1.Strategy of the diagnosis and treatment for epistaxis and guideline for clinical application of MasterPillar.

Epistaxis is a common otorhinolaryngological emergency with complex etiological factors and varied clinical manifestations. The key to epistaxis treatment is accurate diagnosis and adequate hemostasis. Electrocoagulation is a reliable, safe and effective treatment for epistaxis. However, there are still several deficiencies in application of the commonly used electrocoagulation surgical products. This paper introduces a new type of radiofrequency head incorporating the dynamic circulatory functions of drip, irrigation, hemostasis and aspiration. We aim to achieve noninvasive, effective and accurate hemostasis in the treatment of epistaxis or nasal sinus surgery.

2.Expression of TRAF6 and its correlation with clinicopathologic characteristics in ovarian carcinomas

Objective:To examine the expression of TRAF 6 in ovarian carcinomas ,discuss the relation between expression level and clinical characteristics of ovarian carcinomas and study the clinical significance .Methods: Specimens from 102 patients with ovarian carcinomas managed in our hospital between Augest 2001 and December 2009 were included in this study .Immunohistochemical staining was used to detect the expression of TRAF 6 in ovarian carcinomas and normal tissues .All the specimens were confirmed by pa-thology for ovarian carcinomas and normal tissues by HE staining .Then, the correlation between the expression and clinical characteristics of patients was analyzed .Furthermore ,survival analyses were performed according to the Kaplan-Meier method.Results:The positive expression rate of TRAF 6 in ovarian carcinomas and normal tissues was 68.6% and 20.5%, respectively.TRAF6 expression was significantly associated with tumor stage and distant metastasis ( P<0.05 ) .According to the survival analysis of 102 ovarian carcinomas patients ,cases in the TRAF6 low-expression group showed poorer overall survival rate when compared with low -expression group ( P<0.001 ) .Conclusion: These results indicate that the expression of TRAF 6 was closely related with in the progression of ovarian carcinomas and may have clinical utility in the prediction of prognosis of ovarian carcinomas .
3.Autophagy and its relationship with tumor proliferation, invasion, and treatment.

Autophagy is a highly conservative biological behavior in eukaryotic cells. This dynamic process involves "wrapping" cytoplasmic components and combining with lysosomes in cells for catabolism. The catabolic effect of autophagy can eliminate toxic substances in cells, maintain homeostasis in the intracellular environment, and produce small molecules, such as amino acids, which nourish cells, thereby allowing them to survive. Autophagy can inhibit the occurrence of tumors by maintaining homeostasis in the intracellular environment. However, it can promote the proliferation, invasion, and metastasis of malignant tumor cells. Autophagy can regulate the microenvironment of tumor cells and has an important role in a series of processes, such as anoikis, tumor dormancy, and epithelial-mesenchymal transition.

4.On the depth of needle insertion.

The statement on the depth of needle insertion is collected from Neiiing (nternal Classic) and Nanjing (Classic of Medical Problems) and the relationship of it is discussed with meridians, diseases, symptoms, location of sickness, qi and blood changes in human body, time, layers of needle insertion, reinforcing and reducing of acupuncture, etc. The factors are explored for the determination of the puncture depth, which provides the important guidance for the depth of insertion, needling sensation, reinforcing and reducing manipulations, acupuncture effect and the improvement of acupuncture efficacy. The complex and mystery of acupuncture technique focus on the control of insertion depth. In clinical practice, the increase of needle insertion depth may not induce the equal increase of efficacy. The depth of needle insertion is rather relevant with stimulation at the effective acupoint in the effective layer.

5.Expression of interleukin-18 in peripheral blood mononuclear cells from patients with rheumatoid arthritis

Objective To further investigate the balance (relative presence) of Th1 and Th2 subsets at the sites of rheumatoid inflammation,and to understand how about the expression of IL-18 in patients with rheumatoid arthritis (RA),and what the relationship exists between expression of IL-18 and rate of Th1/Th2 in RA,and between IL-18 level and activity of the disease.Method Expression of IFN-γ,IL-4,and IL-18 mRNA in peripheral blood mononuclear cells (PBMC) from 16 patients with RA and 15 healthy subjects was detected by semi-quantitative RT-PCR.Results ① PBMC from patients with RA was found to contain greater levels of IL-18 mRNA than that from healthy subjects (P<0.01).② IL-18 mRNA levels were correlated with IFN-γ mRNA (relative coefficients:r=0.836,P<0.05).③ IL-18 mRNA levels were associated with the disease activity as assessed by levels of erythrocyte sedimentation rate (r=0.753,P<0.05).Conclusion IL-18 is among a matrix of inflammatory cytokines produced abundantly in patients with RA and is associated with induction of IFN-γ and activity of the disease.
6.Chronic scaphoid nonunion treated with vascularized bone graft and external fixator

Objective To evaluate the medium-term follow-up outcomes of treatment of chronic scaphoid nonunion with vascularized bone graft based on 1, 2 intercompartmental supraretinacular artery.Methods From February, 1995 to November, 2006, 13 cases of chronic scaphoid nonunion were treated with vascularized bone graft based on 1, 2 intercompartmental supraretinacular artery in our hospital. Nine cases were complicated with avascular necrosis of the proximal pole. The interval between injury and treatment averaged 22. 2 months. The nonunions were stabilized by screw or external fixator. Procedures of arthrolysis were applied and early wrist mobilization was initiated under the protection of external fixator. Radiographic finding and Mayo scoring system were used to evaluate the clinical outcomes. Results The mean time of follow-up was 82.9 months. The union time for the 13 cases averaged 11.2 weeks. At the final follow-up,there were 3 excellent, 8 good and 2 fair evaluations;the range of motion of the wrist, Mayo score, grip strength, intrascaphoid angle and scapholunate angle were significantly improved. Conclusions Chronic scaphoid nonunion can be well treated with vascularized bone graft based on 1, 2 intercompartmental supraretinacular artery. A simple arthrolysis during the procedure can produce satisfactory results. External fixation plays a critical role in early mobilization and functional restoration of the wrist.

8.Treatment of chronic scaphoid nonunion with vascularized bone grafts from dorsal distal radius

Objective To evaluate the medium and long term follow-up results of vascularized bone grafts dorsal distal radius in treatment of chronic scaphoid nonunion. Methods A retrospective study was carried out in 13 patients with chronic scaphoid nonunion treated with vascularized bone grafts dorsal distal radius in our hospital from 1995 to 2006. Nine patients were accompanied by avascular necrosis of the proximal pole. The vascular pedicle was 1,2 intercompartmental supraretinacular artery. The nonunion was stabilized by screws or external fixators. Radiographic finding and Mayo scoring system on wrist mobilization were used to evaluate the clinical outcomes. Results The interval between injury and treatment was average 22.2 months. All patients were followed up for average 76.6 months, which showed that 12 patients obtained fracture union, with average union time of 11.2 weeks. Functional evaluation results were defined as excellent in three patients, good in eight and fair in two. The last follow-up revealed significant improvement of the range of motion of the wrist, Mayo scores, grip strength, intrascaphoid angles and scapholunate angles. Conclusions Vascularized bone grafts from dorsal distal radius has relatively surgical procedure and can achieve satisfactory results in management of scaphoid nonunion. The perioperative arthrolysis and early postoperative mobilization of the wrist under exernal fixation play critical role in restoration of wrist function.
9.Assessment on Equalization of Eseential Medical Services in Ningxia:Base on Theil Index

Objective: To measure the equalization of urban and rural essential medical services in Ningxia. Methods: To apply the Theil Index and its decomposition by using population as the weight and the distance as the weight of index of essential medical resource allocation. Results: Significant difference was found in urban and rural essential medical service in Ningxia, the contribution rate among regional differences was high. Conclusion: Non equivalence of essential medical service is serious between urban and rural areas in Ningxia. It is suggested to improve the quality of the rural medical staffs, implement the training and qualification appraisal system, set up a remote consultation system to increase the medical resource sharing, implement medical and health care policies and open essential medical function of essential medical services.
10.Clinical effects of treatment of gonadotropin releasing hormone agonist combined with oral contraceptive on endometriosis

Objective To investigate the treatment effect of gonadotropin releasing hormone agonist and oral contraceptive on endometriosis diseases.Methods One hundred and eighty-six cases with endometriosis who were hospitalized in Traditional Chinese Medicine Hospital of Fangshan District of Beijing from Jan.2012 to Dec.2012,were randomly divided into control group (93 cases) and observation group (93 cases).Patients in control group were given Marvelon by oral from the first day of menstruation after operation to 6 months,1 tablet/day,and patients in observation group were given Marvelon combined with Zoladex after laparoscopic conservative treatment (3.6 ml subcutaneous injected Zoladex from the first week after operation,4 weeks/time,3 times).The total subjective symptom scores,efficacy rate,Pregnancy rate and adverse reaction were calculated and recorded.Results Subjective symptom score of observation group was (0.91 ± 0.32),significantly lower than that of the control group (1.64 ± 0.74 ; t =8.73,P < 0.01).The efficiency rate in observation group were 95.70% (89/93) was significantly higher than that in control group(82.80% (77/93) ;t =4.328,P <0.05).Adverse reactions rate and reoccurrence in observation group were 4.30% (4/93) and 3.23% (3/93),lower than that of the control group(16.13% (15/93) and 10.75% (10/93) ; x2 =4.854,5.168 ; P < 0.05).After treatment,the pregnancy rate in infertile patients of observation group the was 85.71% (12/14),significantly higher than that of the control group (61.54% (8/13),x2 =3.987,P < 0.05).Conclusion Gonadotropinreleasing hormone agonist combined contraceptive is worthy of recommendation due to alleviating clinical symptoms,reducing adverse reactions occur,the relapse rate and improving the chances of conceiving infertility patients.
