1.The diagnosis and treament of pseudoaneurysm in 17 cases

Objective To evaluate the diagnosis and treatment of pseudoaneurysm .Methods The clinical data of 17 cases of pseudoaneurysm from Jan .2007 to Dec .2011 were reviewed retrospectively ,10 cases were caused by iatrogenic ,6 cases were drug caused by injection of poison ,and 1 case was was caused by trauma .2 cases of brachial artery pseudoaneurysm ,and 15 cases of arte-ria femoralis pseudoaneurysm .1 case was treated by simple compressive therapy ,the other 16 underwent surgical treatment and the pseudoaneurysm was removed ,in which 3 cases were treated by artery patch repair ,7 were treated by bypass with prosthetic grafts , 2 were treated by bypass with saphenous vein ,4 were treated by bypass with superficial femoral vein branch .Results There was no death except 1 case died because her family gave up ,All patients with signs and symptoms were significantly alleviated ,and there was no severe ischemia limb leading to amputation in postoperative follow-up .Conclusion The revascularization of the artery in pseudoaneurysm patients can make a significant improvement in the long-term quality of life .
2.Management and practice of intra-laboratory Turn-Around-Time in emergency testing laboratory

Objective To master the actual work efficiency situation of emergency inspection,analyze the existing problems,and provide objective basis for continuous improvement through the statistic analysis of intra-laboratory turn-around time (TAT) median,coincidence rate of inspection report within the specified time limit,report timeout reasons and other indexes of different items of emergency inspection group in 2015.Methods All kinds of original data from the different items of emergency inspection groupswere derived from LIS system,according to the different types of projects in different segments.Microsoft Excel2003 software was used to count the actual TAT time of various items,and function was used to calculate TAT median;The non-conformance rate (inspection report timeout rate) and coincidence rate within the prescribed time limit by inspection report were calculated,and gather the statistical analysis on the timeout reasons of daily recorded report.Results With the exception of electrochemical luminescence class,the TAT of other items decreased in different degrees of emergency inspection group in 2015 when compared with 2014.The average coincidence rate of inspection report of various items within the time limit was 94.8%,non-conformance rate of inspection report of prescribed time limit was 5.2%.The top four inspection timeout reasons of various items were more number of submitted samples(59.6%),redo the reinspection (12.4%),smear reinspection (12.0%) and equipment failure (10.0%) in turn.Conclusion The time limit of the emergency report meets the specified requirements and statistical analysis of the causes of overtime can find out the problems and take appropriate measures to provide an objective basis for continuous improvement.
3. One male case with accessory breast

To introduce the diagnosis and treatment of one male case with accessory breast. The patient was a 27-year-old male. An additional nipple on the left posterior axillary line was found on birth, which enlarged and became darken since adolescence. The clinical diagnosis was accessory breast, and a complete surgical excision was performed. Pathological examination proved the diagnosis of accessory breast.
4.Left atrial dimension and atrial fibrillation in surgical heart disease patients

Objective The effect of left atrial (LA) dimension on the occurrence of atrial fibrillation (AF) has been examined in some small studies.Less is known about the relationship of LA dimension,hemodynamic with AF during echocardiographic evaluation,especially,the flow dynamics in LA poorly described.The objective of this study was to investigate the relationship between LA dimension and the occurrence of AE Methods Two hundred and forty-five consecutive patients with heart disease scheduled to undergo open heart surgery were prospectively enrolled in the study.Patients were divided into 2 groups according to atrial fibrillation:AF group (n=148,99 men and 49 women,with a mean age 59.3+8.4 years),and no-AF group (n= 97,60 men and 37 womem).Echocardiography was performed before surgery.All measurements were performed following the American Society of Echocardiography recommendations.Results There were more patients with congestive heart failure in AF group than in no-AF group (45.9% vs 39.1%,P <0.05).The mean LA volume was 49.2±12.2 ml/m2 in AF group and 33.1±10.8 ml/m2 in no-AF group.There were also significant differences between two groups in left atrial end systolic dimension (LAESD) (50±13mm vs 27±14mm),left atrial end diastolic dimension (LAEDD) (79±17mm vs 53±13mm),PA pressure ( 41.3+11.6 mmHg vs 37.5±10.4 mmHg),and ratio of mitral E velocity and septal mitral annulus motion velocity (E/E') .The percentage of abnormal diastolic function grades (DGF) was also higher in AF than in no-AF group (89.9% versus 59.8% );.Conclusion Atrial fibrillation is associated more frequently with an increased LA dimension and more severe atrial hemodynamics disorder.(J Geriatr Cardiol 2008;5:11-4)
5.Minimally invasive hybrid surgery for Stanford B aortic dissection involving the aortic arch

Objective:To investigate the effect and safety of one-staged hybrid surgery in the treatment of complicated Stanford B aortic dissection.Methods:246 consecutive patients who underwent one-staged hybrid technique with complicated type B aortic dissection (cTBAD) between January 2014 and July 2020 were retrospectively reviewed. The perioperative mortality and morbidity of the hybrid technique was assessed and the early results of follow up were evaluated.Results:The mean age of patients was(57.1±11.2)years old, and 175 (71.1%) were male. There were 166 cases of left common carotid artery-left subclavian artery vessels bypass + TEVAR (A group), operation time(62±8)min, post-operated hospitalization(5.2±1.3)days; 62 cases of right common carotid artery-left common carotid artery vessels bypass+ left common carotid artery-left subclavian artery vessels bypass + TEVAR (B group), operation time(88±12)min, post-operated hospitalization(6.1±1.8)days; 18 cases of debranch + TEVAR, operation time(236±36)min, post-operated hospitalization(8.8±2.1)days. 246 patients underwent hybrid procedure successfully (The technique success rate was 100%). 1 patient(0.4%)with traumatic aortic dissection (with cerebral trauma) died after operation 7 days. The average follow-up period was 5 years(30.3±7.1)xmonths. 236 patients(96%) without endoleak, 10 patients(4%)with proximal endoleak, 5 patients with regular follow-up, 5 patients with surgical treatment during one year (3 patients with re-TEVAR; 2 patients with opening operation); 25 patients(10.2%)incision hematomas, all these patients settled with closely observing and conservative treatment, and all these patients without reintervention. The rate of second intervention was 2% (5/246) the patency of bridging vessel was 99.7% (343/344). There was no paraplegia and stroke during perioperative period.Conclusion:One-staged hybrid surgery has a good short-term and med-term efficacy and safety for complicated Stanford B aortic dissection.
6.Application of fusion imaging in the treatment of complex aortic pathology

Objective:To explore the value of fusion imaging in the treatment of complex aortic pathology.Methods:A retrospective analysis was conducted of 29 patients with complex aortic pathology who underwent treatment with endovascular aortic repair using fusion imaging (FI+ ) technique or without FI (FI-) between June 2015 and June 2021. The perioperative outcomes and morbidity of the FI was assessed and the early results of follow up were evaluated.Results:The mean age of patients was (70.3±7.3) years old, and 24 (82.8%) males. Technical success was 96.5% (28/29). The FI+ group patients had lower procedure time[FI+ , (209±53) min vs. FI-, (306±24)min, P=0.005]and ionic contrast medium[(169±23)ml vs. (201±20)ml, P=0.040]. Contrast-induced acute kidney injury (CI-AKI)[3.4%(FI+ 0 vs. FI-6.7%, P=0.33)], and operation-related reintervention[6.9%(FI+ 0 vs. FI-13.3%, P=0.16)] were similar. There were no significant differences in blood loss, fluoroscopy time. Conclusion:FI technique improves the accuracy during positioning in complex endovascular aortic repair, could reduce aortic related reintervention rate, operation time and contrast dose. Further studies and development are needed to obtain optimal image quality and higher precision.
