1.Pharmacokinetics and relative bioavailability of Gliclazide capsules in healthy volunteers

Objective:The relative bioavailability and pharmacokinetics of Chongqing Lisi(LSGL) or Shanxi Fenghe Gliclazide capsules(FHGL) were studied in 20 healthy male volunteers. Methods:A single dose of 50mg of LSGL or FHGL was administered by randvmized crossover way in 20 volunteers and the plasma concentrations of Gliclazide(GL) were determined by HPLC.The pharmacokinetic parameters were calculated with 3p97 pharmacokinetic program and the bioequivalency was evaluated.Results:The concentration-time curves of two proparations fitted to one-compartment model.The peak plasma levels (C max ) of LSGL and FHGL were 4.50?0.83 and 4.54?l.04?g/ml,respectively;the peak time (T max ) was 4.05+0.89 and 4.05?0.76h,respectively;and AUC (0~T) were 80.30?26.87,82.59?26.20?g?h?ml -1 ,respectively.The relative bioavailability of LSGL was 97.23?12.58%.Conclusion:The result of two one-side tests suggest that the LSGL is bioequivalent to the FHGL.
2.Effect of nandrolone phenylpropionate on androgen receptor-mediated transcriptional regulation of target genes in rat scald models

BACKGROUND:Moderate to severe burn and trauma, treatment of which has been paid equal attention to wound surface, is always a difficulty of clinical systemic treatment and has a poor prognosis. Anabolic hormones have acquired secure and good results in the treatment of burns in both animals and clinical patients. Although use of anabolic hormones is restricted by the doping management, but its androgen receptor and nuclear receptor coregulators are the newly emerging areas of interests in the field of gene regulation mechanism in recent years. OBJECTIVE:To investigate the effect of nandrolone phenylpropionate on androgen receptor-mediated transcriptional regulation of target gene in rat scald models. METHODS: Thirty-six rats were randomly divided into nandrolone phenylpropionate, model and control groups. In the nandrolone phenylpropionate and model groups, rats were subjected to a 20% total body surface area second-degree scald injury by hot water. Two days after model preparation, rats in these two groups were intramuscularly injected with nandrolone phenylpropionate and saline, once every other day, for a total of 21 days. RESULTS AND CONCLUSION:The gene expression levels of steroid receptor coactivator-1 and insulin-like growth factor 1 in the rat livers and gonads (testes, ovaries) were significantly different between the nandrolone phenylpropionate and model groups (P < 0.05), except steroid receptor coactivator-1 in the liver. There were no significant differences in steroid receptor coactivator-1, c-myc and insulin-like growth factor 1 in the rat livers and gonads (testes, ovaries) between model and control groups (P < 0.05). The results demonstrate that nandrolone phenylpropionate has different effects on the gene expression of steroid receptor coactivator-1, c-myc and insulin-like growth factor 1 in different tissues under different physiological and pathological conditions.
3.Prediction of lymph node metastasis in stage Ⅰ endometrial cancer by evaluation of risk factors pre-and intra-operation

Objective To selectively resect the lymph nodes in the endometrial cancer by risk factors evaluation pre-and intra-operation.Methods Three hundred and seventy-four cases who in the Obstetric and Gynecology Hospital Affiliated to Fudan University from Oct.2010 and Dec.2012 were clinically diagnosed as stage Ⅰ pre-and intra-operation,grouped as one (staged Ⅰ A,294 cases) and two (staged Ⅰ B,80 cases),and the situation of lymph nodes metastasis and the related risk factors were analyzed.Results (1) Lymph nodes metastasis were positive in 6 cases in group one (2.0%) and 12 cases in group two(15.0%),and the difference was significant between two groups (x2 =23.054,P =0.000).(2) University analyses showed that pathologic type,grade,myometrial invasion,lymphovascular invasion and cervical gland invastion were the risk factors contributed to the lymphatic metastasis in early staged endometrial cancer (P< 0.05).Multivariate analysis showed that myometrial invasion was independent risk factor affecting early metastasis of lymph node in endometrial carcinoma (regression coefficient =2.339,standard error =0.585,OR =10.897,95% CI 3.273 ~ 32.367,P =0.000).(3)Based on the gynecology oncology group test and risk factors that recent used in different countries and clinical centers,superficial myometrial layer invasion,G2,age> 60 years,tumor ≥ 2 cm in diameter as the medium risk factors,and deep myometrial layer invasion,G3,lower portion of the uterus or cervical gland invasion as high risk factors,and patients with two or more medium risk factor (106 cases) and with one or more high risk factors(138 cases) were inclined to develop lymph node metastasis((5.66% (6/ 106),0(0/130),P =0.04;8.70% (12/138),2.54% (6/236),P =0.007).(4) Two hundred and thirteen patients were found with superficial layer invasion by frozen section,and 11 of them were upgraded to Ⅰ B by postoperative pathologic examination.Interestingly,all of the 11 cases were accompanied by two medium or one of high risk factor.Conclusion Low incidence of lymph nodes involvement is found in stage Ⅰ endometrial cancer,especial stage Ⅰ A.Myometrial invasion is closely linked to lymph node metastasis,thus,lymphadenectomy should routinely be carried out in endmetrial cancer diagnosed as Ⅰ B whether pre-or intraoperation.As in stage Ⅰ A,unless two medium and one high risk factors are found.
4.Minimally invasive techniques in arthroscopic resection of soft tissue cysts

Objective To evaluate the techniques and effects of arthroscopic resection of cystic lesions of soft tissue. Methods Twenty-six patients with soft tissue cysts underwent arthroscopic shaving of cyst wall from February 1998 to February 2001. Results Follow-up observation ranged from 6 months to 12 months, with a mean of 8.6 months. The total effective rate reached 96.2% (25 of 26), and no postoperative complications occurred. Recurrence was only seen in 1 patient with thecal cyst on the right dorsal wrist. Conclusions Arthroscopic resection of cystic lesions of soft tissue can be a less invasive method, with fewer complications, a high effective rate and a rapid recovery.
5.An assessment of the efficacy of arthroscopic debridement for osteoarthritis of the knee

Objective To study the curative efficacy of arthroscopic debridement in the management of osteoarthritis of the knee. Methods A retrospective analysis was made on clinical records of 44 cases of knee osteoarthritis (62 knee joints) treated with arthroscopic debridement. Results All the patients were followed for 2~2.5 years. The rates of excellent or good results were 85.5% (53/62) at 6 months after the surgery, 77.4% (48/62) at 1 year after the surgery, and 66.1% (41/62) at 2 years, respectively. The curative results were significantly worse at 2 years after the surgery than at 6 months ( ? 2=6.332, P =0.012). Conclusions Arthroscopic debridement is effective for the relief of painful and swollen knee joint, and postpones the appearance of articular cartilage degeneration. The efficacy of the treatment is apparent in mild cases, but the treatment will be less effective as time goes on.
6.The adjunctive therapeutic effect of recombination growth hormone on the elderly patients with pulmonary infection

Objective To investigate the adjunctive therapeutic effect of recombination growth hormone(rhGH) on the elderly patients with pulmonary infection.Methods 53 elderly patients with pulmonary infection were randomly divided into rhGH group(n=25) and control group(n=28).The rhGH group were received hypodermic introl group were received standard therapy.Clinical efficacy were compared between two groups.The level of albumin (ALB),growth hormone (GH),insulin-llke growth factor-1 (IGF-1),leptin (LP) were measured and the body mass index(BMI) was observed in all cases before and after treatment.Results The overall efficacy of rhGH group and control group was 88.0% versus 60.7%.The results showed significant statistical difference between both groups(P<0.05).After treating with rhGH for 10 days,the level of BMI,ALB,GH,IGF-1 and LP were increased in the rhGH group,which were(26.1±4.1)kg/m2;(38.4 ±6.6)g/L;(7.0 +0.9) μg/L;(27.3 ±6.1)μg/L;(6.9 ± 1.1)μg/Lrespectively.And the level of BMI,ALB,GH,IGF-1 and LP were (21.8 ± 3.4 ) kg/m2 ;( 29.5 ± 5.1 ) g/L;( 4.0 ±0.4) μg/L;( 22.0±3.8 )μg/L;( 3.8±0.8 )μg/L in the control group after treatment.The results showed significant statistical difference between both groups(P<0.05).Conclusion There was obvious adjunctive therapeutic effect of rhGH to the elderly patients with pulmonary infection by improving the nutritional state of these patients.
7.Effect of ginsenoside rb1 on survival of dorsal random-pattern skin flap with large length-to-width ratio in rats

BACKGROUND: Ginsenoside Rb1 has been extensively used in the protection and treatment of heart, encephalon, lung, kidney and liver damage. However, its application in skin flap is rare.OBJECTIVE: To investigate the effects of intraperitoneal injection of ginsenoside Rb1 on the survival of the dorsal random-pattern skin flap with large length-to-width ratio in rats.METHODS: Healthy adult SD rats were randomly divided into experimental and control groups. A caudally based dorsal random pattern skin flap, 80 mmx20 mm (length: width = 4:1), was symmetrically made. Ginsenoside Rb1 (2 mg/kg) was intraperitoneally injected into the experimental group rats, and the same volume of normal saline was injected into the control group. Malonyl dialdehyde (MDA) and nitric oxide (NO) level of the flaps were tested 1 day after operation; the amount of viable tissues of the flaps were examined by planimetry 10 days after operation. Specimens from the proximal, middle and distal flaps were harvested for HE staining to examine the microstructure.RESULTS AND CONCLUSION: At the first day after operation, NO level was higher in the experimental group than the control group (P< 0.01), while MDA level was lower than the control group (P< 0.01). At the 10th day after operation, the survival rate of the flap was significantly greater in the experimental group than the control group (P < 0.001). Histological observation showed that compared with the control group, the edema and inflammatory cells infiltration were less, while the fiber hyperplasia and the microvascular growth were more obvious in the experimental group. Results show that intraperitoneal injection of ginsenoside Rb1 can enhance the blood supply of the flaps and improve the survival of the random-pattern skin flaps with large length-to-width ratio in rats. This may involve its effects of improving NO activity, decreasing lipid peroxidation, and promoting angiogenesis of skin flaps.
8.Development of a new breathing control system of tumor target for precision radiotherapy

Objective:It is more difficult to conduct precision radiotherapy for organs of the human body with the respiratory movement. It is necessary to compensate a certain degree of deviation which is produced by many thoracic and abdominal organs with breathing exercises. To develop a new breathing control system of tumor target for precision radiotherapy is the practical demand in hospital.Methods: According to the current active breathing control system, there are gasbag, control box, handle switch for the patients, computer, automatic gasbag controller, respiratory sensor and communication tools. The Pneumotach PowerCube pulmonary function respiratory sensor and C++ high level programming language were selected to program the breathing control system.Results: The system could make patients conduct respiratory control better, reduce the deviation of pulmonary tumor caused by respiratory movement and improve the accuracy of treatment.Conclusion: The system had a lot of functions, such as dispersion, oscillation, ventilation and so on. It is refitted on the current instruments and successful to clinical application.
10.The Changes of Main Oxidase and Antioxidase Activities in The Pathological Scars

In order to study the changes of main oxidase and antioxidase in the pathological scars,the tissues of hypertrophic scar(10 cases),keloid(10 cases)and normal skin(8 cases)were obtained.The concentration of malonaldehyde(MDA)and the activities of xanthine oxidase(XO),copper,zinc-superoxide dismutase(CuZn-SOD),catalase(CAT)as well as glutathione peroxidase(GPX)were detected by spectrophotometric method.Compared with normal skin tissues,the concentration of malonaldehyde and xanthine oxidase activity were significantly higer in pathological scars(P
