1.Retrospective analysis on efficacy and safety of interventional therapy for pulmonary disease with central airway stenosis:a report of 389 cases

Objective To investigate the efficacy and safety of interventional therapy for central airway stenosing pulmonary disease. Methods In 389 petients diagnosed as central airway stenosis, their primary diseases were handled timely and correctly. On the basis of handling the primary disease, cetral airway stenosis was treated by interventional techniques, including electrosurgical unit (ESU), laser, argon plasma coagulation, freezing, high-pressure balloon expansion, stent implantation, local injection and toilet with lavage. Following the American College of Chest Physicians (ACCP) guidelines, the therapeutic effects were evaluated based on correction of stenosis after interventional treatment and the dyspnea scores. Results Of the 389 patients, 154 were treated with electrosurgical unit under bronchoscopy, 18 with laser, 76 with freezing, 69 with argon plasma coagulation, 35 with high-pressure balloon expansion, 21 with tracheobronchial stent implantation, and 16 with local injection and laving clearance. Evaluation of the therapeutic effects was made one month after the interventional treatment. As a result of the interventional treatment, a complete relieve was observed in 152 cases (39.1%), partial relieve in 138 cases (35.5%), mild response in 67 cases (17.2%), no response in 32 cases (8.2%), and with 17 deaths (4.4%). [Objective]To evaluate the effectiveness of recombinant human bone morphogenetic protein-2 in the repair of rabbit rotator cuff injury.[Method]Forty-eight male New Zealand rabbits,aged 8 months,received an rotator cuff acute injury and reconstruction of the insertion of supraspinatus tendon on greater tuberosity of humerus.The rabbits were randomly divided into 3 groups postoperatively:(1) rhBMP-2 group:fibrin sealant(FS) containing rhBMP-2 was applied to the interface between the bone-tendon interface; (2)FS control group:only FS was applied;(3)blank control group:untreated after the surgery.Harvested 36 specimens underwent biomechanical analysis at the 2nd,4th,8th postoperative weeks respectively.Harvested 12 specimens underwent histological analysis at the 8th postoperative week.[Result]Histological examination showed that Sharpey's fibers were found in the interface with the formation of four-layer indirect insertion in the rhBMP-2 group at the 8th postoperative week.In the FS control group and blank group,the tendon-bone interface was filled with granulation tissue and part of Sharpey's fibers and the newly generated bone tissue.Biomechanical analysis displayed that the tensile strength and stiffness of bone-tendon interface increased time-dependently in all groups,and it in the rhBMP-2 group was significantly higher than those in the other two groups at any time-points (both P

Objective To investigate the clinical management of Spontaneous rupture of hepatocellular carcinoma(SRHCC) . Methods This was a retrospective review of the clinical data of patients with SRHCC who underwent treated in the affiliate hospi‐tal of Luzhou medical college from January 2001 to December 2014 .Results Among 104 patients ,small hepatocellular carcinoma (<5 cm) were found in 11 cases ,and large hepatocellular carcinoma(>5 cm) in 93 cases .Thirty‐one cases which underwent surgi‐cal treatment ,were cured;44 underwent transcatheter arterial embolization (TAE) ,5 cases died of liver function failure;29 cases were treated conservatively ,11 cases died of huge bleeding ,18 cases gave up discharged .Twenty‐two small and medium‐sized SRH‐CC cases underwent hepatectomy survived 1to‐10 years ;8 huge sized SRHCC cases survived 5to‐13 months;one case who under‐went partial filling pressure hemostasis and hepatic artery ligation ,but died of tumor rupture again after 34 days .Sixteen cases un‐derwent TAE were followed up ,14 cases survived 3to‐10 moths ,the survival time of two cases were 3 years and 5 years ,respective‐ly .Conservative treatment group has not been followed up .Conclusion The tumours should be surgical resection as soon as possi‐ble in those whose lesions confined to the liver and may be removed ,systemic condition is good;TAE should be used for other pa‐tients .
4.Effect of Relevant Training on Understanding of Plant Reproduction in Preschoolers Aged 5 to 6

Objective:To explore effect of relevant training on understanding of theory of naive biology for plant reproduction in 5- and 6-years old,and to attempt to provide demonstration for a debate of different views between domain universality and domain specificity cognitive development.Methods:48 preschoolers who did not comprehend concept of plant reproduction screened from 64 preschoolers ages 5 to 6 were surveyed by clinical method.Half of the 48 children were assigned to intervention group,the other to control group.The effects of intervention were compared by the experimental before-after design.Results:Relevant biological knowledge might widely accelerate 5- and 6-year-olds' understandings for plant reproduction,and 6-year-olds had more enhancement than 5-year-olds.Pass rates of intervention group were more 29.2% than control group in 5 year olds,and more 54.1% in 6 year olds.Conclusion:Acquiring specific knowledge will generally improve preschoolers' cognition on plant reproduction.Children' cognitive development is influenced jointly by both domain-specific and domain-general thinking.
5.Experimental study of TGF-β2 antisense oligodeo- xynucleotide as an anti-scarring agent in glaucoma surgery

AIM: Currently available anti-scarring regimens for glaucoma filtration surgery have potentially blinding complications and safer alternatives would be beneficial. This experiment is to investigate the effect of TGF-β2 antisense oligodeoxynucleotide on differentiation, proliferation of subconjunctival fibroblast following glaucoma filtration surgery.METHODS: Glaucoma filtration surgery were performed on both eyes of 28 rabbits. TGF-β2 antisense oligodeoxynudeotide was subconjunctivally injected in the right eyes (A group), and TGF-β2 missense oligodeoxynucleotide (B group)or PBS(C group) was used at the same method in the left eyes as controls. Rabbits were killed at 4,7,14 and 28 days after surgery. Intraocular pressure (IOP), bleb characteristics were recorded at different time point. Subconjunctival fibroblasts were examined by immunohistochemistry and electron microscopy.RESULTS: The IOP of rabbits in group A was significantly lower at 14 days (6.74± 1.18 mmHg) and 21 days (8.15± 1.97mmHg) after operation than the IOP in group B (8.53± 1.04,9.72± 1.09 mmHg)(P <0.01) and group C(8.79± 1.21, 9.43±1.27 mmHg) (P <0.05). The mean bleb survival time was longer (17.2 days) in group A than that of group B (14.5 days) and group C (13.5 days)(P<0.05). The population of the cells expressing α -smooth muscle actin(α -SMA) and proliferating cell nuclear antigen (PCNA) was significantly reduced in group A compared with the group B and C. The ultrastructure of fibroblast was not altered by TGF-β2 anti-sense oligodeoxynucleotide.CONCLUSION:TGF-β2 antisense oligodeoxynucleotide can prevent the scar formation after glaucoma surgery by inhibit the differentiation and proliferation of subconjunctival fibroblast. It could be a potentially useful anti-scarring alternative for the prevention of late surgical failure.

8.Expression of periostin and MMP-2 in colorectal carcinoma and their clinicopathological significance

Objective To investigate the clinicopathological significance of periostin and MMP-2 in colorectal carcinoma.Methods Immunohistochemistry was used to detect the expression of periostin and MMP-2 in colorectal carcinoma,tubular adenoma,villous adenoma and normal colorectal tissue.The correlations between the expression of periostin and MMP-2 with clinicopathologic parameters were analyzed.Results In colorectal carcinoma,tubular adenoma,villous adenoma and normal colorectal tissue,the positive rates of periostin were 83.7 % (41/49),40.0 % (6/15),32.1% (9/28),0 (0/15),respectively,while the positive rates of MMP-2 were 71.4 % (35/49),60.0 % (9/15),64.3 % (18/28),0(0/15),respectively.There were significant differences among the groups (x2 =41.252,P =0.000; x2 =24.811,P =0.000).The expression of periostin in colorectal carcinoma tissue were not correlated with sex (x2 =0.002,P =0.961),age (x2 =2.267,P =0.132),tumor sites (x2 =1.506,P =0.220),differentiation status (x2 =0.875,P =0.350) and lymphatic metastasis (x2 =3.315,P =0.069).The expression of MMP-2 in colorectal carcinoma were correlated with lymphatic metastasis (x2 =5.800,P =0.016),whereas not correlated with sex (x2 =0.562,P =0.453),age (x2 =0.138,P =0.711),tumor sites (x2 =0.408,P =0.532),differentiation degree (x2 =1.335,P =0.248).The expression of periostin and MMP-2 were positively correlated in colorectal carcinoma (r =0.332,P =0.020).Conclusion Periostin and MMP-2 are correlated closely with the development and progression of colorectal carcinoma.It might be helpful for evaluating the biological properties and prognosis of colorectal carcinoma,and it would be more accurate to use a combined analysis of the two indicators.
10.Commissioning of Mobetron mobile intra-operattve radiotherapy accelerator

Objective To commission a Mobetron intra-operative mobile accelerator and analyze the characteristics of its electron beams. Methods The dosimetrie characteristics of the electron beams genera-ted by Mobetron accelerator were measured and compared with those generated by conventional accelerator (Primus, Siemens). M oberton accelerator can generate electron beams of nominal energies of 4,6,9 and 12 MeV. The measurement items were as followings : percentage depth dose perpendicular to water phantom sur-face and beam profiles parallel to water phantom surface, output factors, applicator leakage, electron beam at-tenuation made by lead blocks,and machine output calibration. The measurement devices included a three-dimensional ( 3 D) water scanning phantom, an electrometer, a 0.6 cm3 Farmer ionization chamber, a parallel-plate ionization chamber and solid water slabs. During measurement, all applicators of different tilt angles and diameters were attached to the machine head,and their ends were adjusted to be tangent to the phantom surface. Results Except for the 12 MeV,skin dose for all energies was no more than 90%. The skin dose was higher for Mobetron accelerator electron beams than for regular electron beams. The Dmax depth in water for a 10 cm flat applicator were 0.7,1.3,2.0 and 2.2 cm for the 4 energies,respectively. The depths of 90% dose were 1.0,1.8,2.7 and 3.6 cm, respectively. The selected flat applicator was just 1 cm larger than the tumor bed. But for the beveled applicators,the field flatness and symmetry became worse,and con-sequently,the applicator size had to be selected based on the isodose distribution. The leakage dose at 1 cm outside the applicator was 1.2% ,5.1%, 10.0% and 9.1%, respectively. The lead thickness for full block was 1.5,3.0,4.5 and 6.0 mm,respectively. Conclusions Through the commissioning of Mobetron accel-erator, the machine characteristics are understood, and the data for clinical implementation and routine quality assurance are acquired.
