Objective To analyze the application of MSCT in A.C.T. Methods MSCT and CR examination results of 41 A.C.T cases were compared and analyzed by 3 doctors. Results Compared with plain flim,MSCT markly increased the fracture detection rate. Conclusion MSCT is precise and safe in the diagnosis of A.C.T.

4.Advances in CRISPR/Cas9-mediated gene editing.

Clustered regulatory interspaced short palindromic repeats (CRISPR) found in bacteria and archaea genome that contains multiple short repeats loci, provides acquired immunity against invading foreign DNA via RNA-guided DNA cleavage. The first inkling of this hot new genetic engineering tool turned up in 1987, when a research team observed an oddly repetitive sequence at one end of a bacterial gene. Now three types of CRISPR/Cas system have been identified: types I, II and III. In the type II CRISPR/Cas9 system, short segments of foreign DNA termed 'spacers' are integrated within the CRISPR genomic loci, transcribed and processed into short CRISPR RNA (crRNA). These crRNAs anneal to trans-activating crRNA (tracrRNA) and direct sequence-specific cleavage in that a double-strand break (DSB) is generated by Cas proteins. Based on these findings, various genetic methods, including gene targeting (Gene disruption), gene insertion, gene correction etc., are being designed to manipulate the genomes of different species at specific loci. Compared with zinc finger nucleases (ZFN) and transcription activator-like effector nucleases (TALEN), CRISPR/Cas9 is simpler with higher specificity and less toxicity. This review summarizes recent progress, discusses the prospects of CRISPR/Cas9 system, with an emphasis on its structure, principle, applications and potential challenges, and provides a useful reference for researchers who are interested in this new technique.

5.A Molecular-Imprinted Sensor for Trace Detection of Gibberellin Based on Ferrocenecarboxylic Acid Multiply Marked Dendrimer

A novel strategy employing dendrimer to multi-label the template molecule to improve the sensitivity of molecularly imprinted electrochemical sensor was proposed. The determination relies on a competition reaction between poly ( diethylene-triaminepenta-acetic acid )-glycol ester ( PDTPA )-ferrocene-carboxylic acid ( FcA ) labeled gibberellins acid 3 ( GA3 ) and GA3 in the sample. Since one cavity corresponds with multiple FcA, instead of only one FcA, the intensity of the detecting signal was greatly enhanced, so was the sensitivity of the sensor. Experimental results showed that the molecularly imprinted electrochemical sensor was sensitive to GA3 detection at a concentration from 2. 0í10-9 to 1. 0í10-7 mol/L, with a detection limit of 9. 3í10-10 mol/L. In addition, the sensor had good reproducibility and its feasibility was verified in the analysis of series of real beer samples.
6.Effects of Early before PCI and Immediate Application of Tirofiban on Coronary Blood Flow and Myocar-dial Perfusion in Patients with High Risk Acute Coronary syndrome

OBJECTIVE:To investigate the effects of early before PCI and immediate application of tirofiban on coronary blood flow and myocardial perfusion in patients with high risk acute coronary syndrome (ACS). METHODS:100 high risk ACS patients were randomly divided into observation group and control group,with 50 cases in each group. Two groups were given tiro-fiban;observation group was given medicine 10 μg/kg within 3 min,4-6 h before PCI,with the velocity of 0.15 μg/(kg·min)till 24 h after PCI. Control group was given medicine at the beginning of PCI,route of administration was same as observation group. The incidence of TIMI blood flow grading,TIMI myocardial perfusion grade (TMPG),cTn Ⅰ,PAR and main adverse cardiac events(MACE)were compared between 2 groups before and after treatment. RESULTS:After operation,the patients of 2-3 grade TIMI blood flow and 2-3 grade TMPG in 2 groups were significantly more than before;the patients of 2-3 grade TMPG in observa-tion group was significantly more than in control group,with statistical significance(P<0.05). After treatment,cTn Ⅰof 2 groups were significantly increased,while PAR were significantly decreased,with statistical significance compared to before operation (P<0.05);but there was no difference between 2 groups (P>0.05). There was no statistical significance in the incidence of MACE between observation group(8.0%)and control group(16.0%)(P>0.05). CONCLUSIONS:Early application of tirofiban before PCI can effectively improve coronary blood flow and myocardial blood supply in high risk ACS patients.
7.Influence of region of interest size on performance of shear wave elastography in breast masses

Objective To explore the influence of ROI size on the diagnostic performance of shear wave elstography (SWE) of breast masses.Methods A total of 225 consecutive patients with 239 solid breast masses underwent SWE before surgery,and then quantitative SWE parameters,including the maximum stiffness (Emax),mean stiffness (Emean),ratio of stiffness of the mass to stiffness of surrounding fat (Eratio) and standard deviation (SD),were measured by two methods,one method was using larger ROI to include the largest part of a mass,and the other was placing fixed ROI=2 mm over the stiffest part of the lesion to measure.The diagnostic performance of the two methods were evaluated by ROC curve,and the cut-off value,sensitivity and specificity of each SWE parameter were obtained.Results There were no statistically significant differences between the performance of Emax,Emean and SD from the two different methods (all P> 0.05),the performance of Eratio of the two methods showed significant difference (P=0.046).The significantly higher specificity of Emax and Emean in the larger ROI were found,and the significantly higher sensitivity of SD and Eratio in the ROI=2 mm was found.Furthermore,the diagnostic performance of combination of SWE and BI-RADS were not greatly affected by the ROI size,the difference had no statistical significance (all P>0.05).Conclusion The ROI size only affects the diagnostic performance of Eratio,and donot affect the diagnostic performance of the quantitative SWE parameters (Emax,Emean,SD) and combination of SWE and BI-RADS.
8.The value of ultrasonography in the differential diagnosis for TTTS and sIUGR

12 patients with inconsistent growth of twins were involved.The acoustic features were analyzed,inclu-ding 2D and Doppler images.The differential diagnosis was verified during follow-up exam in some patients.8 cases were diagnosed with sIUGR by ultrasonography,meanwhile 3 were diagnosed with TTTS.The rest 1 case could not be etiologically diagnosed due to the bad image conditions.We can differentiate the two diseases by evaluating the umbilical cord insertion,the amiotic fluid and the hemodynamics of the other twin.Ultrasonography is helpful in the differential diagnosis for TTTS and sIUGR.

10.The Advance of Diagnosis and Therapy on Elderly Patients with Coexistence of COPD and CHF

The prevalence of obstructive pulmonary disease (COPD) and chronic heart failure (CHF) increased gradually with age. The coexistence of COPD and CHF was very common but often neglected. To avoid overlooking COPD in patients with known CHF, pulmonary function tests should be routinely obtained. Likewise, to avoid overlooking CHF in patients with known COPD, left ventricular function should be routinely assessed. Plasma brain natriuretic peptide was a useful index to differentiate COPD exacerbation from CHF decompensation in patients presenting with acute dyspnea. Skeletal muscle metabolic alterations and its atrophy were the common mechanisms which resulting in deterioration of function capacity in elderly patients with CHF and COPD. Contrary to the conventional belief, long-term beta adrenergic blockers should be given to elderly patients with CHF and COPD. Exercise training reverses rapidly progress of skeletal muscle metabolic alterations and atrophy and promotes independence and life quality in elderly patients.
