1.Study on impact mechanism of four different anesthetics on middle cerebral artery blood flow velocity

Objective To compare the impacts of four different intravenous anesthetic agents on middle cerebral artery blood flow velocity(V‐MCA) during the anesthesia induction period .Methods Totally 80 cases were randomly divided into four groups (n=20) ,maintenance drugs of anesthesia were propofol 2 .00 mg/kg ,etomidate 0 .30 mg/kg ,midazolam 0 .15 mg/kg and dezocine 0 .20 mg/kg respectively ,the bispectral index (BIS) value was dropping to below 50 ,the endotracheal intubation and mechanical ventilation were performed .The transcranial Doppler (TCD) monitoring was adopted to monitor and record middle cerebral artery mean flow velocity (Vm‐MCA) ,mean arterial pressure (MAP) ,heart rate (HR) ,systolic blood pressure (SBP) ,diastolic blood pressure (DBP) in the four groups before induction after entering operation room (T0 ) ,at1 min before intubation (T1 ) ,immediate intubation (T2 ) ,at 1 min after intubation (T3 ) ,3 min after intubation (T4 ) ,5 min after intubation (T5 ) .Results Except for the midazolam group ,Vm‐MCA at T1 in the other three groups were significantly lower that that in the T0 group (P< 0 .05);Vm‐MCA ,SBP ,DBP after intubation in the midazolam group and the etomidate group were significantly increased compared with the basic values ,while the difference between the propofol group and the dezocine group had no statistical significance (P>0 .05) .Con‐clusion midazolam and etomidate are weaker than propofol and dezocine in the aspect of inhibiting the middle cerebral arterial blood flow fluctuations caused by intubation .
2.The Effect of Jianweiyuyang Granule on Gastric Ulcer and Expression of VEGF and PDGF in Rat

Objective To study the effect of Jianweiyuyang(JWYY) granule on gastric ulcer of rats and its effective mechanism. Methods The rat model of gastric ulcer was set up by Okeba's method with some modification, and the expression of vascular endothelial growth factor (VEGF) and platelets-derived growth factor (PDGF) in the gastric ulcer tissues was examined by immunohistochemical method. Results JWYY granule could elevate the expression of both VEGF and PDGF, obviously decrease the ulcer index, increase the thickness of gastric mucosa, and promote the healing of gastric ulcer. Conclusion The results indicated that Jianweiyuyang granule can increase the expression of VEGF and PDGF, which may be one of the mechanism of its promoting the healing of the gastric ulcer.
3.The EvaIuation of Auditory and VerbaI AbiIity in 107 Pre-LinguaIIy Deaf ChiIdren with CochIear ImpIantation

Objective To evaluate the auditory and verbal abilities of cochlear implantees using question_naires ,to explore basic rules of the development of auditory and verbal abilities ,and to analyze their relationships with some influencing factors .Methods A retrospective analysis of 107 cases of pre-lingually deaf children with cochlear implantation was conducted with Infant-Toddler Meaningful Auditory Integration Scale/Meaningful Audi_tory Integration Scale (IT -MAIS/MAIS) ,categories of auditory performance (CAP) and speech intelligibility rat_ing (SIR) for the evaluation of auditory and verbal abilities .An analysis was performed to assess influencing fac_tors ,such as postoperative time ,age at implantation ,physical age at follow -up ,time with hearing aids before im_plantation .SAS 9 .1 .3 was used in statistical analysis .ResuIts A total of 107 pre-lingually deaf children (48 males and 59 females) participated in this study .The postoperative time ranged from 0 to 49 months ,with an average of 9 .0 ± 10 .9 months .The results showed the postoperative time and age at implantation had significant influences on the scores of IT -MAIS/MAIS ,and postoperative time and physical age at follow -up had significant influences on the scores of CAP/SIR (P<0 .05) ,and hearing aids (HA) usage before implantation had no significant influences on these three questionnaires (P>0 .05) .ConcIusion The ages at implantation ,postoperative time and physical age had significant influences on the auditory and verbal abilities of pre-lingually deaf children with cochlear implanta_tion .Further studies are needed to explore their relationships with hearing aids usage before implantation .
4.Femoral compression variation in osteoporosis rats after intervention with different drugs

BACKGROUND:The mechanical index is an important method for the evaluation of the therapeutic efficiency of drug treatment for osteoporosis animal models. OBJECTIVE: To explore the effects of various drug treatments on osteoporosis through a mechanical performance test about the femoral compression of rats. METHODS: Thirty-six Wistar female rats were randomized into six groups: normal control group, model group, Dan Qiparticles group, alpha-D3 group, premarin group, ipriflavone group, with six rats in each group. Osteoporosis models were made in al groups except for the normal control group, and after modeling, the rats in different groups were treated withDan Qi particles, alpha-D3 group, premarin and ipriflavone, respectively. After 15 weeks, the rats were kiled by abdominal aortic bloodletting to take out the left and right femurs that were placed on a universal testing machine to perform a compressive test at a speed of 5 mm/min. RESULTS AND CONCLUSION:The maximum load, maximum stress, maximum displacement, maximum strain, and elastic modulus were significantly lower in the model group than the other four groups (P < 0.05). There was no difference in different mechanical parameters between alpha-D3 group and model group as wel as between Dan Qi particles group and normal control group (P> 0.05). These findings indicate that osteoporosis leads to the variation of compression mechanical properties of the femur. There are good compression mechanical properties of the femur after treatment with premarin and ipriflavone, andDan Qi particles has the best effect.
5.Cloning of the pneumolysin gene

OBJECTIVE To prepare pneumolysin(Pn)by genetic engineering and thereby establish the basis for the study of vaccines against otitis media. METHODS A pair of primers including two restriction sites was designed based on the pneumolysin gene sequence reported by Walker in 1987. The pneumolysin gene was PCR-amplified from pneumococcal DNA. The resulting fragment, digested by restriction enzymes, was ligated into the vector PET-28a and then transformed into host cell E.coli JM109(DE3). RESULTS The sequence of the inserted pneumolysin gene was confirmed by DNA sequencing. CONCLUSION The pneumolysin gene was successfully cloned into the host cell.

7.Study of categories of auditory performance and speech intelligibility rating of post-lingual cochlear implantes.

OBJECTIVE:
To evaluate the verbal communication skills of post-lingual cochlear implantees with categories of auditory performance (CAP) and speech intelligibility rating(SIR), and to explore their relationships with the duration of hearing loss, the postoperative time and preoperative hearing aid usage.
METHOD:
A retrospective analysis was conducted on 23 cases of post-lingual patients with cochlear implantation. CAP and SIR were applied in the evaluation of verbal communication skills. IBM SPSS Statistics 19 was used in statistical analysis.
RESULT:
The average score of CAP was 6.13, and statistical analysis showed CAP score was not significantly correlated with the duration of hearing loss, the postoperative time or the time of wearing a hearing aid preoperatively. The average SIR score of all these post-lingual patients was 4.91.
CONCLUSION
It showed that CAP score was not significantly correlated with these three factors. This may due to the low accuracy of CAP or the small sample size in the present study. SIR was not suitable for the evaluation of post-lingual cochlear implantees.

8.Effect of oleuropein on IL-1β-induced rat chondrocytes

AIM:To investigate the effect of oleuropein on interleukin-1β( IL-1β)-induced SD rat articular chondrocytes .METHODS:The SD rat articular chondrocytes were isolated by 2 step enzyme digestions .The chondrocytes were cultured in vitro.Inverted microscopic observation was performed during the culture .Alcian blue staining and type II collagen immunohistochemical staining were used to identify the chondrocytes .The effects of oleuropein on the viability of chondrocytes were determined by CCK-8 assay.The cells in 3rd passage were pretreated with oleuropein at 10, 50 or 100 μmol/L and subsequently stimulated with IL-1βat 10 μg/L for 24 h.Production of prostaglandin E 2 ( PGE2 ) and ni-tric oxide (NO) were evaluated by the Griess reaction and an enzyme linked immunosorbent assay (ELISA).The mRNA expression of matrix metalloproteinase ( MMP)-1 and MMP-13 was measured by real-time PCR.The protein levels of in-ducible nitric oxide synthase (iNOS), cyclooxygenase-2 (COX-2) and nuclear factor-kappa B (NF-κB) were detected by Western blotting .RESULTS:The cell viability of chondrocytes was not significantly impaired by treating with oleuropein at concentration of 10, 50 or 100μmol/L for 24 h compared with control group .Pretreatment with oleuropein significantly in-hibited the production of PGE 2 and NO induced by IL-1β.Oleuropein also significantly decreased the IL-1β-stimulated MMP-1 and MMP-13 mRNA expression in articular chondrocytes .Pretreatment with oleuropein inhibited the IL-1β-media-ted activation of NF-κB by suppressing the degradation of its inhibitory protein IκBαin the cytoplasm .CONCLUSION:Oleuropein inhibits IL-1β-induced inflammatory gene expression by suppressing NF-κB activation at the transcriptional le-vel, suggesting a new mechanism for the anti-inflammatory effects of oleuropein as a novel agent on treating with osteoarthri-tis.
9.Comparative analysis of the efficacy of ERCP and Transoral pancreaticoangioscopic technique in the placement of biliary stent in hilar cholangiocarcinoma

Objective To evaluate the efficacy of two kinds of biliary stent placement in the treatment of high bile duct carcinoma.Methods the clinical data of 136 patients with hilar cholangiocarcinoma who underwent biliary stent implantation in Nanyang Central Hospital from January 2021 to September 2023 were analyzed retrospectively.According to the mode of biliary stent placement,the patients were divided into ERCP group(n=86)and transoral cholangiopancreatoscopy group(n=50).In this study,SPSS26.0 was used for analysis.The age,related test indexes,days of hospitalization,postoperative complications,incidence of liver failure,remission rate of jaundice,stent material and patency rate were observed in the two groups.Results all patients completed the operation successfully.The two groups compared the incidence of postoperative liver failure,postoperative remission rate of jaundice,different material stents,postoperative complications and short-term patency rate,there was no significant statistical significance(P>0.05).There were significant differences in postoperative complications between the two groups(t=12.02,P<0.05).Compared with postoperative biliary tract infection and postoperative bleeding,the postoperative biliary tract infection and postoperative bleeding in ERCP group were 26.7%and 11.6%respectively,which were significantly higher than those in transoral cholangiopancreatoscopy group 10.0%and 2.0%(x2=5.42,3.94;P<0.05).Conclusion of the mean hospitalization days between the two patient groups,Transoral cholangioscopy(10.78±2.17)d<ERCP(12.91±2.96)d.Compared with the 7th day after operation,the liver function of the two groups was significantly improved and the related test indexes decreased significantly.There was no significant difference in postoperative complications and short-term patency rate between plastic stent and metal stent(P>0.05).When the patency time was more than 6 months,the patency rate of metal stent was significantly higher than that of plastic stent(P<0.05).Conclusion The placement of bile duct stent by ERCP and Oral choledochoscopy is effective in relieving clinical symptoms of high cholangiocarcinoma and can improve the quality of life of patients.Transoral cholangiopancreatoscopy has low complication rate,faster recovery,greater advantage,but higher cost;plastic stents are beneficial for patients with short expected survival time,and metal stents are more suitable for patients with long expected survival time.
10.Clinical experience in facial nerve tumors:A review of 27 cases

Objective:To analyze the clinical manifestations and the diagnosis of the facial nerve tumor according to the clinical information, and evaluate the different surgical approaches depending on tumor location.Method:Twenty-seven cases of facial nerve tumors with general clinical informations available from 1999.9 to 2006.12 in the Shanghai EENT Hospital were reviewed retrospectively.Result:Twenty(74.1%) schwannomas,4 (14.8%)neuofibromas ,and 3(11.1%)hemangiomas were identified with histopathology postoperatively.During the course of the disease,23 patients(85.2%)suffered facial paralysis,both hearing loss and tinnitus affected 11 (40.7%)cases,5(18.5%)manifested infra-auricular mass and the others showed some of otalgia or vertigo or ear fullness or facial numbness/twitched. CT or/and MRI results in 24 cases indicated that the tumors originated from the facial nerve.Intra-operative findings showed that 24(88.9%)cases involved no less than 2 segments of the facial nerve,of these 24 cases 87.5%(21/24)involved the mastoid protion,70.8%(17/24)involved the tympanic protion, 62.5%(15/24)involved the geniculate ganglion, only 4.2%(1/24)involved the internal acoustic canal(IAC),and 3 cases (11.1%)had only one segments involved. In all of these 27 cases ,the tumors were completely excised,of which 13 were resected followed by an immediate facial nerve reconstruction,including 11 sural nerve cable graft,1 facial nerve end-to-end anastomosis and 1 hypoglossal-facial nerve end-to-end anastomosis.Tumors were removed with preservation of facial nerve continuity in 2 cases.Conclusion:Facial nerve tumor is a rare and benign lesion,and has numerous clinical manifestations.CT and MRI can help surgeons to make a right diagnosis preoperatively.When and how to give the patients an operation depends on the patients individually.
