2.Diagnostic value of multilayer spiral CT for chronic pyelonephritis

Objective To analyze the characteristics of multilayer spiral CT (MSCT) in chronic pyelonephritis,and to explore its clinical value in the diagnosis of chronic pyelonephritis.Methods 30 patients with chronic pyelonephritis were examined by MSCT enhancement scan,intravenous pyelogram (IVP) and double kidney color dopplar ultrasound.25 healthy people were selected as the control group.Two different seniority doctors evaluated the results separately.Results The diagnosis of two physicians had high consistency(K=0.8 1,P < 0.05).In addition,compared with IVP and color dopplar ultrasound,the diagnostic rate of MSCT enhancement scanning in the renal parenchyma sag sign,renal pelvis wall thickening,strengthen and calyces mild deformation were significantly higher (P < 0.05).Conclusion MSCT can clearly observe kidney form change in patients with chronic pyelonephritis,diagnostic rate is higher,it is worth clinical promotion.
3.The high-risk factors and audiology characteristics analysis of 952 cases of neonatal failed hearing screening

Objective To analyze the related factors in new born failed in hearing screening and its audiological characteristics . Methods From May 2009 to Febrary 2012 ,952 infants within the 3 to 6-month-old with detailed birth records and hearing screen-ing records were reviewed in the study .They were born in Chongqing and surrounding areas ,but failed in the first and second hear-ing screening .They were tested by auditory brainstem response (ABR) ,distortion product otoacoustic emissions (DPOAE) ,tympan-ometry and stapedius reflex .Results The normal ratio of tympanogram ,normal group(74 .25% ) was higher than the high-risk group(69 .61% )(P<0 .05);The pass proportion of DPOAE in normal group(34 .57% ) was higher than high-risk group(27 .94% ) (P<0 .05);In normal group ,ABR response threshold >30 dBnHL was 55 .80% ,60 .36% in high-risk group .High-risk group′s hearing abnormality rate was significantly higher than the normal group (P<0 .05);In normal group ,mean threshold of ABR was (47 .64 ± 22 .86)dBnHL .While it was(50 .58 ± 25 .02)dBnHL in high-risk group .The threshold was higher in normal group than high-risk group(P<0 .05) .Conclusion premature birth ,low birth weight(<1 500 g) ,hyperbilirubinemia ,neonatal asphyxia ,pal-ace the virus infection ,mechanical ventilation ≥ 5 days ,≥ 2 high-risk factors are risk factors for hearing loss .Newborn hearing screening is a long and arduous task ,We should enhance screening efforts in infant with high risk factors ,and actively prevent and treat neonatal perinatal risk factors and reduce the incidence of hearing loss .

5.Preliminary study of MRI in diagnosis of heat stroke

Objective To investigate MRI features of heat stroke (HS) and 1H-MRS in monitoring clinical outcome.Methods Six cases(4 males,2 females) of HS patients,aged 43-75 years old were retrospectively studied.The patients were divided into 4 severe cases (2 cases with coma for 4 hours,2 patients with persistent coma) and 2 moderate cases according to initial Glasgow coma scale (GCS) at the time of hospital admission.All patients underwent T1WI,T2WI,FLAIR,DWI,SWI,multi voxel 1H-MRS scans and the ratios of NAA/Cr,Cho/Cr,and NAA/Cho were calculated.One case of severe coma died on the next day.Follow-up imaging and brain 1H-MRS were performed on other 5 patients on the 7th and 15th day after the first examination.The imaging results were compared with clinical outcome.Results MRI features of HS for severe coma patients were iso-intensity on T1WI,hyper-intensity on T2WI,FLAIR and DWI,heterogeneous signal intensity on ADC.The infratentorial lesions distributed in the cerebellum,dentate nucleus and cerebellar arm (3 cases),and the supratentorial lesions distributed in the parahippocampal gyrus (2 cases),parietal lobe (2 cases),and temporal lobe (1 cases).The lesions could be multiple,and the cerebellum and the parahippocampal gyrus were most common involved.SWI showed focal hemorrhage in 2 cases,which located in the brainstem and parietal lobe.No obvious MRI abnormality was found in 2 cases of moderate coma patients.The ratios of NAA/Cr,Cho/Cr,and NAA/Cho for severe coma patients were 0.67 ±0.09,0.94 ±0.16 and 0.70±0.12,respectively,which for moderate coma patients were 0.87±0.12,0.95±0.13,and 0.94±0.08,respectively.The NAA/Cr and NAA/Cho ratios decreased and Cho/Cr mild decreased in the severe coma patients when compared with the moderate coma patients.One patient with persistent coma had expanded lesions on follow-up study.NAA/Cr,NAA/Cho and Cho/Cr were 0.63± 0.07,0.67 ±0.10,and 0.96 ±0.05,respectively.Two cases had smaller lesions on follow-up study with NAA/Cr and NAA/Cho ratio increased,which were 1.02±0.13 and 0.96±0.11,respectively.GCS was turned to 14.NAA/Cr and NAA/Cho for moderate coma patients were 1.17±0.10 and 1.21 ±0.07,respectively.Conclusions Severe HS patients present with cerebral edema and hemorrhage.1H-MRS can be an important index for monitoring the severity of HS.The longer the duration of coma,the worse prognosis may happen.
6.Endoscopic ultrasonography-guided fine-needle aspiration in diagnosis of pancreatic cancer

Objective To evaluate the diagnostic value of endoscopic ultrasonography-guided fineneedle aspiration(EUS-FNA)for pancreatic occupying lesion,especially pancreatic cancer.Methods From year of 2005,37 patients with suspected pancreatic cancer by means of uhrasound,CT or MRI received EUS-FNA.Amylase and tumor markers(CEA,CA19-9 and CA125)in cyst fluid were analyzed if applicable.The patients were followed up till July 2008,and the results of EUS-FNA were compared with those confirmed during the follow-up.Results The EUS-FNA yielded diagnosis of 16 cases of pancreatic duetal adenocarcinoma,1 metastatic:renal cancer,5 suspicious malignancy,6 atypia,6 normal pancreatic tissue,and 3 normal non-pancreatic tissue.During the follow-up,25 cases of pancreatic cancer and 10 benigh lesions,including 4 chronic panereatitis,4 cyst-adenoma and 2 pseudocyst,were confirmed,and the other 2 cases still remained un-determined.The sensitivity,specificity,positive predictive value and negative predictive value of EUS-FNA were 80.0%(95% CI:59.0-93.0),100.0%(95% CI:60.0-100.0),100.0%(95% CI:80.0-100.0),and 55.6%(95% CI:27.0-79.0),respectively.No severe procedurerelated complication was observed.Conclusion EUS-FNA is a safe and effective medality for diagnosis of pancreatic occupying lesions,especially pancreatic cancer.
7.Effect of sevoflurane on brain injury in a pig model of hemorrhagic shock in hypothermia environ-ment

Objective To evaluate the effect of sevoflurane on brain injury in a pig model of hem?orrhagic shock in hypothermia environment. Methods Twenty?four Bama miniature pigs, weighing 21-25 kg, aged 3-5 months, were equally randomized into 3 groups using a random number table: sham opera?tion group (group Sham);hemorrhagic shock (group HS); sevoflurane group (group Sev). The animals were anesthetized, tracheostomized and mechanically ventilated. Bilateral femoral arteries were cannulated for continuous mean arterial pressure, and heart rate monitoring, blood?letting and blood sampling. A cath?eter was inserted into the right internal jugular vein for body temperature monitoring. After the animals were awake, they were placed in an environment at-15℃. Hemorrhagic shock was induced by withdrawing 40%of blood volume from the right femoral artery within 15 min ( 30 ml∕kg) in HS and Sev groups. The animals inhaled 2% sevoflurane for 30 min after establishment of the model in group Sev. Before hemorrhagic shock, and at 30, 60, 90, 120, 180 and 240 h after hemorrhagic shock ( T0?6 ) , blood samples were collected from the femoral artery for determination of plasma tumor necrosis factor?alpha ( TNF?α) , interleukin?6(IL?6), nuclear factor kappa B (NF?κB), S100β protein and neuron?specific enolase (NSE) concentra?tions. After blood sampling at T6 , the animals were sacrificed, and brains were removed for microscopic examination of pathological changes, and for determination of Toll?like receptor 4 ( TLR4) expression by Western blot. Results Compared with group Sham, the plasma NSE, S100β protein, TNF?α, IL?6 and NF?κB concentrations were significantly increased at T2?6 , and TLR4 expression was up?regulated at T6 in HS and Sev groups ( P<0?05) . Compared with group HS, the plasma NSE, and S100βprotein concentra?tions were significantly decreased at T4?6 , the plasma TNF?α, IL?6 and NF?κB concentrations were de?creased at T2?6, and TLR4 expression was down?regulated at T6 (P<0?05), and the pathological changes were significantly attenuated in group Sev. Conclusion Sevoflurane can mitigate brain injury in a pig mod?el of hemorrhagic shock in hypothermia environment, and the mechanism may be related to inhibited TLR4∕NF?κB signaling pathway and attenuated inflammatory responses.

The underlying effect of different concentrations of neogenin on proliferation, apoptosis and the related proliferative factors in human trophoblasts was explored in order to understand the function of neogenin during placentation. TEV-1 cell line was cultured and the expression of netrin-1 was detected by using indirect cellular immunofluorescence. Exponentially growing TEV-1 cells were treated by different concentrations of neogenin (0, 1, 5, 10, 50 ng/mL) for 24 h. Cell viability was measured by 3-(4,5-dimethyl-2-thiazolyl)-2,5-diphenyl-2H-tetrazolium bromide (MTT) assay. TEV-1 cell apoptosis was assessed by flow cytometry (FCM). The expression of netrin-1 mRNA and protein in TEV-1 cells was examined by using real-time PCR and Western blot, respectively. It was found that immunoreactivity for netrin-1 was observed in cytoplasm of the trophoblasts. Immediately after treatment with different concentrations of neogenin for 24 h, the netrin-1 expression began to increase. Real-time PCR revealed that the expression level of netrin-1 mRNA was 37.59+/-10.25 times higher than control group when TEV-1 cells were exposed to 50 ng/mL neogenin (P<0.01), and the same tendency was seen by using Western blot. MTT results showed that proliferation of TEV-1 cells was independent of neogenin. Meanwhile, apoptosis was significantly increased to (22.15+/-6.15)% at 50 ng/mL neogenin and (6.55+/-0.25)% without neogenin (P<0.01). It is suggested that neogenin regulates proliferation and apoptosis of TEV-1 cells. And it can enhance the ability of TEV-1 cells to express netrin-1 in a dose-dependent manner. Neogenin may play an important biological role in the normal human pregnancy and contribute to the physiological pregnancy process.

10.Design and medical application of T-shaped shoes with chuck regulator

Objective To design T-shaped shoes with a chuck regulator to facilitate the orthopedics patient in lower limb immobilization,nursing and rehabilitation training.Methods Each of the shoes was composed of the body of low temperature thermoplasticized plate,a regulator,two cross bars controlled by the regulator and a circular silica gel liner at the heel.The stability of triangles kept the limb involved at the middle position,and prevented the lower limb from inward turning,outward turning or dropping.The shape of the triangles was modified by adjusting the regulator to form an individualized fixation posture for each patient.Results The shoes facilitated clinical nursing,enhanced the patient comfort and decreased the complications.Conclusion The shoes gains advantages in wearing,low cost and repeated use,and thus is worthy promoting practically.
