4.Analysis of 3 cases misdiagnosised with myasthenia gravis

The clinical data of 3 cases with myasthenia in Children′s Hospital of Fudan University were retrospectively analyzed.All the 3 patients were diagnosed with myasthenia gravis and given pyridostigmine bromide, but the treatment effect was unsatisfactory.One patient had left lower limb weakness and motor development impairment, and was diagnosed with centronuclear myopathy by gene testing.One patient had blepharoptosis, accompanied by developmental delay, facial deformity, and toe deformity.This patient was diagnosed with chromosome 18p syndrome by chromosome analysis.One patient had weakness of both legs, slight development retardation, and significantly high lactic acid levels.Basal nuclei lesions were observed under head magnetic resonance imaging (MRI). Whole exome sequencing (WES) and mitochondrial genome testing results revealed Leigh syndrome.Through summing up experience and lessons, strengthen and improve the clinical identification ability of clinicians for myasthenia gravis, and avoid misdiagnosis.
5.Decellularized whole liver bioscaffold repairs liver injury

BACKGROUND:Decel ularized scaffolds are special for retaining the tubular structure used for nutrition transport, and providing a similar inner environment for cel growth. OBJECTIVE:To study the preparation of the decel ularized whole liver bioscaffold and to explore its repair outcomes for liver injury. METHODS:Livers from 12 Sprague-Dawley rats were used for preparing the decel ularized whole liver bioscaffold by chemical detergent-enzymes decel ularized technology. Models of liver injury were established in another 24 Sprague-Dawley rats and randomized into two groups:the decel ularized whole liver bioscaffold was implanted into the rat liver lesions in experimental group, and controls were given the injection of normal saline. Thirty days later, the serum levels of alanine aminotransferase and glutamic-oxaloacetic transaminase were detected, and liver tissues were removed for hematoxylin-eosin staining. RESULTS AND CONCLUSION:Hematoxylin-eosin staining showed that extracel ular matrix-like structures existed in the decel ularized bioscaffold;cel components were completely removed from the liver, the col agen fibers in the scaffold arranged regularly and were not dissolved under electron microscope. The serum levels of alanine aminotransferase and glutamic-oxaloacetic transaminase in the experimental group were significantly lower than those in the control group (P<0.05). Hematoxylin-eosin staining showed a large number of blue-stained and dense distributed nuclei, and pink distribution of col agen fibers that had no overt breakages in the control group, while pink and dense structures in the experimental group. These results suggest that the decel ularized whole liver bioscaffold is easy to obtain, and can promote the injured liver repair.
6.Repeated CT scan in improving the reproducibility of grass tumor volume for moving target

Objective To find a method to improve the range accuracy of moving target such as peripheral lung tumors, since a single CT snapshot may not be accurate during the treatment process.Methods A simple harmonic motion phantom, embedded with a cube and a circular ball, was used to simulate the tumor motion. Individualized moving targets were scanned 24 times with different amplitudes and frequencies. Then the images were fused from every 1, 2 or 3 sets of CT scans. The GTV volume variation of circular target and the length variation of the cube target along the z axis were contoured and analyzed. Results As motion amplitude increased, the maximum of both circular target volume and cube target length was increased, while the minimum of the factors was decreased. Motion frequency affected the target volume less than amplitude. For a cube target with the length of 3.3 cm at stationary phase, when motion frequencies was 20 and motion amplitude was 2 cm, the maximal length was 2. 4 times of the minimal length (5. 1 cm vs. 2. 1 cm). When it came to the cube target groups fused from every 1,2 and 3 sets of CT scans, the average length and standard deviation were (3.77 ± 1.20) cm, (4.18 ±0. 91)cm and (4.52 ±0. 59) cm, respectively. With the increase of fused scan number, targets became bigger, the standard deviation decreased, and the change of center positions was decreased. Conclusions The motion amplitude, frequency and the number of CT scans are the main factors affecting target definition, though, the optimized scanning phase is not certained. When 4DCT and respiration gating technique are not available,the efficient and practical method to solve this problem is to scan the target three or more times and fuse them in planning system, which will generate a larger, more reproducible GTV volume for moving targets.
7.Application of an image navigation system in nasal endoscopic surgery in treatment of basicranial tumor.

OBJECTIVE:
To investigate the therapeutic effects of the image navigation system in nasal endoscopic surgery in treatment of basicranial tumor.
METHOD:
Retrospective analysis was carried out in 17 patients with basicranial tumor who underwent nasal endoscopic resection via image navigation system. The accuracy of image navigation system, operation procedure time, curative effect and complications were analyzed and summarized to evaluate the effect.
RESULT:
The image navigation system provided an accurate anatomical localization 0.5-2.0 mm localization error (mean 1.2 mm). The operation procedure time was 1.5-3.0 h. Postoperative complications occurred in 3 of 17 patients including cerebrospinal fluid rhinorrhea and injury of sieve plate. Two patients recrudesce within half a year.
CONCLUSION
The image navigation system in combination with a nasal endoscope is helpful in the resection of basicranial tumor with minimal trauma, increasing the curative effect and avoiding complications.

9.Pathological study of rabbit femoral after high-intensity focused ultrasound irradiation in vivo

Objective To inquire into the hurt-efforts to the rabbit femoral in vivo irradiated by high intensity focused ultrasound(HIFU). Methods Using a HIFU with the operating frequency of 0.8 MHz; the focal distance of 135 mm and the sound-intensity of 8000 W/cm 2. The average interal diameter of the rabbit femoral artery was about 1.1 mm.Thirty rabbits were divided into 3 groups randomly and unilateral-femoral was chosen at random. To observe by using the method of pathology and victorian blue of elastic fibre. Results After irradiation all the endothetial cells of blood vessels of the rabbit peeled off,disappeared and tiny thrombuses were formed in the vessels the core of smooth muscles withered, cell disorder appeared. Conclusions Irradiation by HIFU could cause injury to the rabbit femoral artery, change blood dynamics and produced contribution to the further research on the energy accumulation of the targets in HIFU system.
10.Research progress of DcR3 in common malignant tumor tissues of digestive system

Decoy receptor 3 (DcR3) is a newly discovered member of the tumor necrosis factor receptor (TNFR) superfamily, expresses highly in the digestive system neoplasms. DcR3 inhibits cellular apoptosis and produces cellular modulation by competitively combining with FasL, LIGHT and TL1A, therefore it deeply relates to the digestive system neophlasm's generation and progression. DcR3 is supposed to be the new tumor-specific marker that may cast the light to the digestive system neoplasms' generation, diagnosis,treatment, prognosis and effect observation. DcR3 is expected to open a new chapter in clinical application.
