2.Analysis of soluble expression,purification and antigenic characterization of P protein of GⅡ.2[P16]norovirus

ObjectiveTo obtain the soluble expression of P protein of norovirus(NV)G Ⅱ. 2[P16]and analyze its antigenic characteristics.MethodsThe recombinant plasmid GⅡ.2-NV-pET-28a was constructed by optimizing the NV GⅡ.2[P16]P protein gene,synthesizing and cloning it into pET-28a(+)vector. The recombinant plasmid was transformed into DH5α competent cells,and the expression strain was optimized to make the P protein express in soluble form. The expressed P protein was purified by Ni-NTA affinity chromatography,of which the antigenicity was detected by Western blot,the purity was detected by size exclusion high performance liquid chromatography(SEC-HPLC),and the antigenantibody binding ability was identified by ELISA.ResultsThe target protein of recombinant BL21 Star(DE3)pLySs was highly expressed in soluble form. After purification by affinity chromatography,the purity of GⅡ.2 P protein was 95. 53%,and the antigenicity was good.ConclusionSoluble NV P protein was successfully prepared by prokaryotic expression strain,and the antigenicity of P protein was confirmed,which laid a foundation of the development of NV G Ⅱ.2[P16]detection kits and the recombinant subunit vaccines.
3.Most common illnesses in outpatient clinic of one primary health center in Shanghai

There were 461939 outpatient visits in Shanghai Tianping Primary Health Center from January to December 2010,the data were retrospectively analyzed based on the electronic outpatient record.The 20 most common illnesses accounted for 73.67% of total outpatient consultations (340310/461 939),hypertension ranked the first position of all illnesses ( 21.06% ) and followed by coronary heart disease (8.36%).Patients aged over 65y accounted for 72.35% of all patients,the frequency of consultations was positively correlated with age of patients(P <0.01 ).The frequency of consultations varies according to the gender,age of the patients as well as the organ system involved (P < 0.01 ).It can be concluded that the most frequently encountered illnesses in primary health center are common illnesses and chronic diseases.There are different needs for primary health service between genders and different age groups.
4.Multi-mode administration of dexmedetomidine for thoracic surgery anesthesia

Objective To investigate the reasonable and effective administration ofdexmedetomidine for thoracic surgery anesthesia.Methods Eighty ASA Ⅰ-Ⅱ patients,aged 18-60 years old scheduled for elective thoracotomy were randomly assigned to 4 groups(each 20 patients).Group A:dexmedetomidine before anesthesia induction + bupivacaine before closed thorax cavity.Group B:dexmedetomidine before anesthesia induction + dexmedetomidine and bupivacaine before closed thorax cavity.Group C:0.9% sodium chloride before anesthesia induction + bupivacaine before closed thorax cavity.Group D:0.9% sodium chloride before anesthesia induction + dexmedetomidine and bupivacaine before closed thorax cavity.Mean arterial pressure (MAP) and heart rate (HR) were measured before infusing dexmedetomidine or 0.9% sodium chloride (T0),after infusing dexmedetomidine or 0.9% sodium chloride (T1),instant time after intubation (T2) and 3 min after intubation (T3),5 min after intubation (T4).The scores of visual analogue scale(VAS) and the consumption of analgesics were compared.Results There was no significant difference including gender,age,weight and operation time among four groups (P > 0.05).Compared with T0,MAP and HR were significantly decreased at T1 in group A and group B (P < 0.05),and were significantly increased at T2 in group C and group D (P< 0.05).Respectively compared with group A and group B,MAP and HR were significantly increased at T2,T3 in group C and group D (P < 0.05).The scores of VAS in group B [(2.47 ± 1.43) scores] and group D [(2.00 ± 1.68) scores] were lower than those in group A [(4.78 ± 1.26) scores] and group C [(4.88 ± 1.62) scores] after operation 12 h.The times of using postoperative analgesics in group B [(0.6 ± 0.4) times] and group D [(0.8 ± 0.1) times] were significantly less than those in group A [(1.3 ± 0.5) times]and group C [(1.5 ± 0.4) times] (P < 0.05).Conclusions Intravenous dexmedetomidine before anesthesia induction can control the effect of double-lumen endobronchial tube responses and make hemodynamics stable.Intercostal nerve block with 0.5 μ g/kg bupivacaine and 0.375% dexmedetomidine can enhance the analgesia effect and prolong the analgesia time.
5.Effect of articular step-off on knee joint contact pressures in fractures of the medial tibiul plateau

Objective To study the effect of articular step-off on knee joint contact pressures in fractures of the medial tibial plateau. Methods Eight specimens of embalmed adult knee joint were used to establish models of articular step-off of the medial tibial plateau by 0,1,2,3 mm respectively (Schatzker Ⅳ fracture) .The technique of pressure sensitive film was used to measure the contact pressures on the weight-bearing area of the knee joint in the 3 models. Results When the step-offs of medial tibial plateau were 0,1,2,3 mm,the average contact pressures on the lateral compartment were respectively 3.34 ± 0.06 MPa,3.48 ± 0.06 MPa,4.19 ± 0.05 MPa,4.19 ± 0.04 MPa,while the average contact pressures on the medial coMPartment were 4.04±0.17 MPa,4.57±0.20 MPa,5.64±0.20 MPa,5.64±0.19 MPa.When the step-off of medial tibial plateau was 2 mm,the average contact pressures on the medial and lateral plateaus increased by 39.6% and 25.4% compared with the 0 mm step-off,with statistically significant differences (P <0.05). Conclusions If the step-off of the medial tibial plateau reaches ≥ 2mm,open reduction and internal fixation should be indicated to prevent secondary traumatic arthritis.When the step-off of the medial tibial plateau is < 2 mm,conservative treatment can be a proper choice.

7.Clinical Observation of Modified Hezhong Powder for Treatment of Children Mesenteric Lymphadenitis

Objective To observe the clinical effect of modified Hezhong Powder for the treatment of children mesentetic lymphadenitis.Methods Sixty children with mesentetic lymphadenitis were randomly divided into treatment group and control group,30 cases in each group.The control group was treated routine anti-inflammatory drug Cefaclor for Oral Suspension,and was given symptomatic treatment when necessary.The treatment group was given modified Hezhong Powder on the basis of treatment for the control group.The treatment course lasted for two weeks.After treatment,clinical efficacy and safety,and effects on relieving primary and secondary symptoms and on shortening the swollen lymph nodes were observed.Results (1) The total effective rate of the treatment group was 96.7%,and that of the control group was 83.3%,the difference being significant (P < 0.05).(2) The treatment group had better effect on relieving the primary symptom of abdominal pain,and secondary symptoms of poor appetite,vomiting,abdominal distention and abnormal stool than the control group.The differences of time for the symptoms disappearing were significant (P < 0.05).(3) After treatment,the swollen.lymph nodes of the two groups were shrunk(P < 0.05),and the shrinkage in the treatment group was superior to that of the control group (P <0.05).(4) During the treatment,no obvious adverse reaction occurred in the two groups.Conclusion Modified Hezhong Powder combined with western medicine is effective and safe for the treatment of children mesentetic lymphadenitis,and its efficacy is superior to that of the western medicine alone.
8.Analysis and Safety Evaluation of the Clinical Efficacy of Laparoscopic Appendectomy for Acute Appendicitis

Objective To investigate the clinical efficacy and safety of laparoscopic appendectomy for acute appendicitis.Methods Nighty-two cases of acute appendicitis were included in this study,randomly divided into observation group (n =46) and control group (n =46) according to the random number table method.The observation group was treated with laparoscopic appendectomy,and control group with the traditional open appendectomy treatment.We compared the two groups of cases in postoperative conditions and the level of WBC changes and 1d and 3d after operation.Results The operation time of the observation group was shorter than that of the control group (t=4.049,P<0.05);there was no significant difference in lymph nodes (t=0.570,P>0.05);the amount of bleeding was less than that of the control group (t=20.748,P<0.05).The get-out-of-bed activity time in the observation group was faster than the control group (t=9.221,P<0.05);eating time was faster than the control group (t=8.794,P<0.05);the anus recovery exhaust time was faster than the control group (t=17.423,P<0.05);hospital stay was shorter than the control group (t=13.479,P<0.05);the complication rate was lower than that of the control group (t=13.883,P<0.05).The levels of 1d and 3d WBC were lower in the observation group than that in the preoperative group (t=7.277,26.911,P<0.05);the levels of 1d and 3d WBC in the observation group were lower than those in the control group at the same time (t=3.6189,17.827,P<0.05);the 3d WBC level of the observation group was lower than that of 1d (t=20.732,P<0.05).Conclusion Laparoscopic appendectomy is a safe and effective treatment for patients with acute appendicitis,and it is worthy of further clinical research.
