2.Effects of different ventilations during the induction of anesthesia on postoperative nausea and ;vomiting in patients undergoing laparoscopic cholecystectomy

Objective To investigate the effects of different ventilations during the induction of anesthesia on postoperative nausea and vomiting in patients undergoning laparoscopic cholecystectomy. Methods One hundred and twenty patients undergoing laparoscopic cholecystectomy were randomly divided into M group and V group according to random number table, 60 patients each group. The patients in M group performed manual ventilation and the patients in V group performed mechanical ventilation. According to different tidal volume, V group were randomly divided into three groups (20 patients each group:V1,V2 and V3 group, respectively recieved body weight × 4, 6 and 8 ml/kg. After tracheal intubation,the operations were completed by maintaining the end of CO2 partial pressure (PETCO2)from 35 to 45 mmHg, 1 mmHg = 0.133 kPa. Tidal volume, peak airway pressure (Ppeak) and PETCO2 in anesthesia induction,and intraoperative gastric insufflation and postoperative nausea and vomiting were recorded and compared. Results The incidence rate of gastric insufflation, postoperative nausea and vomiting in V group were significantly higher than those in M group:50.0%(30/60) vs. 21.7%(13/60), 45.0%(27/60) vs. 18.3%(11/60), 26.7%(16/60) vs. 10.0%(6/60), P<0.05. The level of Ppeak in M group, V2 group and V3 group were signicantly higher than that in V1 group: (13.55 ± 1.95), (12.05 ± 1.23), (18.35 ± 3.71) cmH2O vs. (13.55 ± 1.95) cmH2O, 1 cmH2O = 0.098 kPa, P<0.05. The level of PETCO2 in M group, V2 group and V3 group were signicantly lower than that in V1 group:(19.60 ± 4.31), (27.75 ± 1.94), (23.60 ± 4.45) mmHg vs. (29.90 ± 2.40) mmHg, P<0.05. Conclusions Manual ventilation during the induction of anesthesia in patients undergoning laparoscopic cholecystectomy is better than mechanical ventilation . Proper manual positive pressure ventilation and volume mechanical ventilation with low tidal (4 ml/kg) may allow for decreasing intraoperative gastric insufflation and postoperative nausea and vomiting.
3.Analysis of 262 Cases of ADR in Our Hospital

OBJECTIVE:To investigate the characteristics and regularity of ADR in our hospital,and to provide reference for clinical rational drug use. METHODS:262 ADR reports of the hospital during 2013-2014 were summarized and analyzed. RE-SULTS:Among 262 cases of ADR,most of ADR occurred in elderly patients aged above 60 years old,accounting for 46.56%. 239 cases were induced by intravenous drip,accounting for 91.22%;anti-infective drugs induced 97 cases of ADR,accounting for 37.02%,among them there were 48 cases of cephalosporin-induced ADR,accounting for 49.48%(total 97 cases);most of them were lesion of skin and its appendants,being 96 cases and accounting for 36.64%. Among 262 cases,132 cases were improved af-ter drug withdrawal(50.38%);no drug sequelae or death cases was found. CONCLUSIONS:To master the characteritics and reg-ularity of ADR contribnte to redcue and aroid the repeat of ADR.

5.Phenytoin promotes collagen deposition and activity of matrix metalloproteinases in ventricle in rat after experimental myocardial infarction

Objective To investigate the mechanisms of phenytoin accelerating healing process.Methods Adult male Wistar rats that survived ligatation of the left coronary artery were randomized to phenytoin group or operation control and compared to sham-operated rats.The time-dependent proteolytic activity of MMP-2 and MMP-9 were detected by gelatin zymography.Picrosirius red staining plus polarized light micrscopy was used for qualitative and quantitative analysis of collagen include collagen volume fraction(CVF) and ratio of typeⅠ/Ⅲ collagen.In addition,infarct thickness and myocyte cross-sectional area were also evaluated by image analysis.Results Fourteen days after myocardial infartion(MI),phenytoin reduced infarct wall thinning.Compared with control group,the rats received phenytoin had less myocyte cross-sectional area.Two weeks after MI,CVF in two groups both had significantly dynamic increase and phenytoin accelerated the change.In contrast to control group,ratio of typeⅠto type Ⅲ collagen in phenytoin increased more quickly.Apart from these results,phenytoin did little to CVF in non-infarcted region.Analysis of MMPs activity in myocardial extracts by zymography demonstrated that infarction-induced expression of proMMPs and active MMPs was both upregulated in phenytoin group and operation control.We found that after MI,MMP-9 activity increased as early as 1 day and reached a maximum then gradually descended,whereas MMP-2 began to increase rapidly and remain elevated for up to 14 days thereafter.Phenytoin seemed toenhance expression of MMP-2 and MMP-9.1 day after MI,active MMP-9 in phenytoin group expressed an increasing trendency compared to MI control.Conclusion Phenytoin can attenuate the degree of post-infarction left ventricular dilation and expansion of the infarct during the early phase of MI healing.MMP-2 and MMP-9 enhanced by phenytoin probably played a prominent role.
6.Impact of Ureaplasma urealyticum and Chlamydia trachomatis on Semen Quality

OBJECTIVE To investigate the impact on semen quality of Ureaplasma urealyticum(Uu) and Chlamydia trachomatis(Ct) infections.METHODS Uu and Ct in semen were detected and the samples of semen were analyzed for 228 cases in infertiliry group and 132 cases in fertility group,The semen quantity,sperm density,survival rate,vitality,liquefacient duration,normal morphologic rate,deformity rate and death rate were compared.RESULTS Among 228 cases in infertility group,173 were positive in Uu and Ct(75.9%),obviously higher than that of fertile males(28.0%).Sperm density,vitality,survival rate and normal morphologic rate in the infertility group were remarkably lower than those of the fertility group(P0.05).CONCLUSIONS The genital infection of Uu and Ct has its impact on the multiple parameters of semen,and semen quality as a whole is decreasing.

9.Effects of Gensenosides Rb1 on proliferation of human acute promyelocytic leukemia (HL60) cells and expression of glucocorticoid receptor ?

AIM: To investigate the effect of Ginsenoside Rb1(G-Rb1) on transcription of glucocorticoid receptor ? (GR?) mRNA and expression of its protein in a acute promyelocytic leukemia cell line. METHODS: Antiproliferation effects of G-Rb1 on HL60 cells were determined by MTT. The transcription of GR mRNA and expression of GR were analyzed by reverse transcription-polymerase chain reaction (RT-PCR) and Western blot, respectively. RESULTS: G-Rb1 inhibited the growth of HL-60 cell line in a time-dependent and dose-dependent manner. After 36 hours treatment with 80 mg/L G-Rb1, the expression of GR? mRNA and GR protein increased. CONCLUSION: G-Rb1 exhibits the inhibitory effects on cell growth in HL60 cells, and increases the expression of GR? mRNA and protein. [
10.Study on the Gene Mutation in a Pedigree with Dowling-Meara Type Epidermolysis Bullosa Simplex by PCR-Heteroduplex Analysis

Objective To study the gene mutation in a pedigree with Dowling-Meara type epidermolysis bullosa simplex (DM-EBS). Methods Using the immuno-histochemistory, electron microscopy, PCR-HA and DNA sequencing. Results The cytolysis was observed throughout the lower basal layer, tonofilaments were decreased; using PCR-HA, the K14 gene mutation was detected. By DNA sequencing, we found gene mutation in this pedigree: K14 1A domain: N123R. Conclusion There is K14 1A domain gene mutation in this DM-EBS pedigree.
