Early stage Hodgkin' s lymphoma (HL) is highly curable with abbreviated chemotherapy plus involved-filed radiotherapy or chemotherapy alone, but the role of radiation remains the subject of debate and disagreement. The management of recurrent or refractory HL remains challenging with limited effective treatments except high dose therapy with autologous stem cell transplantation.Emerging new safe and effective drugs such as brentuximab vedotin might promisingly improve the outcome of these patients in the future. The interim positron emission tomography (PET-i) scan has important prognostic value in patients with early and advanced stage HL,especially PET scan after 2 cycles. However, the PET-i guided treatment decisions are not currently recommended outside clinical trials.

The role of consolidating radiation therapy (RT) is very controversial in Hodgkin lymphoma (HL),especially in adolescents and young adults (AYA) HL.The key problem is how to achieve better therapeutic effect but bear less toxicities in chemotherapy and radiotherapy, which will be hopefully solved by clinical trials adopting interim positron emission tomography (iPET) scanning to guide therapy for HL.The new technologies including whole genome amplification (WGA),high-throughput gene sequencing (NGS) assay and genome-wide association study (GWAS) further illustrate abnormal signaling pathways in HL,such as NF-κB,JAK/STAT and PI3K pathway, which may provide new therapeutic targets for the disease.

Hodgkin's lymphoma(HL)is a highly curable malignancy for most patients during the recent decades.Frontline treatment is generally ABVD alone or in combination with other chemotherapy regimens or radiation. To achieve a higher control of disease, ABVD chemotherapy has been challenged by more intensive treatments, such as Stanford V and BEACOPP regimen. In the 53rd ASH annual meeting,ABVD is still widely accepted as standard regimen for HL, even in elderly people or patients with HIV infection. Relapsed or refractory HL is a challenging problem for clinician, and high dose chemotherapy followed by autologous stem cell transplantation(HDC/ASCT)is the preferred treatment for chemoresistant patients. Many novel treatment strategies were explored for these patients.Positron emission tomography (PET) scanning provides important prognostic information in patients with HL receiving chemotherapy or pre/posttransplant,but the utility of long term surveillance radiological studies is not recommended in the meeting.
5.Experimental Study on Antitussive, Expectorant and Antiasthmatic Effects of Fortunella Hindsii Leaves
China Pharmacist 2017;20(2):256-258
Objective: To study the antitussive, expectorant and antiasthetic effects of Fortunella hindsii leaves. Methods: The antitussive effects were observed by the method of ammonia-induced cough in mice, the expectorant effects were observed by the method of phenolsulfonphthalein excretion in mice, and the antiasthmatic effects were observed by the method of histamine phosphate spray in guinea pigs. Results:The leaves of Fortunella hindsii at low, medium and high doses could decrease the cough times and prolong the cough latent period (P<0. 05 or P<0. 01), high dose could promote the phenol red excretion (P<0. 05), and high and medium do-ses could prolong the incubation period of guinea pigs for asthma caused by histamine phosphate (P<0. 05 or P<0. 01). Conclu-sion:The leaves of Fortunella hindsii have promising antitussive, expectorant and antiasthmatic effects.

Objective To evaluate the clinical application of thoracoscope mediastinal tumor resection with the laryngeal mask anesthesia. Methods 40 cases of mediastinal tumors were randomly divided into two groups from 2012 to 2015, 20 cases in each group. Laryngeal mask group performed thoracoscope mediastinal tumor resection with laryngeal mask anesthesia and the Intubation group performed thoracoscope mediastinal tumor resection dou-ble-lume nendo tracheal intubation anesthesia, then compare the overall effect. Results All the patients were suc-cessfully completed with the thoracoscope surgery, no transfer to open chest cases. The preoperative anesthesia time of laryngeal mask group was shorter, blood pressure and pulse range of wave was smaller in the process of anesthesia and surgery, move restlessly was less, postoperative recovery time was shorter, and lower incidence of postoperative pharynx ministry unwell and sore throat, so the laryngeal mask group shows statistically significance comparing with the intubation group (P < 0.05); Operative time, operative field exposure satisfaction, intraoperative blood loss, post-operative extubation time and hospitalization days have no statistical significance (P >0.05). Conclusion The laryn-geal mask airway intravenous anesthesia in thoracoscope mediastinal tumor resection in practical good, in the preop-erative anesthesia and postoperative recovery time and postoperative sore throat was better than that of intubation group, so should be extended in clinical treatment.

Emergency in public health is one major issue relating to the national economy and the people's livelihood. In dealing with the key technologies of detects, diagnosis, disinfection, prevention and control,it is wise to syncretize the correlated subjects in nucleus, chemistry and biology, improve the support condition of teaching, strengthen the reform of teaching contents, add some lessons of emergency treatment, make emergency equipments, and form the ability of rapid diagnosis on the spot and precise identifying in lab,for cultivating the students'capability of spot handling, organization and command, and furthermore exploring and practicing the training pattern of the public heath talent with high quality and emergency ability in our country.

Objective To investigate the application value of prophylactic bilateral uterine artery catheterization and embolization in treatment of patients with pernicious placenta previa and accreta. Methods Data of 16 patients with pernicious placenta previa and accreta who underwent prophylactic bilateral uterine artery catheterization and uterine artery embolization and cesarean section were retrospectively analyzed. The amount of intraoperation blood loss, the amount of blood transfusion, cesarean hysterectomy rate, fluoroscopy operative time, radiation dose, complications and neonatal outcome were recorded. Results The technical success rate of the combined treatment was 93.75% (15/16).The mean amount of blood loss was (1 575.00 ± 1 040.83)ml.The mean amount of blood transfusion was (3.44 ± 2.34)U leukoreduced red blood cells. The mean fluoroscopy operative time and radiation dose before delivery were (0.89±0.24)min and (7.17 ± 2.12)mGy.One newborn was diagnosed as severe asphyxia. The mean Apgar score of another 15 newborns was (9.38± 0.89) at 5 min after birth. One patient underwent hysterectomy due to postpartum hemorrhage complicated with disseminated intravascular coagulation.Buttock pain was found in 2 cases. Conclusion Prophylactic bilateral uterine artery catheterization and embolization can be used for the treatment of pernicious placenta previa and accreta. The combined treatment can control postpartum hemorrhage during placental dissection and reduce transfusion requirements and hysterectomy rate with few minor complications and low radiation exposure dose.
