2.The study on duodenogastric reflux after cholecystectomy

Objective To observe the alteration of duodenogastric reflux after cholecystectomy so as to explore the basis for diagnosis and therapy. Methods Intragastric bile reflux during 24 hours was assessed using ambulatory bilirubin monitoring and 24-hour pH monitoring techniques in 20 cholecystectomy patients, 10 cholelithiasis patients and 15 healthy volunteers. Results Bile reflux and alkaline reflux of cholecystectomy patients did not increase compared with those of cholelithiasis and the control. Conclusion Duodenogastric reflux of patients receiving postcholecystectomy patients does not increase.

4.Investigation in lower extremiy injuries of new recruits in a Chinese special military force

[Objective] To study the features in lower extremiy injuries of new recruits in the Chinese special military force during Basic Training Program,and supply scientific precautionary measures to reduce the military training injury.[Method]A special military force group had been chosen and the circumstance of military injuries in lower extremiy had been observed from Jan.to March in 2006.The population selected for study included all 281 new recruits of the unit.A questionnaire was designed to document all of the correlated information,and all injuried soldiers were examined and inquired about the case history.All data had been classified for analysis.[Result]In all 281 samples,75 cases occurred lower extremiy injuries(up to 26.69%).In all lower extremiy injuries,35 cases were leg injuries(up to 46.67%),17 cases were ankle injuries(up to 22.67%),11 cases were foot injuries(up to 14.67%).In all injuries,20 cases were acute injuries(up to 26.67%),55 cases were excess fatigue injuries(up to 73.33 %).In all injuries,38 cases were bone and joint injuries(up to 50.67%),37 cases were soft tissue injuries(up to 49.33%).The reasons for injuries were training field(l 8 cases),exercise overload(61 cases).In all training programs,64 cases were occured in 3 kilo run.[Conclusion]Lower extremiy injuries are common and frequently occurring illness.The features and methods of injuries are different and complicated.In this study,excess fatigue injuries caused by exercise overload were the most important.After comprehened the occurring rule,it should be emphasized on health propaganda.During basic military training,we should pay attention to soldiers' health condition.Training should be carried out step by step,to reduce the occurrence lower extremiy training injuries.
5.Indication of video-assisted thoracoscopic decortication in treatment of tuberculous empyema

Objective To evaluate the efficacy of decortication by video-assisted thoracic surgery (VATS) in pa﹣tients with tuberculous empyema, and discuss its indications. Methods 60 patients with tuberculous empyema who underwent decortication by VATS for surgical management from December 2010 to December 2015 were included. Under a thoracoscope, we cleaned up the pus, separated adhesions, scraped granulation tissues and caseous necrosis on the inner wall of the abscess cavity, and stripped the thickened fiberboard of the parietal and visceral pleurae. Af﹣ter the procedure, sufficient drainage and antituberculosis therapy were carried out. Results All the patients in this group were operated successfully. All the patients were cured without perioperative death and complications. No re﹣currence of empyema was observed at the follow-up examination from 2 months to 5 years, and suffered pulmonary reexpansions were better. Conclusions The decortication by VATS for tuberculous empyema is safe, effective, mini﹣mally invasive. The imaging manifestations of pleural thickening in 1 cm, no obvious calcification, no serious lesions in the lungs are the indications for the operation.
6.Effects of penehyclidine hydrochloride pretreatment on expression of nitric oxide synthase and cell apoptosis in rats with endotoxin-induced acute lung injury

Objective To evaluate the effects of penehyclidine hydrochloride pretreatment on the expression of nitric oxide synthase (NOS) and cell apoptosis in lung tissues in rats with endotoxin-induced acute lung injury (ALI).Methods Forty adult male Sprague-Dawley rats,weighing 220-270 g,were randomly divided into 5 groups (n =8 each) using a random number table:control group (group C),endotoxin-induced ALI group (ALI group),and penehyclidine hydrochloride 0.03,0.10,0.30 mg/kg groups (Pi-3 groups).ALI was induced with lipopolysaccharide (LPS) 5 mg/kg which was injected via the caudal vein.In P1-3 groups,penehyclidine hydmchloride 0.03,0.10 and 0.30 mg/kg were injected intraperitoneally,respectively,at 1 h before LPS injection,while the equal volume of normal saline was administered in C and ALI groups.The animals were sacrificed at 4 h after ALI models were successfully established and pulmonary specimens were obtained for determination of the cell apoptosis (by TUNEL),expression of NOS mRNA (using PT-PCR),and Bcl-2 and Bax protein (by Western blot),NOS activity (using chemical colorimetry) and NO content (by using nitrate reductase method) and for examination of pathological changes (by light and electron microscopes).Apoptotic index (AI) and the ratio of Bcl-2/Bax was calculated.Results Compared with group C,NOS mRNA and Bax protein expression was significantly up-regulated,NOS activity,NO content and AI were increased,Bcl-2 protein expression was down-regulated,the ratio of Bcl-2/Bax was decreased (P < 0.01),and the degree of pathological changes of the lung was aggravated in ALI and P1-3 groups.Compared with ALI and P1 groups,NOS mRNA and Bax protein expression was significantly down-regulated,NOS activity,content of NO and AI were decreased,Bcl-2 protein expression was up-regulated,the ratio of Bcl-2/Bax was increased (P < 0.01),and the degree of pathological changes of the lung was alleviated in P2-3 groups.There was no significant difference in the indexes mentioned above and results of pathological changes of the lung between group ALI and group P1,and between group P2 and group P3 (P > 0.05).Conclusion Penehyclidine hydrochloride pretreatment ameliorates endotoxin-induced ALI by inhibiting NOS expression and cell apoptosis in rat lung tissues.
7.Cochlear implantation in patients with middle ear inflammation

OBJECTIVE Explore the waysof cochlear implantation(CI)surgery technique for bilateral profound sensorineural hearing loss patients with otitis media and mastoiditis. METHODS CI was performed in 29 patients with different of otitis media and mastioditis,26 of them underwent a single-stage operation,and 3 had a two-stage operation,according to the degree and extent of lesion. For patients with mild inflammation,cochlear implantation was performed after complete eradication of inflammation at the same protocol the patients with severe inflammation underwent an intact canal wall tympanomastoidectomy or reconstructed it initially,cochlear implantation was performed after the initial procedures. RESULTS One- stage or tow-stage operations of CI were carried out for 29 patients with otitis media and mastoiditis. All electrodes were implanted successfully,in which the CI went normally and electrode array were protected well. None of the cases showed recurrence of infection during an average follow-up period of 2-8 years. CONCLUSION The CI could be performed in otits media and mastoiditis patients after reversionary surgery . Individual management based on the degree and extent of the local lesion was emphasized.
8.Therapeutic effect of hesperidin on adjuvant arthritis in rats and its mechanisms

Aim To study the therapeutic effect of hesperidin (HDN) on adjuvant arthritis (AA) in rats and its mechanisms.Methods Freund's complete adjuvant (FCA) was used to induce AA in rats. Secondary paw swelling of AA rats was measured with volume meter. Splenic lymphocyte proliferation response induced by concanavalin A (ConA) or lipopolysaccharide (LPS) was examined with MTT assay. IL-2 production of splenic lymphocytes and IL-1, IL-6,TNF-? productions of peritoneal macrophage (PM?) were determined by radio-immunity assay.IL-10 production of PM? was estimated by enzyme linked immunosorbent assay (ELISA).Results The secondary inflammation of AA rats appeared on the 12th day after injection of FCA. At the same time (d 12), HDN (40,80,160 mg?kg-1,?12 d) were given to AA rats by intragastric administration. It was found that HDN(80, 160 mg?kg-1) could significantly inhibit the secondary paw swelling of AA rats from the 20 th day.The suppressed lymphocyte proliferation and IL-2 production of splenic lymphocytes in AA rats were reversed by treatment with HDN. Meanwhile,HDN could remarkably down-regulate IL-1,IL-6,TNF-? productions of PM? and up-regulate IL-10 production of PM?.Conclusions The results suggested that HDN had therapeutical effect on AA rats. Its mechanisms may be related to adjusting abnormal immune function in AA rats and keeping the balance of cytokine network.
9.Protective effect of ulinastatin on acute lung injury induced by orthotopic liver transplantation

Objective To evaluate the effects of ulinastatin on acute lung injury during ortho- topic liver transplantation(OLT).Methods Twenty ASAⅢ-Ⅳpatients with end-stage liver disea- ses.undergoing OLT were randomly divided into two groups.Ulinastatin group received intravenous infusion of ulinastatin(3?10~4 IU in 100 normal saline)after skin incision and every 4 h thereafter(n =10).Control group received same amount of normal saline instead of ulinastatin(n=10).Blood sam- pies were taken before skin incision,120 min after skin incision,30 min after liver was removed,5 min and 60 min after reperfusion of the graft and at the end of operation for determination of plasma IL-6, IL-8,TNF-?,MDA concentrations and SOD activity.Respiratory index(RI)[P_(A-a)DO_2/PaO_2]was cal- culated before skin incision,30 min after liver was removed,5 min and 60 min after reperfusion of the graft and at the end of operation.After anesthesia was induced,cardiac output,mixed venous oxygen saturation and central venous temperature were continuously monitored during operation.ECG,CVP, SpO_2,P_(ET)CO_2,radial artery and MPAP were also continuously monitored during operation.P_(ET)CO_2 was maintained at 35-40 mm Hg during operation.Blood temperature was maintained above 35.5℃during operation.Results In group C plasma IL-6 and IL-8 concentrations were significantly increased from 120 min after skin incision to the end of operation as compared with the baseline values(P0.05).RI was significantly lower at 60 min after reperfusion of the graft and at the end of operation in group U than in group C(P
10.Immunomodulative effect of hesperidin on immunodepressed mice

Aim To investigate the immunomodulative effect of hesperidin on immunodepressed mice.Methods The immunosuppressed mice were induced by cyclophosphamide (Cy, ip). Indexes of immune organs were calculated. Phagocytosis of mononuclear macrophage was determined by cleaning carbon particle method. Spectrophotography was used to estimate levels of serum specific IgG, IgM (HCIgM, HCIgG). Plaque forming cell (PFC) was determined with quantitative haemolysis of SRBC (QHS). Splenic lymphocytes proliferation was measured by MTT method. The mouse delayed type hypersensitivity (DTH) model induced by dinitoflruorobenzene (DNFB) was used to study the effect of hesperidin on the level of DTH and subset of T lymphocyte. Results Hesperidin remarkably increased indexes of spleen and thymus, the rate of clearance and clearance index, but had no significant impact on HCIgM, HCIgG and PFC. In addition, it could enhance the proliferation of splenic lymphocytes and reverse DTH response to normal level. Conclusion Our results indicated that hesperidin had an enhanced effect on nonspecific immunity and specific cellular immunity in immunodepressed mice, while specific humoral immunity wasn′t significantly changed.
