Objective To study the changes of expression levels of vascular endothelial growth factor(VEGF) and its receptors(FLT-1,FLK-1) mRNA in equal area of penumbra in diabetes rats with cerebral infarction(DMCI).Methods Middle cerebral artery occlusion(MACO) model was made in diabetes rats.The expression levels of VEGF and FLT-1,FLK-1 mRNA in equal area of penumbra were measured with situ hybridization in different times after MACO(1 h,3 h,6 h,12 h,24 h).The difference was compared between DMCI group and no diabetes cerebral infarction(NDMCI) group.Results The expression level of VEGF mRNA was lower in DMCI group than that of NDMCI group at every time point(P

Objective Video-assisted thoracoscopic surgery is widely used in clinical practice,due to the characteristics of minimal invasion,quick recovery,low incidence of postoperative chest pain and the same effect of lobectomy as thoracotomy.However,the surgical itself has a certain degree of difficulty,and due to the effect of lymph node,intraoperative blood loss,pleural adhesions,conversion to thoracotomy surgery is needed in some cases. Understanding the risk factors of conversion to thoracotomy can help thoracic surgeons figure out the right time to operate the surgery.
3.Management of Drug Account in Emergency Pharmacy with Excel

OBJECTIVE:To probe into the approach of managing drug account in emergency pharmacy by Excel.METHODS:Drug account in hospital emergency pharmacy was established by means of Excel,the drugs in the emergency pharmacy were kept under control by means of Excel’s multi-functions like searching,sorting,selecting statistics,etc.RES_ ULTS:Through excel,a clear shift records account of emergency pharmacy drugs was achieved,the recording and statistics of prescriptions become more convenient and fast,both the working efficiency and the congruity between the account and the drugs were greatly enhanced.CONCLUSION:Intelligentized management of emergency pharmacy drugs can be effectuated by means of Excel.
5.Tissue engineering combined with mosaicplasty to promote healing and integration of the osteochondral defects

Objective To develop a new method, the bone marrow stromal cells (BMSCs)-mediated tissue engineering technique combined with mosaicplasty, for repair of osteochondral defects and integration of gaps. Methods BMSCs from 12 Chinese goats were cultured and proliferated in vitro. Prior to the BMSCs harvest, osteochondral defects, 5 mm in diameter and 3 mm in depth, were created in the femoral medial condyles of both the goat's hind limbs. When the mosaicplasty (osteochondral autograft transplantation) was performed, the BMSCs, which had been harvested and compounded with hyaluronic acid, were injected into the gaps between the osteochondral autografts in the left hind limb. The right hind limb which only received osteochondral autograft transplantation without BMSCs served as a control. At four, eight and 16 weeks post-operatively, samples of the repaired defects were harvested and assessed by histological evaluation, immunohistochemical analysis and glycosaminoglycan (GAG) quantification. In both groups 16 weeks post-operatively, the GAG quantification was analyzed by one-way ANOVA and least significant difference (LSD) method. Results At all the time points, the cartilage autografts in both groups survived as hyaline cartilage and presented no significant difference from the surrounding native cartilage. In the group filled with BMSCs compound, the gaps were replaced by regenerated hyaline cartilage and disappeared; however, in the control group, the osteochondral autografts were still distinct from the surrounding normal cartilage, though the gaps were replaced by fibrous tissue or fibrous cartilage. Immunohistochemical analysis of typeⅡcollagen showed positive staining in the matrix of transplanted and regenerated cartilage. The Alcian blue method also confirmed a significantly less GAG content in the regenerated tissue in gaps in the control group than in the treatment group and in the normal cartilage. Conclusion Since tissue engineering combined with mosaicplasty can promote gap integration and cartilage healing, the method can be an ideal way for osteochondral defect repair.
6.A prospective study of the cognitive changes of type 2 diabetes mellitus patients complicated with stroke

Objective To investigate the characteristics of cognitive changes in type 2 diabetes mellitus pa-tients complicated with stroke. Methods One hundred and one stroke patients were recruited and allocated to thetype 2 diabetes mellitus group (34 patients) and sixty-seven patients in the control group (67 patients) according tobeing with or without type 2 diabetes mellitus. All the patients were evaluated, after 3 weeks, 6 months and 12months of onset of stroke, with regard to their cognitive functions by using the Mini-mental State Examination(MMSE) and Montreal Cognitive Assessment (MOCA). Results Average scores obtained in MMSE and MOCAtests were significantly lower at 3 weeks and 6 months after onset in the type 2 diabetes mellitus group when comparingwith control group. In the type 2 diabetes mellitus group, no significant difference was observed between 3 weeks and6 months after onset of stroke with regard to scores of MMSE and MOCA ( P > 0.05 ), however, the score at 6 monthswas significantly lower than that at 12 months after onset (P <0.05). On the other hand, scores of MMSE (P >0.05 ) and MOCA (P > 0.05 ) in the control group were not any statistically different among various time points ofobservation. Conclusion Type 2 diabetes mellitus will aggravate cognitive dysfunctions of stroke patients, whichstarted to recover in 6 months of onset of the stroke.
7.Clinical significance of ultrasound-guided systematic biopsy in patients with prostate cancer

4 ng/ml and/or with prostate nodule found on DRE or TRUS . Methods 197 patients suspected of prostate carcinoma underwent ultrasound guided sextant biopsy and 2~4 more cores in the nodule. Four patients whose initial biopsy was negative but subsequent PSA levels were continuously abnormal underwent repeated biopsies. Results Of the 107 cases with prostate nodules,in 34(31.8%) of them prostate cancer was detected,while in 90 cases without a prostate nodule, only 11(12.2%) were positive. Stratified analysis on PSA found that the higher the PSA, the higher the positive biopsy rate of prostate carcinoma. In the 4 cases with repeated biopsies, 1 prostate cancer was detected. Conclusions For patients with prostate nodule and/or PSA over 4 ng/ml,systematic biopsy should be taken, with 2 or more cores toward the nodule.In the patients with continuous high PSA levels,if the first biopsy is negative, rebiopsy should be taken.

9.Pituitary adenoma resection by single-nostril transsphenoidal approach

Objective To summarize the experience of pituitary adenoma resection by single-nostril transsphenoidal approach and analyze the indications,intraoperative and postoperative common problems in order to improve the technique and quality of life.Methods Summarize the clinical data of 611 pituitary adenoma resections by single-nostril transsphenoidal approach from January 2005 to June 2010 retrospectively,reanalyze the choice of operative indications,analyze the bleeding during the operation and sums up the relevant pro cessing suggestions,and summarize the reasons and countermeasures of postoperative visual impairment.Results The tumors were totally removed in 538 cases,sub-resections in 59 cases,sub-totally removed in 14 cases,and no deaths.Eleven cases were poor gasification or concha sphenoid sinus in 11 totally removed patients.The bleeding came from diploe,emissarium,mucosa of sphenoid sinus,diploe,epidural space,dura matter of the sella turcica region,intercavernous sinuses,tumor or the tumor bed.The bleeding was controlled effectively with different approaches.There were 3 patients who underwent visual impairment,and the impairment was improved after treatment.Conclusion Following the correct operation principle,we can improve the rate of total resection; the single-nostril transsphenoidal approach is also suitable for the poor gasification; The prognosis can improve after deal with the intraoperative and postoperative common problems.
10.Clinical Administration of Partial Parenteral Nutrition in Premature Infants

Objective To evaluate the influence of partial parenteral nutrition on serum osmotic pressure,blood glucose,(biochemistry),bilirubin metabolism,immune function,growth and development of premature infants.Methods Seventy premature infants were randomly divided into control group and study group.On the base of enteral feeding,study group were offered parenteral nutrition, while the control group were supplied 10% glucose, fluid and electrolytes. Simultaneously, relevant indices were measured in 2 groups.Results 1.There were no significant difference in serum osmotic pressure,blood glucose and biochemistry before and after parenteral nutrition. 2.There were no significant difference in emerging and lasting time of jaundice between 2 groups.3.Serum IgG,IgA,IgM,C_3,CD4 and CD4/CD8 in study group were significantly higher than those in control group. 4.In study group the time of hospitalization and birth-weight regain were significantly shorter than those in control group.Conclusions There is no significant influence on serum osmotic pressure,blood glucose, biochemistry and bilirubin metabolism during partial parenteral nutrition. Parenteral nutrition may help gain weight, shorten the time of hospitalization, and improve immunological function of neonates.
