OBJECTIVE: To analyze the requirement of pharmaceutical care among physicians, nurses and patients and to explore work mode of clinical pharmacists. METHODS: The requirement of pharmaceutical care in concrete cases was summarized by combining with experience of author as clinical pharmacist in cardiology department. RESULTS: The advantage of clinical pharmacist and convenient consultation for selection of drugs contribute to complementary with physicians’ work. CONCLUSION: Clinical pharmacist provides various pharmaceutical care according to the requirement of doctors, nurses and patients.
4.Clinical effect of itraconazole oral solutions on pulmonary candidiasis albicans in ICU patients

Objective To evaluate the clinical effect of itraconazole oral solutions on pulmonary candidiasis albicans in ICU patients.Methods A total of 71 patients with candida ablicans in their sputum clutures were randomized into two groups.The itraconazole group(n= 36)received itraconazole 200 mg,p.o.(including nasal feeding),q12h for 14 days.The fluconazole group(n=35)received fluconazole 400 mg for the first dose,then 200 mg,i.v.drip,qd for 14 days.Results There were 31 patients in the itraconazole group and 29 paitents in the fluconazole group who completed their treatment.In the itraconazole and fluconazole group,the clearance of candida ablicans was 64.52%(20/31)vs.65.55%(19/29),cleaning time(10.12?2.57)vs(8.87?1.95)days,effective rate 58.06%(18/31)vs 62.07%(18/29),recovery rate is 19.35%(6/31)vs 17.24%(5/29),occurrence of hepatic dysfunction is 25.81%(8/31)vs 24.14%(7/29),and withdrawal for inefficacy was 3.23%(1/31)vs 6.90%(2/29),respectively.These parameters did not have statistical significance.Conclusion Itraconazole oral solutions can be used effectively and safely in the treatment of pulmonary candidiasis albicans in ICU.Patients with renal dysfunction do not need to adjust the dosage.
5.Correlation bewteen associated genes and biofilm phenotype in Staphylococcus epidermidis

Objective To investigate the relationship of icaA,aap ,atlE,sarA gene with biofilm phenotype in Staphyloccus epi-dermidis .Methods Tissue culture plates assays were used to assess biofilm-forming ability of 78 Staphyloccus epidermidis strains.The presence of icaA,atlE,aap ,sarA gene was amplified with PCR method,the relationship between these genes and bio-film phenotype was evaluated by χ2 test and the difference of the biofilm OD′s value in TSB and TSB+3% NaCl by icaA + strains and icaA - strains was evaluated by Wilcoxon sign test,respectively.Results The positive rates of icaA,atlE,aap ,sarA gene were 24.4%(19/78),79.5%(62/78),73.8%(57/78),82.1%(64/78),respectively.40 biofilm-positive strains were detected(51.3%), among which 16 strains carried icaA gene,24 strains showed weak biofilm-forming ability.Those genes above with biofilm forma-tion were statistically correlated.Moreover,there was a relation between the icaA gene and the high biofilm-producing phenotype. There was a significant difference between the biofilm OD values in TSB and TSB+3% NaCl by icaA + strains and icaA - strains, respectively.Conclusion Multiple genes are involved in Staphylococcus epidermidis biofilm-positive phenotype,but the icaA gene contributes to the high biofilm-forming phenotype.Biofilm phenotype is also influenced by environmental factors.
7.Analysis of the effects of mobile payment in outpatient service

Objective To learn the applications and patient experience of mobile payment technology as used in the hospital outpatient service.Methods 342 outpatients using mobile technology (APP)were chosen from the hospital information system for survey,and 300 outpatients registering in the conventional process were chosen as a control.Data of the doctor-seeing experiences of the two groups were analyzed statistically.Results Waiting queue reminder function is found with the highest patient usage among the three officially launched APPs,accounting for 83.6%.The appointment and cellphone payment functions exert the greatest impact on patient experience,boasting a patient satisfaction of 90%. All these functions can save totally one hour for an outpatient.Conclusions The mobile payment technology can significantly reduce outpatient waiting time,and improve outpatient experiences.Such technology therefore needs more promotions and connection with medical insurance system for the sake of outpatients.
8.Compatibility Rules of Traditional Chinese Medicine in Prevention and Treatment of Scleroderma Based on Data Mining

This study was aimed to analyze compatibility rules of Chinese drugs and prescription design in the pre-vention and treatment of scleroderma. The database of scleroderma treated by Chinese medicine was set up. And then, association rules were used to analyze the prescription compatibility rules. The results showed the highest fre-quency of drugs in the treatment of scleroderma were Astragalus, Angelica sinensis and Salvia. The tonifying and re-plenishing medicinal, blood-activating and stasis-resolving medicinal and exterior-releasing medicinal were with the highest frequency. The commonly used formula in the treatment of scleroderma was Y ang-He decoction. For herbal groups, occurrence of the highest was the combinations of Ramulus Cinnamomi-Astragalus and Carthamus tinctorius L.-Astragalus. Three kinds of herbals were commonly coupled, such as Polygonum multiflorum Thunb. and Caulis Spatholobi with Salvia, P. multiflorum Thunb. and R. Cinnamomi with Salvia, P. multiflorum Thunb. and Astragalus with Salvia, prepared Radix Rehmanniae and Codonopsis with C. tinctorius L., prepared Radix Rehmanniae and Codonopsis with R. Cinnamomi, Codonopsis and Radix paeoniae rubra with R. Cinnamomi. Four kinds of herbals such as R. Cinnamomi, Glycyrrhiza uralensis Fisch., Astragalus and Codonopsis were commonly coupled. It was con-cluded that compatibility rules of Chinese medicine in the prevention and treatment of scleroderma laid the founda-tion for herbal medicine selection and prescription design.
9.Clonality analysis for differentiating multicentric origin and intrahepatic metastasis in multiple hepatocellular carcinomas

Objective To explore the differential diagnostic significance of clone analysis for multicentric occurrence (MO) and intrahepatic metastasis (IM) in hepatocellular carcinomas (HCCs).Methods Loss of heterozygosity (LOH) and microsatellite instability (MSI) were analyzed by microsatellite polymorphism test and the integration sites of HBV were assessed by Southern blot in each nodule of the HCCs. The clonalities were then compared between each nodule of the same patient and the diagnosis of MO or IM was concluded. Finally, the results based on clonality analysis were compared with those according to clinicopathological and imaging data. Results According to the results of LOH and MSI in 79 nodules and nontumorous tissue from 35 cases of mutiple HCCs, 5 (14.3%)and 29 cases (82.9 %) were divided into MO and IM, respectively. Both MO and IM presented simultaneously in 1 patient (2.9%). The integration sites of HBV could be analyzed in 77 nodules from 34 multiple HCCs. Among them, 6 (17. 6%) and 27 cases (79.4%) were divided into MO and IM, respectively. Both MO and IM presented simultaneously in 1 patient (2.9%). The classification results of microsatellite polymorphism test and HBV integration sites analysis demonstrated a significant positive correlation (rs = 0.909, P<0.001). Nevertheless, neither the classification of microsatellite polymorphism test nor that of HBV integrate sites analysis was correlated with the discrimination according to clinicopathologic and imaging data (rs=0. 133, P=0. 468, rs =0. 262, P=0. 155, respectively). Recurrence in patients in the MO group occurred significantly later than that in IM cases who were diagnosed by clonality analyses (P=0. 001). Conclusion The clonality analysis based on the results of LOH and MSI or assessments of HBV integrate sites is useful for the differential diagnosis of MO and IM and their treatment and prognosis.
10.Study on hNIS mediated transferring gene99mTcO-4 imaging and mI treatment in xenografted ovariancancer

Objective To establish radioiodine therapy in nonthyroid tumor and to investigate 131Ⅰ treatment effect on xenografted ovarian cancer. Methods Based on previous test, xenografted ovarian cancer nude model were established in nude mice. The effects of radioactive isotope 99m TcO-4 imaging and radioiodine 131Ⅰ treatment on xenografted ovarian cancer in vivo were investigated. Results After transferring human sodium/iodide symporter (hNIS) gene, the xenografted ovarian cancer in nude mice was imaged by isotope 99m TcO-4 Moreover,131Ⅰ exerted inhibitory effect on the proliferative activity. Conclusion After the transfection of hNIS gene, 131Ⅰ has inhibitory effect on proliferative activity of xenografted ovarian cancer.
