Objective To determine the prediction and clinical characteristics of intravenous immunoglobulin (IVIG) treated Kawasaki disease (KD) failure in initial dose.Methods Retrospective analysis was performed with the clinical data of 1 11 children with KD.The paticnts were divided into sensitive group and unresponsive group according to initial effect of IVIG.The clinical manifestations,laboratory examination and radiological features of the children were compared.Logistic regression analysis was performed in significant different indicators to determine independent correlation factors.In order to seek the reference indexes which indicate unresponsive to IVIG,a receiver operating characteristic (ROC) curve was made,of which the diagnostic cut-off was nine independent correlation factors while grouping was made according to patients' different responses to IVIG.Results (1)There were 90 cases (81.1％)in effective group and 21 cases (18.9％) in unresponsive group.(2) Compared with the sensitive group,hyperpyrexia cases[66.7％ (14/ 21 cases) vs 34.4％ (31/90 cases),x2 =7.334,P =0.007] and the chances of coronary artery lesions [47.6％ (10/ 21 cases) vs 23.3％ (21/90 cases),x2 =4.989,P =0.026] were significantly higher in the unresponsive group.(3)Compared with the children administered twice with gamma globulin,the children of single-dose treatment significantly reduced the unresponsive probability [12.5 ％ (9/72 cases) vs 30.8 ％ (12/39 cases),x2 =5.504,P =0.019],and there was no difference in the chances of coronary artery lesions[23.6％ (17/72 cases) vs 30.8％ (12/39 cases),x2 =0.672,P =0.412].(4)Comparing the sensitive group and the unresponsive group,the percentage of neutrophils count [(0.72 ± 0.06) vs (0.76 ± 0.04),t =-2.84,P =0.005],platelet counts [(352.38 ± 42.18) × 109/L vs (373.14 ±36.93) × 109/L,t =-2.076,P =0.040] and C-reactive protein (CRP) [(74.38 ± 12.92) mg/L vs (92.05 ± 11.17) mg/L,t =-5.780,P =0.000] were significantly higher in the unresponsive group,but the level of serum albumin[(34.09 ±3.19) g/L vs (31.61 ±2.03) g/L,t =4.442,P =0.000] was lower.(5) Multivariate Logistic regression analysis indicated that the percentage increase of neutrophils count (P =0.018),CRP (P =0.000) increase and serum albumin(P =0.040) decrease were independent risk factors for unresponsive treatment with gamma globulin.(6)After the area under the ROC curve was calculated,the percentage of neutrophils count,CRP and serum albumin could be effective predictors to IVIG treatment failure in initial dose,and the critical values were 0.72,78.5 mg/L and 33.11 g/L,respectively.Conclusions Treatment with IVIG for the first time may be ineffective in some situations such as the percentage of neutrophils count ≥0.72,CRP ≥78.5 mg/L or serum albumin≤33.11 g/L.

The papers regarding moxibustion published in science citation index (SCI) journals for the recent 5 years were searched to explore the international tendency of moxibustion researches, which provided references for moxibustion to have a better internationalization. With methods of internet search and database search, a total of 116 papers regarding moxibustion were included. These papers were published in 40 kinds of journals, mostly in Britain and the United States. The journal with the highest impact factor was Stroke, which had 5729 points. The number and impact factor of these journals were inferior to those of acupuncture journal. Compared among these journals, the depth and width of moxibustion research were increasing year by year. The category of diseases related with moxibustion is mainly digestive system diseases, motor system diseases and urinary-genital system diseases. The type of papers was characterized with clinical observation and mechanism research. Researches related with moxibustion included moxibustion dose and safety. It was believed that the international recognition of moxibustion effectiveness, standardization of moxibustion manipulation, standardization research, etc. were needed to be solved in the further.

Objective To explore the characteristics of semantic priming effects in Chinese words with different association strength in patients with aphasia by auditory stimulation.Method Stimulus-response word pairs with different association strength including strong,moderate,weak,and no association categories were chosen from word association thesaurus as experiment materials.Both patients with aphasia(n=11)and normal subjects (n=16)were requested to finish an auditory lexical decision task for target words.Semantic priming effects were investigated by means of measuring reaction time(RT)and error rate of each word-pair.Results In patients with aphasia and normal subjects,the mean RTs were significantly shorter in strong,moderate and weak association strength words than in no association strength words(patients with aphasia(1270.20±47.70)ms,(1340.50±266.25)ms,(1429.70±317.07)ms vs(1549.00±325.87)ms,P＜0.05 and normal subjects(1140.2±274.48)ms,(1196.50±284.06)ms,(1262.10±274.31)ms vs(1391.20±315.68)ms,P＜0.05).In strong,moderate,and no association strength words,the mean RTs were no significant differences between two groups.In the weak association strength words,mean RTs were significantly longer in patients with aphasia than in normal subjects((1429.70±317.07)ms vs(1262.10±274.31)ms,P＜0.05).In two groups,mean error rates were significantly less in strong,moderate and weak association strength words than in no association strength words(patients with aphasia:7.73±6.07,4.55±7.23,6.82±8.15 vs 14.09±12.41,P＜0.05 and normal subjects:3.44±4.37,2.81±3.64,5.31±5.91 vs 10.94±11.14,P＜0.05).However,in strong association strength words,mean error rates were significantly higher in patients with aphasia than in normal subjects(7.73±6.07 vs 3.44±4.37,P＜0.05).In moderate,weak and no association strength words,there were no significant differences between two groups.Conclusion The patients with aphasia follow gradient of the association strength words like normal subjects and have semantic priming effects in the strong,moderate association strength words.

Aim To investigate the effect of TAL and MAPK signal transduction on alveolar macrophage (AM) apoptosis and activity of chronic bronchitis (CB) rats.Methods CB model was established by BCG+LPS method and the in vitro and in vivo experiments were used. MTT method was used to detect the AM activity,and the apoptosis of AM was observed by electron microscope.Results The number of AM in BALF of CB rats was increased than that of normal group (P<0.01).The activity of AM was increased in model group than in control group.The apoptotic rate of AM in CB group was much lower than that in the control group [(13.93±3.34)% vs (5.37±1.38)%] (P<0.01).ERK inhibitor PD98059 induced the apoptosis of cultured AM while JNK inhibitor Curcumin reduced the apoptosis.TAL could inhibit ERK MAPK phosphorylation in AM of CB rats.Further investigation showed that Bcl-2 protein expression was significantly increased while Bax evidently decreased in AM of CB rats.TAL could significantly decrease Bcl-2 expression and increase Bax protein expression, which might be the mechanism of its effect.Conclusions There is an increased activity and decreased apoptosis of AM in CB rats compared with normal rats. TAL can inhibit AM activity and increase apoptosis of AM in CB rats which may be related to the therapeutic effect of CB. ERK and JNK MAPK signal transduction participates in the apoptosis of AM. Regulation of Bcl-2/Bax imbalance and MAPK phosphorylation in AM of CB rats might be the mechanism of its effect.
