1.Investigation and reflection about cognition and attitude of clinical nnrses for carrying out holisticnursing

Objective To investigate the attitude of clinical nurses to the practice of holistic nursing and provide basis for establishing harmonious relationships between patients and nurses.Methods primary nurses were randomly selected and underwent investigations on quality check of nursing work quarterly by serf-designed inventory.Results 24 nurses considered it necessary to carry out holistic nursing but 70 of them were reluctant to undertake it.They thought it boring because of the high requirement of holistic nursing and much literal document in practice.Conclusion Nursing services calls out seeking truth and dealing with concrete matters at present.The holistic nursing is not equipped with professional title since serious shortage of nursing staff.The emaphasis should be focused on the health education according to different situations in hospitals.Besides we should practice holistic nursing step by step instead of by the modality.
2.The application of fast track surgery theory in thoracoscopes pulmonary bulla resection

Objective To explore the feasibility, safety and advantage of thoracoscopes pulmonary bulla resection without thoracic closed drainage tube according to the fast track surgery (FTS) theory. Methods The clinical data of 53 patients with pulmonary bulla were retrospectively analyzed. Among them, 29 patients underwent the thoracoscopes pulmonary bulla resection without thoracic closed drainage tube (FTS group), and 24 patients underwent the thoracoscopes pulmonary bulla resection with thoracic closed drainage tube (tradition group). The oxygenation index at 24 h after operation, visual analogue score (VAS) at 24, 48 and 72 h after operation, postoperative complications, postoperative hospital stay and hospital costs were compared between 2 groups. Results There was no statistical difference in oxygenation index at 24 h after operation between FTS group and tradition group:(312.4 ± 16.4) mmHg (1 mmHg=0.133 kPa) vs. (330.6 ± 13.2) mmHg, P>0.05. The VAS at 24, 48 and 72 h after operation in FTS group was significantly lower than that in tradition group:(2.6 ± 1.1) scores vs. (4.3 ± 1.6) scores, (1.6 ± 0.9) scores vs. (3.8 ± 1.4) scores and (0.8 ± 1.0) scores vs. (2.9 ± 1.4) scores, and there was statistical difference (P<0.05). The incidence of postoperative complications, postoperative hospital stay and hospital costs in FTS group were significantly lower than those in tradition group:13.8%(4/29) vs. 37.5% (9/24), (3.2 ± 0.9) d vs. (6.2 ± 1.2) d and (1.3 ± 0.3) × 104 yuan vs. (1.5 ± 0.4) × 104 yuan, and there were statistical differences (P<0.05). Conclusions FTS used in thoracoscopes pulmonary bulla resection is feasible and safe, and has certain advantages.
3.Study on Bone Metabolism in Children with Leukemia and Its Relationship to Tumor Necrosis Factor-Alpha Pre-and Post-Treatment

Objective To investigate the bone metabolism of children with leukemia and its relationship to tumor necrosis factor-?(TNF-?)pre-and post-treatment.Methods Serum bone gla protein(BGP),the excretion rate of urianry deoxypyridinoline(DPD)and TNF-? were measured in 35 children with leucocythemia pre-and post-treatment and 30 normal children(control group).Results The results showed that BGP,the excretion rate of DPD and TNF-? in children with leucocythemia before treatment were significantly higher than those in control group.However,the excretion rate of DPD decreased after chemotherapy but still significantly higher than that of control group(P0.05).Conclusions In children with leukemia,there are bone absorption and bone dissolution before chemotherapy,while there is osteogenesis dysfunction and bone absorption after chemotherapy.TNF-? can be used as a parametre for evaluating treatment effect and monitoring relapse.
4.Efficacy of transversus abdominis plane block for postoperative analgesia in pediatric patients undergoing operation on inguinal region

Objective To evaluate the efficacy of transversus abdominis plane (TAP) block for postoperative analgesia in the pediatric patients undergoing operation on the inguinal region.Methods Sixty-four pediatric patients of both sexes, aged 1-3 yr, weighing 8.5-23.6 kg, of American Society of Anesthesiologists physical status Ⅰ , undergoing elective unilateral high ligation of the hernia sac or high ligation of the processus vaginalis, were equally randomized into either intravenous analgesia group (group VA) or TAP block group (group TAP).In group TAP, after induction of anesthesia, the pediatric patients received ultrasound-guided TAP block with 0.2% ropivacaine 1 ml/kg injected locally.After the laryngeal mask airway was inserted, the pediatric patients were mechanically ventilated, and then inhaled 2%-3% sevoflurane for maintenance of anesthesia.In group VA, postoperative analgesia was performed with fentanyl 0.35 μg · kg-1 · h-1 until 24 h after operation.The level of pain was rated using the FLACC pain scale.When FLACC score>5, fentanyl 0.25 μg/kg was injected intravenously as rescue analgesic.The occurrence of TAP block-related adverse events, time for removal of laryngeal mask airway, and occurrence of fentanyl-related nausea and vomiting, respiratory depression, and emergence agitation were recorded.Results No TAP block-related adverse events were observed in group TAP, and no respiratory depression was found in the two groups.Compared with group VA, the time for removal of laryngeal mask airway was significantly shortened, and the requirement for rescue analgesics and incidence of vomiting and emergence agitation were decreased in group TAP (P< 0.05).Conclusion TAP block with 0.2% ropivacaine 1 ml/kg provides good efficacy for postoperative analgesia with good safety in the pediatric patients undergoing operation on the inguinal region.
5.Early removal of the urethral catheter after transurethral plasma kinetic resection of the prostate in the treatment of BPH.

OBJECTIVETo investigate the feasibility and effect of early removal of the urethral catheter after transurethral plasma kinetic resection of the prostate (PKRP) in the treatment of benign prostatic hyperplasia (BPH).
METHODSWe equally randomized 128 BPH patients treated by PKRP to an experimental group and a control group, urethral catheters removed at 1 -2 days for the former and at 5 -7 days for the latter. We compared the relevant indexes and clinical effects between the two groups.
RESULTSThe baseline data were not significantly different between the two groups. Compared with the controls, the experimental group showed a significantly shorter postoperative hospital stay ([6.8 +/- 1.9] d vs [3.7 +/- 1.5] d, P < 0.05) and lower infection rate (25.0% vs 10.9%, P < 0.05). All the patients were followed up for 3 -6 months postoperatively. At 3 months after surgery, both the experimental and the control groups showed remarkable improvement in the International Prostatic Symptoms Scores (4. 9 +/- 2. 2 vs 5. 3 +/- 2. 3), maximum urine flow rate ([21.5+/- 5.6 ] ml/s vs [19.1 +/-4.9 ] ml/s) , and residual urine ( [ 16.8+/- 10.3 ] ml vs [18.9 +/- 12.3 ] ml), but with no significant differences between the two (P > 0.05) , and no significant differences were observed in postoperative complications (P >0.05).
CONCLUSIONEarly removal of the urethral catheter after PKRP, with its advantages of shorter postoperative hospital stay, lower infection rate, and no influence on the long-term effect, deserves to be recommended as a routine method in the treatment of BPH.

7.Blocking efficacy of 23 cases of mother to child transmission of acquired immunodeficiency syndrome in the First Hospital of Changsha

Objective To observe the blocking efficacy of mother to child transmission (MTCT) in pregnant women with positive human immunodeficiency virus(HIV), and explore proper MTCT blocking mode for acquired immunodeficiency syndrome.Methods Clinical data of 23 HIV-positive pregnant women in a hospital from 2005 to 2015 were retrospectively analyzed.Results All 23 HIV-positive pregnant women received highly active antiretroviral therapy (HAART) and comprehensive intervention for blocking MTCT of HIV.Among these women, 12 got pregnant after receiving HAART, 10 were detected positive HIV in early pregnancy (within 28 weeks) and then received HAART, 1 was detected positive HIV 28 weeks after pregnancy and then received HAART.23 HIV-positive pregnant women all delivered normal newborns, follow-up observation of babies found no HIV infection.Conclusion HAART for HIV-positive pregnant women is the key to block MTCT of HIV, combined with preventive medication and artificial feeding of newborns, HAART can effectively prevent MTCT.Mutual blocking mode, such as HAART for HIV-positive pregnant women by specialists, pregnancy check-up, and preventive medicine for infants provided by maternity and child care hospital, is highly efficiency.
8.Cloning and Expression of EGFR Tyrosine Kinase and Construction of the Screening Model for the Enzymatic Inhibitors

As a member of tyrosine kinases(TK) family,Epidermal growth factor receptor(EGFR) has an activity of intrinsic protein tyrosine kinase,which plays an essential role in the regulation of signal transduction in the cells.Due to the abnormal expression of EGFR-TK,the certain type cancers may developed and progressed.Based on that,the inhibitors of EGFR-TK could be effective medicines for the treatment of cancer.The EGFR-TK domain was amplified by RT-PCR with RNA of HUVCEs cells as the template and expressed in E.coliBL21(DE3) using plasmid pET30a as vector.The recombinant protein was purified with the affinity chromatography(Ni-NTA),which was identified to have kinase activity catalyzing the substrate phosphorylated with ATP in the enzymatic reaction.Using the recombinant EGFR-TK as target,the screening model for enzymatic inhibitors was constructed.
9. Research on strategy of China pharmaceutical companies coping with competition of authorized generics in American drug market

OBJECTIVE: To study the principle, feature and impact of the authorized generics strategy in American drug market, thus to provide some references for Chinese scholars who research the authorized generics strategy and for China pharmaceutical companies that would face competition in American pharmaceutical market. METHODS: By case study and policies analysis, the ways how the brand-name companies used the authorized generics in American drug market were summarized. RESULTS and CONCLUSION: Successful authorized generics strategy can maintain a large market share of the brand-name companies products. In American drug market, there are some particularities about the authorize generics strategy due to the features of American drug policies and regulations. China pharmaceutical companies should implement different authorized generics strategy according to their own conditions in American drug market at present.
10.Influence of Laparoscopic Surgery on the Expression of Potential Metastasis-related Molecules and Implantation of Endometrial Cancer
