Multiple modality treatment has been adapted for the treatment of various cancers,the outcome has been dramatically improved,but some of unique side effects due to the modalities like chemotherapy combined with radiotherapy have been observed in the clinic. Radiation recall dermatitis(RRD) is one of the side effects. In this paper,the etiology,clinical features,diagnosis,treatment and related factors of the radiation recall dermatitis are discussed.

BACKGROUND: The main difficulties of whole-cell patch clamP are cell culture and whole-call current recording. OBJECTIVE: To reduce the difficulty of patch-clamp experiment by combining acute cell isolation with current separation technique. METHODS: A total of 40 wistar rats aged 4-7 days, irrespective of genders, were selected. The pallium of wistar rats were cut into slices with 400-600 μm, rested for 1 hour in artificial cerebrospinal fluid with gas mixture, simultaneously put in oxygen. The brain tissues were placed into artificial cerebrospinal fluid containing 16 u/mL(type X) and 2 u/mL(type XIV) proteinase, incubated for 60 minutes, and cleared the digestive enzyme. Under the whole-cell voltage clamp mode, the potential was hold at -80mV, depolarizing pulse stimulation from -60 mV to 60 mV, 10 mV step and 160 ms width. The total Tran membranes current was recorded, and then 70 mmol/LCsCI, 70mmol/L CsF was 11 μmol/L Na~+ channel blockers tetrodotoxin, 30 mmol/L tetraethylammonium, 1 mmol/L 4-AP was successively added into extracellular fluid. The inward sodium current, transient outward potassium current and delayed rectifier potassium current was recorded, and then the result was analyzed using clampfit. MAIN OUTCOME MEASURES:①Cell morphological observation; ②Whole-ceil current recording;③Inward sodium currentrecording; ④Outward potassium current recording.RESULTS AND CONCLUSION: The cell had clearly three-dimensional structure, smooth surface and whole neurodendrite or neuraxon. The cell viability could maintained for 8-10 hours at 25 ℃ temperature. The added 11 tetrodotoxin in extracellular fluid could block sodium Current. The outward potassium current could be blocked by tetraethylammonium with 30 mmol/L and 4-AP with 1 mmol/L. The calls harvested by modified rapid dissociation have good functions. Using current separation method, only specific blockers are needed, without extracellular fluid or electrode solution replacing, which can record the inward sodium current, transient outward potassium current, as well as delayed rectifier potassium current. This method can obviously improve the work efficiency than traditional one.

OBJECTIVE: To explore the application of the Dexmedetomidine utend drug induced sleep endoscopy. METHOD: Forty-four OSAHS patients diagnosed by PSG randomly were divided into group A (Dexmedetomidine group) and group B (Propofol group), each group of 22 cases. Group A: pump the Dexmedetomidine (1 microg/kg) over 15 minutes. Once the patient reached a satisfactory level of sedation, the electronic nasopharyngoscope was introduced into the nasal cavity group B: the propofol (2 mg/kg) was intravenous injected, use the same exmination after the object patients falling asleep. If not, double the injection dose of the two group. Observe and record the patient vital signs, EEG and sleep time, and handle the complications. RESULT: The study was successfully completed both in group A and B. Mean arterial pressure (MAP) in group B was lower than group A during the examination significantly (P < 0.05). The time falling to sleep in two groups were (13.4 +/- 2.5) min and (6.6 +/- 1.2) min, and the time in group A was much longer than that in group B. The lowest oxygen saturation during the examination in two groups were 0.835 +/- 0.096 and 0.691 +/- 0.095, and the difference was statistically significant. There was no incidence of adverse reactions in two groups. CONCLUSION Compared with propofol, Dexmedetomidine is a safer sleep-inducing drug, and it can be used for clinical sleep endoscopy.

Objective To evaluate the result of radiotherapy for primary cardiac neoplasm.Methods A patients with primary malignant mucinous Schwannoma in th left ventriclular wall had been treated by conservative surgery. He,then,received fast neutron radiotherapy one year after wards. Radiotherapy was given through opposing anterior and posterior fields of 7.8 cm?7.8 cm. The total radiation dose was 1?004.95 ncGy with 11 fractions of 100 ncGy in 48 days.Results The function of the patient's heart recovered after treatment. The patients has been followed for 5 years without evidence of recurrence or distance metastased. Conclusion Fast neutron radiotherapy is effective in the treatment of primary cardiac neoplasms by surgery combined with postoperative radiotherapy. It could improve the prognosis of patients with primary cardiac neoplasms.

Objective To investigate the signaling transduction mechanisms of inhibiting effect of radiation on vascular smooth muscle cells(VSMC).Methods Cell culture in vitro and semi quantitative reverse transcription polymerase chain reaction(RT PCR) were used to observe the dynamic change of VSMC focal adhesion kinase(FAK) on the mRNA expression level and result of the control group as compared to those of the irradiation ones at 2.5,10,15,20 Gy.Results Dose of 2～20?Gy irradiation down regulated the expression of FAK mRNA. Objective To retrospectively analyze the relationship of the classification and risk stratification in senile hypertension with benign prostatic byperplasia (BPH).Methods Totally 376 male senior patients,including 233 senile hypertensive patients and 143 non-hypertensive patients as a control,were enrolled in this study.There were 35 cases of hypertension at level 1,82 cases at level 2,116 cases at level 3.Based on risk stratification of hypertension,there were 3 cases of low-risk,28 cases of medium risk,75 cases of high-risk,127 cases of very high risk.All candidates accepted the International Prostatic Symptom Score (IPSS) assessment before the treatment.Fasting blood glucose (FBG),total cholesterol (TC),triglycerides (TG),low density lipoprotein cholesterol (LDL-C),high density lipoprotein cholesterol (HDL-C) and prostate specific antigen (PSA) were determined.Body mass index (BMI) and prostate volume (PV) were calculated.Relationship of classification and risk stratification in hypertension with BPH were analyzed.Results The levels of systolic blood pressurc (SBP),diastolic blood pressure (DBP),body weight,BMI,FBG and TC were higher (t=3.883,2.498,2.161,3.399,2.200,2.370,P＜0.05 or P＜0.01),while serum HDL-C were lower (t=2.036,P＜0.05) in hypertensive patients than in control group.Compared with the control group,IPSS and PV was increased (t =3.432,3.381,both P＜0.01) in hypertension group.Risk rate of hypertensive patients with BPH was 2.03 times (95％CI:1.33-3.11,P＜0.01) as compared with control group.In hypertensive patients,PV and IPSS were higher in level 2 or 3 grade group than in level 1 group (F=6.890,7.576,all P＜0.01).PV and IPSS in high risk and very high risk groups were enhanced as compared with those in low-medium risk group (F=30.608,19.804,all P＜0.01).Pearson analysis showed that PV was positively correlated with SBP,FBG and TC(r=0.223,0.251,0.305,all P＜0.05),while negatively correlated with HDL-C(r =-0.235,P＜0.05).Similarly,IPSS was positively correlated with SBP,DBP and FBG (r=0.396,0.273,0.224,all P＜0.01),while negatively correlated with HDL-C (r =-0.288,P＜ 0.01).Conclusions High incidence of BPH appears in the elderly men with hypertension.The increased PV and IPSS coexist with conventional risk factors of essential hypertension.Development of BPH is closely related to higher blood pressure and risk stratifications of hypertension.

Objective To explore the effects of amlodipine,perindopril and valsartan on plasma adiponectin and retinol binding protein 4 in elderly patients with essential hypertension.Methods From March 2007 to July 2010,238 elderly patients with essential hypertension were selected and 193 cases completed this study.Patients were randomly divided into 3 groups:amlodipine group (n=68),perindoprilgroup (n=60) and valsartan group (n=65).Patients in each group were treated with amlodipine,perindopril and valsartan respectively for at least 12 weeks.The changes in blood pressure,heart rate,body height,body mass index (BMI),abdominal circumference,waist circumference (WC),levels of blood lipids,plasma adiponection and retinol binding protein 4 were observed before and after treatment.Results Compared with pre-treatment,systolic blood pressure in 3 groups were significantly decreased after treatment (all P＜0.01).There were no significant differences in blood pressure among 3 groups after treatment (all P＞0.05).Compared with pre treatment,plasma adiponectin level was significantly increased in perindopril group and valsartan group after treatment [(7.4±1.8) μg/L vs.(8.3± 1.8) μg/L,(7.5±1.7) μg/L vs.(8.4±1.9)μg/L,both P＜0.01].Plasma adiponectin level was higher in perindopril group and valsartan group than in amlodipine group after treatment [(8.3±1.8) μg/L vs.(7.6±1.8) μg/L,(8.4±1.9) μg/Lvs.(7.6±1.8) μg/L,both P＜0.05].Compared with pretreatment,plasma retinol binding protein 4 level in 3 groups were all decreased after treatment,and the decrements had significant differences in perindopril group and valsartan group (both P＜0.01) but had no difference in amlodipine group (P＞0.05).Plasma adiponectin retinol binding protein 4 levels were lower in perindopril group and valsartan group than in amlodipine group after treatment[(36.6± 14.2) μg/L vs.(42.7± 13.8) μg/L,(36.3±14.1) μg/L vs.(42.7±13.8) μg/L,respectively,both P＜0.01].Conclusions Perindopril and valsartan play important roles in cardiovascular protection beyond the antihypertensive effects by increasing plasma adiponection level and decreasing plasma retinol binding protein 4 level in elderly patients with hypertension.

Objective To study and contruct competency characteristic evaluation models of nursing supervisors in secondly hospitals.Methods Using the matron semi-structured interviews,literature review,questionnaire survey and expert questionnaires to discuss the competency of nursing supervisors in secodary hospitals.Results We found that the competency model of nursing supervisors in secondary hospitals could better reflect the competency of nursing supervisors.The model had 16 competencies which included knewleage skills.management and individual characters.Conclusions Through the study of the model of competency nursing supervisors in secondary hospitals,we form a set of standard for nursing supervisor selection based on their competency.The standard can help to select the most suited nursing supervisors who have great potentials.
