1.Rheological Properties and Retention Ability of Octoxynol-9 Vaginal Thermosensitive in Situ Gel

Objective: To observe the rheological properties of octoxynol-9 vaginal thermosensitive in situ gel [(O-9)-VTG] for predicting the gelation behavior in vivo, and evaluate the retention ability in vagina.Methods: The rheological parameters of (O-9)-VTG and (O-9)-VTG diluted by stimulant vaginal fluid (SVF) were measured by a Haake Rheomix to characterize the rheological properties.The vaginal samples after the administration of self-made (O-9)-VTG and O-9 gel were withdrawn, and then the concentration of octoxynol-9 in the samples was determined to evaluate the retention ability.Results: (O-9)-VTG was Newtonian fluid with low viscoelasticity under room temperature and converted to gel at 32.6℃.The formula could still transform into gel at body temperature after diluted by SVF, and resided in the vagina of mice above 8 h.Conclusion: (O-9)-VTG has suitable gelation temperature and rheological properties.Compared with the self-made octoxynol-9 gel, (O-9)-VTG has satisfactory retention in vagina, which meets the requirements for vaginal topical use.
2.Preparation and Quality Control of Compound Mupirocin Film Forming Gel

Objective:To prepare compound mupirocin film forming gel and establish the quality control methods. Methods:The compound film forming gel was prepared using tannic acid and salicylic acid as the esterifying agents interacting with hydroxypropyl cel-lulose followed by cross-linking with tartaric acid and sorbitol to form a new film forming material, and then antimicrobial mupirocin and anesthetic dyclonine were added to prepare compound mupirocin film forming gel. The contents of mupirocin and dyclonine were deter-mined by HPLC. The column was Hypersil ODS2 (250 mm × 4. 6 mm, 5 μm) and the mobile phase consisted of methanol-ammonium acetate buffer (1. 15 g ammonium acetate, 300 ml water, 1 ml glacial acetic acid) (75 :25). The flow rate was 1. 0 ml·min-1 and the detection wavelength was 230 nm. The column temperature was 30℃ and the injection volume was 20μl. Results:The film form-ing gel was yellow, brown and gelatinous at the normal temperature, and formed transparent film after coating on the skin. The linear range of mupirocin and dyclonine was 20-400μg﹒ml-1(r=0. 9999) and 10-200μg﹒ml-1(r=0. 9996), respectively. The aver-age recovery was 99.9%(RSD=0.54%, n=9) and 99.6%(RSD =1.45%, n =9), respectively. Conclusion: The preparation process is reasonable, simple and controllable in the preparation of ideal film forming gel.
3.MRI characteristics of solid papillary carcinomas in situ of breast

Objective To evaluate MRI characteristics of solid papillary carcinomas (SPCs) in situ of the breast.Methods A retrospective study included 5 patients with pathologically confirmed SPC in situ was performed.MRI data before operation including conventional MRI,dynamic contrast enhanced MRI (DCE-MRI) and DWI were analyzed.Results All the lesions showed iso/hypointensity on T1 FSPGR sequence,iso/hyperintensity on FSE T2WI sequence and STIR sequence.Mass enhancements were observed for all lesions with oval or irregular shapes on DCE-MRI.The margin of lesions were circumscribed,and internal enhancements were homogeneous or heterogeneous.Time intensity curve appeared a rapid increase in initial contrast phases and platform or outflow types in delayed phases.All the lesions on DWI showed slightly hyperintensity with the ADC value range from 1.34 × 10-3 mm2/s to 1.96)× 10-3 mm2/s.Conclusion MRI manifestations of SPC are characteristics,which may provide valuable information to distinguish SPC in situ from other invasive breast carcinomas.
4.Study on the Stability of Fuke Zhidai Capsule

OBJECTIVE:To study the stability of Fuke zhidai capsule at accelerated testing and room temperature. METHODS:HPLC was conducted to determine the content of berberine hydrochloride in the preparation:the column was Diamonsil C18 with mobile phase of B methanol-0.7% triethylamine(pH adjusted to 3 by phosphate)(50:50,V/V)at a flow rate of 1 ml/min;detec-tion wavelength was 265 nm,with a column temperature at 45 ℃,and the injection volume was 5 μl;its moisture,disintegration time,accelerated stability and long-term stability were studied according to the 2015 edition of Chinese Pharmacopoeia. RE-SULTS:The linear range of berberine hydrochloride was 0.0217-0.1736 mg/ml(r=0.9999);RSDs of precision,stability and re-producibility tests were lower than 2%;recovery was 97.24%-101.73%(RSD=1.72%,n=6). CONCLUSIONS:The moisture,dis-integration time,accelerated stability and long-term stability were all in line with the quality standards.
5.Special Review of 2596 Prescriptions Including Potassium Chloride Injection in Neurosurgery Department of a Hospital

Objective:To discuss the management norms for the rational use of potassium chloride injection in neurosurgery department of a hospital.Methods:A special review of the medical advice including potassium chloride injection from January to June in 2016 in neurosurgery department was carried out.Results:In the 2 596 prescriptions including potassium chloride injection,the irrational drug use rate was 17.87%,and among them,92.67% were irrational compatibility,which showed serious risk of drug use.Conclusion:Some defects still exist in the application management of potassium chloride injection in neurosurgery department of this hospital.In order to avoid the risk of clinical medication,it is necessary to establish standardized management norms for the use of potassium chloride injection in this hospital to further promote the management of high-risk drugs with intravenous administration commonly used in clinics.
6.Effects of lidocaine in combination with sevoflurane on myocardial protection in patients undergoing off-pump coronary artery bypass grafting

Objective To evaluate the effects of lidocaine in combination with sevoflurane on myocardial protection in patients undergoing off-pump coronary artery bypass grafting (OPCABG).Methods One hundred ASA physical status Ⅱ or Ⅲ and NYHA class Ⅰ or Ⅱ patients of both sexes,aged 45-70 yr,weighing 63-82 kg,scheduled for elective OPCABG,were randomly assigned into 4 groups (n=25 each):control group (group C),lidocaine group (group L),sevoflurane group (group S),sevoflurane combined with lidocaine group (group SL).Tracheal intubation was performed after induction of anesthesia.Anesthesia was maintained with iv infusion of propofol and intermittent iv boluses of sufentanil and pipecuronium.Bispectral index value was maintained at 45-55.Lidocaine 1.5 mg/kg was injected after intubation,followed by infusion at 2 mg/min until the end of surgery in L and SL groups.Sevoflurane was inhaled with end-tidal concentration of 2.2%-2.5% starting from the end of intubation until the end of operation in S and SL groups.Before induction of anesthesia,at skin incision,immediately after transsection of internal mammary artery,after completion of anastomosis of the proximal aortovein and distal coronary artery,at the end of operation and at 24 h during operation,venous blood samples were obtained to detect the levels of plasma creatine kinase (CK),creatine kinase isoenzyme-MB (CK-MB) and cardiac troponin Ⅰ (cTnI).Lidocaine-related side effects such as arrhythmia,bradycardia or cardiac arrest were recorded during operation and within 24 h after operation.Results Compared with group C,the plasma CK,CK-MB and cTnI levels were decreased in the S,L and SL groups.The plasma CK-MB and cTnI levels were significantly lower in SL group than in L group.The plasma CK,CK-MB and cTnI levels were significantly lower in SL group than in S group.In L and SL groups,no patient developed lidocainerelated side effects.Conclusion Lidocaine 1.5 mg/kg injected intravenously after intubation,followed by infusion at 2 mg/min in combination with sevoflurane end-tidal concentration about 2.2% ~2.5% provides myocardial protection in patients undergoing OPCABG,the efficacy is stronger than that of either alone.
7.Survey on risk factors for stroke in population with family history in Zhengzhou rural area

Objective To investigate the risk factors for stroke in population with family history in rural areas of Zhengzhou. Methods A cluster random sampling method was used to collect 386 residents with family history of stroke from 5 villages of Zhengzhou. The participants were divided into stroke group and non-stroke group according to diagnosis. IBM SPSS STATISTICS 21.0 and SPSS Modeler 14.1 software were used to analyze the data.Results Hypertension, dyslipidemia, atrial fibrillation or arrhythmia, smoking and drinking in stroke group were significantly higher than those of the non-stroke group (P < 0.05). Logistic regression analysis indicated that hypertension (OR=35.705, 95% CI9.045~140.950, P =0.000) and dyslipidemia (OR=2.669, 95% CI 1.077~6.614, P = 0.034) were linked with stroke. Artificial neural network (ANN) model indicated the independent variables in predicting stroke were hypertension, dyslipidemia, smoking, drinking and atrial fibrillation or arrhythmia according significance, respectively. The accuracy of the model was 82.0%. Conclusion Among population with family history of stroke, those who had hypertension, dyslipidemia, atrial fibrillation or arrhythmia, smoking and drinking were more susceptible to develop this condition.
8.Wernicke's encephalopathy misdiagnosed as schizophrenia: a case report

This article presents a case report of Wernicke's encephalopathy misdiagnosed as schizophrenia. The patient was hospitalized and diagnosed with schizophrenia at a local psychiatric hospital over a month ago. Twelve days ago, the patient gradually developed symptoms such as salivation, difficulty in eating, trembling hands, unsteady walking, glossoptosis and so on. The above symptoms were interpreted as drug-induced adverse reactions and managed accordingly, whereas the patient showed a poor response to treatment, and began to experience intermittent consciousness disorder. After referral to our hospital, findings were diagnostic for Wernicke's encephalopathy and peripheral polyneuropathy. Due to the progression of the disease, the family members abandoned further treatment and requested discharge from the hospital, and the patient died outside the hospital. This case report aims to improve the prognosis of Wernicke's encephalopathy in patients with mental disorder by raising the awareness of the disease and enhancing the early prevention.
10.Liver and spleen stiffness in diagnosing esophageal varices and predicting high risk of esophageal and gastric varices bleeding

Objective To explore the value of liver and spleen stiffness in diagnosing esophageal and gastric varices (EGV) and predicting high risk of EGV bleeding in patients with hepatitis B virus (HBV)-related liver cirrhosis.Methods Totally 71 patients with HBV-related liver cirrhosis who had undergone endoscopy were prospectively recruited.Then acoustic radiation force imaging (ARFI) was performed.The severity of EGV was graded and ROCs were drawn on the liver shear wave velocity (LSWV) and spleen shear wave velocity (SSWV) to detect the value of liver and spleen stiffness in diagnosing EGV and predicting the high risk of EGV bleeding.Results There were significant differences of LSWV and SSWV between patients with EGV and without EGV (all P<0.001).Taking endoscopy results as golden standards,the areas under ROC curve of LSWV and SSWV were 0.877 and 0.910 (both P<0.001) in diagnosing esophageal varices,and the optimal cut-off values were 2.01 m/s and 2.84 m/s (sensitivity 93.5% and 76.1%,specificity 76.0% and 92.0%),respectively.Areas under ROC curve of LSWV and SSWV in predicting the high risk of EGV bleeding were 0.882 and 0.914 (both P<0.001),and the optimal cut-off values were 2.27 m/s and 2.94 m/s (sensitivity 77.1% and 85.7%,specificity 83.3% and 91.7%),respectively.Conclusion Liver and spleen stiffness are useful in diagnosing EGV and predicting the high risk of EGV bleeding in patients with HBV-related liver cirrhosis.
