Aim To investigate the effects of Metacycloprodigiosin(MP) on tumor metastasis-associated cell behavior in vitro.Methods The effects of MP on the proliferation of HUVEC and 4TO7 cells were observed by MTT assay.Migration abilities of HUVEC and 4TO7 cells were measured by wound healing assay.Tube formation of HUVEC in the presence of MP for 10 hours was evaluated on matrigel basement membrane matrix.The abilities of adhesion and invasion of 4TO7 with MP for 24 hours were measured quantitatively by cell adhesion and transwell assay.Results MP did not affect proliferation of HUVEC and 4TO7 cells in concentration less than 10 μmol·L-1.MP inhibited HUVEC cells migration and formation of the tube in the concentrations of 0.01 to 1 μmol·L-1.In these concentrations MP could also inhibit 4TO7 cells migration,adhesion and invasion.Conclusion sThese results demonstrate that MP effectively inhibits tumor metastasis in vitro.All these findings suggest its potential as an anti-tumor metastasis compound in vivo.

Objective T_O analyze the electrophysiological features of shoulder-hand syndrome(SHS)fol- lowing stroke and investigate the relationships between peripheral nerve damage and the factors causing SHS.Meth- ods Fifty-eight stroke patients were divided into an SHS group(39 patients with shoulder-hand syndrome)and a model group(19 patients without shoulder-hand syndrome).Standard sensory and motor nerve conduction studies were performed with 58 of the patients,including sensory nerve conduction velocity(SCV),amplitude of the sensory nerve action potential(SNAP),distal motor latency(DMI)and the amplitude of the compound muscle action poten- tial(CMAP)of the median nerve.A needle electromyogram(EMG)test was performed on the abductor pollicis of the affected side with both groups.Results The needle EMG showed abnormal insertion potential,fibrillation po- tential and positive sharp waves in all 39 cases of the SHS group,which was significantly higher than in the model group.The amplitude of the sensory nerve action potential(7.77?4.34 mV)and the amplitude of the compound muscle action potential(10.13?3.15 mV)were significantly lower than those of the model group.Abnormal ampli- tude was more severe in sensory nerves than in motor nerves.Conclusion Peripheral nerve damage was found in the shoulder-hand syndrome patients,and this damage was mainly the dystrophy of axonal neuropathy.The damage severity was more in sensory nerves than in motor nerves.The study could offer an useful clue of prevention and treat- ment of shoulder-hand syndrome.

Gut microbial metabolite trimethylamine-N-oxide (TMAO) is associated with type 2 diabetes (T2DM). Decreased insulin sensitivity is a significant etiological factor of T2DM. Adipocytes, myocytes, and hepatocytes are the three major target cells for insulin. This study aims to investigate the effects and mechanisms of TMAO on the insulin sensitivity of these target cells. Research results indicate that in different ages of db/db diabetic mice, plasma TMAO levels were increased. TMAO significantly inhibits the insulin signaling pathways in these three major insulin target cells, reduces glucose uptake in 3T3-L1 adipocytes and L6 myocytes and downregulates genes related to gluconeogenesis in primary mouse hepatocytes. Furthermore, in mice with normal insulin sensitivity, elevating plasma TMAO levels to those seen in db/db mice using a minipump results in impaired glucose tolerance and hyperinsulinemia. All animal experiments were carried out with approval of the Experimental Animal Welfare Ethics Committee of the Institute of Materia Medica (Chinese Academy of Medical Sciences and Peking Union Medical College). Mechanistic studies suggest that TMAO exposure increases the levels of endoplasmic reticulum stress-related proteins in these three major insulin target cells. In summary, TMAO directly attenuates insulin sensitivity in insulin target cells, and its mechanism of action may involve enhancing endoplasmic reticulum stress.

OBJECTIVETo evaluate the effect of the oral-appliance in the treatment of ED induced by obstructive sleep apnea-hypopnea syndrome (OSAHS).

METHODSThis study included 40 cases of OSAHS-induced ED. Based on the apnea-hyponea index (AHI) and lowest SaO2, we divided the patients into groups A (AHI < or = 30/h and lowest SaO2 > or = 80%) and B (AHI > 30/h and lowest SaO2 < 80%), and treated them with the oral-appliance for 3 months, followed by evaluation of the therapeutic effect using polysomnography, IIEF-5 questionnaire and Sexual Encounter Profile (SEP) diaries.

RESULTSThirty-seven of the patients accomplished the treatment, 16 in group A and 21 in group B. Compared with the baseline, the AHI was significantly decreased in both groups A (21.4 +/- 7.3 vs 14.2 +/- 6.6, P < 0.05) and B (51.4 +/- 9.5 vs 29.7 +/- 8.1, P < 0.05) after treatment, the IIEF-5 scores increased, even more remarkably in B than in A (3.36 +/- 2.48 vs 1.12 +/- 1.27, P < 0.05). The positive rates of SEP2 and SEP3 were markedly lower in A than in B (37.5% vs 76.2% and 25.0% vs 61.9%, P < 0.05).

CONCLUSIONThe oral-appliance can improve the erectile function of OSAHS-induced ED patients, particularly for those with severe OSAHS.

Objective To establish a successful model for pancreaticoduodenal allotransplantation with enteric drainage in piglet and surveillance acute rejection by biopsy.Methods We implemented the model same as protocol of clinical pancrease transplantation.40 piglets were divided into donors and recipients randomly.20 times experimental operation were finished. The total pancreas with a short segment of duodenum was transplanted to a recipient by anastomoses of vessels to the recipient iliac vessels and small bowel segment to the jejunum with enteric drainage. During operation, mean arterial pressure(MAP), central venous pressure(CVP)were monitored. The levels of amylase activity in blood were investigated.Doppler color blood flow ultrasonography was used to evaluate blood flow of pancreatic allografts,and ultrasound-guided percutaneous pancreas allograft biopsy and histopathologic examination were performed.Results The successful rate of operation was 90%. Objective To explore therapeutic efficacy of endoscopic biliary stent placement in the management of bile leakage following hepatobiliary surgery.Methods 11 cases of bile leakage following hepatobiliary surgery were reviewed retrospectively from January 2001 to December 2005.New clinical classifications of bile leakage were proposed: type I,leakage from cystic duct;type II,leakage from extrahepatic bile duct(diameter of leakage less than 1/3 diameter of biliary duct was type IIA;more than 1/3 was type IIB);type III,leakage from intrahepatic bile duct(leakage from intrahepatic bile duct cecum was type IIIA;leakage from intrahepatic bile duct under draining hepatic segments was type IIIB;above draining hepatic segments was type IIIC);type IV,leakage from Luschka duct.Results In 11 cases,there were 6 cases of type I,2 cases of type IIA,1 case of type IIIA,and 2 cases of type IIIB.4 cases were placed only stents,and Oddi sphincterotomy combined with stent placement were performed in 7 cases.Stents were taken out at 3 weeks(6 cases),1 month(2 cases),2 weeks(1 case),6 months(1 case) and 9 months(1 case) postoperatively.Abdominal pain was relieved in 11 cases after endoscopic treatment,with disappearance of biliary drainage from abdominal cavity and subsidence of jaundice.1 case was followed up for 6 months,and other 10 cases were followed up for 1-3 years,with no recurrence of abdominal pain,fever,jaundice and seroperitoneum,and hemobilirubin became normal.Conclusions Endoscopic biliary stent placement is one of effective therapeutic procedures in the management of most bile leakage following hepatobiliary surgery.New classification of bile leakage may be taken as a guide to select therapeutic procedures.

Objective To compare the efficacy of oral rehydration salts(ORS),health education and autonomic nervous activity training with health education and autonomic nervous activity training on children with postural orthostatic tachycardia syndrome (POTS).Methods Fifty-two children with POTS based on head-up test or head-up tilt test were divided into ORS treatment group and conventional treatment group.ORS treatment group included 33 children,and they received ORS and health education and autonomic nervous activity training therapy.Conventional treatment group included 19 children who received health education and autonomic nervous activity training therapy only.Telephone follow-ups were made after the therapy for 1-3 months.The contents of follow-up were the compliance of ORS treatment,the implementation of health education and POTS related symptom frequency.Results 1.The compliance of doctor's advice in ORS treatment group was better than that in the conventional treatment group.2.There was no significant difference in the symptom scores before therapy between ORS treatment group and conventional treatment group [(6 ± 3) scores in ORS treatment group vs (5 ± 3) scores in conventional treatment group,t =0.799,P ＞ 0.05].After 1 to 3 months therapy,the symptom scores were significantly lowered in both groups.The symptom scores in ORS treatment group was lower than those of the conventional treatment group[(2 ± 1) scores in ORS treatment group vs(3 ± 1)scores in conventional treatment,t =-2.189,P ＜ 0.05].3.If the therapy was determined to be effective when the symptom scores decreased by at least 2 scores,the effective rate in ORS treatment group was significantly higher than that of the conventional treatment group(78.8％ in ORS treatment group vs 42.1％ in conventional treatment group,x2 =7.169,P ＜ 0.01).Conclusions The efficacy of ORS,health education and autonomic nervous activity training therapy is better than that of the health education and autonomic nervous activity training therapy only in POTS children.

Objective To evaluate the operation of laparoscopic splenectomy(LS),its safety and clinical effects.Methods Literature of the advances of LS were reviewed and analyzed.Results With the development of technology and surgical technique,the indications for LS were widened,the size of spleen plays an important role in the clinical outcome of LS.Conclusion LS has all the advantages of minimally invasive surgery.The application of hand-assisted technique is safe and feasible for giant spleen.With the accumulation of surgical experiences and technique development,LS will be extensively used in clinic.

Objective:To observe HSP70 expression during experimental tooth movement in rats.Methods:Orthodontic appliance was placed between the maxillary right first molar and maxillary central incisors of 30 adult SD rats.The rats were killed at 1,3,5,7,14 d after orthodontic force application,samples of experimental teeth with periodontal tissue were prepared for immunohistochemical examination of HSP70 expression.Result:Strong expression of HSP 70 in periodontal ligment was observed at 1 and 3 d after orthodontic force application,positive at 5 d and weak positive at 7 and 14 d.Conclusion:HSP 70 is expressed in periodontal ligment at different levels in different stages during orthodontic tooth movement.
