2.Advance on the research and clinical treatment of gastrointestinal stromal tumors

Gastrointestinal stromal tumors(GISTs) are rarely one of gastrointestinal carcinomas that originate from mesenchymal tissue.GISTs are characterized by expression of CD117 receptor in cells and have variable biological phenotypes ranging from benign to highly malignant.The most common complaints of patients with GISTs are abdominal pain,peptic ulcer,bleeding or/and obstruction in digestive tract.GISTs have an extraordinarily high rate of recurrence after surgical resection and are highly resistant to radiation and standard chemotherapy.The imatinib mesylate(Gleevec) is a selective inhibitor of the tyrosine kinase receptor.The outcome for the patients with GISTs treated by Gleevec was promising in terms of immediate response and survival fraction.Recent research shows that a new targeting therapeutic agent,SU11248,can prolong the survival time of the patients with GISTs who were resistant to Gleevec therapy.
3.Effect of Immediate Hyperbaric Oxygen on Infarct Size in Rats after Focal Cerebral Ischemia

Objective To investigate the effect of immediate hyperbaric oxygen on focal cerebral ischemia of rats. Methods Sprague-Dawly rats were randomly divided into control group (n=9), hyperbaric air group (n=9), hyperbaric oxygen group (n=9) and sham group (n=5). The middle cerebral arteries were occluded in the former 3 groups, while the hyperbaric air group and the hyperbaric oxygen group operated in the hyperbaric chamber, and the hyperbaric oxygen group accepted hyperbaric oxygen immediately after operation. All the rats were sacrificed 6 hours after operation and the percentage of volume of infarction in the brain slices was calculated. Results The percentage of infarction decreased in both hyperbaric oxygen group and hyperbaric air group compared with the control group (P<0.001), and decreased more in the hyperbaric oxygen therapy than in the hyperbaric air group (P<0.001). Conclusion Immediate hyperbaric oxygen therapy can reduce infarct size in rats after focal cerebral ischemia.
4.Lumbar fusion research comparing the effect of unilateral and bilateral internal fixation for lumbar disc herniation

Objective:To compare lumbar fusion effect of unilateral and bilateral internal fixation for lumbar disc herniation, to explore more effective, reliable treatment. Methods:112 cases of lumbar selects my courtyard 2007 April-2010 year in April admitted to the disc herniation were randomly grouped, unilateral group and bilateral group with 56 cases in each group, respectively, to take unilateral or bilateral internal fixation for the treatment of lumbar fusion operation, compared two groups of patients and the therapy effect. Results:the unilateral group operation time, amount of bleeding during the operation and hospitalization expenses below the bilateral group, there was significant difference(t=8.857, t=9.620, t=12.338;P<0.05), the two groups had no significant difference (P>0.05);the two group patients after VAS and ODI were significantly decreased (P<0.05), before and after the treatment of VAS and ODI the comparison between the two groups were not statistically significant difference (P>0.05);the unilateral group at the end of the follow-up of 50 cases of intervertebral fusion, the fusion rate was 96.2%, the bilateral group interbody fusion in 52 cases, the fusion rate was 100%, two groups of patients at the end of the follow-up intervertebral fusion rate has no obvious statistical difference (x2=0.372, P<0.05);unilateral group were no significant complications, bilateral group were found 1 cases of intervertebral fusion device implant bone absorption, 1 cases highlight the root symptoms, complication rate was 3.8%, the two groups of patients with complications were no significant statistical difference. Conclusion:Lumbar unilateral and bilateral internal fixation can effectively improve the patients with lumbar disc herniation pain symptoms, and has good fusion degree, high security, and unilateral internal fixation operation time is shorter, less blood loss and the burden of patients is smaller, need operation treatment for severe lumbar disc herniation, fusion and unilateral internal fixation for the treatment of recommend the use of lumbar.
5.The significance of blood lactic acid level and lactic acid clearance in evaluating the prognosis of septic shock

Objective To explore the significance of blood lactic acid level and lactic acid clearance in evaluating the prognosis of septic shock.Methods The blood lactic acid concentration of 64 patients with septic shock EICU ICU admission 1h inside,6,12,24 and 48h was recorded,and the lactic acid clearance was calculated 12 hours after treatment,and according to lactic acid clearance of outcome,64 patients were divided into two groups,live group and death group;Then compared blood lactic acid level and lactic acid clearance between the two groups.Results The blood lactic acid concentration of live group and death group when enter the ICU was (7.2 ±5.4)mmol/L,(9.2 ±5.6)mmol/L respectivly,and was higher than normal,and of live group after 6h began to fall,and it reached normal level after 24h [ (2.2 ± 1.3 ) mmol/L ],but of death group after 48 h [ ( 8.0 ± 3.8 ) mmol/L ] was still significantly higher than normal,until death.There was significant difference between the two groups (t =3.7816,3.8257,7.5263,4.5875,5.8765,all P < 0.05 ).The lactic acid clearance of death group after 12 hours was (5.83 ±3.92)mmol/L,and significantly lower than that of live group[ ( 14.32 ±2.68) mmol/L] (t =6.2565,P <0.05).Conclusion Blood lactic acid concentration was positively related to the prognosis of patients with septic shock,and dynamic response of disease progression by detecting lactic acid clearance rate could be early,sensitive,quantitative indicators to determine the severity of critical illness.
6.Analysis and treatment of explicit and occult Descemet's membrane detachment after phacoemulsification

AIM: The reasons of explicit and occult Descemet's membrane detachment ( DMD ) after phacoemulsification were analyzed and the therapeutic measures were further provided.
METHODS:A total of 31 cases diagnosed as explicit and occult DMD between Jan. 2013 and Apr. 2016 received the injection of sterile air into anterior chamber by 1-3 times, and were followed up for 3mo to observe the recoveries of cornea.
RESULTS: A total of 30 cases recovered with no visual impairment and other related complications. One case which refused being treated resulted in bullous keratitis.CONCLUSION:Both the internal and external factors can influence the operation effect of explicit and occult DMD after phacoemulsification. Early detection and timely treatment play important roles in improving patients'visual acuity. Early examination of anterior segment OCT or UBM for intractable corneal edema is helpful to find the occult DMD with different degree and work out the targeted therapies.
7.Analysis of the Utilization of Antitumor Drugs in 42 Hospitals from 3 Cities during 2012-2016

OBJECTIVE:To analyze the clinical application and trend of antitumor drugs in 42 hospitals from Beijing,Shang-hai and Chengdu,and to provide reference for rational drug use in clinic. METHODS:The utilization of antitumor drugs in 42 hos-pitals from Beijing,Shanghai and Chengdu during 2012-2016 was analyzed statistically in respects of consumption sum,DDDs. RESULTS:The consumption sum of antitumor drugs in 3 cities,Beijing ranked the first;annual growth rate of Shanghai was the highest. In the list of consumption sum of antitumor drugs of 3 cities in 5 years,immunosuppressive agents as tacrolimus,mycophe-nolate mofetil were among top 10 antitumor drugs in the list of consumption sum. Top 1 antitumor drug in the list of consumption sum in Beijing,Shanghai and Chengdu were thymopentin,tacrolimus and thymalfasin,respectively. Among subtype of antitumor drugs,other antitumor drugs and adjuvant drugs in Beijing and Chengdu during 2012-2014 took up the first place in the list of con-sumption sum;the consumption sum of antimetabolite during 2015-2016 occupied the first place,and that of Beijing was higher than that of Chengdu. The cost of antimetabolite in Shanghai during 2012-2016 was the highest. The consumption sum of other antitumor drugs and adjuvant drugs were the highest in 3 cities during 2012-2014;that of antimetabolite was the highest during 2015-2016. Among related drugs,immune enhancer,immunosuppressive agents and analgesics ranked the top in the list of consumption sum. Among top 10 antitumor drugs in the list of DDDs,most were antineoplastic medicine,and only cyclophosphamide among alkylat-ing agents entered the ranking. CONCLUSIONS:The growing trend of antitumor concumption sum is stable. Antimetabolite,hor-mones and plant-derived antitumor are becoming larger. Molecular targeted therapies have a good prospect for application.
8.Effect of NIP regime in treatment of non-small cell lung cancer

This research is to investigate the effect of NIP regime in the treatment of non small cell lung cancer (NSCLC) and its side effects.Thirty two patients with NSCLC (stageⅢ 28 cases, include Ⅲa 6 cases and Ⅲb 22 cases, stageⅣ 4 cases) received chemotherapy regime NIP.Vinorelbine(NVB)25 mg/m 2 V.D,first day and eighth day ;Ifosfomide(IFO) 1.6 g mg/m 2 V.D, from first day to third day; Cisplatin(DDP)25 mg/m 2 V.D,from first day to third day.Four weeks was a cycle, every patients was given 2 cycles. RR was 53.13%(CR 1/32,PR 16/32,SD 4/32,PD 9/32). Main side effect was mylosuppression.NIP regime is effective and safe for patients with advanced NSCLC.
9.Changes of ATM-Akt signaling in myocardium around infarcted zones and its intervention by phenylephrine in rats

Objective To investigate the changes of ataxia-telangiectasia mutated(ATM)-Akt in the myocardium around the infarcted zones and its intervention by phenylephrine in rats.Methods Myocardial infarction(MI) and sham operation were established in Sprague Dawley male rats by Litwin's method.Three days after operation,surviving rats were divided into MI group(n=8),sham operation group(n=8),MI phenylephrine intervention group [n=8,phenylephrine 0.65 mg/(kg?d),i.p.,for 8 weeks),and sham operation phenylephrine intervention group(n=8,phenylephrine administered as above).Cardiac left ventricular posterior wall end-diastolic thickness(LVPWd),left ventricular end-diastolic diameter(LVDd),fractional shortening(FS) and ejection fraction(EF) were examined by Doppler ultrasonography.Twelve weeks after treatment,the expression of p-ATM substrate protein and p-Akt protein in the myocardium around the infarcted zones were detected by Western blot analysis and immunohistochemical assay respectively.Results In 12 weeks after treatment,LVDd(0.79?0.06) and(0.54?0.06) cm,FS(17.31?2.60)% and(38.58?2.56)%,and EF(67.75?3.54)% and(70.25?4.33)%(P0.05),and was(0.99?0.07) and(0.65?0.04) in MI phenylephrine intervention group and sham operation phenylephrine intervention group respectively(P0.05);and were(98.80?3.17) and(75.88?2.77) in MI phenylephrine intervention group and sham operation phenylephrine interventin group respectively(P
10.The therapeutic effect of pertuzumab on HER2 positive breast cancer patients and its effect on the expression of serum nectin-4 and TK-1

Objective:To investigate the effect of pertuzumab in the treatment of human epidermal growth factor receptor 2 (HER2)-positive breast cancer patients and its effect on the expression of serum nectin-4 and thymidine kinase 1 (TK-1).Methods:110 HER2 positive breast cancer patients treated in Longfu Hospital of Beijing from June 2014 to December 2016 were selected and divided into observation group and control group according to treatment method, 55 cases in each group.The control group was given docetaxel combined with platinum (TP) neoadjuvant chemotherapy, and the observation group was given TP neoadjuvant chemotherapy combined with paltuzumab. After 18 weeks of treatment, the therapeutic effect, serum tumor markers [carbohydrate antigen (CA), CA153 and CA199], immune function, levels of nectin-4 and TK-1 and adverse reactions were observed.Results:The total effective rate of the observation group was 83.64%(46/55), which was significantly higher than that of the control group 58.18%(32/55), with statistically significant difference ( P<0.05). After treatment, the levels of CA125, CA153 and CA199 in the two groups were decreased, and the difference was significant in the observation group ( P<0.05); after treatment, the levels of CD3 + , CD4 + , CD4 + /CD8 + in the two groups were increased, while the levels of CD8 + was decreased, with no significant difference between the two groups ( P>0.05). After treatment, the levels of serum nectin-4 and TK-1 decreased in both groups, and the the decrease in the observation group was significant ( P<0.05). There was no significant difference in digestive system, white blood cell count and cardiac toxicity between the two groups ( P>0.05). Two years follow-up showed that the mortality of the observation group was 3.64%(2/55), and that of the control group was 10.91%(6/55). The mortality of the group was significantly lower than that of the control group ( P<0.05). Conclusions:Pertuzumab combined with TP neoadjuvant chemotherapy has better effect on HER2 positive breast cancer patients, it can effectively inhibit the growth of tumor tissues, reduce the level of tumor markers and the level of nectin-4 and TK-1, and has less influence on the immune response of patients with fewer adverse reactions and lower mortality rate.
