1.Relationship of susceptibility of primary angle-closure glaucoma with glutathione S-transferase T1 and M1 polymorphisms

?AlM:To investigate the relationship of susceptibility of primary angle- closure glaucoma with glutathione S-transferase T1 ( GSTT1 ) and M1 ( GSTM1 ) polymorphisms.
? METHODS: Totally, 300 cases were collected from primary angle-closure glaucoma patients and 300 health volunteers were served as control group. The observation group were divided into chronic and acute primary angle-closure glaucoma groups, then multiplex PCR technology was used to detect the genetic polymorphisms of GSTM1 and GSTT1.
?RESULTS:The distribution frequencies of GSTT1-null genotype were 54. 3%, while it was 54. 0% in the control group, statistically no significance between control group and observation group (χ2 = 0. 053, P > 0. 05 ) ; The frequency GSTT1 - null genotype in chronic group of primary angle-closure glaucoma was 54. 9%, while it was 48. 6% in the acute group of primary angle - closure glaucoma, statistically no significance between control group and acute group(χ2=0. 064, P>0. 05), and chronic group (χ2=0. 037, P>0. 05); The distribution frequencies of GSTM1-null genotype was 59. 0%, while it was 55. 7%in the control group, statistically no significance between control group and observation group (χ2=0. 013, P>0. 05);The distribution frequencies of GSTM1-null genotype was 62. 3% and 58. 1% in acute and chronic group of primary angle - closure glaucoma patients respectively. Acute group of primary angle - closure glaucoma has no significant compared with control group (χ2 = 0. 005, P>0. 05), Chronic group of primary angle-closure glaucoma had no significant compared with control group (χ2 = 0. 047, P>0. 05).
?CONCLUSlON:No statistic significance relationship is found between primary angle-closure glaucom patients and the null genotypes of GSTM1 and GSTT1 susceptibility.
2.Content Determination of Glucosamine Hydrochloride and its Preparations by HPLC

Objective: To establish an HPLC method for determining the content of glucosamine hydrochloride and its prepara-tions. Methods:A Phenomenex Luna NH2 column was used, and the mobile phase was ammonium phosphate buffer (dissolving 3. 5 g dipotassium phosphate and 0. 25 ml ammonium hydroxide into 1 000 ml water, adjusting pH to 7. 5 with phosphoric acid) – acetoni-trile (25 ∶75). The flow rate was 1. 5 ml·min-1 and detection wavelength was 195 nm. The column temperature was 35℃ and the injection volume was 20 μl. Results:The linear range of glucosamine hydrochloride was 1. 6-16. 0 mg·ml-1(r=0. 999 9). The av-erage recovery of glucosamine hydrochloride tablets and capsules was 99. 3% and 99. 5%, respectively, and RSDs were 0. 3% ( n=9). Conclusion:The method is simple, accurate and reliable, and suitable for the quality control of glucosamine hydrochloride and its preparations.
3.Cochlear implantation combined with subtotal resection of temporal bone in treatment of nasopharyngeal carcinoma after radiotherapy of temporal bone necrosis

Objective To investigate the effects of cochlear implantation combined with subtotal resection of temporal bone in treatment of nasopharyngeal carcinoma after radiotherapy of temporal bone necrosis. Methods A prospective study method was used, and 76 cases of nasopharyngeal carcinoma after radiotherapy of temporal bone necrosis from February 2013 to October 2015 in our hospital for diagnosis and treatment were selected. According to the open control paired principle,the patients were equally divided into observation group and control group, each of 38 cases. Both groups received subtotal temporal bone resection in the treatment, and the observation group received cochlear implant therapy. The surgical effect and hearing improvement of two groups were observed. Results All patients successfully completed surgery. In the observation group, the patients showed normal reactions to intraoperative electrode detection and postoperative electrode impedance, without electrode slippage. There were no statistical differences between the two groups in postoperative pneumothorax and other complication (P>0.05). The sound intensity level of hearing test in postoperative 1 month in the observation group and control group were (21.23±5.22) dB and (28.42±4.19) dB, which was significantly lower than that in the preoperative 1d [(38.24 ±4.98) dB and (38.12 ±5.00) dB], with significantly statistical difference (P<0.05). The postoperative 1 month of hearing and speech score in the observation group were (87.24 ±2.98) points and (82.10 ±3.91) points respectively, which were significantly higher than those in the control group [(73.02 ±5.30) points and (71.84 ±3.11) points] (P<0.05). The two groups of postoperative 1 months of hearing and speech scores were also significantly higher than those in the preoperative 1 d [observation group: (34.29±3.49) points and (32.10±5.30) points; control group: (33.20±4.14) points and (31.98±4.92) points] (P<0.05). Conclusion Cochlear implantation combined with subtotal resection of temporal bone in treatment of nasopharyngeal carcinoma after radiotherapy of temporal bone necrosis shows high safety and success rate, which can promote the improvement of hearing and speech ability, and be widely used in clinic.
4.Value of a Rapid One Step Test for the Detection of Helicobacter Pylori Antigen in Stool Samples.

Objective To evaluate the sensitivity and specificity of a rapid one-step test for the detection of Helicobacter pylori antigen in stool samples. Methods 13C-urea breath test (13C-UBT) and rapid urease being taken as “gold standard”, Helicobacter pylori stool antigen (HpSA) was detected with Atlas rapid one-step test among patients with syndromes in upper alimentary tract. These patients were divided into two groups. One was pretreatment group that included 62 cases and the other one was post-treatment group with 42 cases. Results In pretreat_ment group, there were 24 Hp positive cases and 38 negative ones according to the golden standard, 27 positive and 35 negative according to one-step HpSA assay, and the sensitivity and specificity was 100% and 92% respectively; In post-treatment group, 15 positive and 27 negative according to the golden standard, 19 positive and 23 negative according to one-step HpSA assay, and the sensitivity and specificity 93%and 81% respectively; the total sensitivity and specificity was 97% and 88% respectively. Conclusions HpSA rapid one-step test has good sensitivity and specificity in diagnosis of Hp infection and judgment of Hp eradication.
5.Local decompression and selective interbody fusion with pedicle screw fixation technique for the treatment of degenerative lumbar scoliosis

BACKGROUND:Conservative treatment is mostly invalid in treatment of severe degenerative lumbar scoliosis-induced low back pain, so surgical intervention is general y required. Repair methods are decompression, rigid fixation, and selective interbody fusion for the“responsible vertebra”to re-establish normal lumbar curvature, spinal balance and good stability. OBJECTIVE:To investigate clinical curative effect of local decompression and selective interbody fusion with pedicle screw fixation technique for the treatment of degenerative lumbar scoliosis. METHODS:Data of 57 cases of degenerative lumbar scoliosis from Armed Police Corps Hospital of Hunan Province from January 2011 to January 2014 were retrospectively analyzed. According to clinical characteristics of the patients, selective fusion, local decompression and pedicle screw fixation were used for repair. The imaging analysis and Suk criteria were used to evaluate fixation, decompression and bone fusion. Japanese Orthopaedic Association scores were utilized to assess therapeutic effect. RESULTS AND CONCLUSION:(1) Operation time was 157-255 minutes, averagely 176 minutes. Amount of bleeding was 480-1 700 mL, averagely 835 mL. Postoperative drainage volume was 140-210 mL, averagely 155 mL. (2) 57 patients were fol owed up for 12-38 months. Postoperative X-ray revealed that Cobb’s angle was (3.8±2.1)° on the coronal plane after treatment, and the correction rate of lordosis was 54%. The sagittal Cobb’s angle was (6.3±10.5)°, and the correction rate of lordosis was 35%. According to Suk standard, the fusion rate of vertebra was 95%. According to low back pain score system recommended by Japanese Orthopaedic Association scores, the effective rate was 91%at the end of fol ow-up. (3) There were 2 cases of cerebrospinal fluid leakage, 1 case of nerve injury, and 1 case of pedicle screw failure, no infection, no blood vessel injury. (4) These findings suggest that according to imaging data and self clinical features, local decompression and selective interbody fusion with pedicle screw fixation technique for the treatment of degenerative lumbar scoliosis can rebuild and stabilize degenerative lumbar scoliosis in the coronal and sagittal planes to different degrees, with satisfactory clinical curative effect.
6.Fishbone diagram reduces the incidence of blood residual after sealing tube indwelling needle

Objective To investigate the influence of fishbone diagram in the incidence of blood residual after sealing tube indwelling needle in cardiovascular disease patients.Methods Two hundred and ninety-three patients(1,035 times)with sealing tube intravenous indwelling needle were assigned into control group and another 272 ones(1,276 times)with the needle into experiment group according to admission sequence. In the former group the method of conventionally sealing the tube for intravenous indwelling needle was applied and in the latter,fishbone diagram was applied to analyze the key factors for blood residuals and regulate pertinent measures for effective sealing of tube.The two groups were compared in terms of incidence of blood residuals.Result The incidence of blood residuals in the experiment group was significantly lower than that in the control group(P<0.05).Conclusion The fishbone diagram is effective in reducing the incidence of blood residuals after sealing the tube for intravenous indwelling needle by way of taking effective interventional measures after screening out the key influential factors for blood residuals.
7.Innovative practice of workshop training based on the CDIO model for the training of new nurses in the operating room

Objective:To explore the effect of CDIO (conceive, design, implement, operate) model workshop training on the training of new nurses in the operating room.Methods:Forty-two nurses with standardized training were selected as the research objects, and they were randomly divided into two groups: observation group ( n=21) and control group ( n=21). The control group used the traditional training method in the operating room, and the observation group used the workshop training based on the CDIO model. SPSS 17.0 was used to statistically analyze the operation skill scores and case scores after training. The measurement data is expressed by (mean±standard deviation), the independent sample t test is used for comparison between the two groups, and the chi-square test is used for the comparison of counting data. Results:After the training, the scores of operating room knowledge assessment of the two groups were (72.31±2.16) points and (73.61±2.18) points respectively, with statistical significance ( P<0.05); the operation skill scores of observation group were (7.42±0.13) points, which were higher than those of the control group (6.62±0.11) points, and the disposal scores of observation group after operation were (7.12±0.20) points, which were higher than those of the control group (6.27±0.16) points, with statistical significance ( P<0.05); the professional quality scores of observation group were (7.41±0.25) points, which were higher than those of the control group (6.55±0.22) points; the adaptability scores of observation group were (7.06±0.22), which were higher than those of control group (6.35±0.21) points, with statistical significance ( P<0.05). Conclusion:The workshop training based on the CDIO model is effective for the training of nurses in the operating room, and the combined application effect is better than the effect of the traditional teaching model, especially for the improvement of practical skills.
8.Ginsenoside-Rg1 protects human dermal fibroblasts against 8-methoxypsoralen plus ultraviolet A-induced photoaging through p53-dependent signaling pathways

Aim To investigate the roles of p53-dependent signaling pathways in the process of ginsenoside Rg1 protection against 8-methoxypsoralen(8-MOP) and subsequent ultraviolet A(UVA) irradiation induced photoaging model in human dermal fibroblasts(HDFs).Methods Photoaging model was established by 8-MOP/UVA in skin HDFs.Flow cytometry, enzyme cytochemistry, immunofluorescence and Western blot were employed.Results Pretreatment with ginsenoside Rg1 could significantly reduce the amount of UVA-generated 8-oxo-dG and relieve the photoaging representation.Compared with 8-MOP/UVA treatment group, pretreatment with Ginsenoside Rg1 could decrease the expression of SA β-galatosides(SA-β-Gal), and down-regulate the level of senescence associated proteins(p16 and p21).Conclusions Ginsenoside Rg1 has prominent dose-dependent antagonism on senescence of skin HDFs induced by 8-MOP/UVA, and its mechanism may be due to its antioxidation which reduces the production of photoproducts to protect telomere against abnormal shortening.
9.Detection of cancer stem cell like circulating tumor cells in peripheral blood of 45 patients with gastric cancer

Objective To explore the expression and significance of CD44 gastric cancer stem cell biomarker positive circulating tumor cells (CTC ) in peripheral blood of patients with gastric cancer . Methods From March 2010 to December 2012 ,45 patients with gastric cancer and 20 healthy individuals were enrolled .Peripheral blood samples of all the subjects were collected .The expression of CTC and CD44 postive CTC were detected by density gradient centrifugation method and double immunofluorescent staining .Chi‐square test and t‐test were performed for statistical analysis .Results CTC in peripheral blood of healthy controls were not detected .Among 45 patients with gastric cancer ,CTC were detected in 27 patients and 19 of them were CD44‐positive .The difference in positive rate of CTC expression was statistically significant between two groups (χ2 =10 .986 , P<0 .05) .Among 27 CTC‐positive patients , 19 were proximal gastric cancer and 16 of them were CD44‐positive;16 patients had lymph node metastasis and 14 of them were CD44‐positive;13 patients had distant metastasis and 12 of them were CD44‐positive;14 patients had recurrence and 13 of them were CD44‐positive .The CD44 positive CTC was correlated with tumor location ,lymph node metastasis ,distant metastasis and recurrence (χ2=5 .891 ,5 .527 ,5 .787 and 7 .052 ,respectively ,all P<0 .05) .Furthermore ,in 13 patients with recurrence and CD44 positive CTC ,the recurrence time was (10 .54 ± 5 .55) months which was shorter than that of one patients with CD44 negative CTC (19 months) ,and the difference was statistically significant (t= -3 .654 , P=0 .004) .Conclusions CTC which express cancer stem cell marker may be associated with recurrence and metastasis of gastric cancer .It has more important clinical significance to detect this subtype of CTC .
10.Studies on Dissolution Determing Method for YakepingⅡ Capsules

Objective: To establish a dissolution determing method for the two primary ingredients atenolol and nifedipine in YakepingⅡcapsules. Methods:A small glass method was adopted with the rotation rate of 50 r·min-1 . According to the dissolution conditions in Japan “quality of medical drugs information set” with appropriate adjustments in accordance with the actual situation of the samples, different YakepingⅡ capsules were determined by HPLC respectively in pH 1. 2 artificial gastric solution ( containing 0. 5% sodium dodecyl sulfate), pH 4. 0 acetate buffer(containing 0. 5% sodium dodecyl sulfate), pH 6. 8 phosphate buffer(contai-ning 0. 25% sodium dodecyl sulfate) and water(containing 0. 25% sodium dodecyl sulfate). Results:The assay displayed a good lin-earity over the concentration range of 10-30 μg·ml-1 for atenolol and nifedipine(r=0. 999 6 and r=0. 999 8), and the recovery of the two components in the different medium was 99. 64%(RSD=0. 73%), 99. 55%(RSD=0. 65%), 99. 53%(RSD=0. 47%)and 99.54% (RSD=0.51%), 99.52%(RSD=0.67%), 99.52%(RSD=0.72%), 99.51%(RSD=0.63%)and 99.61%(RSD=0. 59%)(n=9). The dissolution of different batches of YakepingⅡcapsules in the four media showed the similar behavior. Conclu-sion:The method is simple, accurate and reproducible in the dissolution determination of atenolol and nifedipine in YakepingⅡ cap-sules.
