1.Value of CT imaging in detection of mesenteric injury in closed abdominal trauma

Objective To investigate the CT manifestations in mesenteric injury.Methods CT imaging data were analyzed retrospectively in 28 patients with mesenteric injury caused by abdominal trauma,of which 26 patients were confirmed by surgery and 2 patients received conservative treatment.Results CT showed simple mesenteric injury in 5 cases,and multiple organ injury in other 23 cases.Only 18 cases were dignosed by preoperative abdominal CT,and the rate of missed diagnosis was 36%.Conclusion Multi-slice CT is an important evaluation method for the detection of acute mesenteric injury.
3.Transforaminal lumbar interbody fusionvia Quadrant system: early correction of lumbar spondylolisthesis

BACKGROUND:Conventional therapies for lumbar spondylolisthesis can result in trauma,bleeding and low back pain.With the vigorous development of spinal biomechanics and novel spinal fixation systems,we have more understanding on the reduction and fusion after spondylolisthesis.OBJECTIVE:To observe the clinical effects of transforaminal lumbar interbody fusionvia the quadrant system on lumbar spondylolisthesis and related biomechanical changes.METHODS:A retrospective analysis was done in 23 patients with lumbar spondylolisthesis undergoing transforaminal lumbar interbody fusionviathe quadrant system admitted from June 2012 to September 2013.Oswestry disability index and visual analog scale score were detected at 3 months and 1 year after treatment,as wel as fusion conditions and internal fixation with or without loosening or breakage.RESULTS AND CONCLUSION:Al patients were successfuly treated,with no cerebrospinal fluid leakage and nerve injury.Incisions were healed wel in al cases except one case suffered from incision infection that wascontroled after 10 days of antibiotic treatment.Al the patients were folowed up.The Oswestry disability indexes and visual analog scale scores were significantly improved at 3 months and 1 year after treatment (P <0.05),but there was no difference in these two scores at 3 months and 1 year after treatment (P>0.05).The improvement rates of Oswestry disability index and visual analog scale score were (65.3±14.8)%and (58.2±12.0)%,respectively.These findings indicate that the transforaminal lumbar interbody fusionvia the quadrant system is safe and effective to correct lumbar spondylolisthesis,maintains the biomechanical stability,improves patient's symptoms,reduces the incidence of low back pain and improves the quality of life.
4.Colon interposition for esophageal benign stricture:58 cases experience

Objective To review the experience of colon interposition in the treatment of benign esophageal stricture.Methods 58 patients who had undergone colon interposition for esophageal replacements were studied retrospectively,including 53 patients with corrosive burn esophageal strictures,3 traumatic esophageal strictures and 2 congenital esophageal strictures.The interposition colon for all patients went through substemum paths.Results There was no postoperative death in the duration of hospital stay.14 cases developed postoperative complication including 2 total colon necrosis,7 anastomotic leak,2 anastomotic stricture and 3 recurrent laryngeal nerve injury.52 patients were followed-up(ranged 1 to 16 years),40 cases were extremely satisfied(1 grade),9 very satisfied(2 grade),2 satisfied(3 grade)and 1 unsatisfied(4 grade).Conclusion Colon interposition is an ideal procedure for esophageal benign stricture.
5.Effect of remote ischemic postconditioning on brain injury after asphyxial cardiac arrest-resuscitation in rats

Objective To investigate the effect of remote ischemic postconditioning on brain injury after asphyxial cardiac arrest-resuscitation in rats.Methods Sixty-nine male Sprague-Dawley rats were randomly divided into 3 groups (n =23 each) using a random number table:sham operation group (group S),asphyxial cardiac arrest-resuscitation group (group CA-CPR),and remote ischemic postconditioning group (group RIPost).The animals were anesthetized with intraperitoneal 1% pentobarbital 40 mg/kg,intubated and mechanically ventilated.Asphyxia was induced by occlusion of the tracheal tube and resuscitation was started 8 min later.In RIPost group,RIPost was produced by 3 cycles of 15 min occlusion of the right hind femoral artery-15 min release of the right hind femoral artery after restoration of spontaneous circulation (ROSC).Neurological deficits were evaluated and scored at 24 h,72 h and 7 days after ROSC.Neuron specific enolase (NSE) concentration in serum was assessed at 48 h after ROSC by ELISA.At 3 days after ROSC,the number of viable neurons in hippocampal CA1 region was recorded (by N issl's staining).Morris water maze test was used to quantify spatial learning and memory deficits after ROSC.Results Compared with group S,the neurological deficit score at each time point and the number of viable neurons in hippocampal CA1 region were significantly decreased,the NSE concentration in serum was increased,the escape lantency was prolonged,and the target quadrant residence time percentage and distance percentage were decreased in CA-CPR and RIPost groups.Compared with group CA-CPR,the neurological deficit score at each time point and the number of viable neurons in hippocampal CA1 region were significantly increased,the NSE concentration in serum was decreased,the escape lantency was shortoned,and the target quadrant residence time percentage and distance percentage were increased in group RIPost.The damage to neurons in hippocampal CA1 region was significantly mitigated in group RIPost as compared with group CA-CPR.Conclusion Remote ischemic postconditioning can reduce brain injury after asphyxial cardiac arrest-resuscitation in rats.
6.Weekly docetaxel combined with cisplatin versus weekly docetaxel combined with oxaliplatin in the treatment of advanced gastric cancer

Background and purpose:It has confirmed that docetaxel, in combination with cisplatin or oxaliplatin have good effect in the treatment of advanced gastric cancer. This study in order to observe the clinical efficacy and adverse reaction of weekly docetaxel combined with cisplatin (DP) versus weekly docetaxel combined with oxaliplatin (DO) as the firrst-line treatment of advanced gastric cancer.Methods:A total number of 76 cases of advanced gastric cancer were randomly assigned 2 arms, 38 per arm. DP regimen group (docetaxel 35 mg/m2,ivgtt,dl,d8 combined with cisplatin 60 mg/m2, ivgtt,dl,repeated every 3 weeks) and DO regimen group(docetaxel 35 mg/m2,ivgtt,dl,d8 combined with oxaliplatin 120 mg/m2 ivgtt,dl,repeated every 3 weeks). Results:No significant difference was found between DP regimen group and DO regimen group on the objective RR(37%vs 41%),PFS (4.9vs 4.4 months) and OS (9.7vs 12.3 months). The main grade 3 or 4 toxicity in the DP and DO groups was neutropenia, DO was less associated with nausea and vomiting, but more associated with peripheral neuropathy than DP group. No signiifcant difference was found between DP regimen group and DO regimen group on the anemia, thrombocytopenia, diarrhea, alopecia (P>0.05).Conclusion:Weekly docetaxel combined with cisplatin (DP) shows similar efifcacy and toxicity compared with weekly docetaxel combined with oxaliplatin (DO) as the ifrst-line treatment of advanced gastric cancer and worthy of further study.
7.Analysis of general curriculum satisfaction survey in local medical college

Objective To learn students' satisfaction with the general education courses, discuss the problems in comprehensive general education courses in local medical colleges , and provide the basis and reference for the medical general education courses reform. Methods Using the stratified random sampling method, self-designed questionnaire to local medical colleges of 575 different professions stu-dents. The questionnaire had the total of 54 topics whose contents were related to the basic situation aware-ness and satisfaction, the demand for general education courses, general education curriculum problems and suggestions. The survey data were analyzed using SPSS 19.0 software and the results were expressed by the relative number(rate and percentage). The differences between different majors and different grade student satisfaction were analyzed by using rank-sum test, showing statistical significance(P<0.05). Results The total satisfaction was not very high, only 219 students(38%) were satisfied with general curriculum;256 students(45%) thought it didn't matter whether to set general education courses;100 students(17%) were not satisfied. In addition, satisfaction with different grades was undifferentiated (P=0.798), but there was a statistical difference among the students of different professions(P=0.007). Conclusion The general education curriculum of medical schools should not blindly pursue being wide and full. Instead, it should be built from three aspects such as core curriculum, flexible teaching methods, teaching methods reform to enhance teaching effectiveness.

Apolipoprotein C3 (APOC3) is a multifunctional protein.Its concentration is positively correlated with triglyceride (TG) levels and is an independent risk factor for coronary heart disease (CAD) prediction and development.Recent studies showed that APOC3 could not only regulate triglyceride-rich lipoproteins (TRL) metabolism,but also regulated endothelial function,that is,it could induce endothelial dysfunction and disorders of lipid metabolism.Gradually,APOC3 induces atherosclerosis (AS),increases the risk of the occurrence of CAD and other related diseases.APOC3 polymorphisms are also closely related to related diseases.

Objective To observe the effect of transplanting the adipose-dervived stem cells(ADSCs) on free radical metabolism and immune function of rat aging model induced by D-galactose from fasiaology perspective;to explore a new method for anti-aging. Methods The ADSCs were cultured in vitro. Thirty male SD rats were randomly divided into 3 groups: normal control group(A), aging model group(B) and treat group(C). Ten rats in each group. Rats in B and C groups were injected D-galactose continually into make the sub-acute aging model rats.After 8-week injections of D-galactose;R3ats in group C were injected ADSCs(3×10~6/ml) through caudal vein. After 2-week transplantions of ADSCs, T-SOD, CuZn-SOD, MDA, NO, IL-2 and spleen index levels in serums of each group were detected and compared among the three groups. Results Compared with the A group, the SOD, NO, IL-2 level and spleen index in serum in group B decreased significantly, while the contents of MDA increased significantly. Compared with group B, the SOD, NO, IL-2 level and spleen index in serum in group C had been improved, and the contents of MDA decreased significantly. Conclusion Transplanting ADSCs can improve the antioxidant ability and strengthen the cellular immune function of aging rats.Further more, it can delay the ageing procedure induced by D-galactose in rats.
