1.Characteristic of class 1 integron-cassette among Shiga toxin-producing E. coli isolates in China

Objective To explore the distribution and characteristic of class 1 integrons among Shiga toxin-producing E.coli(STEC) in China,and to elucidate the type of cassettes.Methods Antibiotics susceptibility was tested by the disk diffusion method,class 1 integron was detected by PCR assay and PCR products were sequenced and analyzed in eight isolates.Results Four isolates were multiple-drug resistant whose antibiogram were ampicillin,tetracycline,erythromycin,sulfamethoxazole/trimethoprim and ciprofloxacin,but two isolates conferred resistance of streptomycin and spectinomycin simultaneously.Only the two isolates carried class 1 integrons ranging in 1 000 bp which located on the plasmid of 7 800 bp.The sequenced PCR product demonstrated that the 1 000 bp integron harbored aad A1 gene cassettes conferred the resistance of streptomycin and spectinomycin;and linked with sul 1 and qacE?1 gene conferred the resistance of sulfamethoxazole and quaternary disinfectants.Conclusion The class 1 integrons exists among STEC isolates in China and determines the resistant antibiotics.
2.Femoral artery ligation in treatment of infected femoral artery pseudoaneurysm caused by addictive drugs injection

Objective To summarize our experience in management of infected femoral artery pseudoaneurysm caused by addictive drugs injection.Methods Clinical data of 45 cases of infeted femoral artery pseudoaneursysm were reviewed retrospectively.When infected femoral artery pseudoaneurysm was diagnosed,the femoral artery or external iliac artery was ligated and the pseudoaneurysm was resected in all of cases.Anticoagulants and antibiotics were used after operation.Results Lower limbs were salvaged in 44 patients.The ambulation function of affected limb was fine in the short-term.One limb was amputated because of lower limb deep venous thrombosis and gangrene.Thirty-four patients(75.6%) were followed up for 3-12 months,the results were satisfactory.Conclusions Femoral artery or external iliac artery ligation is an effective procedure for management of infected femoral artery pseudoaneurysm.

4.Effectiveness of mucolytic agent treating with chronic rhinosinusitis:a randomized clinical research

Objective To investigate the effect of mucolytic agents during endoscopic sinus surgery.Methods 84 patients of chronic rhinosinusitis were employed in the study,which were randomly divided into the control group(41 subjects) and study group(43 subjects) according to the digital table.The patients in the control group adopted regular therapies,and study group were received regular therapies as well as mucolytic agents(eucalyptol-limonene-pinene soft capsule,300 mg,three times a day) application for 8 weeks.Followed visiting lasted for 6 months.The VAS scores,Lund-Mackay scores and efficient rate were compared.Results The VAS scores and Lund -Mackay scores in the control group after treatment were (3.63 ± 1.71)points and (8.34 ±4.68) points,and which in study group were (2.03 ± 1.67) points and (6.48 ± 3.59) points,and the difference was statistic significant (t =4.338 and t =2.049,P < 0.05).According to CPOS-2012,in the control group,19 cases were controlled,14 cases were partly controlled,and 8 cases were uncontrolled,the whole efficient rate was 80.5%.in the study group,24 cases were controlled,17 cases were partly controlled,and 2 cases were uncontrolled,the whole efficient rate was 95.3%.Compared to the control group,the efficient rate of the study group was higher,the result showed statistical significance (x2 =4.420,P < 0.05).Conclusion Using mucolytic agent after surgery can improve the controlled rate and affect the success of treatment of chronic rhinosinusitis.
5.Study on Improvement of Quality Standard of Shuangshen Capsule

OBJECTIVE:To improve the quality standard of Shuangshen capsule. METHODS:TLC was conducted for the qual-itative identification of Curcumnae radix and Rehmannia glutinosa in the preparation. HPLC was used for the content determination of salvianolic acid B in the preparation:the column was Diamonsil C18(2)with mobile phase of acetonitrile-0.1% H3PO4(22:78, V/V) at a flow rate of 0.5 ml/min,detection wavelength was 286 nm,column temperature was 30 ℃,and the injection volume was 2 μl. RESULTS:The TLC spots of C. radix and R. glutinosa were clear and well separated;the linear range of salvianolic ac-id B was 0.0792-0.792 μg(r=0.9999);RSDs of precision,stability and reproducibility tests were lower than 1%;recovery was 100.71%-101.82%(RSD=0.50%,n=6). CONCLUSIONS:The established standard can be used for the quality control of Shuang-shen capsule.
6.Inhibitory effect of antisense oligodeoxynucleotide c-myc on the growth of lens epithelial cells induced by galactose

Objective To investigate the effects of antisense oligodeoxynucleotide c-myc (c-myc ASODN) on the growth of rabbit lens epithelial cells (LECs) induced by galactose. Methods Based on the translation initiation region of the second extron of c-myc gene, oligonucleotides were synthesized and modified with phosphorothioate. Lens epithelial cells were treated with galactose and c-myc ASODN. The effects of c-myc ASODN on the growth of LECs, cultured in high concentration of galactose, were observed by cytometry and MTT. Results The results demonstrated that c-myc ASODN at the doses of 2.5-10.0 ?mol/L inhibited the growth of LECs induced by galactose in a dose-dependent manner. Conclusion These results indicate that c-myc ASODN can inhibit the growth of LEC induced by galactose.
7.Clinical effects of needle free (INJEX30) versus traditional injection of botulinum toxin A for bromhidrosis

Objective To investigate the clinical efficacy of bromhidrosis with botulinum toxin A by needle free injection.Methods A total of 40 patients with bromhidrosis were collected and divided into needle free group and needle group.A comparative study was carried out in those subjects.Patients in needle free group received needle free injection of botulinum toxin A by INJEX30;Needle group received traditional injection by 26 G needles.Results In needle free group,16 cases were cured,3 cases were effective,1 case was ineffective,no significant pain,hematomas or necrosis were found.In needle group,15 cases were cured,4 cases were effective,and 1 case was ineffective;localized hematoma occurred in 2 cases.And cases in group needle experienced pain more intensely than cases in needle free group.Conclusions The INJEX30 improves the security,comfort and therapy compliance of botulinum toxin A injection in the bromhidrosis patients,with similar efficacy with traditional injection.
8.Inhibiting effects of three components of Astragalus membranaceus on oxidative stress in Chang Liver cells.

The main objective of this research is to investigate the effects of astragaloside IV, calycosin separately glucoside, formononetin on oxidative stress in Chang Liver cells induced by H2O2. In the experiments, Chang Liver cells (a kind of normal human hepatocytes) were used as the research object, bifendate which has a clear hepatoprotective effect was used as the positive control drug, then the oxidative damage model of Chang Liver cells were established by H2O2. Cells were divided into six groups: blank control group, oxidative stress group, astragaloside IV group, calycosin separately glucoside group, formononetin group and positive control group. Then endogenous antioxidant system related indexes were detected by micro plate and colorimetric method; intracellular reactive oxygen species (ROS) were detected by DCFH-DA fluorescent probe; and the expressions of CYP2E1 were evaluated by liver microsomes, mRNA, and protein, respectively with spectrophotometry, Real-time PCR method, and Western blot technique. Results showed that H2O2 decreased antioxidant activity, and increased ROS level and expression of CYP2E1. The above oxidative stress status had been changed with protections of the three components of Astragalus membranaceus (compared with oxidative stress group, P < 0.05, P < 0.01), which taken as a whole had equivalent effects as the drug of positive control group( bifendate). Taken together, three Astragalus membranaceus ingredients all had significant or extremely significant inhibiting effects on oxidative damaged Chang Liver cells which were induced by H2O2, and the oxidative damage of Chang Liver cells had been relieved.

9.Factors affecting the success rate of artificial insemination with donor sperm.

OBJECTIVETo explore various factors affecting the clinical pregnancy outcomes of artificial insemination with donor sperm (AID).
METHODSWe retrospectively analyzed 15,744 cycles of AID in 6302 women and investigated the association of the clinical pregnancy outcomes of AID with the treatment protocols, the times of insemination per cycle, the age of the infertile women, the status of the oviduct, and the number of AID cycles.
RESULTSThe pregnancy rate of AID was higher in the chlomiphene-treated women than in those of the natural cycle group (P = 0.003) but showed no significant differences either between the chloramiphene and human menopause gonadotropin (HMG) or between the HMG and natural cycle groups (P > 0.05), and so was it in the women that had received AID twice per cycle before and after ovulation (26.3%) than in those that had undergone only once before (7.0%) or after ovulation (23.7%) (P < 0.05). However, the pregnancy rate was remarkably lower in the women aged 35-40 years (16.5%), especially in those over 40 years (1.2%), than in those under 35 years (26.0%) (P < 0.05). There was no significant difference in the success rate of AID between the women with oviductal adhesion and those without (27.4% vs. 28.1%, P > 0.05). The pregnancy rate of the first cycle of AID (27.6%) was markedly higher than those of the second (24.7%), third (23.9%), and fourth (23.1%) (P < 0.01), but with no significant differences among the latter three cycles (P > 0.05), while that of the fifth cycle (19.0%) was remarkably lower than those of the first four (P < 0.01).
CONCLUSIONThe age of the infertile women is an important factor affecting the success rate of AID. AID twice per cycle is better than once only. For those without oviductal factors, at least 4 cycles of AID are required before in vitro fertilization.

10.Monoclonal gammopathy of undetermined significance with systemic lupus erythematosus: a case and literature review

Objective To better understand the clinical features and the diagnosis of monoclonal gammopathy of undetermined significance (MGUS) associated with systemic lupus erythematosus (SLE).Methods A case of MGUS with SLE were described including clinical manifestations and pathologic data.Literatures were also reviewed.Results The patient was admitted because of proteinuria.Laboratory findings showed monoclonal gammopathy.However,both bone marrow exam and iconography showed no signs of multiple myeloma.Lupus nephritis Ⅳ + Ⅴ was proved by kidney biopsy.Prednisone and tacrolimus were used with significant clinical improvement.Conclusion MGUS associated with SLE is not rare.MGUS criteria based on 2003 international MM working group should be used to differentiate MGUS from MM.Monoclonal protein level,plasma cell in bone marrow and free light chain are risk factors for MM progression.Treatment is based on lupus disease activity and organ damage severity.
