4.Advancement in Intestinal Barrier Dysfunction of Severe Acute Pancreatitis

Objective To summarize the recent progress in pathogenetic,diagnostic and therapeutic researches on the intestinal barrier dysfunction(IBD) of severe acute pancreatitis(SAP).Methods The advancement of IBD in SAP,which was published recently at home and abroad,was collected and reviewed.Results The pathogenesis of IBD in patients with SAP was complex.Ischemia-reperfusion injury,endotoxin,inflammatory mediators and gastrointestinal hormone played an important role in the process of IBD.There were many ways to detect IBD,and the ratio of lactulose and mannitol,plasma diamine oxidase were relatively ideal markers.Medical therapies,such as treatment of SAP and maintaining the perfusion of intestines,were essential to cure IBD.On this basis,the propulsives,nutritional support and traditional Chinese drugs should be administered reasonably.Conclusions IBD is a sophisticated process of pathophysiology.In recent years,abundant of animal experiments and clinical researches have provided new clue for prevention and cure of IBD,but further researches are still needed on the mechanism of the cells and molecules implicated.
5.The Effect of Human Placenta Lipopolysaccharide and Dexamethasone on Spontaneous Resorption of Herniated Intervertebral Disc

Objective To explore the effect of human placenta lipopolysaccharide (LPS) and dexamethasone (DXM) on spontaneous resorption of herniated intervertebral disc. Methods The animal model of intervertebral disc herniation was set up by placing the harvested disc into the epidural space at L4 or L5 of the same rahbit. Then the 24 rabbits were randomly divided into three groups. In human placenta LPS group, rabbits were intraperitoneally injected 2ml of human placenta LPS at the second, 6th and 10th days after surgery. In DXM group, rahbits were intravenously injected 1mg/kg of DXM at the second to 15th days after surgery. In control group, rubbits were intravenously injected 3ml/kg of normal saline at the second to 15th days after surgery. All rahbits were killed, and herniated intervertebral disc was removed for histological and immunohisto- chemical examination at the 12th weeks after surgery. Results A great amount of newly formed vessles and inflammation cells were founded in the disc tissues in the LPS group. In the control group, a lot of granular tissues were founded in the disc tissues. In the DXM group, small amount of granular tissues were founded in the disc tissues. Objective To investigate the expression and significance of vascular endothelial growth factor(VEGF) in the various types of degenerated lumbar discs and the normal control.Methods Forty-one cases of degenerated intervertebral disc tissue(the contained group with 17 cases and uncontained group with 24 cases) from surgical intervention for lumbar disc herniation and 13 cases of normal disc tissue from anterior approach for scoliosis and lumbar fracture were stained with hematoxylin and eosin.The expression of VEGF was tested by immunohistochemistry method.Results There was no capillary infiltration and VEGF positive expression in the control group.The VEGF positive rate was 41.2%(7/17) in the contained group,and 79.2%(19/24) in the uncontained group.In the contained group and uncontained group,the VEGF positive rate was higher than that in the control group(P
7.Role of functional abnormality of sphincter of Oddi on formation of intrahepatic calculi in rabbits

Objective To investigate the relationship between motor abnormality of sphincter of Oddi(SO) and hepatolithiasis.Methods The hepatolithiasis rabbit models were established in adult rabbits(n=64) which were randomly divded into 2 groups:control group(n=16)and experimental group(n=48),including 2 wk,4 wk and 8 wk group(n=16 per group).During the formation process of hepatolithiasis,bile character and ratio of stone formation were both observed,then the myoelectric waves of SO were recorded.Finally,The SO segment was sliced and observed by transmissional electron microscope.Results The rate of true stone formation in group 8 wk(68.75%,11/16)was higher than that in group 4wk(12.50%,2/16)and in group 2wk(0.00%,0/16).Compared with the control group,there was significant decrease of both SO myoelectric peak amplitude and SO myoelectric frequency in the experimental group.Electron microscopy showed that SO cells in control group and 2wk group had no change,but the 4wk and 8wk groups underwent changes including fibrolysis of myofilament,increase of collagen fibers,fusion and thickening of microfilament,decrease and array disorder of macula densa,swelling of chondrosome and endoplasmic reticulum,widening of perinuclear space and deviation and pyknosis of nucleus,and formation of inclusion body in cytoplasm;and the degeneration degree in 8wk group was obvious than that in group 4wk.Conclusions The functional abnormality of SO may contribute important effects on the formation of hepatolithiasis.Fibrous degeneration of SO maybe the pathogenesis of functional abnormality of SO.
8.Analysis of the tasks of and demands for health human resources in the laboratory specialty of the hospital

Objective To determine the tasks that health human resources of technicians in the medical laboratory specialty ought to fulfill, the factors that affect performance and the estimation of demands. Methods Such methods as analyzing medical records, tasks and health demands were used. Results The current situation of health human resources of technicians is as follows: Those with intermediate, junior or no professional titles respectively account for 46.5%, 36.5% and 11.6%; those with a master's or a bachelor's degree or a degree from a polytechnic college or school respectively account for 2.0%, 8.2%, 42.9% and 40.8%; and others account for 6.1%. The average utilization ratio of laboratory service by inpatients and outpatients is respectively 96.7% and 49.5%; and the average utilization of laboratory service per inpatient and per outpatient is respectively 12.5 and 2.2 times. Of the average daily service volume, emergency, outpatient and inpatient, microbiological, immune serum, and biochemical tests respectively account for 8.3%, 3.0%, 8.1%, 7.8% and 72.8%. The chief tasks of the technicians are performing laboratory operations and assisting the physicians with the reporting of the laboratory results. The demand for health human resources of technicians in the laboratory specialty in the hospitals across the country numbers 89 920 people. Conclusion The results of the study will provide health administrative departments, medical institutions and medical education departments with basis for strengthening human resources management and formulating educational plans.
9.Platelet Activating Factor Induced Rat Lung Injury Depends on Extracellular Calcium

The concentration of 1.8 ng/ml platelet activating factor (PAF) caused the elevation of pulmonary vascular permeability and a steady increase of lung weight gain (LWG) in isolated perfused rat lung. The LWG, fluid filtration coefficient and extravasation amount of T-1824 labelled albumin were lower of 89. 2%, 70. 9% and 75. 5%, respectively, in PAF perfusion without Ca2+ group than those in PAF perfusion with Ca2+ group, within 35 min. Mean pulmonary capillary pressure and pulmonary arterial pressure decreased from + 0.725 ? 0.389 kPa and +1.187 ? 0.320 kPa to -0.708 ? 0.315 kPa and - 0.025 ? 0.279 kPa, respectively. Calcium free perfusate without PAF slightly increased vascular permeability in isolated rat lungs. The results suggest that PAF-induced rat lung injury depends on the existence of extracellular calcium ion.
