1.Advances in Study on Clinical Application of Proximal Transparent Cap in Digestive Endoscopy

Proximal transparent cap of endoscope has an important effect in endoscopic diagnosis and treatment, including endoscopic submucosal tumor ligation, endoscopic mucosal resection, endoscopic removal of foreign body, endoscopic variceal ligation and sclerotherapy, endoscopic submucosal dissection, peroral endoscopic myotomy, endoscopic submucosal dissection through tunnel.This article reviewed the effect of different transparent caps in endoscopic diagnosis and treatment.
2.Progresses of ultrasound in fetuses with restricted and premature closed foramen ovale

The foramen ovale (FO) is an important intra-atrium communication in fetuses.FO restriction or closure in fetuses with or without congenital heart defects can cause hemodynamics abnormality in utero.Fetal echocardiography plays an irreplaceable role in diagnosis of disease about FO.Progresses of ultrasound in fetuses with restricted or premature closed FO were reviewed in this article.
3.Diagnostic and therapeutic value of blue laser endoscopy combined with endoscopic submucosal dissection for low grade intraepithelial neoplasia

Objective To evaluate the blue laser imaging (BLI) magnifying endoscopy combined with endoscopic submucosal dissection (ESD) in diagnosis and treatment of low grade intraepithelial neoplasia detected by normal endoscopic biopsy.Methods A total of 76 patients were diagnosed as having LGIN by preoperative biopsy.But 3 months later,BLI endoscopic re-examination showed that the lesions did not heal or even worsened.Patients who received ESD therapy were included in the retrospective study during January 2014 to January 2016 in Remin Hospital of Wuhan University.The efficacy and complications of ESD,and the consistence rate of diagnosis between general endoscopic biopsy,BLI magnifying endoscopy and pathologic diagnosis of total tumor after ESD were analyzed.Results The complete resection rate and curative rate of ESD were 100.0% (76/76) and 97.4% (74/76) respectively.The incidences of hemorrhage and perforation during the operation were 3.9% (3/76) and 1.3% (1/76),and there was no delayed bleeding or perforation.The consistence rates of diagnosis of common endoscopic biopsy and BLI were 55.3% (42/76) and 92.1% (70/76) respectively,with significant difference (x2 =51.987,P<0.001).Conclusion Low-grade intraepithelial neoplasia of the upper digestive tract may be easily underestimated by ordinary endoscopy biopsy.For these underestimated low-grade intraepithelial neoplasia lesions,ESD can not only be used to confirm the diagnosis,but also give safe and effective treatment.The examination of BLI combined with magnifying endoscopy is recommended before ESD,because BLI endoscopy classification of the lesions has positive effects on ESD of upper gastrointestinal mucosa lesions.
4.Effect of acclimation training on physiological changes in a randomized controlled trial in hot-humid environment.

OBJECTIVEThis study was aimed to explore the physiological changes and the effect of heat acclimation training via a randomized control trial study.
METHODSForty healthy male volunteers were chosen and divided into experimental group and control group randomly. Those in experimental group received heat acclimation training including but not limited to meditation, unarmed run, yoga, and stepping in hot lab environment. And then, subjective feeling, rectal temperature, average skin temperature, and sweat electrolytes concentration were detected in order to describe their physiological changes. Before and after the training, both groups received some tests and their 3 000 m run-race time, nervous reaction time and subjective perception scores were recorded to evaluate the effect of acclimation training.
RESULTS(1) There was no difference in 3 000 m between the 2 groups in the same environment. Subjects' 3 000 m race time in experimental group was obviously shortened than that in control group in room temperature environment (t = 2.326, P < 0.05). And subjects' 3 000 m race time in experimental group was obviously shortened than that in control group in hot-humid environment (t = 4.518, P < 0.01). (2) Subjects' reaction time (RT) in experimental group was shortened than that in control group in room temperature environment (Z = 11.258, P < 0.05). And Subjects' RT in experimental group was sharply shortened than that in control group in hot-humid environment (Z = 6.519, P < 0.01). (3) No difference between the experimental and control groups was observed in subjective perception score (SPS) in room temperature environment. But subjects' SPS in experimental group was obviously lowered than that in control group and in hot-humid environment (t = 17.958, P < 0.01).(4) Anal temperature (AT) was lowered during training, while the change of mean skin temperature (MST) was not significant. Sweat sodium concentration (SSC) was lowered during training. SPS continued to decrease and entered plateau on the 13th day after training.(5) After acclimation training, the working capacity of the experimental group in hot-humid environment was over 85% of that in room temperature environment. While subjects' working capacity in control group in hot-humid environment was about 80% of that in room temperature environment.
CONCLUSIONHot-humid environment acclimation training improved the working capacity. After training, subjects' working capacity in hot-humid environment remained over 85% of that in room temperature environment, which was higher than that of those subjects who did not take part in training.

5.Imaging fusion in the diagnosis of prostate cancer.

Ultrasonography (US) is extensively used in the diagnosis of prostate cancer; but with its disadvantages of poor sensitivity and specificity, frequently causes underdiagnosis, and over 50% of the results of transrectal ultrasound (TRUS)-guided biopsy fail to accord with those of surgical pathology. In addition to contrast-enhanced ultrasound and real-time elastography, various new ultrasonic techniques have been developed nowadays, such as prostate histoscanning, brachytherapy template-guided 3-dimensional mapping biopsy (3DMB), and MRI/US fusion. Prostate histoscanning is an ultrasound-based technology that uses computer-aided analysis to identify and characterize the foci of prostate cancer. 3DMB, involving the whole gland, has an even higher accuracy. MRI/US fusion integrates the advantages of pre-interventional MRI with those of peri-interventional ultrasonography, and offers high-resolution images and convenient puncturing. Constant development of ultrasonography will continue to add to its clinical application value.

6.Chinese herbal medicine and therapeutic exercise for relieving spasticity in hemiplegic patients

Objective To observe the effectiveness of Chinese herbal medicine plus movement therapy in treating upper limb spasticity in hemiplegic patients. Methods Sixty-six patients were divided randomly into two groups. Thirty-five patients in the therapy group were treated with Chinese herbal medicine and movement therapy, and 31 patients in the control group were treated with movement therapy alone. The modified Ashworth scale and sur- face electromyography were applied to evaluate spasticity. Results The total effectiveness rates were 94.3% in the therapy group and 64.5% in the control group. After treatment, the average integrated electromyogram (IEMG) val- ue in the therapy group was lower than that in the control group. The difference in IEMG values before and after treat- ment in the therapy group was significantly larger than in the control group. Conclusion Chinese herbal medicine plus movement therapy is more effective than movement therapy alone in relieving spasticity.
7.Recent progress of tissue engineering scaffold materials for ACL reconstruction

The ligament deficiency has very high incidence, which can lead to disability and progressive degeneration of other structures of the joint. There is no efficient clinical treatment can fully reconstruct the injured ACL. Tissue engineering scaffold provides a novel technique to regenerate new ligament tissue ingrowth both in vitro and vivo. Thus the materials for scaffolds are especially important. The scaffolds must be biocompatible, absorbable, and primarily can induce new tissue ingrowth. This review focuses mainly on collagen and silk fibroin, which showed better proprieties for the ligament reconstruction, and also glances at several other scaffold materials. There are some progresses in the scaffold materials, but it is far away from the clinical use. Finally, the trend for the future development is outlined here.
8.Multivariate statistical analysis of external medicine for hydrocephalus therapy with TCM

Objective:To research and summarize the characteristics and medication regulars of external medicine for hydrocephalus therapy with TCM. Methods:Retrieved literatures of explicitly recorded external medicine in all ages,input its drugs into the Excel sheet specifing and specified various terms used for drug,adopted spss17.0 frequency analysis,principal factor analysis,cluster analysis, and sought its regulars.Results:Frequency analysis of external medicine from higher to lower in order as Radix Ledebouriellas,Rhizoma Bletillae,Arborvitae seed,Cinnamon bark,Ackinthepulpit tuber,Rhizoma Zingiberis ect. Withdrawed 16 main factors from 50 original targets, used cluster analysis made 16 main factors into 10 types as eliminating phlegm for resuscitation,activating blood and disinhibiting fluid etc. Conclusion: External medicine for hydrocephalus therapy mainly as drugs for activating blood and dissolving stasis, expelling wind with diaphoresis, warming yang,reinforcing kidneys to strengthening bones,regulating qi.Multivariate statistical analysis can objectively and comprehensively reflect the characteristics and regulars of external medicine and provided some exploratory thoughts.
9.Functions of semaphorin 4D in tumor progression

Semaphorin 4D (SEMA4D), also known as CD100, is a protein that belongs to class IV semaphorin. Its physiologic role in the nervous system has been extensively explored. However, the roles of SEMA4D have extended beyond the traditionally studied territories. Collective data proved that SEMA4D has an important role in the regulation of immune system, angiogenesis, and tumorigenesis, among others. Specifically, SEMA4D enhanced the angiogenesis in malignant diseases. This review summarized the latest progression in the research on SEMA4D, including the structure, receptors, mechanism, and biological functions in tumor angiogenesis and progression.
10.Roles of epithelial CTGF expression in airway remodeling in asthmatic rats

Objective To investigate the roles of connective tissue growth factor(CTGF) in airway remodeling in asthmatic rats by observation of the changes of CTGF expression in the lungs of asthmatic rats. Methods Rats were sensitized and challenged continually with ovalbumin(OVA) for the purpose of induction of asthma in rats. The distribution and types of collagens were detected by Cirius red staining. Lung hydroxyproline content was determined by acid hydrolysis. CTGF expression in rat pulmonary tissues was detected by SABC immunohistochemistry. Results After continual challenging with OVA for 21 days, collagen hyperplasia was found in the airway wall and CTGF positive expression in airway epithelium.
