1.Effect of Allopurinol on the Function of Blood Vessel Endothelium in Patients with Essential Hypertension Complicating Hyperuricemia

OBJECTIVE: To study the effect of allopurinol on the function of blood vessel endothelium in patients with essential hypertension complicating hyperuricemia. METHODS: 65 cases with essential hypertension complicating hyper uricemia were randomized into treatment and control group. The control group was given the classic non- pharmacotherapy: reduced intake of sodium, more exercise, weight loss etc. The treatment group was treated wilh 100mg allopurinol b.i .d for 4 weeks besides the classical non - pharmacotherapy as stated for the control group. Serum uric acid level, brachial artery flow -mediated diastolic (FMD) function, the base internal diameter of brachial artery, nitrogen oxide (NO), von Willebrand' s factor(vWF) were compared between two groups before and after treatment. RESULTS: As compared with prior treatment, the treatment group had decreased level of uric acid and decreased vWF level, increased level of NO and a significantly increased level of FMD, all had statistical significances(P0.05) . CONCLUSION: Allopurinol can ameliorate the function of blood vessel endothelium in patients with essential hypertension complicating hyperuricemia.
2.Role of PI3K/Akt signaling pathway in mild head hypothermia-induced reduction of global cerebral ischemia-reperfusion injury in rats

Objective To evaluate the role of phosphatidylinositol 3-kinase/protein kinase B (PI3K/Akt) signaling pathway in mild head hypothermia-induced reduction of global cerebral ischemiareperfusion (I/R) injury in rats.Methods Sixty male Sprague-Dawley rats,weighing 250-280 g,were randomly divided into 5 groups (n=12 each) using a random number table:sham operation group (group S),group I/R,mild hypothermia + I/R group (group H),mild hypothermia + I/R + solvent control group (group DM),and mild hypothermia + I/R + PI3K inhibitor LY294002 group (group LY).Global cerebral I/R was induced by modified four-vessel occlusion method described by Pulsinelli.In H group,when the hippocampal temperature was decreased to 33℃ using nasopharyngeal cooling,the bilateral common carotid arteries were occluded for 15 min followed by reperfusion,and hippocampal hypothermia was maintained at 32.5-33.5 ℃ for 1 h.In DM and LY groups,DMSO and LY294002 5 μl were injected into the left ventricle,respectively,and 20 min later the other procedures were similar to those previously described in group H.At 8 h of reperfusion,6 rats were sacrificed,and hippocampal specimens were obtained to detect the expression of pFoxO3a,Bcl-2 and Bax (by immuno-histochemistry).The expression of phosphor-Akt (p-Akt) was determined by Western blot.Results Compared with group S,the expression of p-Akt,pFoxO3a and Bcl-2 was significantly up-regulated in I/R,H and DM groups,the expression of Bcl-2 was down-regulated,and the ratio of Bcl-2/Bax was increased in H and DM groups,and the ratio of Bcl-2/Bax was decreased in I/R and LY groups.Compared with group I/R,the expression of p-Akt,pFoxO3a and Bcl-2 was significantly up-regulated,the expression of Bcl-2 was down-regulated,and the ratio of Bcl-2/ Bax was increased in H and DM groups.Compared with H and DM groups,the expression of p-Akt,pFoxO3a and Bcl-2 was down-regulated,the expression of Bax was up-regulated,and the ratio of Bcl-2/ Bax was decreased in group LY.Conclusion PI3K/Akt signaling pathway is involved in reduction of global cerebral I/R injury by mild head hypothermia in rats.
3.Reliability of Amsterdam Preoperative Anxiety and Information Scale score in evaluating preoperative anxiety of Chinese people

Objective To evaluate the reliability of Amsterdam Preoperative Anxiety and Information Scale (APAIS) score in evaluating the preoperative anxiety of Chinese people.Methods One hundred sixty Chinese patients of both sexes, aged 18-60 yr, of American Society of Anesthesiologists physical status Ⅰ-Ⅲ, scheduled for elective surgery, were enrolled.Preoperative anxiety of the patients was assessed with APAIS score and Spielberger's State-Anxiety Inventory (S-AI) questionnaire during the preoperative interview.Cronbach's alpha of total anxiety and need for information scores was calculated.Four thresholds of total anxiety score in identifying preoperative severe anxiety was set as 10, 11, 12 and 13.S-AI questionnaire was considered as the standard, and the correlation between total anxiety score and S-AI questionaire was tested.Results The Cronbach's alpha of total anxiety and need for information scores was 0.84 and 0.71, respectively.When the threshold of total anxiety score in identifying preoperative severe anxiety was 12, the total anxiety score was highly correlated with S-AI questionnaire, the Kappa value was 0.62, 95% confidence interval was 0.46-0.78, the sensitivity was 0.71, the specificity was 0.90, and the positive predictive value was 0.78.Conclusion APAIS score can be used to assess the preoperative anxiety of Chinese people.
4.Chemerin/ChemR23 promotes high glucose-induced IL-6 and TNF-α expressions in glomerular endothelial cells via p38 MAPK

Objective To observe the role and related mechanism of chemerin and its receptor ChemR23 in glomerular endothelial cells (GEnCs) stimulated by high glucose.Methods Mouse GEnCs were cultured and divided into control group,20.0 mmol/L high glucose group,40.0 mmol/L high glucose group and mannitol control group.Then the expressions of interleukin-6 (IL-6) and tumor necrosis factor-α (TNF-α) in cell culture supematant as well as the expressions of intracellular protein and mRNA of chemerin,ChemR23,IL-6 and TNF-α were detected.Lentiviral transfection targeting ChemR23 was applied before high glucose-or Chemerin-stimulated,and expressions of supernatant and intracellular mRNA of IL-6 and TNF-α were measured.Meanwhile whether p38 mitogen-activated protein kinase (p38 MAPK) pathway was activated by high glucose was detected.The specific inhibitor of p38 MAPK was added prior to high glucose-stimulated,then supernatant and intracellular mRNA expressions of IL-6 and TNF-α was detected.The supernatant expressions of IL-6 and TNF-α were measured by ELISA.The intracellular protein expression and p38 MAPK phosphorylation activity were detected by Western blotting.The mRNA expression was detected by real time PCR.Results Compared with those in the control group,in high glucose groups the expressions of IL-6,TNF-α and chemerin were significantly increased (all P < 0.05),however,the expressions of ChemR23 did not change (all P > 0.05);the supernatant and mRNA expressions of IL-6 and TNF-α were also elevated in the chemerin group (all P < 0.05).Lentivirus baring shRNA could efficiently suppress ChemR23 expression,and the Chemerin-or high glucose-induced expressions of IL-6 and TNF-α were reduced (all P < 0.05).Also it could significantly reduce the expression of phosphorylated-p38 MAPK (p-p38 MAPK) induced by high glucose (P < 0.05),as high glucose group had higher p-p38 MAPK than control group (P < 0.05).While the high glucose-elevated expressions of IL-6 and TNF-α were significantly attenuated by p38 MAPK inhibitor (all P < 0.05).Conclusions High glucose stimulation can induce the expression of chemerin in GEnCs.By binding to ChemR23,chemerin activates p38 MAPK signaling pathway,and then promotes the expressions of IL-6 and TNF-α.These inflammatory cytokines aggravate inflammation of GEnCs.
5.Effect of individualized nursing intervention in schizophrenic patients

Objective To analyzed individualized nursing intervention in schizophrenic patients.Methods A total of 108 schizophrenia patients were randomly divided into observation group and control group,with 54 cases in each group.The control group was given routine nursing,while the observation group was given individualized nursing intervention.The clinical efficacy,the compliance and nursing before and after the social function of the two groups were compared.Results The total effective rate,the excellent rate of treatment compliance in observation group were significantly higher than the control group (P < 0.05);Before nursing,social function scores of the two groups showed no significant differences (P > 0.05),and the observation group after nursing was significantly higher than the control group (P < 0.05).Conclusion Individualized nursing intervention can effectively improve the mental compliance behavior in patients with schizophrenia,enhance the clinical curative effect and promote the recovery of social function.

7.Correlations between serum BDNF,IL-18 and hs-CRP levels in patients with acute cerebral infarction and vascular cognitive impairment

Objective:To explore the correlations between serum levels of brain-derived neurotrophic factor(BDNF),interleukin-18(IL-18)and hypersensitivity C-reactive protein(hs-CRP)in patients with acute cerebral infarction and vascular cognitive impairment(VCI),and to provide some clinical ba-ses for early prevention of VCI.Methods:A total of 160 patients with acute cerebral infarction admitted in Department of Neurology of Jincheng People's Hospital from May 2019 to April 2020 were enrolled in this study and were devided into three groups according to whether or not combined with cognitive impair-ment,including no cognitive impairment group(NCI,57 cases),vascular cognitive impairment no de-mentia group(VCIND,56 cases)and vascular dementia group(VaD,47 cases).The cognitive function of all the patients were evaluated by Montreal cognitive assessment(MoCA).The National Institute of Health stroke scale(NIHSS)was used to assess the degree of neurological deficit(mild-,moderate-,se-vere-neurologic deficit group).The infarct size was calculated by Pullicino's method(small-,middle-,large-infarct group).The levels of serum BDNF and IL-18 were measured by enzyme-linked immunosor-bent assay(ELISA),and serum levels of hs-CRP were measured by immunoturbidimetry during the acute phase(0-7 d),recovery period(15-30 d)and 6 months after cerebral infarction.The effects of varying degrees of neurological deficits and different size of infarction on BDNF,IL-18 and hs-CRP were observed.The levels of serum BDNF,IL-18 and hs-CRP in the patients of the three groups with acute,convalescent and six-month cerebral infarction were compared,and their correlations with VCI were ana-lyzed.Results:Serum BDNF level and MoCA scores in mild-neurologic deficit group and small-infarct group were significantly higher than those in moderate-and severe-deficit group,middle-and large-infarct group,respectively(P<0.05).Their levels of IL-18 and hs-CRP were significantly lower than those in moderate-and severe-deficit group,middle-and large-infarct group,respectively(P<0.05).The levels of serum BDNF in NCI group,VCIND group and VaD group during the acute phase,convalescence and 6 months after cerebral infarction were in a significant decline,and the differences during the acute phase and recovery period were statistically significant(P<0.05).The levels of IL-18 and hs-CRP during the acute phase,recovery period and 6 months after cerebral infarction showed a significant increasing trend with significance(P<0.05).Correlation analysis revealed that the levels of BDNF was positively correlated with MoCA scores but negatively correlated with the severity of cognitive impairment while the expression levels of IL-18 and hs-CRP were negatively correlated with MoCA scores but positively correlated with the severity of cognitive impairment.Conclusion:Serum BDNF,IL-18 and hs-CRP are involved in the patho-logical process of occurrence and development of VCI in the patients with acute cerebral infarction.BDNF has a protective effect on VCI while IL-18 and hs-CRP cause severe cognitive impairment.The levels of se-rum BDNF、IL-18 and hs-CRP in the patients with acute ischemic cerebral infarction are closely related to the severity of cognitive impairment and can be used as biomarkers of early diagnosis of VCI.

Objective:To investigate the efficacy and safety of SIMI mouthguard paint(test group) in the treatment of enamel demineralization during orthodontic therapy with fixed applince.Methods:152 cases underwent orthodontic therapy with fixed applince were included in a randomized,open,positive control trial,and were treated by SIMI and Duraphant fluoride toothpast (control group) respectively(n =76).The enamel opaque spot was observed before and 3 months after using the products.The oral mucosa reactions,asthma attacks or stomach nausea and other adverse events were recorded.Results:150 cases (n =75) completed the trial.The results showed that the test group was non-inferior compared with the control group.No adverse event was found in both groups.Conclusion:SIMI mouthguard paint is effective in control of enamel demineralization during orthodontic therapy with fixed applince.
9.Clinical features and prognosis in MLL-AF10 positive acute leukemia.

OBJECTIVETo analyze the clinical features and prognosis of acute leukemia patients with the mixed lineage leukemia(MLL)gene rearrangements AF10 positive.
METHODS6 cases with MLL-AF10 positive were analyzed retrospectively, related literatures were reviewed to clarify MLL-AF10 patients'clinical features and prognosis.
RESULTSThe median age of 6 cases was 19.5 years old, 5 patients with fever onset, the onset white blood cells of 4 patients were less than 10×10⁹/L. 5 cases were as M₅ and 1 case M₄ according to FAB classification, the level of fusion gene(RQ-PCR)was 0.23%-22.60% when diagnosed, 4 cases had concomitant WT1 gene mutation, flow cytometry disclosed myeloid phenotype. Of 5 evaluated patients achieved the first complete remission after conventional chemotherapy, 2 cases of complex karyotype died, one case died of sepsis in induction, another died from failing of transplantation. 4 out of 5 transplant recipients gained long term survival.
CONCLUSIONThe MLL-AF10 positive patients were mostly young men, the majority FAB classification was M5 or M4, often onset with fever, low white blood cells and low level of fusion gene, usually associated with WT1 mutation. Conventional chemotherapy produced a high response rate, but easy to relapse, while the complex karyotype had a poor prognosis, allo-HSCT may have the potential to improve the prognosis of MLL-AF10 positive patients.
