Objective To evaluate the influence of cytochrome P450 (CYP2C9 and CYP4F2) polymorphisms on anticoagulant intensity of warfarin after cardiac valve replacement. Methods A total of 136 patients tak ing warfarin after cardiac valve replacement were identified and classified into 4 groups:CYP2C9 wild type group (CYP2C9*1*1),CYP2C9 mutated type group (CYP2C9*3),CYP4F2 rs2108622 wild type group (CC) and CYP4F2 rs2108622 mutated type group (CT or TT).The patients' baseline data, initial dose of warfarin and base INR measurement resuhs were recorded and then the follow-up was conducted. The initial administration of warfarin to INR standard time for the first time, total amount of warfarin and the average daily amount were recorded. Results Patients carrying CYP2C9* 1* 1 had increased time to reach INR target value for the first time (P < 0.05);and the total warfarin doses and average daily dose when INR reached target value were higher than those carrying CYP2C9*3 (P < 0.05).When compared with those in two wild type groups, patients carrying CYP2C9 and CYP4F2 rs2108622 mutated type needed the shortest time when INR reached target value for the first time, and the total warfarin doses and average daily dose when INR first reached target value was the lowest, which showed significant difference (P < 0.05).And when compared with CYP2C9 mutated type group, the INR average time to reach the first target was shortened and the total warfarin dose of patients carrying CYP2C9 and CYP4F2 rs2108622 mutated type was lower (P < 0.05).Conclusion The gene polymorphisms of CYP2C9 and CYP4F2 are significant hereditary factors influencing warfarin dose.Detection of CYP2C9 and CYP4F2 genotypes prior to medication and predicating warfarin dosage may result in lower incidence of over-anticoagulation and reduce the dosage-adjusting time of warfarin.

ObjectiveTo explore the value of color Doppler echocardiography in diagnosing apical hypertrophic cardiomyopathy( apical hypertrophic cardiomyopathy, AHCM). MethodsRetrospective analysis of 28 cases with apical hypertrophic cardiomyopathy by echocardiography images was carried out. Results28 cases AHCM patients, apical myocardial thickness 17 ~29mm,5 cases of simple apical myocardial thickening;10 cases of ventricular apex、left ventricular myocardial wall thickening in the apical segment;6 cases of ventricular apex、left ventricular myocardial thickening of apical segments of inferior;6 cases of Ventricular apex, left ventricular free wall thickening of apical myocardial; 1 case of ventricular apex、right ventricular free wall thickening of apical myocardial. All patients were diagnosed. ConclusionColor Doppler echocardiograph was an effective method for diagnosis of apical hypertrophic cardiomyopathy.

Objective To study the effect of fat emulsion intravenous infusion on serum free fatty acids(FFAs) in rats. Methods 24 male Wistar rats were divided randomly into 3 groups,8 rats in each group.(1)Control group(NS),the rats were infused with normal saline intravenously and regular chow;(2)Group LCT,infused with 10% intralipid fat emulsion intravenously;(3)Group MCT/LCT,infused with 10% lipofundin fat emulsion. Group LCT and group MCT/LCT were continuously received equal calorie, nitrogen and volemin in 'All-in-One'solutions. Serum samples were drawn on the 8th day after PN for fatty acid determination. Results The FFAs in Group LCT and group MCT/LCT were remarkably higher than that in control group, but no difference between Group LCT and group MCT/LCT. Conclusions Fat emulsion intravenous infusion can increase the serum free fatty acids considerately.

OBJECTIVE To study the clinical usability of virtual endoscopy(VE) in tympanoplasty.METHODS A total of 102 patients (204 ears) were observed by virtual endoscopy. Tympanoplasty was performed in 72 cases (75 ears) including 53 cases (55 ears) with chronic otitis media and 19 cases (20 ears) with congenital microtia and middle ear dysmorphia. Ossicular chain reconstruction was conducteded in 65 ears at the same time. RESULTS The ossicular chain was showed eroded in 19 ears of 23 patients with cholesteatoma otitis media pre-operatively by VE, but was found eroded in all of 23 ears during operation. The ossicular chain was showed eroded in 29 ears of 32 patients with osteitis otitis media pre-operatively by VE, but during operation it was found 23 ears with malleus and incus eroded, 11 ears without head of stapes or up-structure of stapes. VE showed 18 ears with congenital microtia and middle ear malformation and 2 ears with small tympanic cavity and no ossicular chain preoperatively, and operation proved 17 ears with severe ossicular malformation, 2 ears without stapes, 1 ear with vestibular window atresia. There were 2 ears with sudden hearing loss after tympanoplasty, the VE showed ossicular displacement. The coincidence rates between VE and operation view were 92 % in patients with otitis media and 100 % in patients with congenital microtia and middle ear malformation. CONCLUSION VE can supply reliability data for evaluation of the damaged ossicular chain and efficacy of tympanoplasty.

AIM To establish an efficient and convenient system for expressing functional proteins. METHODS Xenopus leavis females were injected with HCG, eggs were harvested next morning and dejellied by 2% cysteine, from which the Xenopus leavis egg cell free translating system was prepared. Through subcloning technique, TFAR19 gene was cloned to pBluescript SK plasmid which was linearized by enzyme, then transcript and capped in vitro to synthesis the cRNA of TFAR19. RESULTS This cRNA was translated in freshly prepared cell free translating system, the product was identified by Western Blot. CONCLUSION The results showed that this translating system could efficiently translate TFAR19 cRNA and be a very useful translating tool.

Objective To compare the features of activity and participation difficulty of children with cerebral palsy, intellectual disability and autism. Methods 42 children with cerebral palsy, intellectual disability, and autism aged 3-6 years were evaluated with ICF-CY Questionnaire. Results There were mild barriers in the domains of learning and applying knowledge, communication, mobility, self-care, domestic life and major life areas, and the moderate barriers in the domains of general tasks and demands, interpersonal interactions and relations. There were more barriers in learning and applying knowledge in children with cerebral palsy and intellectual disability than those with autism. Conclusion There are various features in activity and participation difficulty in children with cerebral palsy, intellectual disability or autism, which required diversity of educational rehabilitation strategies.

ObjectiveTo evaluate the efficacy and safety of hydroxychloroquine(HCQ) in pregnant patients with systemic lupus erythematosus (SLE).Methods Twenty-four pregnant patients with SLE treated with HCQ during pregnancy from May,2006 to February,2011 were studied retrospectively. All babies were followed up during early infancy for growth development.ResultsOf them,22 patients were treated with HCQ throughout the whole pregnancy with no lupus flare occurred in 21 patients (95.4%),while temporary discontinuation of HCQ precipitated a flare of disease in two patients.Three patients ( 12.5% ) had premature delivery, and pregnancy induced hypertension happened in 3 patients (12.5% ).No congenital abnormalities occurred and mean follow-up of 26 months ( range 1 - 47 months) revealed no abnormalities in these children. Conclusion Our findings reinforce the safety of HCQ therapy during pregancy and HCQ should probably be maintained throughout the pregancy in patients with SLE.
