1.Comparative study on the effect of rt-PA and urokinase on the expression of MMP-9 and hemorrhagic transformation in a rat embolic stroke model

Objective:To study the influence of rt-PA and urokinase on the expression of matrix metalloproteinase(MMP-9) and hemorrhagic transformation after thrombolysis therapy in rats.Methods:A focal cerebral ischemia model of rat was established through embolization of left middle cerebral artery with autologous blood clots.rt-PA and urokinase were intravenously administrated 6 hours after ischemia at a dose of 10 mg/kg and 4 000 ?/kg.At the 24th hour after ischemia,all rats were killed.The expression of MMP-9 was detected with immunohistochemical staining and cerebral hemorrhage was quantified by spectrophotometer assay.Results:The expression of MMP-9 was increased after ischemia,and MMP-9 was significantly further upregulated after rt-PA and urokinase had been intravenously administrated.The average optical density(AOD) of MMP-9 in the sham operated group was 0.086?0.008,and increased to 0.158?0.010 after cerebral ischemia for 24 hours and then further increased to 0.225?0.034 and 0.228?0.022 in the rt-PA and urokinase thrombolytic therapy groups.Transformation and quantified intracerebral hemorrhage were significantly higher in the therapy groups as compared with the ischemic group.However,there was no difference between rt-PA group and urokinase group.Conclusion:rt-PA and urokinase treatment both increase the levels of MMP-9 after ischemia.MMP-9 might be involved in the hemorrhagic transformation after thrombolytic therapy with rt-PA and urokinase.
2.The clinical significance of CD46 expression after neoadjuvant chemotherapy for the prognosis of ad-avnced breast cancer patients

Objective The purpose of this study is to investigate CD 46 expression in advanced invasive breast cancer and in normal breast tissues , and to analyze the correlation between CD 4 expression and effect of neoadjuvant chemotherapy ,exploring its clinical significance for the prognostic value of neoadjuvant chemotherapy in breast cancer .Methods Immunohistochemistry was employed to detect CD 46 expression of tissue samples from 60 patients with invasive breast cancer and 30 normal breasts .We further analyzed the relationship between effect of neoadjuvant chemotherapy and CD 46 expression ,and the prognostic significance for the survival .Rseults CD46-positive rate in breast cancer was apparently higher than that in normal breast tissues (81.67% vs. 46.67%,P<0.05).After neoadjuvant chemotherapy,the CD46 expression was significantly decreased (P <0 .05).There was a negative correlation between CD 46 expression and efficacy of chemotherapy ( P<0.05). CD46 expression in breast cancer could be affected in cancer invasion ,lymph node metastasis(P<0.05).Con-clusion CD46 expression in breast cancer patients is higher than that in healthy people ,and was negatively cor-related with efficacy of adjuvant chemotherapy .The increasing rate of CD 46 positive expression has predictive val-ue for the poor prognosis of patients .
3.Efficacy comparison of two different surgery incisions on original corneal astigmatism in cataract surgery

Objective To compare the efficacy of two different surgery incisions on original corneal astigmatism in cataract surgery. Methods 300 patients (300 eyes) of cataract surgery from August 2012 to August 2013 in our hospital were included in the study,of which 150 patients in the observation group treated with transparent cornea incision in the steep axis of astigmatism,and the control group of 150 pa-tients received lateral ( right) or on the nose by the temporal side ( left) transparent cornea incision. Results In the third day,first month, third month,sixth month and 12th month after surgery,the average corneal astigmatism in the observation group was significantly lower than that before the surgery,which after a month the lowest ( +0. 79 ± 0. 92)D;average corneal astigmatism of patients in the control group were significantly higher than that before the surgery,corneal astigmatism after 3 days to reach the highest,( -5. 65 ± 0. 89)D,3 days after sur-gery,1 month, 3 months, 6 months and 12 months,patients in the observation group were significantly higher visual acuity patients,the differ-ence was statistically significant (P<0. 05). Conclusion Phacoemulsification clear corneal incision along the steep axis IOL implantation for cataract surgery can correct astigmatism original extent a certain extent,and to promote the recovery of visual acuity in patients with a grea-ter degree.
4.Advances in molecular genetic study on pathological myopia relevant gene loci

Pathological myopia(PM)is one of the global leading causes of blindness because of its severe optic complications.Previous researches implicated that hereditary factors play crucial roles in the pathogenesis of PM,and plenty molecular genetic studies,such as gene mapping and candidate gene screening,have been done to identify gene loci linked to PM.The newest advances in molecular genetic study on pathological myopia relevant gene loci were reviewed,including non-syndrome PM and system disease associated with PM.

6.Treatment of Brachial Plexus Injury by Exterior- and Interior Meridian Acupoints

Exterior-and interior-meridian acupoints were used, mainly by selection of local acupoints along meridian. According to the state of disease, electroacupuncture and acupoint injection were combined to treat brachial plexus injury. Among 12 patients, 6 cases were basically cured, 4 cases got effectiveness and 2 cases had no effectiveness. The therapeutic effect of exterior- and interior-meridian acupoints in brachial plexus injury was satisfactory.
7.Y-shape plate in the treatment of comminuted fractures of the distal humerus among 18 cases

AIM: To observe the adverse effect and clinical efficiency of Y-shape plate internal fixation for treating comminuted distal humeral fracture. METHODS: A retrospective analysis was conducted in 18 patients with the comminuted distal humeral fracture,who were admitted in the First Affiliated Hospital of Nanyang Medical College from January 2003 to December 2006. All patients were treated with Y-shape stainless steel or Ti-alloy plate in 3-7 holes,then the response of materials and host was observed. The curative efficiency was assayed with Cassebaum score,and the healing of fractures was detected with X-ray film. During follow-ups,the pain and joint motion were observed. RESULTS: The mean operational time was 134 minutes,the mean value of intraoperative blood loss was 172 mL,and all the incisions were healed well. All the patients were followed up for 6 months,and the mean time for the fracture healing was 14 weeks. One case appeared delayed healing in the resection lesion,and Kirschner's wire was loosened in 2 cases. There was no infection,local inflammatory reaction or rejection reaction occurred. Among 18 patients,11 got excellent results,4 good,2 fair,1 poor,and the rate of excellent and good efficiency was 83.3%. CONCLUSION: Y-shape plate after implantation shows good biocompatibility with the human body,with the manifestations of good reduction of comminuted distal humeral fracture,firm internal fixation,and satisfactory functional recovery of elbow joint.

9.Progress of hematopoietic stem cell transplantation for treatment of multiple myeloma in the era of novel agents

Hematopoietic stem cell transplantation (HSCT) has become the frontline therapy for patients with multiple myeloma (MM), and for young patients with primary MM, early autologous stem cell transplantation (ASCT) is a standard treatment. In the era of novel drugs, the application of new pharmaceutics in the consolidation and maintenance therapy before and after HSCT can improve the efficacy of HSCT. The novel agents pose a challenge to the status of HSCT in relapsed and refractory myeloma. This article summarizes the recent clinical studies of HSCT in the era of novel agents to clarify the role of HSCT in treatment of MM.
