and the latest flexible vesiculovasoscopy (FVV) or vasoscopy techniques may further improve the diagnosis and treatment of EDO.
