1.Optimization of diagnosis indicator selection and inspection plan by 3.0T MRI in breast cancer

Objective:To optimize 3.0T MRI diagnosis indicator in breast cancer and to select the best MRI scan program.
Methods:Totally 45 patients with breast cancers were collected, and another 35 patients with benign breast tumor served as the control group. All patients underwent 3.0T MRI, including T1-weighted imaging (T1WI), fat suppression of the T2-weighted imaging (T2WI), diffusion weighted imaging (DWI), 1H magnetic resonance spectroscopy (1H-MRS) and dynamic contrast enhanced (DCE) sequence. With operation pathology results as the gold standard in the diagnosis of breast diseases, the pathological results of benign and malignant served as dependent variables, and the diagnostic indicators of MRI were taken as independent variables. We put all the indicators of MRI examination under Logistic regression analysis, established the Logistic model, and optimized the diagnosis indicators of MRI examination to further improve MRI scan of breast cancer.
Results:By Logistic regression analysis, some indicators were selected in the equation, including the edge feature of the tumor, the time-signal intensity curve (TIC) type and the apparent diffusion coeffcient (ADC) value when b=500 s/mm2. hTe regression equation was Logit (P)=-21.936+20.478X6+3.267X7+21.488X3.
Conclusion:Valuable indicators in the diagnosis of breast cancer are the edge feature of the tumor, the TIC type and the ADC value when b=500 s/mm2. Combining conventional MRI scan, DWI and dynamic enhanced MRI is a better examination program, while MRS is the complementary program when diagnosis is diffcult.

Stem cell research is one of the most notable fields in life science at the end of 20th century and the beginning of 21st century,which has made a considerable contribution to every aspect of life science,especially to the regenerative medicine.However,a great debate on ethics has also arisened all over the world on the research of human embryonic stem cells.This paper gives the cause of the debate,provides the analysis of the stem cell legislations among various countries,which have different impacts on the research of stem cell and finally points out its enlightenment to our country.
3.Assessment of pulmonary function using pixel indexes of multiple-slice spiral CT low-dose two-phase scanning in chronic obstructive pulmonary disease

Objective To explore the values of pixel indexes (PI) with multiple-slice spiral CT low-dose two-phase scanning for assessing the pulmonary function in chronic obstructive pulmonary disease (COPD).Methods Thirty-six patients with COPD (COPD group)and 30 healthy people (control group)underwent pulmonary function test (PFT).Chest 64-MSCT low-dose (50 mAs) scanning at full inspiration and expiration,routine scanning (100 mAs) at inspiration were performed.The effective dose (ED) was calculated.The lung was divided into three regions ( upper,middle,lower).PI of lung were divided into five groups:-960--1024, -910--960, -800--910, -700--800,-400--700.ThePI-910 (sum of the PI under -910 HU) of low-dose scanning at each region were measured and calculated using pulmo software. All PI included PIin-910,PIex-910,PIin-910-PIex-910, PIex-910/PIin-910 and (PIin-910-PIex-910)/PIin-910.All patients underwent PFT within 3 days after 64-MSCT canning,FEV1% and FEV1/FVC were selected for comparison. Results The PIin in three regions ( - 960 - - 1024,-910 - -960,-800 - -910) were statistically significant between normal and COPD groups( U =0.00,57.00,20.50,P < 0.01 ). The PIex in all regions were statistically significant ( U =0.00,0.00,71.52,191.00,6.00,P < 0.01 ). PI-910--1024 at expiration and inspiration were correlated with FEV1% and FEV1/FVC( r =- 0.548,- 0.664,- 0.752,- 0.781,P < 0.01 ).PIin -910,PIex -910,PIex-910/PIin -910,( PIin -910-PIex -910 )/PIin -910 had a good correlation with FEV1% and FEV1/FVC ( r =- 0.548,- 0.664,- 0.752,- 0.781,- 0.674,- 0.642,0.674,0.642,P < 0.01 ).Conclusion Pixel indexes of 64-MSCT low-dose two-phase scanning can be used to evaluate pulmonary function in COPD patients.
4.An Experimental Study on the Early Changes in ~(18)F-FLT and ~(18)F-FDG Uptake of Tumor Cells after Chemotherapy

Purpose:The efficacy of evaluation of changes of tumoral uptake of 3'-deoxy-3'-[~(18)F] fluorothymidine (FLT) was comparatively analyzed with that of ~(18)F-FDG at early stage after anticancer chemotherapy.Materials and Methods:Cells derived from human lung adenocarcinoma were incubated with cisplatin (CDDP),5-fluorouracil(5-FU),doxorubicin (Dox),for 1,4,24 and 72h.The doses(CD-DP: 67 μM; 5-FU 1,540 μM;MTX: 440 μM;) were determined corresponding to a estimated 10% - 95% proliferation inhibition.The cells were allowed to recover before FLT or FDG being added into the culture media for 60 min.Cell counts,viability,estimated by MTT method,were used to evaluate the cytotoxic effects of chemotherapy.Results: FLT uptake was increased significantly at 1 and 4 h after treatment with 5-FU( 145 ± 12%,150 ± 14%,P <0.01).decreased at 24 h and 72 h.In contrast,FLT accumulation was significantly reduced at cytostatic concentrations of CDDP at different time.The uptake of FDG did not change significantly at early time points after treatment,but decreased at 72 h.Conclusion: The tumor cell uptake of FLT revealed specific changes depending on the auti-cancer drug used at much earlier time than FDG after chemotherpay.
5.Simultaneous determination of five constituents in Jiangzhi Granules by HPLC

AIM To establish an HPLC method for the simultaneous content determination of five constituents in Jiangzhi Granules (Salviae miltiorrhizae Radix et Rhizoma,Polygoni cuspidati Rhizoma et Radix,Artemisiae scopariae Herba,etc.).METHODS The analysis of methanol extract of this drug was performed on a 30 ℃ thermostatic Hanbon-C18 column (4.6 mm × 250 mm,5 μm),with the mobile phase comprising of acetonitrile (A) 0.1% phosphoric acid (B) flowing at 1.0 mL/min in a gradient elution manner,and the detection wavelengths were set at 286 nm (0-30,40-50 min) and 203 nm (30-40 min).RESULTS Gypenoside A,chlorogenic acid,polydatin,salvianolic acid B and emodin showed good linear relationships within the ranges of 0.176-2.46,0.056-0.84,0.36-5.40,0.192-2.88 and 0.12-1.80 μg (r >0.999 0),respectively,whose average recoveries were 97.17%-102.59% with the RSDs of 1.9%-2.9%.CONCLUSION This accurate and simple method can be used for the quality control of Jiangzhi Granules.
6.Impact of Bayesian penalized likelihood algorithm on the metabolism parameters and image quality in 18F-FDG PET/CT background tissues

Objective:To investigate the impact of Bayesian penalized likelihood (BPL) PET reconstruction method on the uptake of 18F-fluorodeoxyglucose (FDG) and signal to noise ratio (SNR) of lungs, liver, aorta and bones. Methods:From March 2019 to June 2019, the 18F-FDG PET/CT images of 60 patients with clinical diagnosed tumors (29 males, 31 females, age: 24-89 (60.4±15.2) years) in Yuhuangding Hospital were retrospectively analyzed. PET images were reconstructed with ordered subset expectation maximization (OSEM), time of flight (TOF)+ point spread function (PSF) and BPL (β=350) algorithms. Volumes of interest (VOIs) were delineated on the right upper lung lobe, the right liver, aortic root and lumbar vertebra. The mean standardized uptake value (SUV mean), maximum standardized uptake value (SUV max), peak of lean body standardized uptake value (SUL peak), standard deviation of standardized uptake value (SUV SD) and the SNR were measured. The percentage of SNR change (%ΔSNR) between the BPL method and non-BPL methods were calculated. The correlations between body mass index (BMI) and %ΔSNR were analyzed by Pearson correlation analysis. One-way analysis of variance and the least significant difference (LSD) t test were used to analyze the data. Results:There were no significant differences of SUV mean and SUL peak in lung, aorta, liver and lumbar vertebra among 3 methods ( F values: 0.04-1.95, all P>0.05). The SUV max in lung, aorta, liver and lumbar vertebra of BPL reconstruction (1.14±0.82, 2.13±0.37, 2.95±0.50 and 2.76±0.87) was significantly lower than those of TOF+ PSF (1.56±0.61, 2.99±0.75, 4.32±0.94 and 4.05±1.48) and OSEM (1.51±0.67, 3.00±0.70, 4.45±1.12 and 3.81±1.06) reconstructions ( F values: 20.59-52.24, all P<0.001) and SUV SD (0.13±0.07, 0.20±0.05, 0.26±0.06, 0.38±0.17) was also significantly lower than those of TOF+ PSF (0.24±0.11, 0.43±0.11, 0.58±0.15, 0.67±0.21) and OSEM (0.21±0.09, 0.42±0.10, 0.58±0.14, 0.63±0.20) reconstructions ( F values: 24.46-124.95, all P<0.001), while the SNR (4.67±1.34, 7.74±2.22, 8.17±1.77, 4.45±1.22) was significantly higher than those of TOF+ PSF (2.54±0.72, 3.55±0.82, 3.77±0.91, 2.49±0.69) and OSEM (2.65±0.64, 3.67±0.80, 3.75±0.87, 2.60±0.67) reconstructions ( F values: 83.04-247.73, all P<0.001). However, there were no significant differences between OSEM and TOF+ PSF reconstructed images in SUL peak, SUV mean, SUV SD and SNR (all P>0.05). In BPL group, SNR increased with the increase of BMI, and there were statistically differences of aortic SNR (7.07±2.21 vs 9.67±2.26) and liver SNR (7.75±1.85 vs 9.32±0.70) between BMI<25 kg/m 2 and BMI≥30 kg/m 2 ( F values: 3.46 and 4.19, both P<0.05). Positive correlations were found between %ΔSNR of lung, aorta, liver and lumbar vertebra in OSEM and TOF+ PSF and BMI ( r value: 0.042-0.354, all P<0.05). Conclusion:In background tissues, BPL algorithm has no significant impact on absolute quantification compared with OSEM and TOF + PSF reconstruction methods but it can significantly improve SNR, especially for the patients with large body weight.
7.Study on Blood-stasis Syndrome Induced by Cold: Target on the Tongue

Objectives:To observe the changes of the tongues in rats with blood-stasis syndrome induced mainly by cold. To explore the evidence that tongues were the target of blood stasis syndrome and the target of medication. Methods: Rats with blood-stasis syndrome induced by cold (BSC group) were dipped into ice water (0 ℃) for 5 minutes every day and lasted 20d individually. Different drugs were given orally after the model establishment. And then we took photos of tongues of all rats by digital camera, analyzed the gray scale value of all rats' tongues using image analysis software, and observed the capillaries in tongues by electron microscope. Results: The tongues of rats which had been frozen in ice water for 20 d (once a day) were dark purple, the same as that of models induced by chemical materials .While as for the normal rats, the tongues were lustrous and ruddy. The changes of color of the tongues persisted about 1 week and were great obvious at the 3rd day after the model establishment among different time-points. After given different drugs, the degree of dark purple tongue degraded, showed significant difference (P < 0.01) from that of model rats. The capillary stegnosis and nucleus turgescence of vascular endothelial cell were observed in tongues in BSC group by using electron microscope. After medication, the above changes recovered. Conclusion: The degree of dark purple tongue indicates the degree of diseases with blood-stasis syndrome. The tongue probably is a target of drug treatment.
8.Clinical Study on Prevention and Treatment of Acute Radiation-Induced Oral Mucositis in Patients with Head and Neck Tumor Using Yangyin Jiedu Decoction

OBJECTIVE To observe the clinical efficacy of Yangyin Jiedu Decoction in the prevention and treatment of acute ra-diation-induced oral mucositis(RTOM).METHODS A total of 80 inpatients who were diagnosed with head and neck tumors by pathological examination and received radiotherapy in the Department of Radiotherapy,Affiliated Hospital of Nanjing University of Chi-nese Medicine from November 2021 to September 2023 were selected and randomly divided into an observation group and a control group with 40 cases in each group.The control group was given mouthwash treatment from the onset of RTOM symptoms until the symp-toms disappeared;the observation group was given Yangyin Jiedu Decoction from the first day of radiotherapy on the basis of the treat-ment of the control group until the end of radiotherapy.During the treatment,the onset time,duration and incidence of grade Ⅱ-ⅣRTOM in the two groups were observed;the pain numerical rating scale(NRS)score,Karnofsky performance status(KPS)score,body mass index(BMI)changes were evaluated;and the levels of serum inflammatory factors[C-reactive protein(CRP),interleukin 6(IL-6),interleukin 1β(IL-1β),tumor necrosis factor α(TNF-α)]were detected.RESULTS After radiotherapy,RTOM ap-peared in both groups to varying degrees.The incidence of grade Ⅱ-Ⅳ RTOM,the onset time,duration and NRS score of RTOM in the observation group were significantly better than those in the control group(P<0.05,P<0.01).After radiotherapy,the levels of se-rum CRP,IL-6,IL-1β and TNF-α in the observation group were lower than those in the control group(P<0.05,P<0.01).After radiotherapy and 1 month of follow-up,the KPS score and BMI in the observation group were higher than those in the control group(P<0.05,P<0.01).CONCLUSION Yangyin Jiedu Decoction can decrease the incidence and severity of RTOM in patients with head and neck tumors,shorten its duration,improve the quality of life of patients,downregulate the level of inflammatory cytokines,and has a preventive effect on RTOM caused by radiotherapy in patients with head and neck tumors.
9.Comparison of Muscle Activation and Co-Contraction Patterns for Trunk Muscles During Four-Point Hand-Knee and Hand-Foot Kneeling Positions

Objective To analyze the similarities and differences between four-point hand-knee position and hand-foot kneeling positions in trunk muscle activation and co-contraction, explore the possibility of hand-foot kneeling position as core stabilization exercises, so as to provide suggestions for actual training.Methods Nineteen healthy volunteers randomly performed exercises of four-point hand-knee position (4 motions) and hand-foot kneeling position (3 motions), while surface electromyography (sEMG) signals were collected from bilateral rectus abdominis, external oblique, erector spine, and multifidus muscles. The average sEMG and muscle co-contraction index (CCI) based on the sEMG signals were analyzed and compared.Results Significant differences were found in the sEMG and CCI within and between the two positions. Under four-point hand-knee position with the right hand and left leg lifting, the activation of all muscles was higher than that in the starting position. In four-point hand-knee position with the left leg lifting, the activation of ipsilateral multifidus muscle was significantly higher than that in hand-foot kneeling position. The activation degree of external oblique muscle and rectus abdominis was higher in hand-foot kneeling position with right hand lifting. A total of 28 muscle matching methods were obtained by pair-to-pair matching of 8 muscles. The starting posture in four-point hand-knee position fluctuated the least, indicating that the spine was the most stable, while the index of other exercises fluctuated in a larger range.Conclusions The possibility of hand-foot kneeling position as core stabilization exercises was proved from two aspects, namely, muscle activation and CCI. The hand-foot kneeling position and four-point hand-knee position can be used for strengthening abdominal muscles and back muscles, respectively. The four-point hand-knee position with contralateral upper and lower limbs lift is a more advanced exercise for trunk muscles, but sports injuries should be avoided.
10.Biomechanical analysis of ankle-foot complex during a typical Tai Chi movement-Brush Knee and Twist Step.

The purpose of this study is to analyze the biomechanics of ankle cartilage and ligaments during a typical Tai Chi movement-Brush Knee and Twist Step (BKTS). The kinematic and kinetic data were acquired in one experienced male Tai Chi practitioner while performing BKTS and in normal walking. The measured parameters were used as loading and boundary conditions for further finite element analysis. This study showed that the contact stress of the ankle joint during BKTS was generally less than that during walking. However, the maximum tensile force of the anterior talofibular ligament, the calcaneofibular ligament and the posterior talofibular ligament during BKTS was 130 N, 169 N and 89 N, respectively, while it was only 57 N, 119 N and 48 N during walking. Therefore, patients with arthritis of the ankle can properly practice Tai Chi. Practitioners with sprained lateral ligaments of the ankle joint were suggested to properly reduce the ankle movement range during BKTS.
