1.A modified cuff insertion technique for portal vein anastomoses in rat liver transplantation

Objective To investigate the effect of a modified cuff insertion technique named “portal vein branch-sling” (PBS) method for portal vein (PV) anastomoses in rat liver transplantation.MethodsA total of 60 cases of orthotopic liver transplantation (OLT) using 2-cuff technique in rats were performed in our transplantation center.The PBS technique and conventional methods were used respectively in study and control groups.Time for PV anastomoses,anhepatic phase (AP) and surgery of recipients,successful rate for PV cuff insertion and operation were statistically compared between the different groups.Results35 cases of OLT using PBS technique in rats were assessed.The successful rate of PV cuff insertion was 94.3%,and the time of PV anastomoses using the modified PBS cuff insertion technique was only 1.9 ± 0.7min.The successful rate of operation was 80%with a 21.8±2.2min AP.The study group showed significant differences (P<0.05) compared with conventional group.The survival rate in the first week was 85.8% with no statistical difference (P>0.05).ConclusionsThe modified PBS cuff insertion technique for PV anastomoses of OLT has advantages in speed,safety and stabilization,and should be considered as one of the best available PV anastomosis technique for OLT in rats.
2.Effects of electroacupuncture at Tianshu (ST25) on pro- and anti-inflammatory cytokines in rats with severe acute pancreatitis.

Objective: To observe the effects of electroacupuncture (EA) at Tianshu (ST25) on pro- and anti-inflammatory cytokines in sodium taurocholate-induced severe acute pancreatitis (SAP) in rats. Methods: Sixty-six male Sprague-Dawley rats were randomly divided into three groups: sham-operation group, SAP group and EA group. An SAP model was established by injecting 3.5% sodium taurocholate at a dose of 1 mL/kg through puncturing the pancreatic duct. In the EA group, EA at ST25 was performed with sparse-dense wave (2/100 Hz, 2 mA for 30 min) immediately after sodium taurocholate injection and 30 min before sacrifice. Rats were sacrificed at 3 (n=7), 6 (n=7) and 12 h (n=8) after operation. Levels of tumor necrosis factor-α (TNF-α), interleukin-6 (IL-6) and IL-10 in serum were estimated by enzyme-linked immunosorbent assay. Pathological changes of pancreatic tissues were examined by hematoxylin and eosin staining and observed under a microscope and also scored. Results: The levels of serum TNF-α and IL-6 of SAP rats were significantly higher than those of sham-operation rats at 3, 6 and 12 h. The level of IL-10 in serum of SAP rats was significantly higher than that of sham-operation rats at 3 and 6 h (P<0.05). EA at ST25 down-regulated the levels of TNF-α and IL-6 in serum and up-regulated the level of IL-10 and the ratio of IL-10/TNF-α, also attenuated the morphological damages at 6 and 12 h (P<0.05) in rats with SAP. The pathological scores of pancreatic tissues in the EA group were significantly less than those in the SAP group at 6 and 12 h (P<0.05). Conclusion: EA at ST25 may have a therapeutic effect on rats with SAP by down-regulating the levels of TNF-α and IL-6 and up-regulating the level of IL-10 to re-establish the balance of pro- and anti-inflammatory cytokines, and to ameliorate the inflammatory response of SAP.

4.Pharmacokinetics of rebamipide in the patients with peptic ulcer

5.Protective effect of propofol preconditioning on limb ischemia reperfusion injury in rats

Objective To investigate the effect of propofol on rat′s limb ischemia reperfusion injury .Methods Sixty healthy SD rats were randomly divided into sham operate group ,ischemia-reperfusion group and propofol group (n= 20) ,each group was divided into 4 subgroups according to the different reperfusion time .To copy the right lower limb ischemia reperfusion model ,5 min before reperfusion ,use propofol injection (50 mg/kg ,intraperitoneal inject) ,various subjects in the corresponding time points (3 ,6 , 9 ,12 h) were sacrificed .TNF-α ,NF-κB of blood and MDA ,SOD of Skeletal muscle were measured ,calculate muscle wet dry weight ratio .Results Compared with ischemia reperfusion group ,propofol could significantly reduce expression of TNF-alpha ,NF-κB lev-els in serum (P< 0 .05) ,inhibit the increase of the MDA level and decrease of the SOD level in muscle (P< 0 .05) ,also reduce the extent of skeletal muscle cell edema(P< 0 .05) .Conclusion Propofol can attenuate limb ischemia reperfusion injury by inhibiting inflammation response and reducing the oxygen free radicals′ damage .
6.Correlation between cingulate white matter and executive function in patient with major depression

Objective To investigate the white matter microstructural changes of cingulation and congni-tive function impairment in patient with major depression. Methods 25 middle-aged first episode patients and 24 nondepressed comparison participants were enrolled,and all the subjects scanned by diffusion tensor imaging and tested by Wisconsin Card Sorting Test. Correlation analysis was done respectively for both two groups with FA and WCST items. Results Male patients with depression decreased FA in the middle frontal lobe ( - 29 24 32,16 voxels),the left superior temporal lobe (-36 -34 16,11 voxels),the sub-gyral of temporal lobe ( -45 1 - 15/ - 47 - 6 - 9; 16/10 voxels) and the right lobe of cingulate (18 - 2 29,22 voxels). And the female patients with depression shows reduced FA in the inferior frontal lobe ( -42 23 4/45 24 6; 15/21 voxels) ,the left precentral gy-rus (39 -44 -15,17 voxels) ,the right medial frontal gyrus (6 16 -18; 10 voxels) ,the left superior temporal lobe ( -55 -31 15; 89 voxels) ,the sub-gyral of temporal lobe ( -38 - 12 - 19;12 voxels) and the right anterior cingulate gyrus (71 26 -9; 15 voxels) (P<0.001 ,cluster > 10 voxels). The Regions of interests analyze with cingulate only found reduced FA in the right lobe of female group(0.26 ±0.05,0.33 ±0.08; P=0.03). Correlation were found between the right anterior cingulate and Rpe (r= -0.49, P = 0. 03) ,the right middle cingulate and Rpe (r= -0.56, P = 0. 01) in depressed patients. No significant association were found in healthy control group(P> 0.05). Conclusion Cingulate white matter fiber is abnormal in first episode depression and these abnormalities may be one of the pathophysiology of major depression.
7.Effects of electroacupuncture at Zusanli (ST 36) on the expressions of small intestinal occludin protein and nuclear factor kappa-B in rats with severe acute pancreatitis.

OBJECTIVETo explore the mechanism of electroacupuncture at Zusanli (ST 36) on regulation of intestinal inflammatory reaction in rats with acute pancreatitis.
METHODSFifty-four SD rats were randomly divided into a severe acute pancreatitis (SAP) group, an electroacupuncture (EA) group and a sham-operation (SO) group, 18 cases in each group. 3.5% sodium cholate was used to made SAP model by retrograde injecting in cholangiopancreatic duct. After the success of model making, the EA group was treated with EA at bilateral Zusanli (ST 36) for 30 min. The SO group and the SAP group were fixed at the same time for 30 min without treatment. All the rats were killed at 3 h, 6 h and 12 h after modeling in batches. The pathological changes of pancreatic tissue and intestinal epithelium were observed, and the expression of small intestinal occludin protein and nuclear factor kappa-B (NF-kappaB) were detected by immunohistochemical SP method.
RESULTSThe pathologic score and the expression of small intestinal NF-kappaB p65 at 3 h, 6 h and 12 h after modeling in the EA group and the SO group were significantly lower than those in the SAP group (all P < 0.05), and the expression of small intestinal occludin protein in the EA group and the SO group were significantly higher than that in the SAP group (all P < 0.05).
CONCLUSIONElectroacupuncture at Zusanli (ST 36) can alleviate pancreatic injury by reducing the expression of NF-kappaB p65 and enhancing the expression of occludin protein in the intestinal epithelium in the SAP model rats.

8.Orexins in regulation of sleep and awakening

The neuropeptide orexins, including orexin A and orexin B, are located exclusively in hypothalamus OX1R and OX2R are receptors of orexin. OX1R is selective to orexin A while OX2R is not. The distributions of orexin neurons in human brain are the same as those of rats. Orexin was closely related with sleep and awakening behavior besides regulating the feeding behavior. Orexin-containing terminals are also found in areas modulating sleep and awakening such as the locus ceruleus, tuberomammillary nucleus, dorsal raphe, and the basal forebrain. In those areas, there are also lots of OX1R and OX2R and their precursors. Orexin neurons are extremely active during rapid wave sleep and awakening. If hypothalamus in some areas is destroyed and orexin neurons are reduced, sleep-awakening dysfunction will appear (for instance: narcolepsy).
9.Orthotopic liver transplantation in the treatment of hepatic cancer:a report of 70 cases

Objective To summarize the clinical experience of liver transplantation in the treatment of hepatocellular carcinoma(HCC).Methods From June 2004 to March 2007,70 consecutive HCC patients underwent liver transplantation in our hospital,including classic orthotopic liver transplantation in 41 cases,and piggyback liver transplantation in 29 cases.All data of patients were retrospectively analyzed.Results All liver transplantation were successfully conducted.The average warm ischemia duration was 4.5 minutes,and cold ischemia duration was 8 hours.There were 3 cases of postoperative deaths.Surgical complications were intra-abdominal hemorrhage in 2 cases,and biliary anastomotic stricture in 4 cases.Sixty-seven cases were followed up for 12-33(average 21) months,10 cases had recurrence of liver cancer after transplatation and 1 dead.Conclusions Liver transplatation can used on selected patients with HCC.Successful liver transplantation relies on good quality of liver graft,and idealized technique of vascular and bile duct reconstruction are key factors of liver transplantation.Proper postoperative management can effectively reduce the complications of operation.
10.Anxiolytics and Antidepressants Contribute to Symptomatic Improvement in Patients with Primary Hemifacial Spasm Coupled with Anxiety and Depression

Objective To evaluate the symptom improvement of anxiolytics and antidepressant to hemifacial spasm(HFS)patients with anxiety and depression. Methods A total of 60 HFS patients with anxiety and depression were enrolled for the study and divided into experimental group and control group. Experimental group were treated with escitalopram and BTX-A ,and control group only received BTX-A. SAS and SDS were used to test all the patients before and after treatment. Results Both experimental group and control group were significantly improved after BTX-A therapy. There was no different between two groups at 1 week and 1 month,respectively. At 2 weeks and 3 months,more obvious improvement was seen in experimental group. The SAS and the SDS standard average scores of experimental group were significantly decreased compared with the control group. There was statistical significance in control group after 1 month and 3 months treatment(P<0.05),and in experimental group,the differences appeared after 2-week treatment(P<0.05). Additionally,after 1 month treatment,there were statistical differences between the two groups(P<0.05),and experimental group shown a longer effective period compared with control group. Conclusion Anxiety and depression state serves as a complication of HFS ,which is positively correlated with HFS. HFS patients with anxiety and depression benefit better from escital-opram and BTX-A combination therapy than BTX-A alone.
