Objective To cure various clinical manifestation of spontaneous pneumothorax(SP) through various measures. Methods BZ According to different clinical manifestation and pneumothorax compression degree. We could use artificial airbleeder, blocking drainage and pleural adhesion to test the effect. Results 74 out of 78 patients had been cured through proper measures. Conclusion SP can be cured by proper measures.

OBJECTIVE To improve the work quality in department of nosocomial infection management,and make the "The Year of Hospital Management" action go on effectively.METHODS To look into and analyze the difficulty point in the work of nosocomial infection management.RESULTS Lots of difficulty points about nosocomial infection management existed in departments of nosocomial infection management of 24 hospitals and affected the management quality of nosocomial infection.CONCLUSIONS Solving the difficulty points about nosocomial infection management,it can make the work of nosocomial infection go on smoothly,and improve the medical care quality.

Infant acute lymphoblastic leukemia B (B-ALL) accounts for 10% of childhood ALL. Eighty percent of infant B-ALL was caused byMLL gene rearrangement (MLL-r). The overall survival rate of ALL was less than 35% in infants with MLL-r. Among infant ALL with MLL-r, infants with positivefusion geneMLL-AF4 (MA4) formed by chromosome t (4;11) had even poor prognosis. Studies in monozygotic twins and archived blood spot at birth had veriifed that fusion gene MA4 originated from antenatal. Whole genome sequencing found that t (4;11) alone might be sufifcient to spawn leukemia. This paper is going to summarize the advances in biological characteristics such as clinical features, cellular origin, genomics and disease models of normalMLL gene and infant B-ALL withMA4.

Results Students’practice of massage,acupuncture and moxibustion can be visualized to meet the requirement of teaching experiment in massage, acupuncture and moxibustion,which shows that this research can innovatively solve the problem of the deficiency of quantity index in the teaching of massage and acupuncture.

Objective:To explore the diagnostic value of mammography and ultrasonography(US)in diagnosis of breast disease with microcalcifications.Methods:Seventy-eight cases with breast microcalcifications were detected by mammography.The detection rate of breast microcalcification at US was analyzed.And the sensitivity,specificity and the accuracy of US in diagnosis breast disease with microcalcifications was compared with these of mammography.Results:The detection rate of breast microcalcification was 66.7%,And US depicted more malignant(87.5%)than benign microcalcification(33.3%).The sensitivity specificity and accuracy of US was 68.7%、83.3%、 74.3%respectively,and the sensitivity specificity and accuracy of mammography was 72.9%、 73.3%、75.6%respectively,there were no significant difference between the two methods.The sensitivity,specificity and accuracy of the combined the two methods was 89.6%,90.0%,92.3% respectively.Conclusion:US can effectively detect and identify the breast microcalicications.The combination can improve the assessment of breast disease with microcalicications,and has a significant clinical practical value in diagnosis of early breast cancer.

Objective To investigate the expression of VEZT and its clinical pathological significance in gastric cancer tissues,and to construct the VEZT over-expression vector.Methods The expression of VEZT was examined in 119 cases of gastric carcinoma and their corresponding normal mucus tissues by SP immunohistochemical staining.The relationships between the VEZT expression levels and its clinicopathological characteristics,prognosis were also investigated.VEZT cDNA was extracted and amplificated from 293 cells by polymerase chain reaction (PCR).Using DNA recombinant technique,the target gene was connected to the pEGFP-N1 vector to construct recombinant plasmid vector after the target gene and pEGFP-N1 were purified.The recombinant plasmid was identificated by 1％ enzyme electrophoresis and DNA sequencing.Results The VEZT-positive expression in gastric carcinoma was 30.3％ and 60.5％ in normal gastric tissues.VEZT expression in high differentiated cancer tissues was higher than that in lower differentiated tissues(x2 =5.002,P ＜ 0.05),expression of VEZT in early gastric cancer was significantly higher than that in patients with advanced gastric cancer (x2 =5.551,P ＜ 0.05),expression of VEZT in patients without lymph node metastasis was significantly higher than that of lymph node metastasis (x2 =5.878,P ＜ 0.05).Patients with positive VEZT expression have better prognosis than the negative patients(x2 =6.908,P ＜ 0.01).The target fragment,consistent with the theoretical value,was verified by gel electrophoresis.DNA sequencing confirmed that the gene sequence had no mutation and the eukaryotic expression plasmid vector pEGFP-N1-hVEZT was successfully constructed.Conclusions VEZT protein was correlated with TNM staging,tumor stage,invasion depth and lymph node metastasis,positive VEZT expression predicting better five year survival.The VEZT over-expression plasmid vector was successfully constructed.

Objective To investigate the effect of astaxanthin on the peripheral blood endothelial progenitor cells (EPCs) apop-tosis induced by oxidative stress in vitro and to explore its underlying mechanism .Methods Human peripheral blood EPCs were in vitro cultured and divided into the control group ,model group with 100 μmol/L tert-butyl hydroperoxide(tBHP) and the astaxan-thin plus tBHP group(with 0 .10 ,1 .00 ,10 .00 nmol/L astaxanthin pretreatment for 24 h ,then adding the final concentration of 100μmol/L tBHP for 6 h continuous culture) .The cell viability was measured by the MTT method .The level of reactive oxygen spe-cies (ROS) was determined by the DCFH-DA method .The changes of mitochondrial membrane potential(MMP) and the apoptosis ratio were detected by the JC-1 method and the DAPI method ,respectively .Results Compared with control group ,100μmol/L tB-HP could obviously caused the apoptosis of EPCs(P<0 .05) ,while astaxanthin could decrease tBHP induced apoptosis ,which man-ifested by the decrease of the apoptosis ratio (P<0 .05) and MMP increase .Conclusion Astaxanthin has the protective effect on the apoptosis of EPCs ,its mechanism may be related with the protection of the mitochondrial function .

Objective To describe the clinical characteristics of one child with primary hyperoxaluria types Ⅲ, and to analyze the potential mutant genes in his family.Methods AGXT, GRHPR and HOGA1 genes were analyzed by direct sequencing analysis in this family.One hundred unrelated healthy subjects were also analyzed as controls.Results The child had early onset of symptoms (0.8 year).His principal clinical manifestation included nephrolithiasis and obstructive nephropathy, however his nephrocalcinosis was mild.And he presented high urine oxalate, high urine calcium, and lower citrate levels.Two novel heterozygous mutations in HOGA1 were identified (compound heterozygous), one mutation was a 2-bp substitution at the last position in exon 6 and the first position of intron 6 respectively (c.834_834 + 1GG ＞ TT);another was a guanine to adenine substitution of the last nucleotide of exon 6 (c.834G ＞ A).Both of these variants found in this study probably acted as splicing mutations.Direct sequencing analysis failed to find these mutations in 100 unrelated healthy subjects.In addition, a SNP (c.715G ＞ A, p.V239I) was found in this family.There were no mutations detected in AGXT and GRHPR.Conclusions Two novel mutations are identified probably in association with PH Ⅲ.This is the first description and investigation on mutant gene analysis of PHⅢ in Asia.

Objective To investigate the effects of left dorsolateral prefrontal cortex with 10HZ , 80%motor threshold repetitive transcranial magnetic stimulation ( rTMS ) on the cognitive function in unipolar depression. Method Sixty-nine patients with unipolar depression were randomly assigned to either the real rTMS group or the sham rTMS treatment( the control group) ,all patients were given extended releasing venlafaxine. the cognitive function was evaluated using P300 and Repeatable battery for the assessment of neuropsychological status ( RBANS-immediate memory, visuospatial, language, attention, delayed memory, total score), Wisconsin card sorting test ( WCST-Right responses, Completed categories, Total errors , Preservative errors, Nonpreservative errors ). The depression severity was assessed with the 24-item Hamilton Rating Scale for Depression(HAMD-24). All of tests were examined before and after six weeks with thirty times rTMS. Results At the end of six weeks treatments, regarding the WCST,real rTMS group showed better improvement in the right responses than control group(33.23±10.29 vs 27.09 ± 9.82, F= 16. 116 , P= 0. 000), besides right responses, real rTMS group had better performance in the rest items than control group(F=4.862 ～ 17.758, P= 0.031 ～ 0.000) ;concerning the RBANS, real rTMS group was significantly superior to control group in total score( (88.83 ± 16.48 ) vs (78.85 ± 13.51 ), F= 8. 425,P = 0. 005 ), besides total score , the real rTMS group had better performance in some rest factors than control group (F= 10.088 ～20. 801, P=0. 002 ～0.000);real rTMS group showed better improvement in the amplitude of P3than control group( (8.27 ± 2.97 ) μV vs ( 7.37 ± 2.66) μV, F= 5. 838 , P=0.018 ) ;real rTMS group demonstrated better improvement in HAMD-24 than control group( (4.7 ±2.4)vs ( 11.2 ±5.1 ), F= 29.537, P=0. 000).Conclusion rTMS can significantly improve cognitive function and depressive symptoms with unipolar depression.
