1.Expression and its clinical significance of cytokeratin 18 and Fas in nonalcoholic fatty liver disease

Objective To assess the diagnostic value of cytokeratin 18 fragment M30 (CK18-M30) and Fas in patients with nonalcoholic fatty liver disease (NAFLD),especially nonalcoholic steatohepatitis (NASH).Methods Among 58 patients with NAFLD,36 patients with NAFLD received liver biopsy.According to NAFLD activity score (NAS) and liver fibrosis score,patients were divided into NASH group (24 cases) and non-NASH group (12 cases).And at the same period,15 healthy individuals were set as healthy control group.The serum level of CK18 M30 and Fas were measured with enzyme-linked immunosorbant assay (ELISA).Rank sum test was performed to analyze the differences in the level of CK18-M30 and Fas between groups.The diagnostic value of CK18 M30 and Fas were assessed by the receiver operating characteristic (ROC) curves.Results The level of serum CK18-M30 of NAFLD group was significantly higher than that of healthy control group (97.24 U/L (86.06 to 113.12 U/L) vs 78.41 U/L (74.29 to 80.76 U/L),Z=-4.206,P<0.01)).The level of serum CK18-M30 of NASH group was higher than that of non-NASH group (111.06 U/L (94.30 to 142.68 U/L) vs 89.00 U/L (83.56 to 106.50 U/L),Z=-2.233,P<0.05)).The area under the ROC curve (AUC) of CK18-M30 in the diagnosis of NASH was 0.73 (0.56,0.90),and the sensitivity and specificity of CK18-M30 in diagnosis of NASH was 79.2% and 58.3%,respectively.The AUC of Fas in diagnosis of NASH was 0.58 (0.38,0.77),while the sensitivity and specificity of Fas in diagnosis of NASH was 54.2% and 66.7 %.The serum level of Fas increased in FAFLD group compared with healthy control group,and in NASH group compared with non-NASH group,however the differences were not signifincant (both P> 0.05).Conclusions The level of CK18-M30 has certain value in the diagnosis of NASH.The diagnostic value of Fas in NASH needs more samples in further study.
2.Human Papillomavirus Infection and Increased Risk of HIV Acquisition:A Meta-analysis of Case-control Studies

Objective To evaluate the impact of human papillomavirus(HPV)infection on the risk of HIV acquisition in the female group.Methods Searched PubMed,EMbase,Coehrane Library,CNKI,wanfang database and Chinese Biomedical Lit-erature Database etc for articles about HIV-acquisition in HPV infected female patients.The quality of the literature were e-valuated according to standards of inclusion and exclusion.Data was extracted and methodologically quality evaluated by two independent investigators.Meta-analysis was accomplished using RevMan4.2 software.Results 6 articles were included,all of them were randomized controlled trials,a total of 9 606 cases studied.HIV infection risk was doubled in women with any HPV infection(OR= 2.02,95% CI:1.48 ~ 2.77),demonstrating High-risk HPV-positive (OR= 2.50,95% CI:1.73 ~3.61)and low-risk HPV-positive (OR=2.10,95% CI:1.48~2.96)respectively.Funnel plot analysis revealed no signifi-cant publication bias on HPV genotypes.Conclusion The analysis of selected research suggests the risk of HIV infection was increased in HPV-positive women.HPV vaccine may have some preventive effect on HIV infection.
3.Analysis of serum chemokines in patients with gastric cancer by cytometric bead array and its clinical significance

Objective To explore the levels of serum chemokine in the patients with gastric cancer using cytometric beads array (CBA) and to find out the laboratory evidence for gastric cancer immunotberapy.Methods Forty-five patients with gastric cancer,thirty patients with benign gastric diseases and forty healthy controls were included.The chemokine levels of interleukin-8 (IL-8),regulated upon activation normal T-cell expressed and secreted (RANTES),monokine induced by IFN-γ (MIG),monocyte ehemoattractant protein-1(MCP-1) and interferon-γ induced protein-10 (IP-10) in serum of these three groups were measured using CBA ,then these data were analyzed using BD CBA analysis software.Results The levels of serum chemokines in healthy controls were (176.4±20.7) μg/L for IP-10,(111.3±17.2) μg/L for MCP-1,(503.9±47.2) μg/L for MIG,(472.4±116.7) μg/L for IL-8,respectively.In gastric cancer patients,the levels were (266.6±24.7) μg/L for IP-10,(100.4,70.8-193.5) μg/L for MCP-1,(1614±275.4) μg/L for MIG,(500.0±164.8) μg/L for IL-8.The levels of serum chemokines in patients with benign gastric disease were (207.9±31.7) μg/L for IP-10,(121.2±23.6) μg/L for MCP-1,(514.5±63.0) μg/L for MIG,(480.2±134.8) μg/L for IL-8,respectively.The levels of IP-10 and MIG among these three groups were significantly different (IP-10:F = 3.52,P < 0.05,MIG:F = 9.27,P < 0.01).The levels of IP-10 were elevated in the cancer patients compared with healthy controls (P < 0.05),while the levels of MIG were elevated significantly in the cancer patients compared with healthy controls and benign disease controls(t=3.29,P < 0.01,t=2.84,P<0.01).Conclusions CBA is a novel method with convenience,sensitivity and accuracy for the detection of serum ehemokines.The concentration of IP-10 and MIG in gastric cancer patients are elevated,indicating its implication in cancer pathogenesis and metastasis.Measurement of serum chemokines will lay the groundwork for therapeutic strategy in gastric cancer by inhibiting and anti-chemokine.
4.The Moderating and Mediating Roles of Psychological Capital in the Associations of Effort-reward Imbalance and Overcommitment with Job Burnout among Iron-steel Workers

Objective To explore the moderating and mediating roles of psychological capital in the associations of effort?reward imbalance and overcommitment with job burnout among iron?steel workers. Methods Totally 1 700 male iron?steel workers from an iron and steel company were enrolled for the study. A set of questionnaires that included demographic and working factors,the Maslach Burnout Inventory?General Survey,the Ef?fort?reward Imbalance Scale and the Psychological Capital Questionnaire,as well as perceived work environment was distributed to the subjects. A to?tal of 1 254 effective respondents(73.76%)were collected. Hierarchical linear regression analysis was performed to explore moderating role,and as?ymptotic and resampling strategies were used to detect mediating role. Results There was no moderating role of psychological capital on the associa?tion between effort?reward imbalance and job burnout. Psychological capital had significant and positive moderating roles on the associations of over?commitment with emotional exhaustion and cynicism. Psychological capital showed significant mediating role in the associations of effort?reward im?balance with emotional exhaustion and cynicism,and the proportion of mediating role was 5.84%and 18.75%,respectively. Conclusion In work?ers with high level of psychological capital,emotional exhaustion and cynicism would be significantly increased with overcommitment,and there is a weak increasing trend of emotional exhaustion and cynicism in workers with low level of psychological capital. Effort?reward imbalance could lead to emotional exhaustion and cynicism through the reduction of psychological capital of iron?steel workers.
5.Expression of galectin-10 in CD4~+ CD25~+ CD127~(low/-) regulatory T cells

Objective To investigate the expression of galectin-10 in CD4+CD25+CD127low/-regulatory T cells(Treg) and the correlation between expressions of galectin-10 and Foxp3.Methods CD4+CD25+CD127low/-Treg cells and CD4+CD25-CD127+ T cells were isolated from PBMC of healthy volunteers and lymph nodes of gastric cancer patients by flow cytometry.Following RNA micro-extraction RT-PCR analysis was used to detect mRNA expression of galectin-10 and Foxp3.Results The mRNA expression of galectin-10 and Foxp3 were dominant in Treg cells,whereas nearly no expression in CD4+CD25-CD127+ T cells.The results from PBMC of healthy volunteers and lymph nodes of gastric cancer patients were consistent between.Conclusion The expression of galectin-10 was obviously high in Treg cells,so it may become a novel marker for phenotypic characterization of Treg cells.
6.Clinical research of damage control theory for the treatment of high-energy Pilon fracture

Objective To investigate the feasibility and clinical effect of damage control theory for patients with high?energy Pilon fracture?Methods Fifty?three cases patients with high?energy Pilon fracture were selected in the Traditional Chinese Medical Hospital of Foshan from March 2013 to June 2014 as observation group ( DCO group ) , 46 cases of non?DCO guidance high?energy Pilon fracture as the control group?Intraoperative blood loss,operative time,hospital stay,complications,fractures and function were compared between two groups?Results After the treatments,the intraoperative blood loss,operative time in the DCO group were significantly better than those in the non?DCO group((88?79±4?96) ml vs?(117?74±6?74) ml,(67?44 ±4?75) min vs?(81?43±3?66) min),theses differences were statistically significant between the two groups(t=3?720,5?601,P<0?05)?The complications in the DCO group were significantly less than those in the non?DCO group(22?6%(12/53) vs?52?2%(24/46)),the difference was statistically significant between the two groups(χ2=5?013,P=0?014)?The excellent rate of in the DCO group were significantly better than those in the non?DCO group( 64?15%( 34/53) vs?41?30%( 19/46) ) ,the difference was statistically significant between the two groups (χ2 =6?097, P=0?000 )?Conclusion Reasonable application of the DCT can be quickly and effectively to save the patient's life and improve stability fracture,and reduce the incidence of complications?It is a safe and effective method.

8.Diagnosis and treatment of abdominal compartment syndrome secondary to fulminant acute pancreatitis

Objective To study the treatment of abdominal compartment syndrome(ACS) secondary to fulminant acute pancreatitis (FAP).Method A retrospective study was conducted from Jan.2009 to Dec.2011 to analyze the therapeutic results of 18 patients with ACS secondary to FAP.Results Three out of 7 patients who received non-operative therapy died (mortality rate 42.9%).Three out of 11 patients treated with early surgery died (mortality rate 27.3%),which was significantly better than those patients treated conservatively.Conclusion A reduction in intra-abdominal pressure improved the function of the viscera in the treatment of ACS secondary to FAP.The decompressive effects of early surgery was efficacious,and it reduced the mortality rate.
9.TCM diagnostic method flow reconstruction and optimized management in the mass injuries emergency previewing reception

ObjectiveTo study the effect of TCM diagnostic method flow reconstruction and optimized management in the mass injuries emergency previewing reception.MethodsWorking mode of TCM optimized receiving and dispatching flow were established and enforced.Results27 batch of 216 cases were received by emergency previewing reception in 2009~2010.Compared to 2009,patient dispatching time was shorten by 3.0 min,area dispatching time was shorten by 2.9 min in average.Emergency treatment satisfaction was elevated from 65% to 95% and rescue successful rate was increased from 82.3% to 89.6%.ConclusionTCM diagnostic method flow reconstruction and optimized management improved the level of management of in the mass injuries emergency previewing reception and obtained better results.
10.Duodenum-preserving pancreatic head resection (DPPHR) in treating patients with benign lesions in the head of the pancreas

Objective To evaluate the procedure and the therapeutic efficacy of duodenumpreserving pancreatic head resection (DPPHR) in treating benign lesions in the head of the pancreas.Methods From February 2003 to August 2011,DPPHR was performed in 21 patients with benign lesions in the head of the pancreas.Thirteen patients were male and eight were female.Age ranged from 30 to 48 years,and the lesions ranged from 2.0 cm to 5.6 cm in diameter.Posterior superior pancreaticoduodenal artery was conserved in all patients.Results There was no hospital death.Pancreatic fistula was the main and the most often morbidity,occurring in 33.3%.After operation all patients with preoperative abdominal pain were completely pain free.Preoperative hypoglycemia in 2 patients turned to eugycemia.There was no recurrences report during following up.Concl~ions DPPHR was safe and effective in treating benign lesion in the head of pancreas.It was important in preserving blood supply to the duodenum by posterior superior pancreaticoduodenal artery.
