1.Physiological factors of anxiety and depression in health check-up populations: Logistic regression analysis

Objective To investigate the relationship between mental status and some physical indicators,so as to identify the influencing factors of anxiety and depression in health check-up populations.Methods A total of 3885 health check-up adults enrolled in this investigation.Self-rating anxiety scale (SAS),self-rating depression scale (SDS),and Symptom Checklist 90 (SCL90) were used to evaluate mental health of the participants.Physical indicators,including body mass index (BMI),systolic blood pressure (SBP),diastolic blood pressure (DBP),triglyceride (TG),low-density lipoprotein cholesterol (LDL-C),serum free three iodine armour gland original acid (FT5),serum free thyroxine (FT4),and thyroid-stimulating hormone (TSH),were retrospectively reviewed.Pearson correlation analysis was used to assess the relationship between mental health and physical signs,and the Logistic regression analysis was performed to confirm the influencing factors of anxiety and depression.Results Increased prevalence of anxiety and depression was found in females or those with lower education level.For the health check-up populations,anxiety and depression were negatively correlated to BMI,SBP,DBP,LDL-C,FT3 and FT4.In Logistic regression analysis,LDL-C,FT3 and FT4 were related to SDS score,while SBP,FT3,and FT4 were related to SAS score.Conclusion Our data suggest that emotion could be affected by gender and education level; SBP,LDL-C,FT3,and FT4 may be protective factors of anxiety and depression.
2.The effects of early and systematic hospital-and-family rehabilitation on the growth of premature infants

Objective To explore the effects of early hospital-family comprehensive rehabilitation on the development of pretenn infants. Methods A total of 256 premature infants were chosen and divided into an early intervention group (n = 148) and a control group (n =108). Besides being given the conventional mothering instruction , the early intervention group was given early assessment, regular visits and hospital-family comprehensive rehabilitation treatment. The control group was only given the conventional mothering instruction. The fine and gross motor growth quotients, adaptation, verbalisation and social behavior and general growth quotient of all the premature infants were assessed periodically using the infant neuropsychology growth scale. Results The growth quotient assessment indicators of the infants receiving the intervention were obviously better at the 6th, 12th, 18th and 24th month after birth, and the differences between the two groups were statistically significant. The incidence of cerebral palsy in the early intervention group was 0.71% (1/148), with only one cerebral palsy infant in the early intervention group who was at level Ⅲ of the gross motor function classification system ( GMFCS) , while the incidence of cerebral palsy in the control group was 5.1% (5/98) , with 5 cerebral palsy infants, one of whom was at GMFCS level Ⅲ and 4 of whom were at level Ⅳ. Conclusions Early systematic hospital-family comprehensive rehabilitation can improve the general growth of premature infants, decrease the incidence of cerebral palsy, and neurobehavior deficits.
3.AG200 and polysomnography in patients with obstructive sleep apnea-hypopnea syndrome.

OBJECTIVE:
To study the clinical value of polysomnography (PSG) and ApneaGraph (AG200) in the diagnosis evaluation of obstructive sleep apnea hypopnea syndrome (OSAHS).
METHOD:
From January to December in 2012, 67 OSAHS patients diagnosed by PSG were examined by AG200. The apnea hypopnea index (AHI), hypopnea index (HI), apnea index (AI), the lowest oxygen saturation (LSaO2) was were detected and the results were analyzed statistically.
RESULT:
Significant differences were observed in AI, AHI, LSaO2 between AG200 and PSG (P < 0.05, respectively). No statistically significant difference was found in HI. The differences in HI between AG200 and PSG were not significant (P > 0.05). AHI, HI and LSaO2 was were significantly correlated between AG200 and PSG (r = 0.870, 0.743, 0.374, 0.716, P < 0.01).
CONCLUSION
AG200 could not replace PSG but could identify the level of upper airway obstruction.

4.Mortality of dementia and its major subtypes in urban and rural communities of Beijing.

OBJECTIVETo identify the mortality and epidemiological pattern of dementia and its various major subtypes among urban and rural senior residents in Beijing.
METHODSBased on Beijing's dementia prevalence survey among residents aged 55 years and above in 1997, respondents were selected by stratified multiple-stage cluster sampling and 12 urban communities and 17 rural village communities were randomly sampled then follow-up in 2001. COX regression was used to analyze relative risks controlling confounding factors on deaths of dementia cases.
RESULTSThe mortality of dement patients in the 55-64 age-group was 0.82/100 person-year. The age-standardized mortality of dement cases was 0.90/100 person-year. The mortality in the 65 and above age-group was 1.44/100 person-year, and the age-standardized mortality was 1.56/100 person-year. Among AD cases, the above two mortalities were 0.35/100 and 0.42/100 person-year respectively, and among VaD cases, 0.34/100 and 0.36/100 person-year respectively. For both AD and VaD cases, their mortality increased with age. Region, gender and age were more significant to survival of AD cases.
CONCLUSIONOne major subtype of dementia, AD, among elderly urban and rural residents in Beijing, has a different mortality and epidemiological pattern from VaD.

5.Prediction of the onset time of acute stroke by deep learning based on DWI and FLAIR

Objective:To evaluate the effect of deep learning based on DWI and fluid attenuated inversion recovery (FLAIR) to construct a prediction model of the onset time in acute stroke.Methods:A total of 324 cases of acute stroke with clear onset time, from January 2017 to May 2020 in Nanjing First Hospital, were retrospectively enrolled and analyzed. The patients were divided into a training set of 226 patients and a test set of 98 patients according to the complete randomization method using a 7∶3 ratio, and the patients were divided into ≤ 4.5 h and >4.5 h according to symptom onset time in each group. The acute infarction areas on DWI and the corresponding high signal area on FLAIR were manually outlined by physician. Using the InceptionV3 model as the basic model for image features extraction, the deep learning prediction model based on single sequence (DWI, FLAIR) and multi sequences (DWI+FLAIR) were established and verified. Then the area under curve (AUC), accuracy of human readings, single sequence model and multi sequence model in predicting the acute stroke onset time from imaging were compared.Results:DWI-FLAIR mismatch was found in 94 cases (94/207) of patients with symptom onset time from imaging ≤ 4.5 h, while in 28 cases (28/117) of patients with symptom onset time from imaging >4.5 h. ROC analysis showed that the AUC of DWI-FLAIR mismatch in predicting acute stroke onset time from imaging was 0.607, and the accuracy was 60.2%. The prediction model of deep learning based on single sequence showed that the AUC of FLAIR was 0.761 and the accuracy was 71.4%; the AUC of DWI was 0.836 and the accuracy was 81.6%. The AUC of predicting stroke onset time based on the multi-sequence (DWI+FLAIR) deep learning model was 0.852, which was significantly better than that of manual identification ( Z = 0.617, P = 0.002), FLAIR sequence deep learning model ( Z = 2.133, P = 0.006) and DWI sequence deep learning model ( Z = 1.846, P = 0.012). Conclusion:The deep learning model based on DWI and FLAIR is superior to human readings in predicting acute stroke onset time from imaging, which could provide guidance for intravenous thrombolytic therapy for acute stroke patients with unknown onset time.
6.Efficacy and safety of HIFU lesion combined with endometrial ablation in treatment of adenomyosis with excessive menstruation

Objective To evaluate the efficacy and safety of high intensity focused ultrasound(HFIU)combined with endometrial ablation in the treatment of adenomyosis(AM)patients with heavy menstrual bleeding(HMB).Method A total of 199 patients with AM combined with HMB who underwent HIFU treatment at the Obstetrics and Gynecology Center of Affiliated Hospital of Chengdu University of TCM from July 2020 to September 2022 were selected and divided into two groups according to the ablation range.The combined ablation group included 80 cases with lesion and endometrial ablation(endometrial ablation rate≥30%),while the lesion ablation group 119 cases with simple lesion ablation.A 1∶1 propensity score matching was performed on the two groups,resulting in 59 patients in each group.A 6-month follow-up was conducted.The clinical manifestations(menstrual volume,degree of dysmenorrhea,uterine and lesion volume),quality of life(MS-QOL,UFS-QOL),and postoperative adverse reactions between the two groups before and 6 months after surgery was compared.Results Menstrual volume,degree of dysmenorrhea,uterine and lesion volume,MS-QOL score,and UFS-QOL score of the both groups were improved 6 months after surgery,when compared to those before surgery(P<0.05).Significantly reduced menstrual volume,degree of dysmenorrhea,and lower MS-QOL score were observed in the combined ablation group when compared with the lesion ablation group(P<0.05),but there was no statistical significance in uterine and lesion volume and UFS-QOL score(P>0.05).There was no difference in the incidence of adverse reactions between the two groups(P>0.05),but the combined ablation group had longer vaginal bleeding and fluid flow time(P<0.05).Conclusion The combined HIFU and endometrial ablation can effectively reduce menstrual flow,alleviate dysmenorrhea,and improve quality of life in patients with AM combined with HMB.Although it increases vaginal bleeding and fluid flow time,reasonable symptomatic management can effectively prevent the occurrence of adverse consequences such as infection.
7.The Connotation and Clinical Significance of “Spleen Governs Time” based on the Zangxiang (藏象) Time-space View

There are different views on the theory of “spleen governs time”, which is still a hot spot in the study of Zangxiang (藏象) theory. Based on Zangxiang time-space view, it is found that the thinking mode of the spleen governing time theory follows space-time logic. It is believed that the different time views of the spleen governing time are all formed based on the space view that the spleen belongs to earth and resides in the center, and the zang time theory is developed with the unified time and space logic. Guided by Zangxiang time-space view, the origin of the spleen belonging to earth and residing in the center is traced, and the theoretical connotation and its clinical application of spleen governing time under different time-space logic are explored with reference to the four season and five zang theory, five season and five zang theory, six season and six zang theory, and eight season and eight zang theory.
