1.Treatment of early avascular necrosis of femoral head:core decompression with tantalum rod implantation is better than core decompression with bone implantation

BACKGROUND:Core decompression with bone implantation in treatment of early avascular necrosis of femoral head may provide insufficient support for subchondral bone and increase the risk of fracture and col apse. Tantalum rod implantation can not only provide good biological support, but also promote the revascularization at necrotic regions, thus repairing the necrosis of femoral head.
OBJECTIVE:To evaluate the efficacy of core decompression, core decompression with bone implantation and core decompression with tantalum rod implantation in treating early-stage avascular necrosis of femoral head. METHODS:A total of 24 cases (28 hips) who suffered from ARCO I/II avascular necrosis of femoral head were treated with core decompression with bone implantation, and 25 cases (29 hips) who suffered from ARCO I/II avascular necrosis of femoral head were treated with core decompression with tantalum rod implantation. Al the subjects were fol owed up for 24 months. The efficacy of two different surgical methods was evaluated before and after treatment by observing the changes in Harris scores.
RESULTS AND CONCLUSION:Al involved patients were fol owed up. Harris score of core decompression with bone implantation group were increased 4.93 points at 6 months after surgery;Harris score of core decompression with tantalum rod implantation group were increased 6.89 points at 6 months after surgery. There were significant differences between two groups before and after surgery (P<0.05). After 12 months, Harris scores in the two groups were both significantly increased and the scores of core decompression with tantalum rod implantation group was higher than that of core decompression with bone implantation group (P<0.05). The overal fine/excellent rate of core decompression with tantalum rod implantation group was 83%, which was better than core decompression with bone implantation group (75%). After 24 months, X-ray score of core decompression with tantalum rod implantation group was significantly higher than core decompression with bone implantation group (P<0.05). Comparing with core decompression with bone implantation, core decompression with tantalum rod implantation can better prevent femoral head col apse, improve hip function and delay the process of osteonecrosis of the femoral head.
2.Breast cancer conservation therapy in breast cancer patients with BRCA1/2 mutations

As an important surgical approach,breast cancer conservation therapy is still controversial in breast cancer patients with BRCA1/2 mutations.Since the risk of unilateral multiple and contralateral incidences is high in the familial breast cancer patients with BRCA1/2 mutations,breast cancer conservation therapy is not recommended but mastectomy and/or immediate mamoplasty.However,if carriers have a strong desire of breast cancer conservation therapy,it is feasible if they are fully informed the risks that may exist.At that time,we should need bilateral oophorectomy,tamoxifen therapy,and more interventions for the prevention of contralateral breast incidences.
3.Application of Multivariate Statistical Analysis in Amino Acids Compositions and Cold-Heat Nature of Traditional Chinese Medicine

This study was aimed to find the correlation between amino acid compositions and Cold-Heat Nature of traditional Chinese medicine (TCM) in order to provide basis for the research of TCM natural theory. A total of 17 kinds of amino acid were determined by application before column derivatization reaction and high-perfor-mance liquid chromatography (HPLC) and the tryptophan was determined by UV method. The data were collected for analysis by Fisher method in PAST software. The best statistical identification model was determined. And the Cold-Hot medicine property markers (CHMP-markers) were determined. The results showed that the discriminant function established by Fisher method based on 18 kinds of amino acid contents has good identification ability, and the accuracy of the Fisher discriminant analysis is 82%. Support vector machine (SVM) is the best statistical identification model . The cold and heat markers were analyzed by SVM . The cold nature material bases in-clude Glu, Gly, Arg, Thr, Ala, Tyr, Val, Ile and lys. And the heat nature material bases contain Asp, Ser, His, Pro, Met, Cys, Leu, Phe and Trp. It was concluded that there is relationship between 18 kinds of amino acid contents and the Cold-Heat nature of TCM .
5.Local injection of simvastatin affected reconstruction of trabecular bone of condyles of femur of osteoporotic rats

BACKGROUND:Local injection with simvastatin can induce osteogenesis, significantly increase the bone mineral density and mechanical strength of femoral neck and femoral condyle of rats with osteoporosis and analyze effects of local injection of simvastatin on trabecular bone of the femoral condyle.
OBJECTIVE:To explore the effects of local injection of simvastatin on trabecular bone of the femoral condyle of osteoporotic rats and provide experimental basis for the application of simvastatin in clinical topical treatment of osteoporosis.
METHODS:Eighteen female Sprague-Dawley rats received bilateral ovariotomy at 3 months, and were used to produce rat models of osteoporosis. They were assigned into three groups. Experimental rats received 5 and 10 mg simvastatin by single injection into right femoral cavity. Control rats received blank vector. The rat models were sacrificed at 1 month after injection and specimens were col ected. Right femoral condyles were taken out for bone histomorphometric analysis by Micro-CT.
RESULTS AND CONCLUSION:One month post-injection, Micro-CT scanning results revealed that cortical bone thickness, trabecular bone density and connection rate were significantly better in the simvastatin group than those in the control group. Results indicated that single injection of smal-dose simvastatin obviously promoted rebuilding of trabecular bone of condyles of femur, improved microstructure of skeleton, strengthened local skeleton, prevented and treated osteoporosis, and provided a further basis for the prevention and treatment of osteoporosis, especial y for osteoporotic fractures.
6.PROTECTION OF TAURINE AGAINST LEAD TOXICITY IN CULTURES OF CORTEX NEURON

Objective:To study the effect of taurine on protection against lead toxicity in cultured cortex neurons. Methods:Put taurine and different concentrations of lead acetate in cultures of cortex neurons and observe the length of dendites and the growth activity of neurons. Results:At the concentration of 10-8 mol/l lead acetate the growth of cultured cortex neurons were decreased and even the cells died. Taurine at the concentration of 1.6?10-3 mol/L could protect against lead toxicity and promote neuron to grow. Conclusion:Taurine can protect from lead acetate toxicity in cultured cortex neurons.

The production of mature functional T cells requires many signals from the thymus such as Wnt,Notch and Hedgehog,et al.The Hedgehog protein family signals for development,patterning and organogenesis of many tissues during mammalian embryogenesis.In recent years,more and more research groups focus their attention on it because of its relationship between tumors.Abnormal differentiation and development of T cells may cause various immunological diseases and tumors.Illuminating the roles of Hedgehog signaling in T cell differentiation and development can provide a theoretical guide in the treatment of tumor.
8.Modified subinguinal approach for treatment of acetabular anterior column and wall fractures

Objective To discuss the efficacy of the modified subinguinal approach for the treatment of acetabular anterior column and wall fractures.Methods A clinical retrospective study was performed of the 5 patients who had been treated at our department from January 2011 to April 2015 for acetabular anterior column and wall fractures and conformed to follow-ups.They were 3 males and 2 females,aged from 19 to 45 years (mean,35.6 years).The duration from injury to surgery averaged 7.6 days (range,from 4 to 10 days).The subinguinal ligament approach was adopted in the 5 cases,through which the inguinal ligament was preserved intact after sharp dissection of its insertion at the anterior superior iliac spine and medial-upper advancement,the advantage of expanded exposure of the first window by the Farid sub-ilioinguinal approach was retained,and additional lesions due to iliac osteotomy were avoided.The operation time,intraoperative bleeding,postoperative reduction and time for fracture union were documented.Merle d' Aubigne and Postel scoring system was used to assess the hip joint function of the affected limb at the final follow-ups.Results The operation time ranged from 110 to 150 minutes (mean,125 minutes);the intraoperative bleeding ranged from 415 to 550 mL (mean,450.2 mL).By the Matta's criteria,the postoperative reduction was rated as excellent in 4 cases and good in one.The 5 patients were followed up for an average of 32.4 months (range,from 6 to 43 months).Their fractures united clinically after an average of 4.6 months (range,from 3 to 6 months).The hip joint function was rated as excellent in 2 and good in 3 by the Merle d' Aubigne and Postel scoring system at the final follow-ups.Follow-ups observed no serious complications like inguinal hernia,internal fixation failure,myositis ossificans,or avascular necrosis of the femoral head.Conclusions As the modified subinguinal approach can provide a broad surgical exposure,lead to minimal injury to the inguinal ligament,avoid lesions caused by iliac osteotomy,and decrease operation time and bleeding,it may be a better approach for acetabular anterior column and wall fractures.
9.pRNA: A Safe,Stable and Large Capacity Molecular Vehicle for Gene Therapy

The molecular delivery vectors used in gene therapy need provide the features of safety,stability, efficiency and capacity. The current studies on the structure and action mechanism of pRNA, a packaging RNA of phage?29, showed that pRNA with multiple binding sites can through cell membrane easily and escort exogenous molecules to target cell, without inducing immune reaction. As an ideal nano-scale gene therapy vehicle, pRNA presents a promising application in delivering multiple therapeutic components to detect and treat human diseases.
