Objective The aim of this study is to assess the value of serum vascular endothelial growth factor-C (VEGF-C) for predicting recurrence and prognosis in patients with esophageal cancer.Methods A total of 65 patients with esophageal cancer,40 healthy controls were recruited in this study.Serum levels of VEGF-C were measured by ELISA methods.Results Serum level of VEGF-C in esophageal cancer group was higher significantly than those in controls [7.2 (1.2 ～ 35.5) μ g/L vs.4.2 (0.2 ～ 12.6)μ g/L,P ＜ 0.01].Receiver operating characteristic (ROC) curve of serum VEGF-C for predicting recurrence was 0.781 (P＜ 0.01); sensitivity and specificity were 73.1％ and 61.5％,respectively.The survival rate in patients with high VEGF-C level was lower than those with low VEGF-C level (P ＜ 0.01).Conclusions VEGF-C may be closely associated with the development and metastasis of esophageal cancer.Serum VEGF-C is a useful marker for predicting recurrence and prognosis in esophageal cancer.

[Objective]To explore the effects on the distal femoral joint by high intraosseous pressure after blocking of proximal and middle femoral medullary cavity with bone cement.[Method]The rabbit model was established whose proximal and middle femoral medullary cavity of left side was infused with PMMA while the right side served as the control.Twenty-six rabbits were cavided randomly into 4 groups,8 for normal control group,the remained modeled rabbits were grouped according to the observing time-point into following groups:4th(T4),8th(T8) and 16th week(T16).The rabbits was killed to obtain the samples of distal femoral articular synovium,cartilage and the bone under cartilage.Then the structural changes of these tissues were observed by HE staining,toluidine blue staining,immunohistochemistry and transmission electron microscope.[Result](1)HE staining:the damage to articular cartilage,synovial tissue and the bone under cartilage aggravated with time.At the 16th week,it showed the articular cartilage was destructed,the bone structure was severely damaged with the synovial tissue proliferation and swelling.(2)Toluidine blue staining(16th week):all the layers of articular cartilage appeared loss of stain while the superficial layers were slightly loss of stain in control group.(3)Immunohistochemistry(16th week):①CollagenⅡ:the chondrocytes were positively stainned.②TGF-?1:chondrocytes stain was positive;the staining of articular synoviocytes appeared positive.(4)Transmission electron microscope:The lesions to articular chondrocytes and bone cells aggravated with time,part of which presented with necrosis and collapse at the 16th week.[Conclusion]The distal femoral intraosseous and intramedullary blood circulation is severely damaged after blocking of the proximal and middle femoral medullary cavity with bone cement,leading to continuous high intraosseous pressure.It makes the bone metabolism disordered in distal femur,which results degeneration and necrosis in distal femur,cartilage and synovial tissue.

Objective To investigate the method and curative effect for the operative treatment of acetabular fractures.Methods Forty-one patients with acetabular fractures were treated operatively from March 2001 to March 2006.All fractures were classified according to the Letournel-Judet classification,22cases were simple fractures and 19 cases were complicated fractures.Kocher-Langenbeck (K-L) approach were adopted in 31 cases (14 cases through osteotomy of greater trochanter additionally),ilioinguinal approach were adopted in 8 cases and anterior combined posterior approach were adopted in 2 cases.Different fractures were fixed by reconstructive plates and screws.Results All the patients were followed up for 16-36 (24.31 ± 5.31 ) months.According to the reduction criteria of Matta radiography,there were 27 cases with anatomic reduction,8 cases with satisfactory reduction,3 cases with poor reduction and 3 cases with joint contour reduction.According to the Merle D' Aubigne clinical criteria for joint functions,14 cases showed excellent results,18 cases were good,5 cases were fair and 4 cases were poor,and the excellent-good rate was 78%( 32/41 ).Necrosis of the femoral head was found in 3 cases,heterotopic ossification in 4 cases,traumatic arthritis in 3 cases,sciatic nerve injury in 4 cases,femoral nerve injury in 1 case,all the nerve injury recovered in 6 months.Lower extremity deep venous thrombosis in 3 cases,all of them were recovered by given anticoagulation.No death,infection or nonunion were found.Conclusions K-L approach or ilioinguinal approach are adopted in the most of the operative treatment of acetabular fractures.Dual reconstructive plates or single.reconstructive plate combine lag screws can fix solid.It can make hip to do early exercise.

p21-Activated kinases including p21-activated kinase 2 contributed to the regulation of actin cytoskeleton and cell dynamics. In order to investigate the function of PAK2 on the maturation of Xenopus oocyte, PAK2-NT(PAK2-N-terminal,PAK2-NT) and PAK2-NTm (PAK2-N-terminal mutation) mRNA were microinjected into Xenopus oocyte respectively. Under fluorescent microscopy germinal vesicle breakdown was observed during cytokinesis. To further observe the relationship of oocyte cytokinesis, polar body formation and Cdc42 activity, confocal microscopy with time-lapse was employed . As a result, occurrences of germinal vesicle breakdown in oocytes were similar to those oocytes injected with PAK2-NT mRNA or injected with PAK2-NTm mRNA,but no cytokinesis and polar body formation were observed in oocytes injected with PAK2-NT mRNA or PAK2-NTm mRNA. These results indicated that PAK2 involved in Xenopus oocytes cytokinesis and polar body formation independent of Cdc42 activity.

Objective To evaluate the clinical value of choledochoscopy combined with electrohydraulic lithotripsy (EHL) for the treatment of pancreatolithiasis. Methods From October 2002 to July 2006, choledochoscopy combined with EHL was used in 11 patients with pancreatolithiasis during a dissection of the pancreatic duct combined with pancreato-jejunum Roux-en-Y anastomosis (EHL group). The data of these patients were compared with the data of 12 patients with pancreatolithiasis, who were treated by routine dissection of the pancreatic duct combined with pancreato-jejunum Roux-en-Y anastomosis from January 1995 to September 2002 (Routine group). Results The intraoperative blood loss and the postoperative rate of residual stones in the EHL group were significantly less than those in the routine group [(145.5?82.0) ml vs (406.7?384.9) ml, t=-2.201, P= 0.039; and 0% vs 41.7%, P= 0.037]. The postoperative rate of pain relief in the EHL group was higher than that in the routine group (90.9% vs 58.3%), however the difference was not significant (P=0.155). In the EHL group, 6 patients had diarrhea before the operation, 4 of them achieved relief after the surgery (4/6), which was not significantly more than that in the routine group (5/9, P=1.000). Before the operation, 7 patients in the EHL group and 9 in the routine group had diabetic mellitus; in each group, the disease was relieved in 2 patients after the operation (2/7 vs 2/9, P=0.665). Conclusions Choledochoscopy combined with EHL is effective for the treatment of pancreatolithiasis. The procedure can increase the rate of stone removal, decrease the intraoperative blood loss, and elevate the postoperative rate of pain relief.

Objective To study the cerebral protective effect of intravenous propofol anaesthesia on the patients with acute severe brain injury.Methods 70 patients with acute severe brain injury were randomly divided into control group and observation group,each group had 35 cases.The observation group was given intravenously propofol 2mg/kg for anesthesia induction,then maintaining anesthetization by 4mg · kg-1 · h-1.The control group was given midazolam,then discontinuous indrawed isoflurane.Diprivan 1.2mg/kg,fentanyl 4μ g/kg and vecuronium bromide 0.15 mg/kg were also used intravenously for the maintenance of anesthesia.The changes of intracranial pressure (IP)and cerebral oxygen partial pressure(COP) before anesthetization,surgery,3 hours after surgery and the waking time of surgery were recorded.Results After treatment,IP of two groups was decreased and COP was increased sharply; the IP of observation group was (18.98 ± 3.98) mm Hg,which was lower than (21.76 ± 4.01)mm Hg of the control group(t =7.20,P ＜ 0.05),COP was (17.09 ± 3.03) mm Hg,which was higher than (14.67 ± 2.63)mm Hg of the control group (t =7.23,P ＜ 0.05).The wake time of observation group was (15.98 ± 2.01) h,which was shorter than (20.01 ± 1.23) h of the control group (t =7.96,P ＜ 0.05).Conclusion The cerebral protective effect of intravenous propofol anaesthesia is good in patients with acute severe brain injury;it is worth to popularize in clinic.

Objective To investigate the efficiency of three methods to detect HPV infection of desquamated cervical epithelial cells and the relationship between methods and TCT.Methods hC2 method was used in disconnecting the HPV DNA chain in the samples to hybrid captures,and examine the 13 types of HPV which were highly related to cervical carcinoma.Q-PCR was first used to extract HPV DNA of samples and expanded nucleotide to be examined on Q-PCR to obtain results directly.MPHC was used to extract HPV DNA and to amplify nucleotide.The products was added to micro-well plate to proceed hybridization,and enzyme label was added,and results were studied in the enzyme labelling apparatus.The results of the latter two methods were the same as hC2.hC2 matched Q-PCR was used to examine 260 eases of desquamated cells from HPV infection against 200 cases of desquamated cells from HPV infection,and the results were compared by using MPHC matching examination.Results 1.With three methods,HPV infection was found to be 100% in all the abnormal cervical squamous epithelial cells.2.The sensitivity of the methods,howerer,was different.The QFCR and MPHC had higher positive rate than hC2.Positive HPV infection rate with Q-PCR and MPHC was the same as that of hC2 in the majority of cases,as in a few cases,it was positive with Q-PCR and MPHC but negative with hC2 alone False positive rate was 2% in MPHC.Conclusion HPV testing combined with the TCT routine is the best screening program for detecting cervical cancer and precursor of cervical careinoma.
