Objective To evaluate the effect of excise training on the pulmonary function in patients with chronic obstructive pulmonary disease(COPD).Methods Totally 55 COPD patients received excise training,the patients’pulmonary function,6-minute walking test(6MWT)and St.George's respiratory questionnaire(SGRQ)were measured before and after the 6 months intervention. Result The pulmonary function,6MWT(exercise tolerance)and the quality of life were significantly improved(all P<0?01). Conclusions Excise training can improve COPD patients’exercise tolerance,relieve the symptoms of dyspnea,and thereby improve patients’quality of life?

Objective The objective of this study was to observe the short-term and long-term curative effects of minimally invasive surgical treatment combined with chemotherapy of localized nasopharyngeal carcinoma in patients,and to clarify the necessity of combination therapy in patients with nasopharyngeal carcinoma.Methods Sixty-two patients with nasopharyngeal carcinoma were enrolled in our hospital from March 2011 to September 2011.The patients were divided into the observation and control groups by random number table method.The patients in observation group were given the application of minimally invasive surgery with postoperative combined chemotherapy with cisplatin and 5-fluorouracil after nasal endoscopic resection.The patients in the control group were given single application of minimally invasive surgical treatment of nasal endoscopic resection.The patients in two groups were observed the efficiency,survival rate and postoperative quality of life after surgery for 1 year or 5 years.Results The effective rate was 83.87％ and 100％ for the survival rate in the observation group after surgery for 1 year;the effective rate was 58.06％ and 87.10％ for the survival rate in the control group.There was significant different between the observation and control groups (P ＜ 0.05).The survival and survival rates were significantly higher than those in the control group after surgery for 5 years (P ＜ 0.05).After surgery 1 year,the scores of the patients in the observation group were significantly higher than those in the control group (P ＜ 0.05).Conclusion The combination of cisplatin and 5-fluorouracil in the treatment of minimally invasive endoscopic nasopharyngeal resection combined with postoperative chemotherapy can significantly improve the short-and long-term survival rates,long-term effective rate,and quality of life in tumor patients after surgery.This is suitable for clinical application.

Objective To quantify iron deposition in Alzheimer’s disease (AD),amnestic mild cognitive impairment (aMCI), and normal old people using susceptibility weighted imaging (SWI).Methods Sixty participants (22 aMCI,20 AD,18 normal con-trols)underwent conventional magnetic resonance imaging sequences and SWI using axial/oblique coronal sequences.Phase images were used to calculate bilateral iron deposition in 1 8 regions of interest (ROI)of every participant.The radian angle values were cal-culated and compared between the three groups.Results The radian angle value difference was significant between the aMCI and control groups in the left (L)-hippocampus,L-head of the caudate nucleus,R/L-lenticular nucleus ROIs.The radian angle value difference was significant between the AD and aMCI groups in the R/L-cerebellar hemisphere,R/L-hippocampus,R-red nucleus, R/L-thalamus and splenium of corpus callosum ROIs.Pearson correlation coefficients of the mini-mental state examination (MMSE)scores and many ROIs were significant.Conclusion Iron depositions in the hippocampus,head of the caudate nucleus,len-ticular nucleus and thalamus are significantly different between individuals with aMCI,AD and controls.Using SWI to quantify the iron deposition is a useful tool in detecting aMCI and AD.

Background Conventional chemical method to fix the entire eyeball results in the edema of retinal nerve fiber layer and therefore influent the evaluation of tissue structure.Research showed that microwave irradiation can avoid this phenomenon. Objective This study was to investigate the appropriate energy and time duration of microwave irradiation for fixation of rabbit eyes specimen and compare the influence of different fixation methods on retinal structure. Methods Twenty-two New Zealand white rabbits were sacrificed by using venous air embolism.The eyeballs were enucleated from the rabbits and immersed in chemical reagents with glacial acetic acid,methyl aldehyde and chloroform for 2 days as the control group.The eyeballs were immersed in 400 ml physiological balance solution immediately,and then were fixed by microwave irradiation for 80,160,240 and 320 seconds under the lower power as the microwave irradiation groups.In the microwave irradiation+chemical fixation group,the specimens were immersed in chemical reagents for 1-2 hours after the microwave radiation.The morphology of retinal structure was examined and area of retinal detachment was calculated by hemotoxylin and eosin staining under the light microscope. Results A better fixation effect was obtained in the retinal section of microwave radiation for 240 seconds in comparison with that of the traditional chemical reagent fixation method for 2 days.The retina detachment ratio was 16.3%±11.5% for simple microwave method,50.0%±24.5% for chemical method,and 6.7%±7.8% for microwave+chemical method.showing a significant difference among 3 groups(F=32.43,P=0.000).Retinal staining was clear and retinal structure was almost normal in the specimens of microwave+chemical fixation group with 200-240 seeonds microwave irradiation and 1-2 hours chemical fixation. Conclusion Microwave irradiation method is a more ideal way for the fixation of retina because of taking short duration,lower toxicity and better staining.The combination of microwave radiation and chemical reagents can acquire an excellent quality of retinal section.

Objective To investigate the effect of triiodothyronine (T3) on neurogenesis in after bilateral common carotid artery occlusion in adult rats. Methods Fifteen adult male Sprague-Dawley (SD) rats were randomly divided into sham operation, 2-vessel occlusion (2VO) and T3 intervention groups (n = 5 in each group ). A chronic cerebral ischemia model was established by the bilateral common carotid artery permanent ligation. Seven days after modeling 5-bromo-2'-deoxyuridine (BrdU) was injectied intraperitoneally and the new proliferating cells were labeled. The BrdU positive cells in SVZ were observed using immunofluorescence staining.Results The numbers of BrdU positive cells in the sham operation, 2VO, and T3 intervention groups were 174. 8 ± 18. 5, 325.0 ± 51.0, and 499. 4 ± 30. 8, respectively. There were significant differences among all the groups (F = 101.68, P ＜ 0. 001 ). The numbers of BrdU positive cells in the T3 intervention group were significantly higher than those in the 2VO and sham operation groups (all P ＜0. 001 ). The numbers of BrdU positive cells in the 2VO group were also significantly higher than those in the sham operation group (p ＜0. 001). Conclusions Thyroid hormones may promote neural progenitor cell proliferation in SVZ in adult 2VO rats.

Nicotinamide phosphoribosyltransferase (NAMPT) is the rate-limiting enzyme in the salvage pathway for the biosynthesis of nicotinamide adenine dinucleotide (NAD+) from nicotinamide. NAMPT is also a cytokine that inhibits the apoptosis of neutrophils under various inlfammatory stimuli, regulates various diseases and closely associates with the progression and prognosis of cancers. However, it is still not clear whether the cytokine-like function of NAMPT is interrelated with the biosynthesis enzyme activity of NAD+. This article aims to provide novel insights for inflammation and cancers treatment by reviewing the function of NAMPT in inflammation, carcinogenesis, cancer progression and its inhibitors, APO866/FK866.

Objective To investigate the feasibility, indications, operating technique and the clinical value of laparoscopeic surgery for ectopic pregnancy with prior history of obstetric and gynecologic operations. Methods 82 ectopic pregnancy patients with prior history of the obstetric and gynecologic operation were subjected to laparoscopic surgery. Among them, 60 patients had undergone prior obstetric and gynecologic operation only once, 21 patients had undergone twice, and the remaining one had undergone obstetric and gynecologic operation three times. The laparoscopic operation was completed in different ways as follows: puncture and injection of drug under laparoscopy, salpingectomy, resection of pregnancy in oviduct, removal of embryo through incision of oviduct under laparoscope, suction removal of embryo from oviduct, and new technique for pregnant surgery at interstitial portion. Results Of these 82 patients undergoing laparoscopic surgery, 73 cases had adhesion in different degree, and the adhesion rate was 89.0% (73/82). In the 60 patients with history of one operation, pelvic peritoneal adhesion was found in 51 patients, the adhesion rate was 85.0%. All 22 patients with two prior operations had pelvic peritoneal adhesion, and the rate was 100.0%. The laparoscopic surgery was successful in 70 cases. The rate of success was 85.4%, while in 12 patients, the operation was changed into laparectomy due to dense adhesion, and the rate was 14.6%. Of these 12 patients, in 5 of them there was endometriosis, in 6 patients chronic pelvic inflammation was found, 9 patients had a surgical history for ectopic pregnancy, 2 patients with the history of caesarean section, and one patient had a history of complicated gynecologic operation. Complications included colonic injury in one patient with ectopic pregnancy, and the operation was changed to laparotomy. Injury to the ureter happened in one patient. The operation time for laparoscopic surgery was 30-180 min. Conclusions The laparoscopic operation is an effective procedure and it has become a routine procedure for ectopic pregnancy with prior history of obstetric and gynecologic operation. Because of chronic pelvic inflammation and endometriosis, it is difficult to perform laparoscopic surgery successfully, and laparotomy should be anticipated at all time.

Objective To explore the influences of different inductive methods on cartilage repair by tissue engineered cartilage with the seed cell of mesenchymal stem cells.Methods Rabbit mesenchymal stem cells were divided into dexamethasone-inducing group and TGF-?_1-dexamethasone co-inducing group.The cartilage defects were repaired by autologous tissue engineered cartilage constructs.The defects of control group were filled with scaffold without cells.Specimens were harvested 6 and 12 weeks postoperatively and assessed by histological grading and in situ detection of apoptosis.Results The repair tissue formed perpendicular column structure resembling that of normal cartilage in TGF-?_1-dexamethasone co-inducing group.The histological score 12 weeks postoperatively was higher in co-inducing group(20.26?1.35) than those in dexamethasone-inducing group(14.52?1.46) and control group(4.12?1.13).But the formation of tidemark was not observed in the repair tissue.Cells of repair cartilage at the bone-cartilage interface showed apoptosis.The ratios of apoptosis in dexamethasone-inducing group were(21.4?4.5) six weeks postoperatively and(7.3?2.2) twelve weeks postoperatively.The ratios of apoptosis in TGF-?_1-dexamethasone co-inducing group were(19.8?4.7) six weeks postoperatively and(6.9?2.0) twelve weeks postoperatively.The ratios of apoptosis at 6 th week were higher than those at 12 th week postoperatively with statistical significance(P

Objective To observe the clinical effect of acupuncture combined with ganglioside and mecobalamine injections on idiopathic facial palsy (IFP).Methods A total of 120 IFP patients were divided into a ganglioside group,a mecobalamine group and a consociation group,each of 40.All were given routine drug and acupuncture treatment,while the ganglioside and mecoblamine groups were additionally provided with the corresponding injections of ganglioside and mecobalamine,respectively.The treatment was administered once daily for a total of 14 days.All the groups were evaluated using the House-Brackmann facial nerve grading system before and after the interventions of 14 days ended.The compound muscle action potential (CMAP) wave amplitude and R1 incubation,the motor nerve conduction velocity (MCV) and the sensory nerve conduction velocity (SCV) of the affected facial nerves were observed and compared before and after the 14 days of treatment.Results The total effectiveness rate of the consociation group (95.0％) was significantly higher than that of the ganglioside (90.0％) or the mecobalamine group (87.5％).After the treatment the CMAP wave amplitude had improved significantly and the R1 latency had shortened significantly in the consociation group compared to the other 2 groups.The average MCV and SCV of the consociation group had also improved significantly compared with the other two groups.Conclusion Acupuncture combined with ganglioside and mecobalamine injections is an effective therapy for idiopathic facial palsy.It can improve the patient's clinical symptoms,shorten the CMAP wave amplitude and R1 latency on the affected side and improve MCV and SCV.
