To identify the clinical efficacy,immunological consequences and adverse effects of anti-CD20 monoclonal antibody in systemic lupus erythematosus.Nearly 100 patients have been reported and a 80% of them achieved marked reduction in global disease activity and clinical remission in lupus nephritis.Anti-CD20 monoclonal antibody is well tolerated in 90% of the patients,though10% of them had hypersensitivity reactions and 4 patients had severe opportunistic infection.

3.Synovial mesenchymal stem cells-based therapy for cartilage repair An issue concerning clinical transformation

BACKGROUND:Cartilage injury is stil one of the clinical problems difficult to be treated completely so far. Recently, the discovery of synovial mesenchymal stem cells (SMSCs) has brought about the new hope to cartilage repair.
OBJECTIVE:To explore the process concerning SMSCs-based therapy for cartilage repair in the past few years, such as the characteristics of SMSCs, culture conditions, preclinical and clinical studies, and then to summarize the literatures published in recent years.
METHODS:A computed-based online search of PubMed and SpringerLink databases was performed using the key words of“synovial mesenchymal stem cells, cartilage repair”for literatures published from January 1993 to May 2013.
RESULTS AND CONCLUSION:Final y, 37 articles were included. SMSCs have a greater proliferative capability, colony-forming potential and chondrogenic potential than other mesenchymal stem cells. The diseases such as osteoarthritis and rheumatoid arthritis can influence the characteristics of SMSCs. Numerous articles have aimed at the studies of cellculture in vitro and celltransplantation in vivo. However, the process of SMSCs therapy is mostly at its preliminary stage. Reports on its unique characteristics, optimal culture conditions and the high-quality clinical studies are stil largely lacking. In a word, though further studies are needed, SMSCs appear to be a promising cellsource for cartilage repair in the future.
4.Surgical cardiac synchronization therapy for the cardiomyopathy heart failure with micro-invasive thoracoscopy techniques

Objective The cardiac synchronization therapy (CRT) was proven to have good treatment for the cardiac conduction disorders patients with serious heart failure. But many disadvantages were gradually be noticed, such as difficulty of sinus electrode implantation, coronary sinus injury and bleeding, still one third CRT cases remain unchanged cardiac function.Recently the epicardial lead CRT therapy by the cardiac surgeons appears promising to provide better clinic resynchronization.The aim of this research is to explore the clinical value of surgical epicardial lead CRT for the cardiomyopathy heart failure with micro-invasive thoracoscopy techniques. Methods During April 2007 to Sep 2009 eleven patients were diagnosed as advanced heart failure with cardiac dysynchronization proven by the ECG and tissue Doppler echo examination. The dysynchronization parameters of tissue Doppler echo includes left ventricle maximize delay time , the left dysynchronization index (Ts-SD), the inter-ventricle mechanical delay time (IVMD), left ventricle end-dilation diameters (LVEDD) and the left ventricle ejection fraction (LVEF). All the patients got the consent agreement before the surgery. In the hybrid operation room the right atrial and ventricle endocardial electrodes were placed firs guided by X ray under the general anesthesia and double lumnen incubation. Then the thoracoscope techniques were used to explore the left ventricle lateral wall. The right atrial and ventricle electrodes were connected into the three chamber pacemaker. The Tissue Doppler Echo TEE technique was used to check which part of the left ventricle lateral wall is suitable for the idea CRT therapy. Usually the latest activated part of the left ventricle wall is the goal place. Then the epicardial electrode was fixed with 4-0 prolene suture at the idea place. All the cases were performed by the same surgeons group in the same medical center. The resynchronization features were examined after surgery and during the follow up. Results The endocardial and epicardial electrodes were implanted successfully without any serious complication. All patients were weaned and discharged without any adverse cardiac episodes. There is no mortality during the follow up period. All the patients received the β-blokers, diuretics, ACEI/ARB and other traditional medicine for the heart failure.The Tissue Doppler showed the E peak wave separated form the A peak which means the good resynchronization between the atrium and the ventricle. After surgery during the follow up the left ventricle maximize delay time decreased from (393.4 ±40.2 ) ms to ( 102.1 ± 34.6) ms, the left dysynchronization index (Ts-SD) decreased from (145.2±29.3)ms to(51.0±21.4) ms, the inter-ventricle mechanical delay time (IVMD) decreased from (59.1 ±23.4) ms to (31.2 ± 11.5 ) ms, left ventricle end-dilation diameters increased from (73.1 ± 13.4) mm to (63.2 ± 6.7) mn and the left ventricle ejection fraction increased from 0.32 ±0.04 to 0.41 ±0.07. Conclusion The micro-invasive surgical synchronization therapy could get good CRT result for the cardiomyopathy heart failure patients. Some patients traditionally in the waiting list for the heart transplant could be considered for the CRT therapy candidates.
5.Research on the electrophysiological mapping and ganglianated plexi ablation techniques during the minimally invasive atrial fibrillation surgery

Objective To conclude the results of electrophysiological (EP) mapping and ganglianated plexis (GPs) ablation during the minimally invasive atrial fibrillation (AF) surgery.Methods During the period of Sep 2007 to May 2010 to-tally 185 paroxysmal atrial fibrillation cases were recruited receiving the minimally invasive AF surgery with video assistance.The EP mapping was used to test the dual direction block after ablation and the distribution of GPs was recorded.The sinus rhythm rate was followed up after surgery.Results All the minimally invasive surgery were performed successfully withoutdeath in the hospital.There was no permanent pacemaker implant and other serious complications.EP mapping results showed92% pulmonary vein (PV) potential to the left atrium (LA) could be isolated with the dual direction block for the paroxysmal atrial fibrillation cases.And there were 89% GPs positive,higher rate than the persistent AF.There were more GPs positive in the right PV area than the left and 86% GPs lied at the LA domain,not the PV orifice.At least 5 or 6 ablation lesions were needed to get the dual direction block and denervation of the GPs.The sinus rhythm rate after surgery after 3,6,12 and 24months is 83.7%,82.4%,85.4% and 83.9%.The 12 months follow up data showed the cases with more GPs positive (>6) had 81.3% sinus rate and the less GPs positive (≤2) group with 52.6%.Conclusion The EP mapping and GPsablation techniques during the minimally invasive AF ablation surgery could get good sinus rhythm rate during the long term follow up.
6.Effects of the amphiphilic peptides on membrane permeability of Acinetobacter baumannii

Objective To investigate effects of the amphiphilic peptides on membrane permeability of acinetobacter bauman-nii .Methods The LysAB3 and LysAB3-D (lack of amphiphilic peptides structure gene) was synthesized and inserted into the vec-tor pET28a(+ ) to construct the recombinant expression plasmid (pET28a-LysAB3 ,pET28a-LysAB3-D) .After expression in E . coli BL21(DE3) and purification with Ni2+-NTA Sepharose .Acinetobacter baumannii was observed by scanning electron microsco-py and fluorescence microscope ,pretreated with LysAB3 and LysAB3-D respectively .Results Under scanning electron microscopy , LysAB3-treated acinetobacter baumannii exhibited not only significant abnormalities ,including deep roughening of the cell surface , but also FITC readily accumulated in bacteria .It was different from LysAB3-D-treated .Conclusion These results indicate that the amphiphilic peptides structure increase membrane permeability of acinetobacter baumannii ,which helps LysAB3 degrade bacteria .

Objective To investigate the life quality and influencing factorin stroke patient.MethodTotally 205 hospi-talized stroke patientin the neurology departmenwere investigated with SF-36 scale .The datwere performed the descriptive sta-tistic,t-tesand multiple linearegression analysi.ResultThe scoreof general healthy statu,physiological function ,physiolog-ical role ,somatipain and social function in the patientwith stroke were significantly lowethan thain the national norm (P<0 . 01) .The recurrence frequency ,age ,comorbidity ,education level and income were significanfactoraffecting the life quality in the stroke patien.Conclusion The level of general health statu,physiological function ,physiological role ,somatipain and social function are decreased significantly .Therefore iirecommended to perform the pertinenintervention in the clinical work .

9.Effects of solution-focused brief therapy on self-perceived burden and quality of life in patients with stroke

Objective To investigate the effects of solution-focused brief therapy on self-perceived burden and quality of life in patients with stroke.methods 60 patients with stroke recruited at the department of rehabilitation of a hospital in Chengde city from January to June 2016 were divided into control group (n=30) and experimental group (n=30).Both groups received conventional nursing care and rehabilitation treatment.In addition,the experimental group received psychological intervention of solution-focused brief therapy,2-3 times a week,30-45 minutes for each time.The self-perceived burden scale(SPBS),memorial university of Newfoundland scale of happiness(MUNSH) and short from 36 health survey questionaire(SF-36) were used to assess the patients before and after intervention.Result s(1)The differences of the scores of SPBS ((39.27±5.83) vs (22.40±4.03),t=16.248,P<0.01) and MUNSH ((4.20±4.50) vs (31.10±3.66),t=-24.136,P<0.01) for the experimental group before and after the intervention were statistically significant.There were statistically significant differences in scores of SPBS ((22.40±4.03) vs (35.30±2.63),t=-14.683,P<0.01) and MUNSH ((31.10±3.66) vs (19.67±3.64),t=12.127,P<0.01) between the experimental group and control group after the intervention.(2)The scores of physiologic functioning,general health,vitality,social functioning,role-emotional and mental health for the experiment group were higher than before(t=-23.427--6.624,all P<0.01).After the intervention,the scores of physiologic functioning,general health,vitality,social functioning,role-emotional and mental health in the experiment group were higher than those in control group(t=5.705-12.806,all P<0.01).Conclusion Solution-focused brief therapy can effectively reduce the self-perceived burden of stroke patients and improve their quality of life.
10.Role of tetramethylpyrazine in protection of homocysteine-induced hippocampal neural apoptosis in mice

Objective To explore the role of tetramethylpyrazine (TMP) in protection of homocysteine (HCY)-induced apoptosis of hippocampal neurons in mice. Methods One hundred healthy adult Kunming mice of clean grade were randomly divided into 5 groups, 20 for each group. HCY was injected into lateral ventricle using stereotaxic technique. FJB staining was used to detect neural apoptosis and the Morris water maze test was applied to assess memory and cognitive abilities. Results FJB staining showed that apoptosis of hippocampal neurons in the mice treated with TMP at different doses decreased significantly as compared with that in HCY group, especially at a dose of 20 g/L. Conclusions TMP can alleviate HCY-induced damage of the hippocampal neurons in mice and improve memory and cognitive functions, which suggests TMP has a potential clinical value in prevention and treatment of Alzheimer′s disease.
