1.Effect and safety of methylprednisolone pulse therapy on spinal nerve function in patients with acute spinal cord injury

Objective To investigate the effect of methylprednisolone pulse therapy on spinal nerve functions in patients with acute spinal cord injury (ASCI) and evaluate its safety. Methods 200 patients with ASCI treated in our hospital from January 2010 to December 2016 were selected and were randomly divided into two groups,with 100 cases each group. The patients in control group were treated with routine basic treatment while patients in the study group were treated with methylprednisolone pulse therapy. On the basis of the American spinal cord injury Society scoring criteria (ASIA), the neurological functional recovery scores were performed were scored before and 6 weeks after treatment between the two groups and the adverse reactions were recorded. Results The scores of sensory and motor function of the patients in the two groups were higher than those before the treatment and there was statistical difference (P<0.05), and the scores in the study group was significantly higher than those in the control group, and the difference between the two grouPs was statistically significant (P<0.05). The incidence of adverse reactions in the study group (25.00%) was significantly higher than that in the control group (10.00%), and the difference was statistically significant (P<0.05). The incidence of lung infection and gastrointestinal reaction in the study group were significantly higher than those in the control group and there were statistical difference (P<0.05). Conclusion Methylprednisolone pulse therapy has significant effect on the recovery of neurological function in patients with ASCI, but the incidence of adverse reactions is high, so it is necessary to strengthen the monitoring and intervention in clinic.
2.Design of hospital appointment registration process based on WeChat public platform

Objective To introduce a new appointment registration way in the outpatient department.Methods A WeChat appointment registration platform based on HIS was established with smart phone and WeChat public platform,which was gifted with the functions of online registration and payment and etc.The processes including preliminary diagnosis and return visit,cancelling appointment,treatment of appointment breaking,and the limitations during the appointment could be accomplished by the appointment registration.Results The platform realized appointment registration,information feedback and etc.Conclusion The appointment registration based on WeChat public platform contributes to satisfying the patient,enhancing medical service and distributing medical resources rationally.
3.Expression of Heat Shock Protein 70 and Bag-1 Protein in Gastroenteric Cancer

Objective To observe the expression of bag-1 and heat shock protein (HSP) 70 in clinical malignant tumors and the correlation between them. Methods RT-PCR and Immunohistochemistry were employed to examine the expression of bag-1 and HSP70 in 69 cases with gastroenteric cancer (54 cancer tissues and 15 non-cancer tissues). Results The expression of Bag-1 was detected in 96.3% of the cases, much higher than that in the control group (P0.05), and no correlation was observed between histological grade and HSP 70 expression. Conclusion There exists definite relationship between the gastroenteric cancer and the expression of bag-1 and HSP70, and there are close correlation between the tumor progress and the expression of bag-1.
4.Effects of stress stimulation on repair process of rotator cuff tear

Objective To explore the role and mechanism of stress stimulation in the repair process of the rotator cuff. Methods The supraspinatus tendon was cut off at the osteotendinous junction (OTJ) and then sutured in situ (Carpenter's model). The experimental group started treadmill running after one week immobility, and the control group were free in the cage. All animals were sacrificed at 4 time points (2, 4, 8 and 16 weeks after operation). Histological changes, immunohistology of yon Willebrand factor and tenascin-C at OTJ were compared between the 2 groups. Results There was no significant difference between the 2 groups in the early repair period. Significant differences between the 2 groups in histology and angiogenesis appeared 8 weeks after operation, and significant differences in expression Of Tenascin-C appeared 16 weeks after operation. Conclusion As stress stimulation can promote angiogenesis and expression of Tenascin-C at OTJ, facilitating the reconstruction of bone and tendon, it plays a critical role in the repair process of rotator cuff tear.
5.Application of clinical pathway teaching model in orthopedics clinical teaching

Objective To examine the efficacy of applying clinical pathway (CP) teaching model in orthopedics clinical teaching.Methods Totally 64 medical undergraduates were randomized into 2 groups.The traditional teaching method and CP model were separately preformed on them.Their scores of theoretical,manipulation and case analysis exams at the end of internship were compared.Meanwhile the questionnaire of satisfaction degree also was conducted.Results Scores of exams and satisfaction degree were better in CP group than in classical group (P < 0.05).Conclusions The novel CP model can promote the standardization and systematization of clinical teaching in orthopedics.It is conducive to evaluating teaching effect and performing teaching reform.Meanwhile,it is in accordance with the problem-based learning and can improve clinical teaching effect and satisfaction degree.
6.Parthenolide inhibits neuroinflammation and promotes neurogenesis in the ischemic striatum following transient middle cerebral artery occlusion in the adult rat brain

AIM: The objective of this study was to test the hypothesis that parthenolide suppresses ischemia-induced neuroinflammation in the MCAO model of adult rat. METHODS: MCAO rats were treated i.p. with parthenolide (500 μg/kg). Brain sections were analyzed for BrdU, BrdU-DCX, BrdU-Tuj-1, BrdU-MAP-2 and BrdU-GFAP staining. Total protein was extracted from ischemic striatum, and Western blot was used to determine TNF-α expression. RESULTS: Cerebral ischemia increases expression of TNF-α in the ischemic striatum. Parthenolide suppressed the expression of TNF-α and enhances the proliferation of newborn cells in the ischemic striatum. The cell number of BrdU~+-DCX~+, BrdU~+-Tuj-1~+, and BrdU~+-MAP-2~+ is increased in the ischemic striatum after parthenolide treatment at 3 d, 7 d or 28 d after MCAO. Furthermore, parthenolide depressed the cell number of BrdU~+-GFAP~+ in the ischemic striatum at 3 d, 7 d and 28 d after MCAO. CONCLUSION: Parthenolide inhibits neuroinflammation induced by cerebral ischemia and promotes neurogenesis in the ischemic striatum. Further study of the effects of parthenolide on inflammatory gene expression using model animal systems as described here are critical to elucidating their mechanisms of action.
9.Management of valgus or varus knee deformity with fixator assist nailing technique and distal femur osteotomy

Objective:To discuss the result of treating valgus or varus knee deformity with fixator assist nailing (FAN)technique and distal femur osteotomy(DFO).Methods:In this retrospective study,14 patients with 17 limbs,12 valgus and 5 varus knee deformity were treated with DFO and FAN.The aver-age age of the patients was 23 (17-44)years .The unilateral external fixator was mounted on the lateral side of femur,the minimal invasive distal femoral osteotomy was performed.After the deformity was cor-rected,the intramedully nail was inserted to fix the femur.None of the patients need bone autograft.The preoperation and postoperation lateral distal femur angle (LDFA ),mechanical axis deviation (MAD ) and range of motion (ROM)were measured and analyzed.Results:All the 14 patients were followed-up for 12 -72 months (average 34 months).The osteotomy site united in 3 -5 months (average 3.5 months)post-operatively.According to Paley’s functional scores,12 patients were excellent,2 patients were good.We achieved desired postoperative MAD (from medial 15 mm to lateral 10 mm)in 15 limbs of 12 patients.The LDFA in 1 1 limbs was corrected to normal (84°-90°).The ROM was not signifi-cantly changed before and after operation in the both groups.No infection or neuro-vascular injury oc-curred.Conclusion:The FAN technique is an effective method to treat valgus knee or varus knee de-formity in young patients with DFO.
10.Efficacy of oxycodone for painless gastroscopy in pediatric patients when combined with propofol

Objective To evaluate the efficacy of oxycodone for painless gastroscopy in pediatric patients when combined with propofol.Methods Sixty pediatric patients of either sex,aged 2-7 yr,weighing 10-28 kg,of American Society of Anesthesiologists physical status Ⅰ or Ⅱ,scheduled for elective painless gastroscopy,were divided into 2 groups (n=30 each) using a random number table:oxycodone group (O group) and sufentanil group (F group).The pediatric patients held by their parents entered the gastroscopy room,and propofol 1.5-2.0 mg/kg and penehyclidine hydrochloride 0.3 mg were intravenously injected.Oxycodone 0.1 mg/kg and sufentanil 0.1 μg/kg were intravenously injected in O and F groups,respectively.When body movement occurred during operation,additional propofol 1.0 mg/kg was intravenously injected.The occurrence of adverse reactions such as intraoperative body movement,laryngospasm and respiratory depression was recorded.The occurrence of postoperative vomiting and pain (FLACC pain scale scores >3) was also recorded.Results The incidence of intraoperative body movement,laryngospasm and respiratory depression and postoperative pain was significantly lower in O group than in F group (P<0.05).Conclusion When combined with propofol,oxycodone produces better efficacy than sufentanil for painless gastroscopy in pediatric patients.
