4.Association between insulin resistance and coronary artery disease

Objective To study the effect of insulin resistance on the development of coronary artery disease ( CAD). Methods In total, 204 nondiabetic inpatients diagnosed by elective coronary angiography were selected in the study and divided into CAD and control groups. Relationship between insulin resistance assessed by homeostasis model ( HOMA-IR) and fasting serum level of insulin and occurrence of CAD and the extent of stenosis of coronary arteries was analyzed. 5.Phage display of human CD137 extra-cellular domain and its bioactivity detect

Objective:To display human CD137 extra-cellular domain on phage using phage display technique and detect its antigenicity and bioactivity.Methods:CD137 extra-cellular gene was amplified from CIS plasmid using PCR and then clone it into pComb3HSS.The recombinant pComb3H-CD137 was transfected into competent bacteria XL1-Blue, then the transfected bacteria was co-infected with help phage VCSM13 and CD137 extra-cellular domain was displayed on the phage surface. The antigenicity and bioactivity of CD137 displayed on phage was detected by ELISA and inhibition assay of lymphocyte proliferation(MTT).Results:CD137 PCR products was 525 bp, the recombined plasmid was named pComb3H-CD137, CD137 was displayed on phage. ELISA detection showed that phage displayed CD137 possesses antigenicity of human CD137. MTT inhibition assay showed that lymphocytes proliferation stimulated by PHA and CD137 could be inhibited by phage displayed CD137, indicating that it has bioactivity of nature CD137.Conclusion:The studies demonstrate that phage display system can display human CD137 extra-cellular domain homodimer with antigenicity and bioactivity.
6.Clinical distribution and antimicrobial resistance of 172 Proteus mirabilis isolates and 68 Proteus vulgaris isolates

Objective To analyze strains of Proteus mirabilis (P .mirabilis )and Proteus vulgaris (P .vulgaris ) isolated in a hospital,detect resistance to commonly used antimicrobial agents,and provide reference for rational ap-plication of antimicrobial agents in clinic.Methods 172 P .mirabilis isolates and 68 P .vulgaris isolates isolated between January 1 ,2011 and June 30,2013 were analyzed,antimicrobial resistance susceptibility testing were per-formed by disk diffusion method,data were analyzed with WHONET 5.4 software.Results P .mirabilis strains were mainly isolated from wound secretion(26.74%),sputum(22.68%)and urine(18.61 %),P .vulgaris were mainly from wound secretion(48.53%),urine(17.65%)and sputum(11 .77%).The resistance rates of P .mirabilis to ampicillin and trimethoprim/sulfamethoxazole were both>45.00%;the resistance rates of P .vulgaris to cefazo-lin and trimethoprim/sulfamethoxazole was 86.76% and 41 .18% respectively;the resistance rates of P .mirabilis and P .vulgaris to piperacillin/tazobactam,cefotaxime,ceftazidime,cefepime,carbapenems (ertapenem and mero-penem)and amikacin were all <20.00%.Conclusion The resistance rates of P .mirabilis and P .vulgaris to pip-eracillin/tazobactam,cefotaxime,ceftazidime,cefepime,ertapenem,meropenem and amikacin are all high,and can be used as the empirical medication for the treatment of related infection.
7.The clinical analysis of primary central nervous system lymphoma in 23 patients

ObjectiveTo explore the clinical characteristics and the relationship between rational therapy and prognosis in primary central nervous system lymphoma (PCNSL).MethodsClinical data of 23 patients pathologically confirmed as PCNSL and treated in our hospital from January 2005 to December 2007 were collected and analyzed retrospectively.SPSS 13.0 statistical analysis software was used to analyze the correlation between therapy,clinical characteristics and prognosis.Among the 23 patient,10 were male and 13 were female,with a median age of 50 (2-75) years old.Eighteen patients were undertaken lumbar puncture,and tumor cells in cerebral spinal fluid (CSF) was found in 4 patients.Tumorectomy was performed in 8 patient,while 15 patients biopsy.Among them,4 accepted whole brain irradiation,6 accepted associated chemotherapy based on a high-dose of methotrexate (MTX)and 13 accepted radiotherapy combined with chemotherapy.Results Kaplan-Meier analysis showed that in this serial patients,the median survival time was 45.0 months and the 3-year survival rate was 56.5%.Log-Rank test revealed that the whole survival time and the non-progression survival time of radiotherapy combined with chemotherapy (43.8 and 30.0 months,respectively) and high-dose MTX chemotherapy ( 39.7 and 29.7 months,respectively )were much longer than those of radiotherapy alone (25.7 and 19.8 months,respectively,all P < 0.05 ).Log-Rank test showed no significant difference between high-dose MTX chemotherapy alone and radiotherapy combined with chemotherapy (P >0.05 ),but the whole survival time was much shorter in high-dose MTX chemotherapy ( P < 0.05 ).ConclusionsThe detection of tumor cells in CSF can confirm the diagnosis,but pathological biopsy is the gold standard.The efficacy of associated chemotherapy based on a high-dose of MTX combined with radiotherapy is much better than radiotherapy or chemotherapy alone.
9.Value of FDG PET-CT in outcome assessment and prognostic evaluation for recurrent nasopharyngeal carcinoma

Objective To investigate the value of FDG PET-CT in the outcome assessment and prognostic evaluation for recurrent nasopharyngeal carcinoma (rNPC).Methods From January 2008 to December 2013,92 rNPC patients were treated in our center,who were histologically or radiologically diagnosed and re-staged according to the 2008 clinical staging system for nasopharyngeal carcinoma in China.The numbers of patients in stage Ⅰ,stage Ⅱ,stage Ⅲ,and stage Ⅳ were 8,11,39,and 34,respectively.According to the recurrent T stage (rT),the numbers of patients in rT1,rT2,rT3,and rT4 were 10,11,38,and 33,respectively.Twenty-eight patients had recurrence in the neck lymph nodes.All patients underwent pretreatment FDG PET-CT for the whole body or head/neck,and treated by radiotherapy with or without chemotherapy.The relationship of maximum standard uptake value (SUVmax) and clinical factors with clinical outcomes was analyzed.The Kaplan-Meier method was used to calculate overall survival (OS),disease-free survival (DFS),local recurrence-free survival (LRFS),regional recurrence-free survival (RRFS),and distant metastasis-free survival (DMFS).The log-rank test was used for survival difference analysis and univariate prognostic analysis.The Cox model was used for multivariate prognostic analysis.Results The 3-year OS,DFS,LRFS,RRFS,and DMFS were 33.6%,32.1%,32.8%,31.8%,and 33.7%,respectively.The median SUVmax was 8.35 (2.7-21.5).The SUVmax of 7.0 was taken as the optimal cut-off value for all patients.Patients with SUVmax ≤7.0 had a significantly higher 3-year OS rate than those with SUVm ax >7.0 (42.0% vs.28.3%,P=0.019).The univariate analysis revealed that patient age,SUV and rN were significantly associated with OS (P=0.023,0.019,and 0.002).The multivariate analysis showed that SUVmax and rN were significant influencing factors for OS,DFS,and DMFS (HR=1.68,P=0.045 and HR=2.23,P=0.003;HR=1.67,P=0.042 and HR=2.39,P=0.001;HR=1.77,P=0.025 and HR=2.40,P=0.001).Conclusions SUVmax may be one of the useful prognostic factors for OS,DFS,and DMFS in rNPC patients.
10.Study of characteristics of three dimensional motion of cervical spine during maximal axial rotation

Objective To determine the three dimensional motion data of each segment of cervical vertebrae and analyze the characteristics of the intervertebral coupled motion during cervical axial rotation under physiological weight bearing. Methods A total of 16 healthy volunteers (ranging from 22 to 29, median age, 23 years) were recruited to our study. Any cervical spine disorder history, pain or other discomfort and malformations were excluded so as to avoid abnormal neck motion. These subjects underwent CT scans of their cervical segments in a supine position, and 3D models of C1-C7 were constructed. Next, each subject was asked to sit up straight and was positioned in the following sequence:maximal left and right twisting, while double oblique images by DFIS were taken simultaneously at each of the positions. Then, the CT models were matched to the osseous outlines of the images from the two oblique views to quantify the position of cervical vertebraes in 3D at each position. Through local coordinate systems at the center of vertebral bodies, changes of position and angle of each cephalad vertebrae relative to the cauddal one were calculated before and after the axial rotation. Results (1) In the axial rotation of the cervical spine, the contribution of C1/2 accounted for the most of the total cervical rotation range. For the lower levels, axial rotation was found to be maximal at C3/4 and C5/6, minimal at C2/3. (2) In cervical axial motion, C1/2 demonstrated a coupled lateral bending opposite to the axial rotation direction, while each segment of C2-7 demonstrated coupled lateral bending towards the same side of the axial rotation. Among these segments the lateral bending angle of C2/3 was smaller than angles of C3/4, C4/5 and C5/6. Conclusion This study investigated the cervical coupling behavior using the noninvasive 2D-3D matching technique and obtained the motion data at each cervical spinal segment. These findings will help to improve the understanding on physiological cervical spine movement and potential biomechanical mechanism and treatment of cervical spondylosis. Also our data may provide useful reference for the prosthesis design.
