1.Effects of fentanyl on GABAA receptor in rat hippocampal pyramidal neurons

Objective To investigate the inhibitory effects of fentanyl on GABAA receptors in hippocampal pyramidal neurons of rat.Methods Pyramidal neurons were acutely isolated from 3-10 day old SD rats of either sex by enzymatic-mechanic method. GABAA receptor mediated currents ( IGABA) were recorded using voltage clamped whole cell patch clamp technique in gap-free mode at the holding potential (VH) of - 50 mV. Current-voltage relationship of IGABA was obtained in ramp protocol ranging from + 30 mV to - 110 mV and lasting for 1 600 ms. Data were collected by using a system consisting of Axopatch 200B patch-clamp amplifier, Pentium Ⅲ computer and Digidata 1200 interface. All experiments were performed at room temperature (22-25℃). Five to twelve neurons were used for each fentanyl concentration. The effects of fentanyl from 1.0 ? 10-5 ?mol?L-1 to 10. 0 ?mol?L-1 were evaluated by the inhibition rate of the peak amplitude of IGABA, the desensitization time constant (?des) of IGABA and the reversal potential (Ecl- ) of IGABA. A ?-opioid receptor selective antagonist CTAP 1 ?mol?L-1 was applied and its effects on fentanyl were recorded. Results (1) GAB A 1-1 000 ?mol?L-1 induced inward currents (IGABA) dose-dependently with an EC50 of 23.73 ?mol?L-1.IGABA induced by GABA 30 ?mol?L-1 was blocked by bicuculline 1 ?mol?L-1. (2) Fentanyl depressed IGABA dose-dependently with EC50 of 0.011 ?mol?L-1 and shortened the rdes of IGABA.(3) The inhibitory effects of fentanyl on IGABA were antagonized by CTAP. (4) Fentanyl 0.01 ?mol?L-1 and CTAP did not influence the reversal potential of IGABA (Ecl- -3.0 mV) .Conclusion Fentanyl inhibits the function of GABAA receptors through ?-opioid receptors in hippocampal pyramidal neurons. Hippocampus may play a role in the neuroexcitatory effects of opioids.
2.Role of microenvironment on in vitro differentiation of mesenchymal stem cells into cardiomyogenic cells

Objective To find the cells in myocardium controlling in cardiomyogenic differentiation of marrow derived-mesenchymal stem cells(MSCs).Methods Rat marrow derived-MSCs,cardiomyocytes(CM)and endothelial cells(EC)were isolated and cultured,then MSCs were incubated with the CM or EC or cultured alone after labeled by 5-Bromo-2-deoxyuridine(BrdU)for 72 hours.The cells were identified by morphology,immunocytochemistry and double immunocytochemistry.Results More than 95% of the cells isolated and cultured belonged to MSCs,CM or EC.Incubated with CM,most MSCs labeled by BrdU showed cardiomyocyte-like morphology.After 4 weeks of cultivation,positive staining of double immunocytochemistry of BrdU and Sarcomeric Actin or Connexin-43(CX-43)were observed in 44% cells.However,there was no obvious change in morphology and no positive staining of double immunocytochemistry was recorded in MSCs incubated with EC or cultured alone after 1、2、3 and 4 weeks of cultivation.Conclusion MSCs can differentiate into cardiomyogenic cells in cardiomyocyte microenvironment and cardiomyocytes may play a crucial role in the differentiation.
3.Proliferation capacity of rabbit adipose tissues-derived mesenchymal stem cells at early stage in vitro

BACKGROUND:Surface marker molecules and differential ability of human adipose tissues-derived mesenchymal stem cells can change with the prolonged culture time.OBJECTIVE:To analyze the morphological characteristics and colony forming capacity of early cultured rabbit adipose tissues-derived mesenchymal stem cells in vitro.DESIGN,TIME AND SETTING:The cytological in vitro study was performed at the Institute of Ophthalmology,and Central Laboratory,Eye and ENT Hospital,Fudan University from January to March 2008.MATERIALS:Nine female New Zealand rabbits aged three months were obtained from the Shanghai Yingenyang Rabbit Room.METHODS:Adipose tissue was harvested from the nape fat pad of the New Zealand rabbits following anesthesia.Primary cultured cells were established using type I collagenase and cell cultures were maintained with DMEM containing 0.1 volume fraction of fetal bovine serum.Cells were passaged when 80% was confluent.The second,third and fourth passages of cells were utilized for the study.MAIN OUTCOME MEASURES:The morphology of adipose tissues-derived mesenchymal stem cells was observed using inverted microscope.Cell phenotype was detected utilizing flow cytometry.The cloning capacity was determined.RESULTS:Adipose tissues-derived mesenchymal stem cells were fibroblast-like and proliferated rapidly in vitro at the second,third and fourth passage.Flow cytometry showed that adipose tissues-derived mesenchymal stem cells expressed CD29 and PCNA.The colony-forming rates were respectively(8.0?0.6)%,(6.7?0.4)% and(4.6?0.5)%(F=12.18,P
4.Research in perioperative nursing methods in patients with OSAHS and type 2 diabetes mellitus

Objective To study the perioperative nursing methods in patients with OSAHS and type 2 diabetes mellitus.Methods From March 2009 to December 2010,a total of 46 patients with OSAHS and type 2 diabetes mellitus were randomly divided into the control group and the experimental group with 23 cases in each group.Conventional care was given to the control group,while the experimental group received systematic,phased and individualized nursing.The sleep quality before and after the operation was compared.The oropharynx wound healing and possible complications,pain were observed after operation.Results In the control group,there was one patient with bleeding during operation,one patient happened blood lost after operation and 2 patients had postoperative infection.The hyperglycemia was found in 3 patients and hypoglycemia occurred in 2 patients.But in the experimental group,all of the patients did not suffer complication.The scores of sleep quality on the day before surgery were significantly higher than those scores on admission,and the scores on discharge from hospital were significantly lower than those scores on admission.There were statistical differences.The scores of the experimental group were significantly improved compared to those of the control group.The score of pain was the highest at the day of surgery,and then gradually decreased.The score of pain of the experimental group was significantly lower than that of the control group,and the difference was statistically significant.Conclusions This comprehensive nursing method during operation period for patients with OSAHS and type 2 diabetes mellitus not only can reduce risk of postoperative complication,but also can improve operation efficacy,quality of nursing and life quality of patients as well as can speed recovery.It approved that this standardized,systematic nursing method during the whole period is practical and effective in OSAHS and type 2 diabetes mellitus treatment and care.
5.The clinical value of PCT levels in bacteria identification in ICU patients with bloodstream infection

Objective To investigate the clinical value of procalcitonin (PCT) levels in bacteria identification in intensive care unit (ICU) patients with bloodstream infection.Methods There were 540 cases of patients with bloodstream infection in our ICU between December 2007 and December 2013.The PCT levels and bacteria were identified.The application effectiveness of PCT levels in the bacteria identification was studied.Results The G+ bacteria infection rate was 49.63% (268/540),G-bacteria infection rate was 38.52% (208/540),and the fungal infection rate was 11.85% (64/540).The patients of G-bacteria had significant difference with G + bacteria and fungal infection (P < 0.05).The PCT average and positive rate of G-bacteria were significantly higher than G + bacteria and fungi group (P < 0.05),respectively.G+ bacteria and fungi infection did not have significant difference (P > 0.05).When PCT > 2.04 ng/ml,the sensitivity and specificity that applying serum PCT level to identify the between G-and G+ bacteria were 82.18% and 76.09%,respectively.When PCT >3.16 ng/ml,the sensitivity and specificity that applying serum PCT level to identify the between G-and fungus bacteria were 59.42% and 65.73%,respectively.Conclusions The identification between G-bacteria and G + bacteria,fungi with applying PCT level in bloodstream infections had high accuracy.When the PCT levels was greater than 2.04 ng/ml,the occurrence of G-bacteria was greater risk of infection.The accuracy of PCT level identifying the G + bacteria and fungi was poor.
7.Expression of MMP-2 and MMP-9 in 53 Cases of Salivary Adenoid Cystic Carcinoma and Its Relationship with Invasion and Metastasis

PurposeTo study the patterns of the expression of MMP-2 and MMP-9 in human salivary adenoid cystic carcinoma and evaluate the relationship between the expression and nerve invasion and lymph node metastasis. MethodsThe immunohistochemical location of MMP-2 and MMP-9 was evaluated by using Envision immunoperoxidase technique. Studies were performed in 53 surgical excised specimens. The immunohistochemical staining results were densitometrically semiquantitatived. ResultsThe expressions of MMP-2 and MMP-9 in salivary adenoid cystic carcinoma were 67.92 % and 79.25 % respectively. The MMP-2 and MMP-9 levels in salivary adenoid cystic carcinoma with nerve invasion were much higher than those without invasion(P< 0.05, P< 0.05 respectively), and the percentage of lymph node metastasis rose with the change of MMP-2 and MMP-9 (P < 0.05, P < 0.05 respectively). ConclusionsThe results indicate that the rise of MMP-2 and MMP-9 correlate with the nerve invasion and lymph node metastasis.
8.Comparison of transcatheter intervention and surgical operation in pulmonary atresia with intact ventricular septum

Objective To compare the results of transcatheter intervention (TI) and surgical operation (SO) in the pri-mary treatment for pulmonary atrcsia with intact ventricular septum (PA/IVS). Methods From January 2006 to May 2009, 25 patients (20 male,5 female) with PA/IVS were treated. The age at treatment was from 2 days to 8 months. The body weight was from 2.1 kg to 6.7 kg. All patients had mild to moderate hypoplasia of the right ventricle (the Z-valvue of the tricuspid valve: from -2 to 1.5) with tripatite right ventricle and without coronary artery-right ventricular fistula. Eight patients under-went TI and 17 patients underwent SO. Results The primary procedure was successful in 7 patients (88%) in TI group and in 16 patients (94%) in SO group. There were 2 deaths (1 in each group). There was no significant difference regarding re-sidual pulmonary stenosis between two groups. The ventilation time and the days of hospital stay were shorter in TI group than in SO group. All the survivors were followed up for 3-36 months. One patient required repeat balloon dilation in TI group. While in SO group, one required reoperation and one required balloon dilation for PS, and 2 patients were waiting for balloon dilation. Twenty-one patients have achieved complete biventricular circulation. Two patients were scheduled for Glenn shunt operation later (1 in each group). Conclusion For PA/IVS patients with mild or moderate right ventricle hypoplasia, tran-scatheter intervention is a better alternative than surgical operation in the primary treatment.
9.Case-control study on cancellous screw and compression bolt in the treatment of calcaneal fractures wih minimally invasive plate.

OBJECTIVETo evaluate percutaneous reduction by Kirschner pin and internal fixation with cancellous screw and plate through minimal incision at the lateral side of heel for the treatment of intra-articular displacement fracture of calcaneus.
METHODSFrom September 2009 to June 2012, 80 patients with close intra-articular displacement fractures of calcaneus were divided into treatment group and control group. There were 40 patients in the control group, including 21 males and 19 females, ranging in age from 23 to 64 years old, averaged (39.1 +/-11.7) years old; and the patients were treated with fixation by compress screws and plates of self-broken type. There were 40 patients in the treatment group, including 24 males and 16 females,ranging in age from 21 to 67 years old, averaged (39.6+/-14.3) years old;and the patients were treated with full thread cancellous screws and plates fixation. The fixation time,intra-operative blood loss and the number of injured medial plntar nerve were compared between the two groups. The Böhler angle, Gissane angle and the correction degree of calcaneal width were measured at 1 year after operation. The internal fixators were taken out after 1 year. The AOFAS ankle-hindfoot score was used to evaluate therapeutic effects.
RESULTSAll the patients were followed up,and the duration ranged from 9 to 32 months, with a mean of 16 months. There were shorter operation time, less injury, satisfactory postoperative Böhler angle and calcaneal width in treatment group compared with control group,which reduced the medial neurovascular injuries, and the plate was easily removed after fracture healing.
CONCLUSIONFull thread cancellous screws and plate fixation for the treatment of calcaneal fractures could get same therapeutic effects as compress screws and plate of self-broken type fixation, and also has follow advantages: shortening the operation time, reducing blood loss, easily to be removed, avoiding the medial neurovascular damage, reducing the incidence of complications.

10.Effects of Nourishing Yin Removing Fire Chinese Herbs on Gene Expression of Hypothalamic Ghrelin and its Receptor in Female Precocious Rats.

OBJECTIVETo observe the effect of nourishing yin removing fire Chinese herbs (NYRF-CH) on the gene expression of hypothalamic growth hormone secretion peptide (Ghrelin) and its receptor growth hormone secretion peptide receptor 1alpha (GHSR1-alpha) at the puberty onset of danazol induced female precocious rats.
METHODSForty female SD rats were randomly divided into 4 groups, i.e., the normal group (N), the model group (M), the normal saline intervention group (NS), and the NYRFCH intervention group (NI), 10 in each group. 300 microg danazol was subcutaneously injected to all rats except those in the N group to prepare precocious rat model. NYRFCH and normal saline was respectively administered to rats in the NI and the NS group from the 15th day old for 7-10 days. No treatment was given to rats in the N group. Time of rats' vulva opening was recorded. Ovary index and uterus index were calculated. Peripheral blood levels of estradiol (E2), follicle stimulating hormone (FSH), and luteinizing hormone (LH), and hypothalamic contents of gonadotropin releasing hormone (GnRH) as well as the gene expression of hypothalamic Ghrelin and GHSR1-alpha were determined. Results Compared with the N group, the vulva opening time was advanced in the model group; peripheral blood levels of E2 and LH, uterus index, hypothalamic contents of GnRH increased; peripheral blood FSH levels and mRNA levels of hypothalamic Ghrelin and GHSR1-alpha decreased (P < 0.05, P < 0.01). Compared with the M group and the NS group, the vulva opening time was not advanced in the NI group; peripheral blood levels of E2 and LH, uterus index and hypothalamic contents of GnRH obviously decreased (P < 0.05, P < 0.01); mRNA levels of hypothalamic Ghrelin and GHSR1-alpha increased (all P < 0.01). But there was no statistical difference in the hypothalamic contents of Ghrelin, or the number and activity of GHSR1-alpha (P > 0.05).
CONCLUSIONNYRFCH had regulatory effect on regulating hypothalamic Ghrelin and GHSR1-alpha at gene transcription levels.
