This paper is to explore the role of philosophy in Book of Changes in constructing harmonious relationship between nurses and patients,proposing that understanding and trust are fundamental and sincerity is the premise for the construction of a harmonious relationship between nurses and patients;the nurses should hold kindheartedness,kindness and discretion in mind and should be self-disciplined by holding the sense of propriety for the achievement of a friendly relationship.
2.Effect of Electroacupuncture on Expression of Ghrelin and mRNA Expression of Its Receptor in Functional Dyspesia Rats.

OBJECTIVETo explore the effect of electroacupuncture (EA) on the expression of Ghrelin and mRNA expression of its receptor in functional dyspepsia (FD) rats.
METHODSTotally 80 rats were divided into the normal group, the model group, the drug therapy group, and the EA group according to random digit table, 20 in each group. FD model was duplicated by clipping tail modeling. Drug containing cisapride [2 mL/100 g, 0.09 g/(kg x d)] was administered to rats in the drug therapy group from the 3rd day after successful modeling, once per day. EA at Zusanli (ST36) (0.3-0.5 cun) and Taichong (LR3) (0.1-0.2 cun) was performed in the EA group. The twirling of needle was performed to the subsidence of needle, and then the needle was connected to HANS-200A Acupoint Nerve Stimulating Device using disperse-dense wave at 2 Hz, 2 mA, 30 min each time, once per day. Six days consisted of one therapeutic course, two courses in total with an interval of one day. The intestinal propulsive rate of ink was observed. Ghrelin protein expression in gastric tissue was detected by Western blot. mRNA expression of growth hormone secretagogue receptor (GHS-R) in stomach, hypothalamus, and hippocampus was detected using Real-time PCR respectively.
RESULTSCompared with the normal group, the intestinal propulsive rate of ink, Ghrelin protein expression in gastric tissue, mRNA expression of GHS-R in stomach, hypothalamus, and hippocampus decreased in the model group (P < 0.05, P < 0.01). Compared with the model group, the intestinal propulsive rate of ink, Ghrelin protein expression in gastric tissue, mRNA expression of GHS-R in stomach, hypothalamus, and hippocampus increased in the EA group (P < 0.01); mRNA expression of GHS-R in stomach, hypothalamus, and hippocampus increased in the drug therapy group (P < 0.01). Compared with the drug therapy group, Ghrelin protein expression in gastric tissue, mRNA expression of GHS-R in hypothalamus increased in the EA group (P < 0.05, P < 0.01).
CONCLUSIONEA could regulate Ghrelin content and GHS-R mRNA expression of FD rat hypothalamus, hippocampus, and gastric tissue, and promote the intestinal propulsive rate of ink.

4.Common problems and countermeasure of ultrasound-guided modified Seldinger technique for peripherally inserted central catheter placement

Objective To investigate the common problems and countermeasure of using ultrasound-guided modified Seldinger technique in peripherally inserted central catheter (PICC) placement Method The clinical data of 140 patients who underwent PICC placement using ultrasound-guided modified Seldinger technique were retrospectively analyzed. Results The success rate of catheterization was 98. 6% among these 140 patients,with 135 (96. 4% ) succeeded after a single attempt and 5 (3. 6% ) after 2 attempts. Besides, 7. 1% (n = 10) had difficulty in inserting the guide wire, 3. 6% (n =5) had difficult catheteration, 2. 1% (n =3) met resistance when inserting catheter into ipsilateral subclavian vein, 2. 1 % (n = 3 ) experienced dystopia of inserting catheter into internal jugulular vein, and 1.4% (n = 2) experienced the bending of PICC in the superior vena cava and axillary vein. All of these problems were handled successfully. Conclusions The common problems of PICC placement include puncture failure, difficulty in inserting guide wire, and difficult catheteration. A good knowledge of these problems will help increase the success rate.
5.Clinical value of miniature ultrasonic probe on a combination of endoscopic varices ligation and Hassab's procedure in the treatment of esophageal varices

Objective To assess the efficacy and prognosis of a combination of endoscopic varices ligation(EVL) and Hassab procedure in patients with portal hypertension by miniature ultrasonic probe. Methods Twenty-three patients with esophageal varices received a combination of EVL and Hassab procedure for variceal eradication. The esophagus vascular structures were examined with endoscopy and miniature ultrasonic probe. Change of vascular structure was evaluated,recurrence and rebleeding of esophageal varices were investigated.Results After treatment,the esophageal varices were obliterated and collateral veins were diminished in size or obliterated. During a short-term follow-up,the recurrence and rebleeding rate of esophageal varices were 5.0 % and 0,respectively.Conclusions The combination of EVL and Hassab procedure can effectively shut off the portoazygous shunt,prevent esophageal varices bleeding. It is a simple and less cost procedure. The miniature ultrasonic probe plays a more important role in evaluating the treatments of esophageal varices.
6.Construction of eukaryotic expression plasmid of Smad-7 and study on its function as TGF-β1 inhibitor

Objective To construct eukaryctie expression vectors stably expressing Smad-7 and study its function as TGF-β1 inhibi- tor. Methods Smad-7 genes were obtained by PCR from fetal liver cDNA library and inserted in to eukaryotic expression plasmid pcD- NA3. 1/myc-his-B (-). The constructed plasmids containing right sequence of target genes were transfected into human mesangial cells by lipofectamine. The ceils were selected by G418, the expression of Smad-7 was detected by Western-blotting. Cell proliferation was detected by MTT method, and cell cycle was detected by Flow Cytometer to study its function as TGF-β1 inhibitor. Results We successfully con- struct Smad-7 expression plesmid. Smad-7 can inhibit the effects of TGF-[M induced GI cell cycle block and apoptosis. Conclusion The eukaryotic expression vector of Smad-7 has been successfully constructed. Over-expression of exogenous Smad-7 can inhibit the effects of TGF-β1 induced GI cell cycle block and apoptosis.

OBJECTIVE:
To explore the application of the Dexmedetomidine utend drug induced sleep endoscopy.
METHOD:
Forty-four OSAHS patients diagnosed by PSG randomly were divided into group A (Dexmedetomidine group) and group B (Propofol group), each group of 22 cases. Group A: pump the Dexmedetomidine (1 microg/kg) over 15 minutes. Once the patient reached a satisfactory level of sedation, the electronic nasopharyngoscope was introduced into the nasal cavity group B: the propofol (2 mg/kg) was intravenous injected, use the same exmination after the object patients falling asleep. If not, double the injection dose of the two group. Observe and record the patient vital signs, EEG and sleep time, and handle the complications.
RESULT:
The study was successfully completed both in group A and B. Mean arterial pressure (MAP) in group B was lower than group A during the examination significantly (P < 0.05). The time falling to sleep in two groups were (13.4 +/- 2.5) min and (6.6 +/- 1.2) min, and the time in group A was much longer than that in group B. The lowest oxygen saturation during the examination in two groups were 0.835 +/- 0.096 and 0.691 +/- 0.095, and the difference was statistically significant. There was no incidence of adverse reactions in two groups.
CONCLUSION
Compared with propofol, Dexmedetomidine is a safer sleep-inducing drug, and it can be used for clinical sleep endoscopy.

9.Study on the Effect of Jiegeng Loading Drug up in Tianwang Buxin Wan

Objective To study the relation between Jiegeng loading drug up in Tianwang Buxin Wan and brain,heart and lung.Method 32 Wister rats were randomly divided into four groups(8 rats in each group),namely,control group,Jiegeng group,Tianwang Buxin Wan group,Tianwang Buxin Wan lacked Jiegeng group.The levels of cyclic nucleotide in the brain,heart and lung were assayed by radioimmunoassay.Result The levels of cAMP in the lung in every drug group were significantly increased compared with that of control group.It was significant on the level of cAMP in the lung between Tianwang Buxin Wan group and Tianwang Buxin Wan lacked Jiegeng group.Conclusion It was the lung that linked the effect of Jiegeng loading drug up in Tianwang Buxin Wan.
10.Application of Q-Syte separated membrane sealed without pin connector in children with blood diseases

Objective To explore the effect of Q-Syte separated membrane sealed without pin connector in children with blood disease. Methods Using random digit table, the children using Q-Syte separation membrane enclosed needle-free connector and heparin cap were divided into the observation group and the control group with 260 children in each group. The two groups were compared in terms of the incidence of using Q-Syte membrane closure cap pin connector and heparin. Results The result showed a significant positive relation between the two groups (P<0.05). The incidence of using Q-Syte separation membrane sealed without pin connector was lower than that of the control group. Conclusion Q-Syte separation membrane sealed without pin connector can reduce the incidence of infusion, which is easy, safe and worth clinical application.
