1.Affect of citalopram on HAMD,SDS scores,hs-CRP,heart rate variability,renin angiotensin levels of patients with coronary heart disease complicated with anxiety-depression

Objective To explore the affect of citalopram on Hamilton Depression Scale (HAMD) ,self rating Depression Scale (SDS) score ,high sensitive C reactive protein (hs‐CRP) ,heart rate variability (HRV) ,andrenin angiotensin systerm(RAS) levels of patients with coronary heart disease complicated with anxiety‐depression .Methods 160 cases of patients with coronary heart disease and anxiety depression in January 2012 to August 2014 were selected ,they were randomly divided into observation group and control group according to the list of number table methods ,the control group of 80 patients used routine treatment methods ,while 80 patients in observation group were added with citalopram on the basis of conventional therapy ,after 6 weeks of treatment ,the clinical efficacy and changes of the HAMD ,SDS score ,score ,and hs‐CRP ,heart rate variability ,and renin angiotensin levels of two groups were compared .Results (1)The total clinical effective rate of the observation group was 93 .75% ,significantly higher than 70 .0% of the control group ,the difference was statistically significant (χ2 =10 .21 ,P<0 .05);(2)HAMD score ,SDS score ,hs‐CRP of the observation group was significantly lower than that of the control group ,the difference was statistically signifi‐cant(t=12 .98 ,9 .43 ,11 .86 ,P<0 .05);(3)HRV of the study group was significantly higher than that in the control group ,there was a statistically significant difference (P<0 .05);(4)Angiotensin II of the observer group were (93 ± 26)ng/L ,Angiotensin Ⅱ of the control group were (37 ± 14)ng/L ,the difference was statistically significant (P<0 .05) .Conclusion Citalopram has a signifi‐cant efficacy in treatment of patients with coronary heart disease combined with anxiety‐depression ,which is safe and reliable that is worth clinical popularizing and applying .
2.Effect of Spleen-stomach Regulating Herbal Medicine in Improving Nutritional Status of Patients Receiving Hemodialysis

[Objective] To observe the effect of spleen-stomach regulating herbal medicine in improving the nutritional status in dystrophic patients receiving maintenance hemodialysis. [ Methods ] Sixty-one dystrophic cases receiving maintenance hemodialysis were randomized into two groups. Group A ( n = 31) and group B ( n = 30) received the same symptomatic treatment and the same maintenance hemodialysis, and group A received spleen-stomach regulating herbal medicine additionally. For those in group A with spleen-stomach deficiency, Liujunzi Decoction was applied; for those with stagnation of phlegm, modified Huanglian Wendan Decoction was used. After treatment, the main symptoms and signs of dystrophy, triceps skin fold (TSF), and middle-arm muscle circumference (MAMC) were observed, as well as serum levels of total protein (TP), albumins (Alb) and transferrin (TRF), hemoglobin (HB), protein catabolic rate (nPCR) and SGA scoring for comprehensively nutritional evaluation. Some immune responses induced by atherosclerosis may aggravate the state of the illness by inducing persistent arterial inflammation, whereas others may inhibit arterial inflammation and atherosclerosis.Thus, the prevention and treatment of atherosclerosis may be achieved by the method of immunomodulation, by which selectively suppress proatherogenic immune responses or activate antiatherogenic ones.
4.Assessment of left ventricular contraction asynchrony in patients with acute myocardial infarction using velocity vector imaging

Objective To assess the left ventricular contraction asynchrony in patients with acute myocardial infarction(AMI) by velocity vector imaging(VVI).Methods Sixty AMI patients and 40 healthy volunteers were included.Using VVI,the time to peak longitudinal systolic velocity (TL-V) was measured in the left ventricular long axis views,the time to peak radial systolic velocity (TR-V) was measured in parasternal short axis views.The deviation of the earliest and the latest TL-V and TR-V (Ts max-min) was measured,and the standard deviation of TL-V and TR-V (TS-SD) was also measured.Results ①TL-V ,TR-V of the infarcted and non-infarcted segments in patients with acute myocardial infarction were longer than those of the normal segments (P <0.05).②The longitudinal Tsmax-min,Ts-SD and the radial Tsmax-min,Ts-SD in infarcted patients were increased compared with the healthy volunteers (P <0.001).③Ts of the infarcted segments was increased in turn from the class Ⅰ to class Ⅳ of the cardiac function (P <0.001).Conclusions VVI could be used to assess the left ventricular contraction asynchrony in patients with AMI.VVI is a new useful method to determine the infarcted segments.
5.Social determinants of the life quality among the elderly in Beijing, China

Objective:To verify the association between social determinants and the quality of life ( QoL) among the elderly in Beijing .Methods:A questionnaire household survey was conducted and 1 685 over 60 dwellers in Beijing were interviewed .The EQ-5D scale was used to measure the elderly ’ s QoL and Time Trade-off model was used to calculate the EQ-5D index.Multiple linear regression and Logistic regression were employed to analyze the association between social determinants and the QoL .Results:There was no statistical significant difference in the QoL between the urban and the rural residents ( P>0 .05 ) .The elderly with higher education level and income or married tended to have higher overall QoL scoresand better performance of five dimensions of the QoL (P<0.05).Conclusion:More policies should be focused on the elderly with lower social determinants in both urban and rural areas in Beijing .
6.Impact of adherence of pus moss on the efficacy of different surgical procedures for perforated peptic ulcers

Objective To study the effects of intraperitoneal adherence of pus mosses on the efficacy of open repair(OR)and laparoscopic repair(LR)for perforated peptic ulcers(PPUs). Methods Intraperitonealad-herence of pus mosses was divided into the first,second and third stage,respectively. Surgical duration,volume of peritoneal drainage ,rate of abnormal wound healing ,number of intensive care unit transfer ,recovery time of bowel function ,ambulation time ,length of hospital stay ,and postoperative treatment abandonment rate were compared. Results There were 78 ,46 ,and 29 patients for the three stages in OR group ,and 71 ,39 ,and 15 patients in LR group,respectively. There were significant differences insurgical duration,volume of peritoneal drainage,rate of abnormalwound healing,number of ICU transfer,recovery time of bowel function,ambulation time,length of hospital stay about the first stage between the two surgical methods(P values were 0.000,0.000, 0.015,0.000,0.000,0.000,and 0.043 respectively)except for treatment abandonment rate(P value was 0.209). There were significant differences in volume of peritoneal drainage ,rate of abnormal wound healing ,number of ICU transfer,recovery time of bowel function,ambulation time,and length of hospital stay(P values were 0.000, 0.033,0.000,0.000,0.001,and 0.028 respectively)for the second stage except surgical duration and treatment abandonment rate(P values were 0.196 and 0.358 respectively) between the two procedures. The differences were significant in surgical duration ,volume of peritoneal drainage ,and length of hospital stay between the two methods (P values were 0.039 ,0.003 ,0.024 ,and 0.002 respectively) for the third stage but there were no significant differencesin rate of abnormal wound healing , number of ICU transfer , recovery time of bowel function ,ambulation time ,and treatment abandonment rate (P values were 0.175 ,0.173 ,0.766 ,and 0.757 respectively). Conclusions LR is superior to OR under the first stage of pus moss and still has advantages under the second stage. However,LR may be inferior to OR under the third stage.
7.Efficacy of postauricular injection of shuxuening combined with lidocaine for intermediate and high-frequency sudden hearing loss

[ABSTRACT]OBJECTIVETo assess the efficacy of postauricular injection of shuxuening combined with lidocaine in treating intermediate and high-frequency sudden hearing loss.METHODS48 patients with intermediate and high-frequency sudden hearing loss were randomly divided into postaurical injection group (n=24) and control group (n=24). The control group was given hormone, microcirculation improvement, nutrition of nerve and other conventional treatment. The postauricular injection group received 3 doses of postauricular inject of 0.5ml shuxuening and 0.3ml lidocaine tertian injection in addition to conventional treatments.RESULTSTwo weeks after administration, the total effective rates of postauricular injection group 87.5%were significantly higher than that of the control group 62.5%. Pure tone average improved (20.74±15.19)dB on average in the postauricular group, and (12.44±9.12)dB in the control group. The difference was statistically significant (P<0.05). CONCLUSIONPostauricular injection of shuxuening combined with lidocaine treatment for intermediate and high-frequency sudden hearing loss has better curative effect than that of the conventional treatment.
8.The effects of aerobic exercise on low-and high-density lipoprotein cholesterol

Objective To observe the effects of aerobic exercise on low-and high-density lipoprotein cholesterol,and to investigate the relationship between exercise intensities and high-density lipoprotein cholesterol (HDL-C) changes.Methods Sixty patients with low HDL-C were randomly allocated into a high intensity exercise group,a moderate intensity group or a low intensity group with 20 in each group.According to each patient's maximal heart rate in a restrictive electrocardiogram exercise test,the different aerobic exercise intensities were formulated as follows.The target heart rate of the high-intensity training group was 90% of each subject's maximum heart rate; in the moderate-intensity group it was 70% ; in the low-intensity group it was 50%.All 3 groups jogged on a medical treadmill for 20 minutes every other day with their heart rates monitored at all times.They maintained the target heart rate for 20 minutes.The experiment lasted 2 months.Results In the high intensity group HDL-C increased from 0.81 ± 0.22 to 1.04 ± 0.33 mmol/L after training; in the moderate intensity group it increased from 0.82±0.20 to 1.03 ±0.37 mmol/L; in the low intensity group it increased from 0.79 ±0.25 to 0.82 ± 0.27 mmol/L.That increase in the low intensity group was not statistically significant.After high intensity or moderate intensity training the HDL-C increases were statistically significant,but the difference between the groups was not.After training,in 15 cases among the high intensity group,13 cases from moderate intensity group and in 1 case from the low intensity group the HDL-C level achieved a normal value (≥ 1.04 mmol/L).Comparing the high intensity group to the low intensity group or the moderate intensity group to the low intensity group,there were significant differences in the number of patients who achieved normal HDL-C readings,but there was no significant difference between high intensity group and the moderate intensity group.Conclusion Low intensity aerobic exercise has no obvious effect on low HDL-C levels.High or moderate intensity aerobic exercise can effectively raise the HDL-C level,but there is no significant difference in the fraction of patients reaching normal values between high and moderate intensity aerobic exercise.So moderate intensity is recommended for improving low levels of HDL cholesterol.
9.Correlation Factors of Nosocomial Infection in Patients after Receiving Craniocerebral Operation

OBJECTIVE To analyze the correlation factors of nosocomial infection in patients after receiving craniocerebral operation.METHODS Totally 388 hospitalized patients after receiving craniocerebral operation from Jan 2006 to Dec 2006 were studied retrospectively.RESULTS From them 45 pateints were suffering from nosocomial infection and total incidence of nosocomial infection was 11.59%.Among them,60.0% was lower respiratory tract infection,11.11% was surgical wound infection and 8.89% was upper respiratory infection.Aging,length of stay and invasive operation(deep vein needle detaining,trachea incision,tracheal intubation,urinary canal and drainage tube dwelling) were the main correlation tactors.CONCLUSIONS Minimizing invasive operation and early removing various kinds of dwelling tubes,using antibiotics correctly,improving hospitalizd environment and strengthening wards management are the measures to reduce nosocomial infection according its correlation factors.
10.Effect of relaxation therapy applied on patients with sleep disorders because of post craniocerebral traumatic syndrome

Objective To study the effect of relaxation therapy on sleeping quality of patients with post cran- iocerebral traumatic syndrome. Methods 90 cases with sleep disorders were randomly divided into the treatment group and the control group. The control group was given routine care and the treatment group was given relaxation therapy combined with routine care. Take pittsburgh sleep quality index (PSQI) as evaluation criteria. Results There was significant difference between the two groups( P < 0.01 ). Sleep quality was more improved by relaxation therapy in the experimental group than that of the control group. Conclusion Relaxation therapy can effectively im- prove sleep quality in patients with post craniocerebral traumatic syndrome.
