Because of the close relationship with stomach cancer and higher rates of carcinoma, gastric epithelial dysplasia associated with chronic atrophic gastritis is considered the most important precancerous lesion of stomach cancer. Due to the absence of recognized treatment methods, periodical follow-up and monitoring were considered an effective method to prevent the occurrence of gastric cancer. Currently, there are numerous areas requiring attention; the authors provide some suggestions on the recognition and key technologies for follow-up and re-examination.

Objective To investigate the value of modified loop ileostomy in laparoscopic low and superlow radical resection of the rectal cancer with reservation of anus. Methods The clinical data of 108 patients who received laparoscopic low and super-low radical resection of rectal cancer with preservation of anus at the Subei Hospital of Jiangsu Province from January 2010 to June 2012 were retrospectively analyzed.Fifty-three patients who received preventive ileostomy were in the ileostomy group, and 55 patients who did not receive preventive ileostomy were in the non-ileostomy group. The operation, incidence of complication and follow-up of the 2 groups were analyzed. All data were analyzed using the t test,chi-square text or Fisher exact test. Results The operation time and blood loss were (185 ± 14)minutes and (31 ± 26)ml in the ileostomy group, and (154 ± 12)minutes and (28 ± 19)ml in the non-ileostomy group, with no significant difference between the 2 groups (t =2.34,1.82,P > 0.05).The time to first flatus, time to liquid diet intake, time to pelvic drainage tube removal, duration of hospital stay, hospital costs were (1.1 ± 0.3) days,(1.1 ± 0.2) days,(5.2 ± 1.0) days,(7.2 ± 1.4) days,(3.0 ± 0.2) ×104 yuan in the ileostomy group, and (4.2 ± 0.6) days,(4.3 ± 0.6) days,(8.4 ± 3.9) days,(12.9 ±4.4)days,(3.8 ±0.7) × 104 yuan in the non-ileostomy group, with significant differences between the 2 groups (t =7.25,28.12,15.34,34.01,7.83,P < 0.05).The incidences of postoperative complications and anastomotic fistula were 5.7% (3/53) and 0 in the ileostomy group, and 21.8% (12/55) and 12.7% (7/55) in the non-ileostomy group, with significant differences between the 2 groups (P < 0.05).Conclusion Modified loop ileostomy in laparoscopic low and super-low radical resection of rectal cancer can effectively reduce the incidence of anastomotic fistula, and helps the recovery of patients.

Objective To explore the value of multivoxel 1H-MRS in the differential diagnosis of corpus callosum lesions. Methods 17 patients with corpus callosum lesions confirmed by clinical or pathological method underwent MRS examination. These patients included 2 cases of lymphoma,4 cases of Marchiafava-Bignami disease,3 cases of glioma, 7 cases of infarction, and 1 case of multiple sclerosis.MRS features of lesions were analyzed. Results 2 cases with corpus callosum lymphoma revealed significantly increased Cho peak, decreased Cr and NAA peak, towering Lip peak.Among 4 cases with Marchiafava-Bignami diseases, 2 cases revealed increased Cho/Cr and decreased NAA/Cr,1 case showed normal,1 case revealed inverted Lac peak. For 3 cases with corpus callosum glioma,NAA peak decreased or disappeared, and Cho peak increased in varying degrees.7 cases with corpus callosum infarction revealed significant inverted Lac peak, and NAA peak decreased in varying degrees.1 case of multiple sclerosis revealed increased Cho peak, decreased NAA peak, and inverted Lac peak during acute period with enhancement, while these features returned normal during inactive period. Conclusion Multivoxel 1H-MRS plays an important role in the differential diagnosis of corpus callosum lesions.

Objective To explore the relationship between the variation of leptin receptor (LR) gene and type 2 diabetes mellitus (DM), obesity and plasma lipid levels. Methods The variation of LR gene exon 20 was detected by polymerase chain reaction (PCR) and restriction fragment length polymorphism (RFLP) and their relations to type 2 DM and obesity were analyzed. Results The variant frequency at 3 057 nucleotide of G to A transversion was 82.09% for total, 87.33% for DM group, 75.42% for control group. The frequency of AA genotype in DM cases was higher than that in controls, but that of GA genotype was lower, and that of GG in cases did not indicate. According to BMI and WHR level, there was no obvious relationship between gene variations and obesity. There were significant differences between AA genotype and GA genotype about the serum levels of TG, HDL and SBP, DBP. Conclusion At nucleotide 3 057 in LR exon 20, a G to A transversion was found, which may be the susceptibility marker of DM in Chongqing population. An allele was associated with high blood lipid and increased blood pressure.

Objective To explore the feasible approach for establishment of a liver cancer model induced by N-nitrosodiethylamine(DENA)with Sprague-Dawley rat,and to provide ideal animal model for imaging diagnosis and interventional therapy. Methods One-hundred and forty male SD rats were administrated with 0.95 g/L DENA for 10 weeks, and MRI was performed for inspecting pathological changes of rat livers on the following week. When the liver tumor was proved then the rat will be the candidate for sequential procedures, otherwise the animal continued under observation until next MRI examination after 4 weeks.DSA was done in 64 rats'for detecting blood supply of liver tumors.Animals were sacrificed with an overdose of chloral hydrate,The representative tumor tissues were fixed in 10% formalin and 2.5% glutaraldehyde for light and electron microscopy analysis respectively.Alpha fetoprotein(AFP) and hepatocyte were assaied by immunohistochemistry technique in order to identify intrinsic trait of the harvested tumors. Results The earliest induced tumor was detected on 11th week and the latest was on 20th week by MRI,and the median period was 13.9 weeks.Tumor size ranged from 2 mm to 40 mm in diameter. The rate of single and multi-induced tumor was 9.7%(7/72)and 90.3%(65/72),respectively.87.7% (57/65)" of the induced multiple tumors was with hepatocirrhosis and 18.1% of these tumors combined with extrahepatic neoplasm or metastasis.Plenty blood supply was proved by DSA in most of those tumors. Tumors not only derived from hepatocyte but also manifested positive expression of AFP.Histological types of these tumors include hepatocellular carcinoma(HCC)(92.0%,66/72),intrahepatic cholangiocarcinoma (ICC)(4.0%,3/72),and combined HCC and ICC(4.0%,3/72),respectively.Electro-microscope analysis indicated that cytoplasm and organelle of induced tumors were abnormal distinctly compared with those of non-carcinomatous cells. Conclusion DENA can induce ideal rat liver cancer as a feasible animal model,MRI is the best approach for scrutinizing pathological changes of rat livers during induced period.

Journal of Huazhong University of Science and Technology (Medical Sciences) 2006;26(5):545-7
The immune mechanism of Graves' diseases (GD) and the roles of regulator T cells were investigated. In 32 patients with GD (GD group) and 20 healthy volunteers (control group), flow cytometry was used to detect the proportion of CD4+CD25+ cells, MACS to isolate CD4+ CD25+ cells, RT-PCR to assay the expression of FOXP3, and ELISA to test the level of IL-10, respectively. It was found that there was no significant change in the proportion of CD4+CD25+ T cells between GD group and control group (P>0.05), while secretion of IL-10 and expression of FOXP3 in GD group were lower than control group (P<0.01 and P<0.05, respectively). In conclusion, though the proportion of regulatory T cells of peripheral blood lymphocytes in the patients with GD, the functions of them were significantly weakened, which might be a pathogenic factor in GD.
