Objective To establish an extended hyperinsulinemic euglycemic clamp for the study of insulin sensitivity in Chinese. Methods Combining glucose clamp, 3 3H labelled glucose tracer technique and indirect calorimetry, an extended hyperinsulinemic euglycemic clamp technique was applied into the study of methodology in 9 normal weight subjects with normal glucose tolerance. Results (1) When a higher level of insulin was created during maintaining euglycemia, hepatic glucose production was completely inhibited, and the counter regulatory hormones (including cortisol, growth hormone and glucagon) and endogenous insulin secretion were not significantly stimulated. 3.DNA-associated autoantibodies to cell membrane for rapid detection of systemic lupus erythematosus

Objective To evaluate the expression of autoantibodies to cell membrane associated with DNA (mDNA) in systemic lupus erythematosus (SLE).The specificity of these antibodies differs from antibodies to double stranded DNA.Method Using indirect immunofluorescence,serum samples were detected giving a characteristic pattern of peripheral membrane fluorescence on cultured HL60.Results This pattern was observed in 81 of 90 serum samplas of SLE patients,but absent in the serum samples of 35 rheumatoid arthritis,31 ankylosing spondylarthritis and 42 blood donors.In 20 Sjgren syndrome′s patients only one showed a positive test.Conclusion This novel rapid immunofluorescence method may serve as an identification test of SLE patients.Due to the sensitivity (90%) and specificity (98 8%),it is better than the other diagnostic tests such as the detection of antibodies to dsDNA and Sm.
4.Ultrasound findings of primary hepatic malignant fibrous histiocytoma

Objective To investigate the diagnosis and differential diagnosis of primary hepatic malignant fibrous histiocytoma (MFH) by ultrasound. Methods A retrospective analysis of clinical data and ultrasonic findings was performed on 16 patients with pathologically proven hepatic MFH from July 2005 to August 2012 in Eastern Hepatobilary Surgery Hospital of the Second Military Medical University. Results Of the 16 patients, all the tumors were single, which including 9 tumors located in the right lobe, 6 tumors located in the left lobe, and 1 tumor located in caudate lobe of the liver. The tumor sizes were larger than 10.0 cm in 3 cases, between 5.0 cm and 10.0 cm in 10 cases, and less than 5.0 cm in 3 cases. Tumor internal echo was heterogeneous, including 1 case with honeycomb pattern and 2 cases with mosaic sign. Most tumors were unclearly-deifned (11 cases) without obvious halo. The pathologic results identiifed 15 cases of primary hepatic MFH and 1 case of primary intrahepatic bile duct malignant ifbrous histiocytoma. Conclusions It is dififcult to make correct clinical diagnosis of hepatic MFH. A comprehensive analysis of ultrasound ifndings and clinical information is needed to obtain an accurat diagnosis of malignant liver tumors.
5.Comparison of the application of sirolimus eluting stents and paclitaxel eluting stents in coronary lesions

Objective To observe the security, major cardiac adverse events and target lesion revascularization (TLR) in patients underwent Sirolimus Eluting Stents and Paclitaxel Eluting Stents implantation. Methods A total of 224 consecutive patients from Dec, 2002 to Dec, 2004 who randomized to receive SES and PES implantation were enrolled with 168 male cases and 56 female cases. The average age was 60.9?11.1 years (35-86 yrs) and the median follow-up period was 13.4?6.7 months (3-27 ms). Results There was no difference in baseline characteristics between both groups. No acute and subacute thrombosis events occurred in both groups but two delayed thrombosis occurred in the PES group. There were six MACEs in the PES group, and among them two patients occurred delayed thrombosis and four patients underwent target lesion revascularization. There was one patient underwent TLR in the SES group. Conclusion Both drug eluting stents reduced MACEs and TLR, but the rate of delayed thrombosis, MACEs and TLR was lower in the SES group than in the PES group.
6.Effects of PPV combined with internal limiting membrane flap surgery on idiopathic macular hole

Objective To compare the efficacy and safety of vitrectomy combined with internal limiting membrane flap and vitrectomy combined with internal limiting membrane peeling for the treatment of idiopathic macular hole with different sizes.Methods A total of 127 consective patients (127 eyes)were divided into two groups according to the size of the hole diameter of the smallest split points by less than or equal to 500 μm (small diameter macular hole group) and more than 500 μm (huge diameter macular hole group).According to different surgical methods the patients were divided into non ILM flap coverage group (peeling 1 group and peeling 2 group) and ILM flap cover group (covering 1 group and covering 2 group).All the patients underwent vitrectomy combined with internal limiting membrane peeling or vitrectomy combined with limiting membrane flap.Preoperative and postoperative best correct visual acuity,closure ratio of macular hole and postoperative major complications were observed and followed up.Results The postoperative best correct visual acuity improved in all the groups,there was no significance difference between small diameter macular hole group and huge diameter macuiar hole group (t =0.112 2,0.750 8;all P >0.05).The closure ratio of peeling 1 group and covering 2 group at postoperative 6 months were all 100%,there was no statistical difference (P > 0.05),which in peeling 2 group and covering 2 group were 84.85% and 100.00%,there was statistical difference (x2 =13.292,P < 0.05).There was no statistical difference in preoperative defect diameter of the inner and outer junction between peeling 2 group and covering 2 groups (P >0.05),there was also no statistical difference between peeling 2 group and covering 2 groups at postoperative 1 months (P > 0.05),but there were statistical differences at postoperative 3 months,6 months and 12 months (all P < 0.05),the covering 2 group were less than the peeling 2 group.Conclusion ILM flap coverage helps to heal macular holes greater than 500 μm diameter,and has no extra effect on healing of diameter less than 500 μm.
7.Paired observation of californium-252 neutron intraluminal brachytherapy combined with external-beam radiotherapy with and without lead shielding for cervical cancer

Objective To compare the efficacy between californium?252 ( 252 Cf ) neutron intraluminal brachytherapy combined with external?beam radiotherapy with lead?shielding pelvic parallel opposing field technique and non?lead?shielding four?field box technique for cervical cancer. Methods A total of 52 patients with stage Ⅱa?Ⅲb cervical squamous cell carcinoma who were admitted to our hospital from 2004 to 2007 were enrolled as subjects and paired by clinical stage, age, tumor size, and degree of anemia. The 26 pairs of patients were divided into lead?shielding pelvic parallel opposing field group (lead?shielding group) and non?lead?shielding four?field box group (non?lead?shielding group). For all patients in both groups, 252 Cf neutron brachytherapy was added in external?beam radiotherapy. The local control (LC), overall survival (OS), and disease?free survival (DFS) rates were calculated using the Kaplan?Meier method and analyzed using the log?rank test. The difference in the incidence of late complications was analyzed using the McNemar method. Results There were significant differences in 5?year LC, OS, and DFS rates between the lead?shielding group and the non?lead?shielding group (85% vs. 81%, P= 0?? 014;89% vs. 73%, P=0?? 013; 89% vs. 73%, P= 0?? 013 ). There was also significant difference in the incidence of late complications between the lead?shielding group and the non?lead?shielding group ( 12% vs. 23%, P=0?? 008). Conclusions When intraluminal brachytherapy combined with external?beam radiotherapy is used to treat cervical cancer, the centers of the front and back fields should be shielded by lead, regardless of whether the parallel opposing field technique or the four?field box technique is used.
8.Experimental study on anti-endotoxin activity of a tetrahydropyrimidine derivative, ZL-5015

[ ABSTRACT] AIM:To investigate the protective effect of 1, 3-dicyclopentyl-1, 2, 3, 6-tetrahydropyrimidine-4, 5-dicarboxylic acid diethyl ester (ZL-5015) on lethal endotoxin-challenged mice and to explore the underlying mechanism. METHODS:Mouse model of lethal endotoxin challenge and endotoxemia were established by intraperitoneal administration of lipopolysaccharide (LPS) at a dose of 70 mg/kg to the C57BL/6J mice.Mouse peritoneal macrophages stimulated with LPS (10 mg/L) were used as an in vitro inflammatory model.The levels of interleukin-1β( IL-1β) , interleukin-10 ( IL-10) and tumor necrosis factor-α(TNF-α) were measured by enzyme-linked immunosorbent assay (ELISA).Real-time PCR was used to evaluate the mRNA expression of the cytokines.RESULTS:Prophylactic treatment of the mice with ZL-5015 (100 and 200 mg/kg, ig) slightly increased the survival rate, extended the survival time, decreased the serum levels of IL-1βand TNF-α, and increased the serum level of IL-10 in the early stage of endotoxemia as compared with model group.The results of in vitro study demonstrated that treatment of the endotoxin-stimulated mouse peritoneal macrophages with ZL-5015 (10, 20 and 40μmol/L) inhibited the expression of IL-1βand TNF-αat both mRNA and protein levels but promoted the expression of IL-10 at both mRNA and protein levels.CONCLUSION: The tetrahydropyrimidine derivative ZL-5015 shows a moderate anti-endotoxin effect by increasing the survival rate and extending the survival time of the mice challenged by endotoxin, which may result from inhibition of the expression of pro-inflammatory cytokines such as IL-1βand TNF-α, and promotion of the expression of anti-inflammatory cytokine IL-10.
9.Effects of curcumin on the expression of heat shock protein 70 in the hippocampus and spatial learning ability in APPswe/PS1△9 transgenic mice

Objective To investigate the effect of curcumin on the expression of heat shock protein 70 (HSP70) in the hippocampus and spatial learning ability in APPswe/PS1△9 transgenic mice,and provide a new theoretical basis for the treatment of Alzheimer' s disease (AD).Methods Mice were divided into wild-type (WT) group,APP/PS1 transgenic group,curcumin-treated group,quercetin combined with curcumin-treated group.The 5-month-old mice in curcumin-treated group received 150 mg · kg-1 · d-1 curcumin by intraperitoneal injection for four consecutive weeks.In addition to curcumin,the mice in quercetin combined with curcumin-treated group also received quercetin 50 mg· kg-1 · d-1 intraperitoneally for four consecutive weeks.The expression of HSP70 in the hippocampus of APP/PS1 mice was detected by Western blot.Learning and memory ability of APP/ PS1 mice were measured by Morris water maze test.The latency of finding the platforms and the number of crossing the platforms were observed.Results The results of Western blot analyses showed that the expression of HSP70 in the hippocampus of APP/PS1 transgenic mice decreased significantly compared with that of WT mice (OD:APPswe/PS1△9:0.31±0.14;WT:1.14±0.51;P<0.01).The expression of HSP70 in curcumin-treated group significantly increased compared with that of APP/PS1 transgenic group (OD:0.91±0.20 in curcumin-treated group;P<0.05).The results of water maze test showed that compared with WT group,in APP/PS1 group,the latency of finding the platforms was significantly longer (APPswe/PS 1 △9:(82.3 ± 6.8) s;WT:(19.5±4.4) s),and the number of crossing tbe platforms were significantly decreased (APPswe/PS1 △9:(2.7± 1.1);WT:(5.2± 2.1)).In curcumin-treated group,the latency of finding the platform ((47.7±7.6) s) was significantly shorter and the number of crossing platform ((4.5±1.9) s) was more than those in APP/ PS1 group.However,in quercetin combined with curcumin-treated group,the latency of finding the platform was significantly longer ((69.7±9.1) s) and the number of crossing platform (3.2±1.6) was decreased compared with those in curcumin-treated group.Conclusion Curcumin can promote the expression of HSP70 in the hippocampus of APP/PS1 transgenic mice,thus improve spatial learning ability of APP/PS1 transgenic mice.These results provide evidences that curcumin may have potential therapeutic value for the treatment of AD.
