2.Treatment of Posterior Ankle Impingement Syndrome with Arthroscopy

Objective To discuss and summarize the effects of arthroscopic treatment of posterior ankle impingement syndrome.Methods The study enrolled 30 patients with posterior ankle impingement syndrome treated from January 2008 to November 2015.There were 18 males and 12 females,aged 19-58 years old (mean,37.6 years old).Right ankle injury was noted in 19 patients and left in 11 patients.A total of 27 patients had a history of obvious ankle injury (7 cases of acute sprains and 20 cases of chronic sprains).Arthroscopic surgery,including osteophyte removal,cartilage formation and joint clearance,was performed through the posterolateral and posteromedial approaches.Postoperative outcome was evaluated by using the American Orthopedic Foot and Ankle Society (AOFAS) scores and visual analogue scale (VAS) scores.Results All the patients were followed up for 8-27 months (mean,22 months).These patients got a good healing without nervous and vascular injuries.At the last follow up,the AOFAS score was (89.8 ± 3.8) points,significantly higher than before operation [(35.0 ± 10.1) points,t =-26.456,P =0.000].The VAS score was (2.5 ± 1.3) points,significantly lower than before operation [(6.6 ± 1.9) points,t =12.427,P =0.000].The ankle joint flexion angle was improved significantly from 29.4° ± 2.3° to 43.4° ± 1.4° as compared to preoperative status (t =-32.625,P =0.000).There were 21 cases of excellent result and 9 cases of good result,with an excellent and good rate of 100%.Conclusion Debridement and treatment of complications under arthroscopy is one of the effective methods for the posterior ankle impingement syndrome,which has advantages of minimal invasion,safety,targeted operation,and fast postoperative recovery.
3.A preliminary discussion about the teaching of medical students in ICU

After summing up the experience of recent years' clinic teaching, ICU department in Fangcun Branch of Second Clinical Medical College of Guangzhou University of TCM comes to a conclusion that, the clinical teaching of ICU should pay attention to and ensure effective implementation of beginning educationg. And following the PDCA method (Plan, Do, Check & Act) will make the work orderly, normative and controllable and meanwhile raise the students' observing and analysis ability as well as their operation and doctor-patient communication ability. In this way we can mostly enhance the effect of teaching.
4.Clinical Study on the Treatment of Renal Colic by Body and Auricular Acupuncture

Objective: To evaluate the clinical effects of combined acupuncture and ear-puncture in treating renal colic. Methods: Sixty subjects were randomized into an acupuncture group and a medication group, 30 in each, to receive acupuncture and Western medicine respectively. The analgesic effects and pain relief time were measured in both groups. Results: Both of the two groups got pain relieved or reduced. The total effective rate was 89.7% in the acupuncture group, significantly superior to 77.4% in the medication group (P<0.05). Conclusion: Combined body and auricular acupuncture is efficient in relieving pain in treating renal colic, and its effective rate was higher compared with medication treatment.
5.A New Phenolic Glucoside from Paeonia lactiflora

Objective To study the chemical constituents from EtOAc extracts of Paeonia lactiflora. Methods Compounds were isolated by various chromatographic techniques and structures were elucidated on the basis of spectral analysis. Results Seventeen compounds were obtained and their structures were identified as l,2,6-benzenetriol-l-O-α-D-glucoside (1), paeoniflorin (2), 4-methylpaeoniflorin (3), albiflorin (4), paeonidanin (5), benzoylpaeoniflorin (6), 4-methylbenzoylpaeoniflorin (7), benzoylalbiflorin (8), paeonidanin A (9), galloylalbiflorin (10), debenzoylalbiflorin (11), 4',5-dihydroxyflavanone-7-O-β-D-glucoside (12), 5,7-dihydroxy flavanone-4'-O-β-D-glucoside (13), (+)-catechin (14), gallic acid (15), vanillic acid (16), and 1,2,3-benzenetriol (17). Conclusion Compound 1 is a new compound named paeoniphenoside. Compounds 12 and 13 are firstly obtained from genus Paeonia L., and compounds S and 9 are isolated from P. lactiflora for the first time.
6.Inhibitory effects of the fusion gene system AdKDR-CDglyTK on the angiogenesis of pancreatic cancer

Objective To study the inhibitory effects of adenovirus-mediated fusion gene system driven by KDR promoter(AdKDR-CDglyTK) on angiogenesis and growth of pancreatic cancer.Methods The nude mice model was reproduced bearing pancreatic cancer cell lines Capan-2.Twenty nude mice were randomly and equally divided into four groups: AdKDR-CDglyTK and 5-FC/GCV were injected to the animals of group Ⅰ;group Ⅱ received 5-FC/GCV injection,group Ⅲ received AdKDR-CDglyTK injection,and group Ⅳ as control,received neither AdKDR-CDglyTK nor 5-FC/GCV.AdKDR-CDglyTK was injected directly into the tumor,while 5-FC/GCV was given by intraperitoneal injection.The treatment efficiency in each group was observed and the tumor microvessel density(MVD) was analyzed.RT-PCR was employed to examine the expression of CDglyTK in tumors.Results Tumor growth was dramatically inhibited in group Ⅰ,while no significant difference was found in group Ⅱ,group Ⅲ and group Ⅳ.The MVD in the four groups were 2.08?0.79,10.01?0.77,9.91?0.63 and 10.39?1.35,respectively(F=93.29,P=0.00).The MVD decreased significantly in group Ⅰ compared to the other three groups(P0.05).RT-PCR showed that a 2.4kB fragment had been amplified in the tumor tissues of groupⅠand group Ⅲ,but not in groupⅡand group Ⅳ.Conclusion AdKDR-CDglyTK with 5-FC and GCV can significantly inhibit the angiogenesis and growth of implanted pancreatic cancer in nude mice.
7.Multi-time DCE-MRI and pathological characteristics of rabbit model bearing liver VX2 tumor

Objective To observe the growth characteristics and multi-time dynamic contrast-enhanced MR(DCE-MR)findings of rabbit VX2 liver tumor on MR imaging, and to provide a reliable basis for the values of MR in assessment of non-surgical treatment effect of rabbit VX2 liver tumor.Methods 10 rabbit VX2 liver tumor models were established by liver tissue embedding through laparotomy under direct vision and DCE-MRI scaning was performed at the 2nd,3rd,4th and 5th week after establishment.The tumor volume (V)and tumor growth rate (TGR)were calculated at different time points and the signal characteristics were analyzed on MR imaging;then all the rabbits were excuted and HE staining were performed to observe the cancer cells in different places with microscope.Results The TGR at the 3rd week after establishment (404.16%±114.64%)was significantly higher than that at the 4th week (223.49%±65.90%)(t=3.417,P<0.05).The tumors with well-boundary were rim-like enhanced significantly and the central parts were not enhanced on DCE-MR imaging,and the parts between the edge and centre of tumors were heterogeneously mild enhanced. The HE staining results showed that 6 cases of tumor were squamous cell carcinoma and the cells were distributed densely in the edge,the cancer cells and necrosis were distributed equally in the parts between the edge and centre of tumors and the centre of tumors were necrosis parts.Conclusion The rabbit VX2 liver tumor grows rapidly at the 3rd week after establishment, and its histological features can be better reflected by DCE-MRI.
8.The effect of umbilical cord mesenchymal stem cells transplantation for the GD mice on regulatory B cells

Objective To investigate the therapeutic mechanism of umbilical cord mesenchymal stem cell(UC-MSCs)trans-plantation for the graves disease(GD)mice .Methods Thirty two mice were divided into 3 groups as following :normal control group (G0) ,GD control group (G1) ,UC-MSCs group(G2) .Enzyme linked immunosorbent assay(ELISA)was used to measure the level of TSAb in blood serum and the expression of FT4 was measured by chemiluminescence .Thyroid sections were stained with hema-toxylin and eosin(HE)for histological examination .Splenocytes were stained with multicolor immunofluorescence and detected by flow cytometry to analyze the percentages of CD1dhiCD5+CD19+ regulatory B cells(Bregs) .Expressions of IL-10 and TGF-βmR-NA in spleen organization were measured by Real-time PCR .Results At 26 weeks ,the level of TSAb in blood serum in G2 was more significantly decreased than in G1(P<0 .05) ,and the level of CD19+ B in spleen in G2 was also more significantly decreased than in G1(P<0 .05) ,however ,the percentage of CD1dhiCD5+CD19+ Bregs splenocytes and the levels of IL-10 and TGF-βmRNA in spleen organization were more significantly increased than in G1(P<0 .05) .The concentration differences of TSAb in serum was negatively correlated with the percentage differences of CD1dhi CD5+ CD19+ Bregs ,however ,positively correlated with the expres-sion differences of IL-10 and TGF-βmRNA in spleen before and after transplantation .Conclusion Activation of Bregs may be one of the mechanisms of UC-MSCs therapeutic effect on GD mice .
9.The clinical research of different dose of esomeprazole-based triple therapies for Helicobacter pylori eradication

Objective To compare the efficacy of different dose esomeprazole-based triplen therapies for Helicobacter pylori (Hp)eradication. Methods Two hundred and forty Hp-infected patients were randomly assigned to undergo high-dose (40 mg) or low-dose (20 mg) esomeprasole combined with clarithromycin (500 mg) and amoxicillin (1g) twice a day for one week. Hp eradication test was performed at 4 weeks after the end of treatment to evaluate the response to therapy. Results One hundred and fourteen patients were followed up in high-dose patients, and 104 were Hp eradication. One hundred and thirteen patients were followed up in low-dose patients, and 101 were Hp eradication. There was no significant difference in eradication rate of intention-to treat analysis and per protocol analysis between high-dose and low-dose patients [86.7%(104/120) vs 84.2% (101/120) and 91.2% (104/114) vs 89.4% (101/113), P>0.05]. There was no significant difference in the rate of adverse effect between high-dose and low-dose patients (8.3%(10/120)vs 6.7%(8/120), P>0.05). Conclusion It demonstrates that low-dose esomeprazole-based triple therapy has a similar Hp eradication rate compared with high-dose esomeprazole-based therapy in China.
10.Epidemiological investigation on pre-hospital trauma first aid in Guangzhou

Objective Through epidemiological analysis of prehospital trauma care in Guangzhou during 1996~2004,to explore common characteristics and trends of it.Methods The data of the prehospital trauma patient were collected by Guangzhou First Aid Service Command Center from 1996 to 2004 and was prospectively analyzed. Results The number of prehospital care had been increased year by year(increased by 3.56 times in the nine years).The number of the trauma patients had been increased by 7.98 times.Head injury was in the first place(6.86%). Most of casualties fell into the age of 21~40 years old.The male patients were the two times that of female patients.Conclusion Trauma has become an serious problem. The improvement of prehospital emergency network, ability of on-the-spot emergency care and popularizing the knowledge of emergency are key to reduce the casualties.
