1.Autogenous femoral head bone grafting combined with total hip arthroplasty for the treatment of old dislocation of hip joint center in 16 cases.

OBJECTIVETo study effects of autogenous femoral head bone grafting combined with total hip replacement for the treatment of old center dislocation of hip joint, and to evaluate the superiority of acetabular defect reconstruction with autogenous bone.
METHODSFrom March 2008 to December 2013, 16 patients (16 hips) with old center dislocation of hip joint were treated with autologous bone graft combined with total hip replacement. There were 11 males and 5 females, ranging in age from 41 to 72 years old, with an average of 56.3 years old. The duration of the disease ranged from 3.6 to 37.2 years. Twelve patients had injuries caused by traffic accidents and 4 patients were caused by falling down. The hip joint pain at the first month after operation, Harris score and hip joint movement of all patients were observed by using electronic case follow-up system.
RESULTSAll the patients were followed up, and the duration ranged from 11 to 78 months, with an average of 27.3 months. The postoperative hip joint movement of (56.2±23.4)° VAS 86.3±7.2 and Harris score 32.6±12.6 were all better than preoperative (181.8±17.6)°, 11.1±2.6 and 86.3±7.2. The joint pain was reduced and the function limitation was improved after operation, and the prosthesis position was good.
CONCLUSIONWhen the autologous femoral head bone grafting combined with total hip arthroplasty is used to treat old hip central dislocation, the initial and long-term stability of acetabular bone grafting is good, and the technique can avoid allogeneic bone complications, bone source rationally and reduce the economic burden of patients.

2.Protective effect of ischemic preconditioning on coronary microcirculation function by intracoronary Doppler guidewire assessment

Objective To investigate the effect of ischemic preconditioning (IPC) on coronary microcirculation function in short-term hibernating myocardium(SHM) animal model.Methods Twelve little domestic Chinese pigs were established as the model of SHM by interventional method (closed-chest) and divided into 2 groups at random: the control group (CON group, n =6) and ischemic preconditioning group (IPC group, n =6).IPC was elicited by 2 cycle of 5 min of ischemia followed by 5 min of reperfusion before the establishment of model.Intracoronary Doppler guidewire was used to measure average peak velocity (APV),diastolic systolic velocity ratio(DSVR) and coronary flow reserve(CFR) of the distal to the stenosis at the baseline and 10 min,30 min,60 min,120 min after the establishment of model.Results APV,DSVR and CFR all decreased significantly in SHM model in both groups( P 0.05 ),at 60 min CFR of the CON group decreased significantly than before( 0.96 ? 0.27 vs 1.74 ? 0.49 , P 0.05 ).Conclusions The coronary microcirculation dysfunction happened at the time point of 60 min after coronary stenosis in SHM model,IPC can protect the coronary microcirculation function in SHM model.

4.Chinese version of the SRS-22 patient questionnaire for adolescent idiopathic scoliosis:transcultural adaptation and reliability analysis

[Objective]To translate the SRS-22 Questionnaire from English edition into Chinese version for the use of Chinese adolescents with idiopathic scoliosis and to evaluate the metric qualities of this questionnair. [Method] Transcultural adaptation of the SRS-22 Questionnaire was carried out according to the International Quality of Life Assessment Project guide-lines,The final version was decided on by a committee of experts.The Questionnaire was completed by 86 adolescents(M=11,F=75) with idiopathic scoliosis.The patients were 10~18 years old(mean,13.9 years).Curve magnitude ranged from 25? to 45?(mean,35.6?).Out of them,30 patients repeated the questionnaire 3~4 weeks later.[Result]The overall alpha coefficient of the questionnaire was 0.88.Coefficients for individual domains were as follows:function/activity,0.70;pain,0.80;self-image,0.80;mental health,0.88;and satisfaction,0.81.The questionnaire as a whole had a test-retest correlation coefficient of 0.97.Test-retest correlation coefficients for individual domains were as follows:function,0.85;pain,0.96;self-image,0.96;mental health,0.95;and satisfaction,0.91.Six common factors were acquired from factor analysis,and the cumulative contribution rate was 72.29%. [Conclusion] The Chinese version of the SRS-22 Questionnaire has eligible reliability and validity.It can be used to measure health-related quality of life of adolescents with idiopathic scoliosis in China.
5.Evaluation of effect of exercise therapy intervention on military patients with low back pain

Objective To evaluate the interventional of bare-handed core muscle strength training and the sling exercise therapy (SET) on low back pain.Methods The soldiers of an Air Force Station who suffered from low back pain participated in the study.One hundred and forty-four patients were equally divided into three groups: the core muscle strength group (using core muscle strength training and the general physical therapy), the SET group (using SET and general physical therapy), and the control group (using the general physical therapy).All the groups were tested for 6 weeks.Questionnaires were distributed to the three groups before intervention, two weeks, four weeks and six weeks after intervention in order to find out about the degree of LBP, lumbar function and activity of these cases.Changes in the lumbar muscle strength were observed.Results The number of the patients who completed the test included 43 cases in the bare-handed core muscle strength group, 43 cases in the SET group and 44 cases in the control group, respectively, the VAS scores were 3.25, the Roland scores were 5.83,the FFD scores were 8.58 of the bare-handed core muscle strength group;the VAS scores were 3.30, the Roland scores were 5.93,the FFD scores were 8.10 of the SET group;the VAS scores were 2.41, the Roland scores were 3.03,the FFD scores were 7.40 of the control group.The LBP intervention efficiency of the three groups was 90.90%, 85.13% and 46.82%, respectively.There was no significant difference in indexes between the test groups(P>0.05).But compared with the control group,there was significant difference(P<0.05).Conclusion The bare-handed core muscle strength training and SET have obvious effects on LBP cases.According to their own infrastructure and facilities, military health care organizations in different environments can intervene in LBP cases with the bare-handed core muscle strength training or SET to alleviate the pain effectively.
6.Speckle Tracking Imaging in Assessing Left Ventricular Function of Obstructive Sleep Apnea

PurposeObstructive sleep apnea (OSA) can lead to structure and function damages of multiple organs. OSA is closely related to cardiovascular diseases. This study aims to evaluate left ventricular function in OSA patients with normal left ventricular ejection fraction.Materials and MethodsSixty OSA patients were divided into mild, moderate and severe group according to the clinical diagnosis, with 20 patients in each group. Another 20 healthy volunteers were recruited as normal control group. Strain (S), systolic strain rate (SRS), early diastolic strain rate (SRE) and late diastolic strain rate (SRA) values in longitudinal, circumferential and radial directions of all the subjects were measured by using speckle tracking imaging. The SRE/SRA rates in three directions were calculated.ResultsLeft ventricular longitudinal systolic function showed significant lower in moderate and severe groups than in the control and mild groups (P<0.05). Circumferential and radial contraction reduced in the severe group, and the difference was statistically significant (P<0.05). The longitudinal, circumferential, and radial diastolic functions of the myocardial fibers in the left ventricle decreased in moderate and severe OSA patients when compared with that of the mild OSA patients and healthy individuals (P<0.05).ConclusionThe speckle tracking imaging has the potential to detect the subclinical left ventricular dysfunction in OSA patients with normal left ventricular ejection fraction and might provide useful information for risk stratification.
7.Protective effect of Danshen Tablet on vascular endothelial function

AIM:To investigate the protective effects of Danshen Tablet on the brachial artery flow-mediated dilatation(FMD)in the subjects accompanied with risk factors of coronary heart disease(CHD). 8.Community-acquired Lower Respiratory Infection in Older Patients with Cerebrovascular Disease Sequelae:Bacterial Etiology and Resistance Analysis

OBJECTIVE To investigate the pathogen spectrum and resistance in older cerebrovascular patients with sequelae,so as to provide the evidence for antibiotic applied clinically in these patients. METHODS Retrospective survey of 320 cerebrovascular patients with lower respiratory infection admitted in respiration department from 2003 to 2004 was conducted.RESULTS The occurrence of Gram-negative bacilli(60.41%) in cerebrovascular patients with lower respiratory infection was obviously high,among which the first places were Pseudomonas aeruginosa(24.03%),Klebsiella pneumoniae(13.20%) and Escherichia coli(9.12%),respectively.The Gram-positive bacteria mainly included oxacillin-resistant Staphylococcus aureus(17.12%).In addition,the occurrence rate of fungal infection was increasing,Candida albicans was still the most frequent one(10.30%). The imipenem was the most sensitive to Gram-negative bacilli and more superior than the cefoperazone/sulbactam and ceftazidime.The oxacillcin-resistant S.aureus was still susceptive to vancomycin.CONCLUSIONS The community-acquired lower respiratory infection in oldercerebrovascular patients with sequelae is mixed with multi-resistant bacteria.
9.The effect of the combination of dexmedetomidine and sufentanil on monitoring anesthesia care during burr-hole surgery for patients with chronic subdural hematoma

Objective To evaluate the effect of the combination of dexmedetomidine and sufentanil on monitoring anesthesia care during burr-hole surgery for patients with chronic subdural hematoma. Methods 96 pa-tients underwent burr-hole surgery for chronic subdural hematoma with MAC were randomly divided into two groups:Group D and Group DS (n=48 in each group). Local anesthetic block was started at least 10 min after DEX and sufentanil infusion. Ramsay sedation scale of the two groups was maintained to 3. Anesthesia onset time, hemo-dynamics, the amount of rescue midazolam or fentanyl, the time to first dose of rescue midazolam or fentanyl, the to-tal number of intraoperative patient movements, postoperative recovery time, patient and surgeon satisfaction scores, and the adverse events were recorded. Results Compared with group D, anesthesia onset time was significantly less in group DS (13.68 ± 3.13 vs. 11.82 ± 2.43 min, P=0.002). More patients in group D required rescue midazol-am to achieve RSS=3 compared with group DS (31.25%15/48 vs. 12.50%6/48, P=0.023). Compared with group D, significantly fewer patients in group DS required rescue fentanyl to relieve pain (10.42%5/48 vs. 27.08%13/48, P = 0.036). Additionally, the total dose of rescue fentanyl in group DS was significantly higher (89.48 ± 23.27 vs. 125.28 ± 33.52μg, P=0.000), and the time to first dose of rescue fentanyl was longer than group D(18.34 ± 4.45 vs. 14.34 ± 3.63 min, P=0.000). The total number of patient movements during the burr-hole surgery was higher in group D than group DS (35.42%17/48 vs. 16.67%8/48, P=0.036). The time to recovery for discharge from the PA-CU (time to an Aldrete score ≥ 9) was significantly shorter in group DS compared with group D (17.54 ± 5.92 vs. 12.57 ± 5.28 min, P=0.000). Results from the patient and surgeon satisfaction scores showed significant differenc-es favoring group DS (P<0.05). More patients in group D showed higher levels of the overall incidence of bradycar-dia (37.50% 18/48 vs. 18.75% 9/48, P = 0.041) and hypotension(37.50%18/48 vs. 14.58%79/48, P=0.011)com-pared with group DS. Conclusions Compared with DEX alone, DEX-sufentanil associated with fewer number of in-traoperative patient movements, less amount of rescue scheme, could be safely and efficiently used for MAC during burr-hole surgery for patients with chronic subdural hematoma.
10.Clinicopathologic changes of gigantomastia

Objective To study the clinical and pathologic changes in gigantomast.Methods Tissue sections were prepared from 180 cases of breast hypertrophy and 45 cases of normal breast tissues.The morphological changes and the expression and localization of ERRγwere evaluated on the HE and immunohistochemistry stained sections between hypertrophy and normal breast tissues.Results Compared with normal breast,hypertrophic breast showed expended ducts and obvious hyperplasia of the duct epithelial papillary.Hypertrophic breast tissues demonstrated strong expression of ERR-γ in ducts and lobules.Conclusions Upregulated expression of ERRγ is identified in hypertrophic breast tissues that may associate with the development of gigantomastia.
