1.The value of T wave alternans in prediction of ventricular arrhythmias in post-op TOF children

Objective To explore the value of T wave alternans (TWA) in prediction ventricular arrhythmias in post-op TOF children. Methods The characteristics of TWA were retrospectively analyzed in 36 post-op TOF patients and 120 age-matched normal children. The relationship of sustained TWA and ventricular arrhythmias was also analysed. Results Compared with normal children, the incidence of sustained TWA was significantly increased in post-op TOF group (χ2=6.98, P<0.05). In post-op TOF patients, the incidence of ventricular arrhythmia was much higher when sustained TWA was present (χ2=10.41, P<0.05). Conclusions There exists electrophysiological basis for ventricular arrhythmia in post-op TOF children. Sustained TWA has important value in prediction of ventricular arrhythmia of post-op TOF.
2.Determination of omeprazole with a new flow-injection chemiluminescence method

Objective To develop a new flow-injection chemiluminescence(FI-CL) method for the determination of omeprazole.Methods Strong chemiluminescence(CL) signal was found generated when omeprazole was injected into the reaction mixture of KMnO4 and luminol.The experimental conditions that affected the CL reaction were optimized by using FI mode.Results Under the optimum experimental conditions(150cm of mixing tubing,0.15mol/L NaOH,7.0?10-5mol/L luminol,2.0?10-5mol/L KMnO4 and 2.8mL/min of flow rate),CL intensity was linearly related to the concentration within the range of 2.0?10-8-1.0?10-5g/mL.The detection limit was 3?10-9g/mL omeprazole,and the relative standard deviations(n=11) were 1.0% and 2.6% for 1.0?10-7g/mL and 1.0?10-6g/mL omeprazole solutions,respectively.Conclusion The proposed method is simple,rapid,sensitive,and suitable for determining omeprazole content in pharmaceutical preparations.
4.Anesthetic management for video-assisted laser operation under rigid bronchoscopy

Objective To discuss the management of anesthesia for video-assisted laser operation under rigid bronchoscopy.Methods A retrospective review was made on anesthetic records of 15 cases of video-assisted operation under rigid laser bronchoscopy in this hospital from September 2002 to December 2006.According to the ASA classification,there were 7 cases of class Ⅱ,7 cases of class Ⅲ,and 1 case of class Ⅳ.The diagnosis included 13 cases of tumor and 2 cases of cicatricial stricture.All the patients had a forced expiratory volume in 1 second(FEV1) predicted lower than 70%. By using intravenous fast induction,the propofol,fentanyl,and vecuronium or rocuronium were injected successively.Then a rigid bronchoscope was inserted into the trachea under orthoptic laryngoscope.During the operation a total intravenous anesthesia(TIVA) was maintained.Before induction and 5~10 min after operation,we monitored artery blood gas(ABG) levels.Immediately following the operation the end-tidal CO2 pressure(PETCO2) was measured.Results Hemodynamics parameters were not significantly different between preoperatively and intraoperatively.During the operation,the pH value was 45 mm Hg in 5.All the patients presented normal blood pressures and heart rates. Conclusions During video-assisted laser operation under rigid bronchoscope,anesthesia with normal frequency jet-ventilation and artery blood gas monitoring is safe and reliable.
5.Experiment study on influence of outward development training on psychological resilence and subjective well-being among college students with civil aviation specialty

Objective To investigate the influence of outward development training on psychological tenacity and subjective well‐being among college students with civil aviation specialty ,in order to provide the theoretically supported coping model and in‐tervention mechanism for improving the mental health level of college students .Methods Totally 139 college students of aviation safety technological management specialty were selected as testees ,including 72 individuals in the experiment group and 67 individu‐als in the control group .The experiment group adopted the outward development training for conducting intervention ,while the con‐trol group conducted the arrangement at the corresponding time period according to the college management .Both two groups were measured by using the Psychological Resilience Scale for Adolescents and the Happiness Index Scale before and after the outward development training intervention .Then the scores in the two groups were compared between before and after intervention . Results The factor scores ,total score of the Psychological Resilience Scale and the total happiness index before intervention had no statistical difference between the two groups (P>0 .05) .The 3‐factor scores of interpersonal assistance ,active cognition and emo‐tional control ,and total happiness index after the outward development training intervention were risen ,showing statistical differ‐ence compared with before intervention (P<0 .05);whereas the 2‐factor scores of target concentration and family support and the total scores of Psychological Resilience Scale had no statistical difference between before and after intervention (P>0 .05) .The fac‐tor scores ,total score of the Psychological Resilience Scale and the total happiness index at corresponding time in the control group had no statistical difference between before and after intervention (P>0 .05) .The 3‐factor scores of interpersonal assistance ,active cognition and emotional control ,and the total happiness index after intervention had statistical differences between the two groups (P<0 .05);but the 2‐factor scores of target concentration and family support and the total score of the Psychological Resilience Scale had no statistical difference between the two groups (P> 0 .05) .Conclusion Conducting the outward development training could effectively promote the shaping and fostering of psychological resilience and subjective well ‐being among college students of civil aviation specialty .
6.Expression of CXCR4/CXCL12 and effect on the expression of CXCR4 by hypoxia in trophoblasts of human first trimester gestation

Objective:To investigate the expression of CXCR4/CXCL12 and effect on the expression of CXCR4 by hypoxia in trophoblasts of human first trimester gestation.Methods:To detect the expression of CXCR4 and CXCL12 in placental tissue and primary cytotrophoblasts by immunohistochemistry and to assess the level of CXCR4mRNA under the condition of hypoxia in the trophoblasts of first trimester gestation by reas-time PCR.Results:The expression of CXCR4/CXCL12 were detected in the villous column,blood vessels and trophoblasts in the early pregnancy;CXCR4 mRNA was significantly higher through 12,24,48 and 72 h incubation periods in the hypoxic condition than that under normal oxygen condition respectively.Conclusion:Human first trimester trophoblasts produced CXCR4/CXCL12,which may play important biological role on the normal human pregnancy;Hypoxia could regulate the expression of CXCR4,which may contribute to the physiological pregnancy process.
7.Introduction of American Military Medical Triage

Because of lots of ethical problems,military medical triage has been unaccepted by general doctors.But it's adopted by American Army in front of facts of the war.The main principles of the military medical triage are justice and efficiency.They practice the military medical triage through tow steps.First,all patients are assigned into urgent,immediate ,delayed,minimal or ambulatory and expectant status in decreasing order of medical urgency.Second,choosing the model of triage depends on nonaustere,austere or extreme conditions.The ethical issues of American military medical triage focus on four aspects - how to deal with minimally injured patient treatment first,how to treat expectants,how to care for noncombatant casualties and who decide the model of triage.
8.Analysis of the various types of hepatitis markers in perinatal women

Objective To investigate the infection status of hepatitis virus among the perinatal women in Nanjing.Methods 523 samples of perinatal women's serum were selected.The hepatitis virus markers were detected by the ELISA kit,the quantitive detection of HBV surface-antibody was detected by electrochemiluminescence immunoassay.Results The positive rate of anti-HAV IgM was 0.19%,47 cases were HBV surface antigen-positive,accounted for 8.99%,183 cases were HBV surface antibody-positive,accounted for 35.00%,224 cases were all negative HBV markers,accounted for 42.83%.The quantitive detection of HBV surface-antibody in high concentration was significant higher than middle concentration and low concentration (all P < 0.05).The positive rate of anti-HCV IgG was 1.34%,anti-HDV IgG was all negative.The positive rate of anti-HEV IgM was 0.57%.The positive rate of anti-HEV IgG was 24.28%.With the advance of age,the positive rate of anti-HEV IgG had an increasing trend,with significant difference among groups (P < 0.05).Conclusion There are important significances in the detection of hepatitis virus markers in pregnancy monitoring,it can improve the quality of the population.

10.Perforin gene mutations in 77 Chinese patients with lymphomas

BACKGROUND: Perforin gene (PRF1) mutations have been reported in patients with lymphoma, but the prevalence and characteristics of PRF1 mutation have not been identified in Chinese patients with lymphoma. METHODS: Seventy-seven patients with lymphoma, including 6 patients with Hodgkin lymphoma and 71 patients with non-Hodgkin lymphoma, were recruited. DNA samples from peripheral blood were used for PRF1 mutation detection by the PCR-sequencing method. RESULTS: Eleven novel PRF1 mutations were found in 8 of the 77 patients with lymphoma. Biallelic or compound monoallelic missense mutations in 3 patients indicated the severe impairment of perforin function, monoallelic missense mutations in 3 patients possibly contributed a genetic predisposition to malignancies, and synonymous mutations in 2 patients showed unknown significance. CONCLUSIONS: The frequency of EBV infection was similar in lymphoma patients with PRF1 mutations and those without the mutations. The same PRF1 mutations were also found in DNA samples from nails or hair follicles from 4 patients with PRF1 mutations, suggesting that these mutations may be of germ-line origin.
