1.Mechanism Study Progress of Gastrointestinal Bypass Operation on Treatment of Type 2 Diabetes Mellitus

Objective To study the mechanism of gastric bypass operation on treatment of type 2 diabetes mellitus,recognize the etiology and pathogenesy of the disease and frame therapy strategy for type 2 diabetes mellitus.Methods The literatures about gastric bypass operation on treatment of type 2 diabetes mellitus,including clinical cases reports and evidence-based studies were reviewed.Results Gastrointestinal bypass operation was regarded as an effective treatment for type 2 diabetes mellitus.There were three hypotheses of therapy mechanism: early delivery of nutrients to the distal intestine,exclusion of the proximal intestine and incretin/anti-incretin.Conclusion Gastrointestinal bypass operation is now considering as an effective treatment,there is still a lack of basic experimental studies to clarify the mechanism.
2.CT and Fibrotic Bronchoscopy(FOB) Evaluation of Solitary Pulmonary Nodules

Objective To evaluate the value of CT and bronchoscopy in diagnosis of solitary pulmonary nodules.Methods The CT,FOB and histologic data of 60 patients with solitary pulmonary nodules were analysed retrospectively.Results 60 pulmonary lesions were all detected by CT scan,the detected rate was 100%.The detected rate by endoscopy was 67%(40/60).Conclusion CT can be used as the routine method in evaluating pulmonary nodules and guiding biopsy by fibroptic bronchoscopy.
3.Analysis of the tasks of and demands for health human resources in the laboratory specialty of the hospital

Objective To determine the tasks that health human resources of technicians in the medical laboratory specialty ought to fulfill, the factors that affect performance and the estimation of demands. Methods Such methods as analyzing medical records, tasks and health demands were used. Results The current situation of health human resources of technicians is as follows: Those with intermediate, junior or no professional titles respectively account for 46.5%, 36.5% and 11.6%; those with a master's or a bachelor's degree or a degree from a polytechnic college or school respectively account for 2.0%, 8.2%, 42.9% and 40.8%; and others account for 6.1%. The average utilization ratio of laboratory service by inpatients and outpatients is respectively 96.7% and 49.5%; and the average utilization of laboratory service per inpatient and per outpatient is respectively 12.5 and 2.2 times. Of the average daily service volume, emergency, outpatient and inpatient, microbiological, immune serum, and biochemical tests respectively account for 8.3%, 3.0%, 8.1%, 7.8% and 72.8%. The chief tasks of the technicians are performing laboratory operations and assisting the physicians with the reporting of the laboratory results. The demand for health human resources of technicians in the laboratory specialty in the hospitals across the country numbers 89 920 people. Conclusion The results of the study will provide health administrative departments, medical institutions and medical education departments with basis for strengthening human resources management and formulating educational plans.
4.THE CHANGE IN mGluR1? IN CA_1 AFTER INFRASONIC DAMAGE AND THE EFFECT OF ITS ANTAGONIST MCPG

To study the change in mGluR1? in CA 1 after infrasonic damage and the effect of its antagonist MCPG,120 SD rats were randomizied into infrasonic damage group and MCPG therapy group.The two groups were subdivided into control group and 1 challenge, 7 chellenges, 14 challenge groups respectively,each group consisted of 15 rats. Rats were exposed to 8Hz 130dB infrasound as designed,2h for each challenge.The mGluR1? was stained by immunohistochemistry method.The changes in mGluR1? and cell pattern were observed under light and electronic microscopes.The results showed the number of mGluR1? positive neurons increased after 1 challenge infrasonic damage( P 0 05).MCPG had an effect to protect neuron from infrasonic damage as identified by morphology.It is concluded that the change of mGluR1? activity could mediate excitotoxicity and was one of the major factors related with neuron injury after infrasound.
5.Analysis of anticardiolipin antibody fluctuation and clinical manifestations in systemic lupus erythema-tosus patients

Objective To investigate the association of fluctuation of anticardiolipin antibody (ACA) levels and clinical manifestations in patients with systemic lupus erythematosus (SLE). Methods Anticar-diolipin antibodies were measured sequentially by commercial enzyme-linked immunosorbent assay (ELISA) in 85 SLE patients. Patients were classified into two groups: group A was persistently positive for ACA, group B was transiently positive for ACA, and the clinical features of the two groups were compared. Results There were 37 patients (44%) in group A, and 48 (57%) patients in group B. 41% patients had occurred thromboeytopenia in group A, 19% patients in group B, the difference was significant (P<0.05); platelet accounts were (24±6)×109/L in group A,and (46±10)×109/L in group B, the difference was significant (P< 0.05), too. 11% patients experienced thrombosis in group A, and none in group B, the difference was significant (P<0.05). 19% in group A and 4% in group B had occurred valvular diseases, the difference was significant (P<0.05.). The prevalence of lupus anticoagulant was 32% in group A, 13% in group B and the difference was significant (P<0.05). There was no significant difference between the two groups in Raynaud's phenomenon, pulmonary hypertension, and lupus nephritis (P0.05). Conclusion Fluctuations of anticardio-lipin antibodies are asso-ciated with thrombosis, thrombocytopenia vascular disease and lupus anticoagulant however, there is no lack of association with other clinical manifestations in SLE patients.
6.Mechanisms of pancreatic injury induced by long-term high-fat diet intake in rats

Objective To clarify the involvement of the free fatty acids(FFA)and lipid peroxida-tion in rat pancreatic tissue during the development of pancreatic injuries inducecd by long-term high-fat diet.Mehtods The male Sprague-Dawley rats (n=72) were randomized into 6 groups (n=12).One group (group control) received standard chow for 18 weeks, the other five groups (group HFD) were fed with a high-fat diet respectively for 2, 4, 6, 10 and 18 weeks.Serum TG and TCH, the his-topathological changes, pancreatic malondialdehyde (MDA) content, superoxide dismutase (SOD) ac-tivity and the concentration of free fatty acids in pancreatic tissues were examined.Pancreatic fibrosis was assessed using Sirius Red staining.The expression of desmin, a smooth muscle actin (α-SMA), platelet-derived growth factor receptor type β (PDGFRβ) and transforming growth factor131 (TGFβ1) was determined with immunohistochemistry.Results Pancreatic MDA content, the number of desmin and α-SMA positive cells were significantly increased in all the HFD groups.The FFA content, PDGFRβ, and TGFβ1 in pancreatic tissues increased in rats of 2, 4 and 6 week HFD groups accompa-nied with typical histological alternations including edema, capillary vessels hyperplasia, and focal aci-nar degeneration, vaculation of acinar and islet cells.In 6, 10 and 18 weeks HFD groups, the lesions had progressed and acinar cell atrophy, fatty replacement, deposition of hemosiderin, and interstitial collagen deposition were observed.Conclusions The increased amounts of FFA and lipid peroxidant in pancreatic tissues are associated with pancreatic cell injuryies and synthesis of collagen by activated PSCs during the chronic high-fat diet intake.
7.Establishment of an ultrasonographic method for evaluating the hepatic lipid accumulation in mice.

To investigate the semi-quantitative method for evaluating lipid accumulation in livers, male C57BL/6J mice (CON), HFD mice characterized with the mild fatty liver induced by high-fat diet, and KKAy mice charactered with the moderately severe fatty liver induced by high-caloric diet were used. The lipid accumulation was estimated by the histological examination (HE staining) and the content of hepatic triglyceride, separately. Echo-intensity of two selected regions along the ultrasound transmission direction was recorded using a small animal ultrasonographic system, and the echo-intensity attenuation coefficient was calculated. Correlation between the echo-intensity attenuation coefficient and the content of hepatic triglyceride was analyzed by the Spearman's rank correlation analysis. The results showed that the lipid accumulation in livers increased significantly in both HFD and KKAy mice compared with CON mice and it was more serious in KKAy mice than that in HFD mice. The values of echo-intensity attenuation coefficient were also increased in sequence according to group. These values were positively associated with the content of hepatic triglyceride (r = 0.744, P < 0.01). In conclusion, the echo-intensity attenuation coefficient is a simple, impersonal, and non-invasive method for evaluating the hepatic lipid accumulation. It can be used to research the process and the treatment of fatty liver diseases in mice.
8.Analysis on the chapter of rehabilitation definition in the fourth edition of rehabilitation medicine textbook

The definition of rehabilitation should reflect the connotation and extension of rehabilitation medicine system,moreover,special attention should be paid to the process of translation,applications and promotion due to its derivation from Latin.The fourth edition of rehabilitation medicine textbook which published by People's Medical Publishing House described the definition of rehabilitation inaccurately,although integrated the foreign authority definitions.The definition of ‘ re-learning’has been described by the textbook without considering logical sequence,which caused difficulty in explaining and comprehending between teachers and students in class.Besides,application of another term of rehabilitation medicine in the textbook is imprudence.
9.APPLICATION OF ENDOSCOPIC ULTRASONOGRAPHY AND MICROVESSEL COUNTING IN PREOPERATIVE STAGING OF COLORECTAL CANCER

30 patients with colorectal carcinoma underwent endoscopic ultrasonography(EUS)and microvessel counting for staging before operation.The accuracy of EUS in assessing depth of tumor in- filtration was 76.7%,that in detecting lymph node involvement being 73.3%.The correctness of pre- operative Duke's staging remained 70%.Microvessel counting correlated with depth infiltration and lymph node invasion.The microvascularity in tumor with serosal involvement was obviously abundant than that without,so as in tumor with and without lymphatic invasion.The results revealed that both EUS and microvessel counting can be considered as an essential evidence for preoperative staging of col- orectal cancer and thereby for predicting prognosis.
10.The teaching strategy and evaluating efficacy in selective medical English course

The lecture, role play, combination of team learning and self-directed learning, group presentation,these multiple pedagogies were adopted in the teaching of selective medical English course.The questionnaire result showed this multiple teaching strategy can foster the students' interest in medical English, enhance their awareness of active learning and increase the ir learning efficacy, which is an effective teaching model, and can be applied and promoted in the medical English teaching.
